1.Post-Stroke Mania – A Case Report

Objective: Psychiatric disorders following cerebrovascular accidents are
common. Post-stroke depression is the most common of these disorders, and
post-stroke mania has been reported on rare occasions. Methods: We report a
case of a 65 year-old elderly male who developed mania secondary to a left sided
cerebral infarction in the territory of the middle cerebral artery. Discussion: It
has been theorized that lesions in the cerebral hemisphere and limbic structures
may produce symptoms suggestive of mania. Emotional and behavioral
disorders after stroke negatively impact rehabilitation, cognition, and long-term
recovery. Conclusion: Post-stroke mania is a rare medical condition that may
occur after neurological deficits.
2.A Case of Amelanotic Malignant Melanoma of the Maxillary Sinus Presented with Intraoral Extension

Melanoma of the sinonasal cavity has a high incidence of amelanotic presentation. Its
diagnosis is difficult for clinicians and pathologists because of its hidden location and lack of
pigmentation at the microscopic level. We reported a case of amelanotic melanoma of the maxillary
sinus that showed aggressive extension to the oral cavity after extraction of the maxillary tooth.
Histologically, the lesion resembled a plasmacytoid tumour. Diagnosis was made through the
positive immunohistochemical staining for S100 and HMB-45.
3. Vasectomy reversal: A clinical update

Vasectomy is a safe and effective method of contraception used by 42-60 million men worldwide. Approximately 3%-6% of men opt for a vasectomy reversal due to the death of a child or divorce and remarriage, change in financial situation, desire for more children within the same marriage, or to alleviate the dreaded postvasectomy pain syndrome. Unlike vasectomy, vasectomy reversal is a much more technically challenging procedure that is performed only by a minority of urologists and places a larger financial strain on the patient since it is usually not covered by insurance. Interest in this procedure has increased since the operating microscope became available in the 1970s, which consequently led to improved patency and pregnancy rates following the procedure. In this clinical update, we discuss patient evaluation, variables that may influence reversal success rates, factors to consider in choosing to perform vasovasostomy versus vasoepididymostomy, and the usefulness of vasectomy reversal to alleviate postvasectomy pain syndrome. We also review the use of robotics for vasectomy reversal and other novel techniques and instrumentation that have emerged in recent years to aid in the success of this surgery.
4.High-sensitivity simultaneous liquid chromatography-tandem mass spectrometry assay of ethinyl estradiol and levonorgestrel in human plasma

A sensitive and simultaneous liquid chromatography-tandem mass spectrometry method was developed and validated for quantification of ethinyl estradiol and levonorgestrel. The analytes were extracted with methyl-tert-butyl ether: n-hexane (50:50, v/v) solvent mixture, followed by dansyl derivatization. The chromatographic separation was performed on a Kinetex C18 (50 mm × 4.6 mm, 2.6μm) column with a mobile phase of 0.1% (v/v) formic acid in water and acetonitrile in gradient composition. The mass transitions were monitored in electrospray positive ionization mode. The assay exhibited a linear range of 0.100-20.0 ng/mL for levonorgestrel and 4.00-500 pg/mL for ethinyl estradiol in human plasma. A run time of 9.0 min for each sample made it possible to analyze a throughput of more than 100 samples per day. The validated method has been successfully used to analyze human plasma samples for application in pharmacokinetic and bioequivalence studies.
5.Bilateral Cricoarytenoid Arthritis: A Cause of Recurrent Upper Airway Obstruction in Rheumatoid Arthritis

We report a case of bilateral cricoarytenoid joint arthritis with history of rheumatoid arthritis, presented with stridor to the outpatient department. Endolaryngoscopy revealed adducted vocal cords and a nodule over left arytenoid which later confirmed to be rheumatoid nodule on histopathologic examination. Initially, although patient responded well to medical treatment, recurrence was noticed after 6 months follow-up.
6.Antioxidant activity of Palmarosa essential oil (Cymbopogon martini) grown in north Indian plains

Objective: The present study deals with the in vitro study of antioxidant activity of essential oil from Palmarosa (Cymbopogon martini) of family gramineae. Methods:The in vitro study of antioxidant activity of Palmarosa essential oil has been done by using DPPH assay, Nitrogen oxide assay, reducing power assay, β-carotene bleaching assay and FRAP method. Results: IC50 values observed for DPPH and NO assay were 0.125 mg/mL and 12.5 μg/mL, respectively.In beta carotene bleaching method, the oil showed 93.15% bleaching for the first hour and it increased to 51.1% in second hour. There was a constant increase in the reducing activities with the increase in concentrations in both reducing activity and FRAP mehods. In all the methods BHT and Gallic acid were kept as standards. Conclusions: The results clearly indicate that Palmarosa essential oil is effective in scavanging free radical and has the potential to be powerful antioxidant.
7.Functional Outcome With Percutaneous Ilio-sacral Screw Fixation For Posterior Pelvic Ring Injuries In Patients Involved In Heavy Manual Laboring

