1.Brain abscess surgical treatment

A retrospective study was carried out on 120 patients with brain abscess treated at Cho Ray Hospital from Oct 2000 to Feb 2003. 91% patients were hospitalized on emergency. The patients were diagnosed easily and quickly brain abscess by CT. 80 patients (70.8%) underwent emergency surgeries and the commonest applied technique (73.3%) was punction and pus drainage. Antibiotic were indicated suitably for patients, corticoids were prescribed for 31.6% patients with severe brain edema, antiepileptic drugs were given for patients with convulsions. Twelve patients (10%) died because of too late hospitalizations with general complications and/or brain hernia. Patients with brain abscess were commonly arrived late; the majority of them were admitted on emergency. Although having CT for accurate diagnosis and surgery and new antibiotic generations, the mortality rate is still high

2.Surgical treatment of brain abscess by abscess ablation

An analysis of 26 patients with brain abscesses treated by abscess ablation has shown that the morbidity rate, complications, relapsed brain abscess, repeated surgery and duration of antibiotic using. The bacteria were isolated in 42% of samples. The abscess ablation was indicated for cases of brain abscess with foreign bodies, brain abscess occurred after cerebrocranial surgery or cerebrocranial injuries. The needle aspiration indicated for cases of brain multiabscess, patients with thalamic perception impairment received needle aspiration in the first stage and abscess ablation in the second stage.

Prosthetic valve endocarditis with aortic root abscess is a serious condition requiring urgent surgical intervention. We present a case caused by an infected Bentall mechanical valve conduit after cardiac surgery in a patient who was referred for a suprasternal pulsatile mass. The patient also had 1 episode of sentinel haemorrhage.

6.Reverse partial pulmonary resection: a new surgical approach for pediatric pulmonary cysts.

OBJECTIVE:
To evaluate the safety and efficacy of reverse partial lung resection for treatment of pediatric pulmonary cysts combined with lung abscesses or thoracic abscess.
METHODS:
We retrospectively analyzed the clinical data of children undergoing reverse partial lung resection for complex pulmonary cysts in our hospital between June, 2020 and June, 2021.During the surgery, the patients lay in a lateral position, and a 3-5 cm intercostal incision was made at the center of the lesion, through which the pleura was incised and the fluid or necrotic tissues were removed.The anesthesiologist was instructed to aspirate the sputum in the trachea to prevent entry of the necrotic tissues in the trachea.The cystic lung tissue was separated till reaching normal lung tissue on the hilar side.The proximal end of the striated tissue in the lesion was first double ligated with No.4 silk thread, the distal end was disconnected, and the proximal end was reinforced with continuous sutures with 4-0 Prolene thread.The compromised lung tissues were separated, and the thoracic cavity was thoroughly flushed followed by pulmonary inflation, air leakage management and incision suture.
RESULTS:
Sixteen children aged from 3 day to 2 years underwent the surgery, including 3 with simple pulmonary cysts, 11 with pulmonary cysts combined with pulmonary or thoracic abscess, 1 with pulmonary cysts combined with tension pneumothorax and left upper lung bronchial defect, and 1 with pulmonary herpes combined with brain tissue heterotaxy.All the operations were completed smoothly, with a mean operation time of 129 min, an mean hospital stay of 11 days, and a mean drainage removal time of 7 days.All the children recovered well after the operation, and 11 of them had mild air leakage.None of the children had serious complications or residual lesions or experienced recurrence of infection after the operation.
CONCLUSION
Reverse partial lung resection is safe and less invasive for treatment of complex pediatric pulmonary cysts complicated by infections.

In oral and maxillofacial surgery, careful management is necessary to avoid infection for old aged person or patient with diabetes mellitus(DM). We experienced a 69 years old female patient with DM, who has liver abscess and appendicitis, caused by Klebsiella pneumoniae, was developed into left mid-facial abscess and peritonitis. We report this case along with review of article for we have gained fairly good results in our attempt to resolve the patient's abscess by means of I & D of the left mid-face and percutaneous catheter drainage of the stomach, supplemented with fluid and antibiotic therapy.

9.Assessment by Using a Water-Soluble Contrast Enema Study of Radiologic Leakage in Lower Rectal Cancer Patients With Sphincter-Saving Surgery.

PURPOSE: This study evaluated the efficacy of a water-soluble contrast enema (WCE) in predicting anastomotic healing after a low anterior resection (LAR). METHODS: Between January 2000 and March 2012, 682 consecutive patients underwent a LAR or an ultra-low anterior resection (uLAR) and were followed up for leakage. Clinical leakage was established by using physical and laboratory findings. Radiologic leakage was identified by using retrograde WCE imaging. Abnormal radiologic features on WCE were categorized into four types based on morphology: namely, dendritic, horny, saccular, and serpentine. RESULTS: Of the 126 patients who received a concurrent diverting stoma, only two (1.6%) suffered clinical leakage due to pelvic abscess. However, 37 patients (6.7%) in the other group suffered clinical leakage following fecal diversion (P = 0.027). Among the 163 patients who received a fecal diversion, 20 showed radiologic leakage on the first WCE (eight with and 12 without a concurrent diversion); 16 had abnormal features continuously until the final WCE while four patients healed spontaneously. Eleven of the 16 patients (69%), by their surgeon's decision, underwent a stoma restoration based on clinical findings (2/3 dendritic, 3/4 horny, 5/7 saccular, 1/2 serpentine). After stoma reversal, only 2 of the 11 (19%) complained of complications related to the rectal anastomosis. CONCLUSION: WCE is helpful for detecting radiologic leakage before stoma restoration, especially in patients suffering clinical leakage after an uLAR. However, surgeons appear to opt for stoma restoration despite the persistent existence of radiologic leakage in cases with particular features on the WCE.

10.Abscess formation in vertebral canal and presacral area following penetrating injury of rectum and sacral vertebra by a steel rod.

Penetrating injury to the rectum, vertebral body and spinal cord by a steel rod is a rare condition. Treatment of this kind of injury is very challenging. Rectal injury requires repair and fecal diversion, while debridement of the spine is difficult, especially when the injury site is very long. Here we report a case of penetrating injury of rectum and sacral vertebra by a steel rod after falling onto the ground from 1 m height. The abscess cavity was irrigated with 3% hydrogen peroxide and physio-logical saline repeatedly. The bony canal was carefully debrided, curetted and bony fragments were removed. Spinal irrigation and drainage lasted for 2 months and sensitive antibiotic (amikacin sulfate) was given 7 days after surgery, but abscess was still formed in the vertebral canal. At 6-month follow-up, the patient was paralyzed without any neurological improvement, and the pain in low back and lower limb still continued.
