Breast cancer is a common malignancy in women and frequently metastasizes to various organs such as liver, lung, brain, bone and so on. But metastasis to gastrointestinal tract is rare. We describe a 73-year-old woman with small intestinal metastasis of breast cancer. She was diagnosed as right breast cancer in stage I, received modified radical mastectomy 6 years ago and had been followed up without any evidence of residual disease. During investigation for lower abdominal pain and weight loss of 9 kg, we found a small bowel mass. The histology of the tissue taken from small bowel mass was adenocarcinoma, poorly differentiated. The immunohistochemical stain of this specimen showed 75% positivity of estrogen receptor and 90% positivity of progesterone receptor. This is a case of small bowel metastasis from breast cancer and we report this case with a review of literatures.

Cutaneous metastases from pancreatic adenocarcinomas are rare lesions. The most common site of cutaneous metastasis is the umbilicus, and this is also known as the 'Sister Mary Joseph' nodule. A 68-year-old Korean male, who was previously healthy and asymptomatic, was seen in the dermatology department for two subcutaneous nodules that he had on his right forearm and his back. Histological examination of the right foreman nodule revealed metastatic adenocarcinoma. Immunohistochemical staining for cytokeratin (CK) 7 and CK 19 were positive, and this strongly suggested the pancreatic duct as being the primary source of the cancer. The abdominal computed tomography findings were compatible with pancreatic cancer. Clinicians should be aware that metastatic cutaneous lesions could be the initial presenting sign for pancreatic cancer. The immunohistochemical staining for CK 7 and 19 may also be helpful in the diagnosis of metastatic pancreatic adenocarcinoma.

A 59-year-old man was admitted to the Department of Ear, Nose and Throat with a complaint of six-month history of left facial numbness and toothache. There was no special previous medical history in addition to smoking. No obvious abnormality in routine electrocardiogram, chest X-ray, abdominal B ultrasound were found. (1) CT scans showed heterogeneous shadows in maxillary sinus with the lesions on the left max- illary bone and evidently destruction of alveolar bone. The histopathological examination revealed bone tissue which was partly covered by an intact adenocarcinoma cell. (2) Immunohistochemical staining foe CK7, CD117, thyroid transcription factor-1, and novel aspartic proteinase A were positive and thus compatible with metastatic lung adenocarcinoma. (3) Chest CT scans showed a 1 cm x 2 cm mass on the superior lobe of the left lung, with destruction of sternum and rib, confirming that the lesions in the paranasal sinuses were lung cancer metastases. Therefore, this patient conclusively diagnosed as lung adenocarcinoma with multiple bone metastases.

Primary small cell carcinoma (SCC) of stomach is a rare and highly aggressive malignancy with extremely poor prognosis. We report a 71-year-old man with upper abdominal pain diagnosed as single hepatic metastasis of SCC from mixed SCC and adenocarcinoma of the stomach. An endoscopic examination showed the presence of Borrmann type 2 gastric cancer, 2 cm in size on the lesser curvature of antrum. An abdominal CT scan revealed a huge dumbbell shaped mass with peripheral arterial enhancement and central low density in left lobe of the liver. Endoscopic biopsies showed solid proliferation of small, monotonous tumor cells with hyperchromatic nuclei and scanty cytoplasm. The neoplastic cells were positive for immunostaining with anti-chromogranin and anti-synaptophysin. There were also other neoplastic cells with gland formation being positive for anti- cytokeratin. On the basis of these findings, we made a final diagnosis of mixed SCC and adenocarcinoma of the stomach. In addition, we also confirmed hepatic metastasis of SCC through the microscopic finding and immunostaining of tissues of liver mass. Conclusively, we report a case of hepatic metastasis of SCC only from mixed SCC adenocarcinoma of the stomach.

OBJECTIVETo report the MRI features of intracranial metastases of lung cancer.

METHODSA total of 858 patients with history of primary lung cancer suspicious of brain metastases was retrospectively reviewed with MRI.

