Tumors of the epididymis are rare. The most common one is adenomatoid tumor. At least more than two hundred cases have been reported in the literature but only one was reported by us last year in this country. We add another one which is involving bilateral epididymis simultaneously and review literature briefly.

Adenomatoid tumors arising in the tunica vaginalis are relative rare neoplasms, and usually benign in nature. An appropriate treatment of an adenomatoid tumor is local excision. Herein, a case of an adenomatoid tumor of the tunica vaginalis, in a 40-year-old man, is described. An orchiectomy was performed to excise the mass, and pathologic evaluation revealed an adenomatoid tumor of the tunica vaginalis.

Journal of the Korean Radiological Society 1996;34(4):539-542
PURPOSE: To evaluate the US findings of epididymal adenomatoid tumor. MATERIALS AND METHODS: Were trospectively reviewed US findings of four patients with histopathologically proven epididymal adenomatoidtumors. Lesions were evaluated for their size, location, margin, shape and echogenicity. RESULTS: The size of thetumors ranged between 0.5cm and 2cm and all occurred on the left side of the epididymis. Of the four cases, three were located at the tail of the epididymis and one at its head. The tumors were well marginated and spherical andecho-texture was variable but homogenous. CONCLUSION: The possibility of and adenomatoid tumor should be considered when the epididymal mass is round and has a clear margin and the echo-texture of ultrasound ishomogenous.

Adenomatoid tumor is the common neoplasm of the paratesticular tissue that generally is considered to be of mesothelial derivation. But, there are only few reports on adenomatoid tumor of the epididymis associated with cryptorchid testis in the world. We report an unusual tumor that occurred in epididymis associated with intraperitoneal cryptorchid testis of a 32-year-old man andwas confirmed as an adenomatoid tumor by light microscopy.

Adenomatoid tumor of uterus is described by Golden and Ash as a tumor that similar to adenoma but found obscured histogenesis. Adenomatoid tumor is a rare benign disease that originated from mesothelium and mostly occurred in male and rarely in female, and can be treated by simple excision of tumor because there is no recurrence or malignant change. We describe the clinical and histologic findings with references to other reported cases. We herein report a case of adenomatoid tumor of the uterus that gets an operation under clinical impression of myoma uteri.

Adenomatoid tumors are rare benign neoplasms thought to be of mesothelial origin. Although most reported cases developed from the epididymis, rare cases have been reported in the testicular tunica, spermatic cord and ejaculatory ducts. Because of the benign nature of this tumor, the treatment of choice is local excision. We report a rare case of adenomatoid tumor of the spermatic cord treated by local excision.

8.Touch Imprint Cytology of Adenomatoid Tumor of the Tunica Albuginea: A Case Report.
Korean Journal of Cytopathology 2008;19(1):47-51
Adenomatoid tumor is a benign neoplasm of a mesothelial origin, and it usually occurs in the reproductive organs, especially in the epididymis. The author experienced a case of adenomatoid tumor involving the tunica albuginea and testicular parenchyme without any evidence of epididymis involvement. The patient was a 36-year-old man with a painless scrotal mass that he had experienced for 2 months, and this mimicked testicular neoplasia, including metastatic carcinoma, or other benign lesions. The imprint cytology of the tumor showed a hypocellular smear with mainly arranged cells in cohesive monolayered clusters along with occasional singly dispersed cells and naked nuclei in a clean background. The cellular clusters formed vague glandular and cord-like structures. The tumor cells were large polygonal to columnar cells with a relatively monomorphic appearance. The nuclei were oval to round shape and they showed vesicular, fine chromatin and inconspicuous nucleoli. The cytoplasm was moderate to abundant, and it contained fine vacuoles in some tumor cells. Mitoses and cellular pleomorphism were not present. Awareness of the cytologic finding of this lesion is necessary to screen or differentiate a testicular or paratesticular mass before and/or during surgery because the cytology may be useful as a diagnostic tool. Pathologists should be aware of the cytologic features of common lesions in this anatomic region so as to avoid performing aggressive and unnecessary surgical procedures.

Korean Journal of Andrology 2003;21(2):111-113
Adenomatoid tumor is an uncommon neoplasm found mainly near or in the epididymis in the male and the fallopian tubes and ovary in the female. We observed a case of adenomatoid tumor of the epididymis, which was found incidentally 20 years ago by patient self examination. A rubbery, non-tender, 2 X 1-cm mass was found in the tail portion of the right epididymis. Scrotal ultrasonography showed a 2.1 X 1.1 X 1.2-cm hypoechoic solitary mass. Mass excisional biopsy with partial epididymectomy was done under local anesthesia. Pathologic examination showed an adenomatoid tumor.

Adenomatoid tumor is the most common paratesticular tumor with an anatomic distribution limited to the epididymis and it rarely invades to the tunica vaginalis, spermatic cord and ejaculatory duct. Adenomatoid tumor is a benign neoplasm that is thought to be of a mesothelial origin. The treatment of choice for adenomatoid tumor is local excision because of its benign nature and the absence of distant metastasis. We report here on a rare case of adenomatoid tumor that was found between the epididymis and the testis, and it was treated by local excision of tumor.