Introduction: Unstable posterior pelvic ring injuries are best
treated with operative methods due to better post-op
functional score. Our patient cohort was involved in heavy
manual laboring frequently required ground level work in
their activities of daily living. There are very few outcome
studies dealing exclusively with such patients.
Materials & Methods: Forty one patients who were treated
with percutaneous sacroiliac screw fixation under
fluoroscopic guidance and were followed-up for at least one
year were analyzed retrospectively for functional outcome
using the Majeed score.
Results: Twenty one (51.22%) and thirteen (31.70%) patients
were found to be in excellent and good categories
respectively and majority of the patients (thirty/73.17%)
were able to return to their original occupation with or
without minor adjustments.
Conclusion: Percutaneous ilio-sacral screw fixation for
posterior pelvic unstable injuries is an acceptable mode of
treatment in patients involved in heavy manual laboring.

8.Does Excision of Heterotopic Ossification of the Elbow Result in Satisfactory Patient-Rated Outcomes?

Introduction: Treatment of heterotopic ossification (HO) of
the elbow is challenging and fraught with complications.
Patients who sustain direct trauma to the elbow joint, the
central nervous system, and thermal burns are at increased
risk for development of HO. There is a paucity of studies and
reports on patient’s self-evaluation after the excision of the
heterotopic ossification.
Materials and Methods: This retrospective study assessed
outcomes after excision of heterotopic ossification around
the elbow in a cohort of ten patients operated from 2012 to
2015. The outcome assessment was done by the Mayo Elbow
Performance index (MEPI) and the American Shoulder and
Elbow Surgeons-Elbow score (ASES-E scores).
Results: The mean follow-up was 18.11 months after the
operation. The Mayo Elbow Performance Score was
excellent in two elbows, good in six and fair in two. The
mean gain in flexion-extension arc after excision of HO was
80 degrees. All of the patients had residual flexion deformity
postoperatively. Eight of the nine patients were able to do
activities requiring flexion at final follow-up.
Conclusion: Excision of HO around the elbow is associated
with satisfactory patient-rated outcomes in spite of failure to
regain full range of motion.
9.Anomalous Bilateral Communication between the Inferior Alveolar Nerve and the Auriculotemporal Nerve: A Rare Variation

Branches of the posterior division of the mandibular nerve show various anomalous communications in the infratemporal region. Understanding such communication has relevance in the management of neuropathies and surgical procedures in this region. This study was conducted to explore such communicating branches, anticipating that they might provide information of clinical significance. A total of 15 human cadavers (30 infratemporal regions) were studied to explore such communicating branches in infratemporal region. The branches of the posterior division of the mandibular nerve were carefully dissected, and these branches were studied and analysed for any abnormal course. In one case, a rare type of bilateral communication between the auriculotemporal nerve and the inferior alveolar nerve, forming a loop with no association with any structure, was observed. It is possible that such communicating branches may be associated with delayed regression of the first arch vessels. The clinical implications of these anomalous communications require further detailed study for improved management of neuropathies and surgical procedures.
10.Prevalence and Predictors of Unmet Needs among the Elderly Residents of the Rural Field Practice Area of a Tertiary Care Centre from Northern India

Background: Surrogate markers simple enough to be used by primary care workers
have not been closely investigated by the community experts in rural Uttar Pradesh. We assessed
the physical disabilities in activities of daily living (ADL) and unmet need in physical disabilities
among rural elderly. Predictors of unmet needs in physical disabilities among the elderly were
also identified.
Methods: A community based cross-sectional study was conducted among elderly
residents of the rural field practice area of a tertiary care centre in rural Uttar Pradesh. Three
hundred and thirty five (335) participants aged 60 years and above from 9 villages were selected
using the Probability Proportional to Size (PPS) sampling technique. Study tools were the
proforma regarding socio-demographic details, socio-economic status and Stanford Health
Assessment Questionnaire. Multivariate logistic regression analysis was performed to identify
predictors of unmet needs.
Results: 185 (55.2%) had physical disability in one or more activity limitation. Gender
wise elderly females had more physical disability in one or more ADL categories than elderly
males (66.8% vs. 42.0%). Almost one third (32.5%) of subjects had unmet need for one or more
physical disabilities. the predictors of unmet needs that were identified in the study were female
gender (P = 0.046), elderly aged 70 years and above (P = 0.032), those living alone (P = 0.035),
low monthly family income (P = 0.044), financially fully dependent elderly (P = 0.0002), and
those having 3 or more physical disabilities (P = 0.033).
Conclusions: The findings of the study highlight that large number of needs of the
disabled are still unmet. Greater, targeted efforts are needed to identify at-risk elderly people
living in the community. These predictors would act as surrogate markers and can be easily used
by primary care workers to plan and provide services to the elderly people in rural communities.
