We describe 5 case in which an island posterior calf fasciocutaneous flap used to cover defects over the knee and tibia. 5 posterior calf fasciocutaneous flaps survived completely. We have found that it offers some advantages over the gastrocnemius. 2.Operative results of malformation of cerebral arterio-venous fistula

Meningocerebral haemorrhage due to the rupture of malformation of cerebral arterio venous fistula is neurosurgical emegency that requires the radical management to prevent from relapsed haemorrhage. Currently in Vietnam, the operation for malformation removal is an unique technique. 4.Hysteroscopic Resection of the Vaginal Septum in Uterus Didelphys with Obstructed Hemivagina: A Case Report.

Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis is a rare congenital anomaly. Excision of the obstructed vaginal septum is the treatment of choice for symptom relief and the preservation of reproductive capability. A 14-yr-old girl complained of persistent vaginal spotting following each menstruation. Pelvic magnetic resonance imaging revealed a uterus didelphys with left hematocolpos and ipsilateral renal agenesis. Instead of conventional transvaginal excision of the vaginal septum, we used hysteroscopic excision under transabdominal ultrasonographic guidance to preserve the integrity of the hymen. The postoperative course was uneventful, and clinical symptoms were completely resolved after this intervention. Resectoscopic excision of the vaginal septum was found to be easy, safe, effective, and appropriate for young women as it preserved hymen integrity. We believe that this is the first Korean report on the use of a hysteroscopy for vaginal septum resection in a patient with uterus didelphys with obstructed hemivagina.

6.Horseshoe kidney with retrocaval ureter: one case report.

Horseshoe kidney and retrocaval ureter are uncommon congenital anomalies of the genitourinary system that are easily diagnosed by typical imaging features. Both anomalies presenting in one patient is a rare disease characterized by isthmus of horseshoe kidney between the abdominal aorta and inferior vena cava. The clinical diagnosis and treatment of horseshoe kidney with retrocaval ureter remain a challenge. Here, we reported a case of a 44-year-old man with the two anomalies who was preoperatively diagnosed by unenhanced computed tomography scanning immediately after retrograde pyelography. The literatures on such combined anomalies are reviewed and the diagnostic evaluation and surgical management of this rare entity are discussed.

7.Surgical treatment of 8 patients with cerebral atteriovenous malformation

Most of arteriovenous malformations have the congenital origin and are formed from the mesoderm. They usually are seen under the 40 age and can be ruptured during any time. Sometimes they are also related to stress. Cerebral angiography keeps to play an important role in the diagnosis of AVM Digital Subtraction Angiography is a new advance in AVM diagnosis, it reveals AVM exactly and clearly. So, it helps the neurosurgeons to assess the treatment procedure appropriately. The characteristics of the surgical indications:* AVM in the surface of the cortex or under the cortex.* Size under 6 cm in diameter. * Noneloquent areas.* Without other diseases. Prevention of seizure with phenytoine is nesessary for pre and post operation. Nowadays, gamma knife is applied in spreading around the world with small and deep AVM or eloquent areas.

8.Surgical treatment of deformity of the nose due to congenital cleft palate by cartilage transplant of ear-rim

The morbidity rate of cleft palate is relatively high (1/800). There are about 87500 children with disease. The plastic surgery for correction of cleft palate aims to early close the cleft, solve the functions of eating and drinking preventing from the shock and regulate the development of upper maxillary bone. 15 patients with nasal deformity due to the congenital cleft palate during 1996-1998 were received the plastic surgery for correction of nose-petal and nose-head by using transplants from the cartilage of the ear-rim. Results: the transplants from cartilage of the ear-rim are suitable for this operation. The ages of children are about 12-13 and above to assure the relatively complete development of cartilage frame and ear-rim and their size are the same as these in adults

9.Results of surgical treatment of 16 patients with menigocele and menigomyelocele in Saint Paul hospital

During 3 years, more 400 cases of the brain diseases in children have been operated at the department of neurosurgery of Hanoi Sain - Paul hospital. Among of them, only 16 cases of meningocele and meningoencephalocele who were congenital malformations of neurology with age of 2 days- 16 month of age. The author have presented the clinical and paraclinical manifestations, the indication, and the method of choice for the treatment, and the use of modified technique in the operation.