RESULTS1. Of the 858 patients, 393 (45.8%) had brain metastases on MRI. The primary tumor was lung adenocarcinoma in 117 (29.8%), small cell lung cancer in 110 (28.0%), squamous cell cancer in 52 (13.2%), adenosquamous cancer in 16 (4.1%), large cell carcinoma in 2 (0.5%) and carcinoid in 1 (0.3%). The histopathological types of the primary tumor were unknown in 95 (24.2%). 2. Meningeal metastasis was found in 19 patients with lung cancer. The primary tumor was of adenocarcinoma lung in 6, small cell lung cancer in 5, squamous cell carcinoma in 4 and the remaining 4 were of unknown histopathological type. 3. Edema around the lesion: in 120 cases, there was no obvious edema; the edema was slight in 98 cases, moderate in 70, serious in 86.

CONCLUSIONThe brain metastasis of lung cancer is of common occurrence. MRI with enhancement is very helpful in the establishment of diagnosis.

Liver metastasis from gastric cancer is a major cause of cancer-related death. Since liver metastasis from gastric cancer is usually associated with other non-curative factors, such as peritoneal metastasis, lymph node metastasis and extensive intrahepatic metastasis, the hepatic resection rate was low and the prognosis was poor. The main risk factors of liver metastasis were serosal invasion, p(+), N3-N4, differentiated histologic type, medullary type of poorly differentiated adenocarcinoma, vascular invasion, and overexpression and mutation of VEGF, P53 and c-erbb-2. The 5-year survival rate after hepatic resection of synchronous and metachronous liver metastasis from gastric cancer ranged from 11% to 42%. Appropriate patient selection for hepatic resection may confer a better prognosis of liver metastasis from gastric cancer.

BACKGROUND/AIMS: It has been reported that the risk of gastric polyp is increased in various colonic polyposis syndromes or in series of patients with sporadic colonic polyps. However, there are only a few large case controlled studies of colon cancer incidence in gastric cancer patients who underwent colonoscopy. The aims of this study were to determine the incidence of colorectal neoplasm and to evaluate the necessity of colonoscopic surveillance in patients with gastric cancer. METHODS: We performed colonoscopy in 105 patients with gastric cancer who agreed to undergo colonoscopy before or after 6 months from gastric resection between January 2002 and December 2004 in Kangbuk Samsung hospital. As a control group, 269 consecutive, age and sex matched patients without gastric neoplasm on gastroscopy who underwent colonoscopy within 6 months for the evaluation of various gastrointestinal symptoms during the year 2004 were included. Endoscopic reports and pathological results were reviewed retrospectively. RESULTS: In the patient group, adenomatous polyps were diagnosed in 24/105 patients (22.9%) and colorectal adenocarcinoma in 10/105 patients (9.5%). In the control group, adenomatous polyps were diagnosed in 78/269 patients (29.0%) and colorectal adenocarcinoma in 2/269 patients (0.7%). The incidence of colorectal adenocarcinoma between the patient group and control group showed significant differences (odds ratio 11.04, p=0.003). CONCLUSIONS: The risk of colorectal adenocarcinoma increases significantly in patients with gastric cancer. We suggest that the patients with gastric cancer might carry a high risk for colorectal cancer whom require surveillance colonoscopy.

OBJECTIVETo investigate the expression of glucose transporter protein 1 (GLUT-1) and desmin in benign and malignant mesothelial lesions, including reactive mesothelial hyperplasia (RMH), epithelioid malignant mesothelioma (EMM) and metastatic adenocarcinoma (MAC).

METHODSOne hundred and forty two pleural biopsy specimens were collected in this study, including 58 cases of RMH, 53 cases of EMM and 31 cases of MAC. Immunohistochemical EliVision method was performed to detect GLUT-1 and desmin expression.

RESULTSThe positive rates for GLUT-1 in RMH, EMM and MAC were 13.8% (8/58) , 81.1% (43/53) and 77.4% (24/31) , respectively, with statistically significant differences between RMH and others (both P < 0.01). The positive rates for desmin in RMH, EMM and MAC were 77.6% (45/58) , 9.4% (5/53) and 0 (0/31) , respectively, with statistically significant difference between RMH and others (both P < 0.01). The combined expression pattern of positive GLUT-1 and negative desmin was found in 1 (1.7%, 1/58) RMH cases, 41 (77.4%, 41/53) EMM cases and 24 (77.4%, 24/31) MAC cases, with statistically significant difference between RMH and others (both P < 0.01).

CONCLUSIONSGLUT-1 and desmin may be used as immunohistochemical markers in separating RMH from EMM. Combined application of two antibodies may improve the specificity.
