Kaposi sarcoma (KS) has attracted much attention recently because of its high incidence among patients with acquired immune deficiency syndrome. Other groups with a high risk of developing KS are renal transplant recipients treated with immunosuppressive drugs and, to a lesser extent, patients with autoimmune diseases. We report the case of a patient who developed KS during corticosteroid therapy for idiopathic thrombocytopenic purpura, which subsequently regressed with combination chemotherapy.

7.How to effectively detect and manage people living with HIV/AIDS in China: establishment of a community-based model.

This study explored a novel systemic community-based model for detecting and managing people living with HIV/AIDS (PLWHA). Both quantitative and qualitative research methods were used in this study. A quantitative questionnaire investigation was conducted in a sample of 1192 subjects which were randomly selected from two areas with high HIV prevalence, Xiangfan City and Shiyan City of Hubei Province, China. Twenty-two medical and health service staffs were interviewed by semi-structured questionnaire focusing on awareness, status, problems, and suggestions about community-based Voluntary Counseling and Testing and Provider Initiated Testing and Counseling (VCT/PITC). And they were organized to discuss about the aforementioned issues in Xiangfan City and Shiyan City, respectively. Our results showed that the accessibility and availability of the general VCT/PITC were bad. About 28.3% had known and only 4.9% had made use of VCT/PITC. Developing community-based VCT/PITC had some special advantages that can overcome some existing problems to remedy the aforementioned defects. We are led to conclude that, to maximize the availability and uptake rate of the VCT/PITC, we plan to detect PLWHA by developing the community-based VCT/PITC through 4 paths. Then we establish the community HIV health care center constituted of 8 sectors to provide an overall management. Thus, we can effectively detect and manage the PLWHA with a new systemic community-based model.

8.Perioperative management of HIV infection/AIDS in gynecological and obstetric patients.

OBJECTIVETo investigate the optimal perioperative management of HIV infection/AIDS in surgical patients in the department of gynecology and obstetrics.
METHODSFifty-seven surgical patients with HIV infection or AIDS were studies. Before the operations, the patients were treated with highly active anti-retroviral therapy (HAART), and the surgical approach was determined according to the level of CD4(+) T lymphocytes. The operations were performed strictly following the standard procedures with careful operation to reduce surgical injuries. Routine antibiotics were administered to prevent postoperative infections. The white blood cell count, hemoglobin and CD4(+) T lymphocytes recorded pre- and postoperatively were analyzed for evaluation of the perioperative management and therapeutic effects.
RESULTSWith these perioperative managements, the white blood cells and CD4(+) T lymphocytes showed no significant changes after the operation (P>0.05) and no wound infections or opportunistic infections were found in these cases.
CONCLUSIONSAppropriate perioperative management can ensure surgical success in patients with HIV infection or AIDS in the department of gynecology and obstetrics.

9.The treatment effect of immunoglobulin in AIDS with Guillain-Barre syndrome.

OBJECTIVETo discuss the treatment effect of immunoglobulin in acquired immune deficiency syndrome (AIDS) with Guillain-Barre syndrome (GBS).
METHODSThe clinical data of AIDS with GBS, diagnosed by clinical and laboratory methods, were retrospectively analyzed, and literature retrieval analyzed.
RESULTSAfter treatment by immunoglobulin and antiviral. The patient's peripheral nerve injury recovered, and the number of HIV decreased.
CONCLUSIONImmunoglobulin has a therapeutic effect for HIV infection related GBS, and beneficial to antiviral treatment.

10.Immune reconstitution related factors in patients with HIV/AIDS.

Immune reconstitution could be found in part of HIV/AIDS patients after receiving highly active anti-retroviral therapy (HAART). Its influencing factors are very complex. Many factors, such as the demographic factor, stage of HIV disease, pattern of therapy, sort of co-infections, baseline of CD; T-cell count, abnormal immune activation, related immune phenotype, and virologic factors, all could influence the immune reconstitution. In view of Chinese medline (CM) may play a positive role in this process, the clinical evidences for above-mentioned factors were summed up briefly in this paper for providing reference to the future studies on immune reconstitution with intervention of CM.
