2.Reduction of intubation rate during newborn resuscitationafter transition from self-inflating bag to T-piece resuscitator: A quality improvement project

Introduction: T-piece resuscitator (TPR) has many
advantages compared to self-inflating bag (SIB). Early
Continuous Positive Airway Pressure (CPAP) during
newborn resuscitation (NR) with TPR at delivery can reduce
intubation rate.
Methods: We speculated that the intubation rate at delivery
room was high because SIB had always been used during
NR and this can be improved with TPR. Intubation rate of
newborn <24 hours of life was deemed high if >50%. An audit
was carried out in June 2010 to verify this problem using a
check sheet.
Results: 25 neonates without major congenital anomalies
who required NR with SIB at delivery were included.
Intubation rate of babies <24 hours of life when SIB was
used was 68%. Post-intervention audit (August to November
2010) on 25 newborns showed that the intubation rate within
24 hours dropped to 8% when TPR was used. Proportion of
intubated babies reduced from 48.3% (2008-2009) to 35.1%
(2011-2012), odds ratio 0.58 (95% CI 0.49-0.68). Proportion of
neonates on CPAP increased from 63.5% (2008-2009) to
81.0% (2011-2012), odds ratio 2.44 (95% CI 2.03-2.93). Mean
ventilation days fell to below 4 days after 2010. Since then,
all delivery standbys were accompanied by TPR and it was
used for all NR regardless of settings. There was decline in
intubation rate secondary to early provision of CPAP with
TPR during NR. Mean ventilation days, mortality and length
of NICU stay were reduced.
Conclusion: This practice should be adopted by all hospitals
in the country to achieve Millennium Development Goal 4
(2/3 decline of under 5 mortality rate) by 2015.

3.Hypopituitarism in a Dengue Shock Syndrome Survivor without known Pituitary Adenoma

Dengue infection is endemic in South East Asia and parts of the Americas. Dengue
hemorrhagic fever is characterized by vascular permeability, coagulation-disorders and
thrombocytopenia, which can culminate in hypotension i.e. dengue shock syndrome.
Hypopituitarism arising as a complication of dengue is extremely rare. Hemorrhagic pituitary
apoplexy of pre-existing pituitary adenomas has been rarely reported in dengue. We describe
an uncommon case of hypopituitarism in a dengue shock syndrome survivor without known
pituitary adenoma. A 49 years old nulliparous lady (from Kuala Lumpur, Malaysia) presented
with typical symptoms of hypocortisolism. Postural hypotension was evident with normal
secondary sexual characteristics. Further history revealed that she survived an episode of
dengue shock syndrome 6 years ago where premature menopause developed immediately
after discharge, and subsequently insidious onset of multiple hormonal deficiencies indicative
of panhypopituitarism. There were no neuro-ophthalmological symptoms suggestive of pituitary
apoplexy during hospitalization for severe dengue. Magnetic resonance imaging of the pituitary
6 years later revealed an empty sella. Autoimmune screen and anti-thyroid peroxidase
antibodies were negative. We describe a rare possible causative association of severe dengue
with panhypopituitarism without known pituitary adenoma, postulating pituitary infarction
secondary to hypotension (mimicking Sheehan’s syndrome), or a direct viral cytopathic effect.
Subclinical pituitary apoplexy secondary to asymptomatic pituitary hemorrhage however
cannot be excluded. Future research is required to determine the need for and timing of
pituitary axis assessment among dengue shock syndrome survivors.
4.PyOncoPrint: a python package for plotting OncoPrints

OncoPrint, the plot to visualize an overview of genetic variants in sequencing data, has been widely used in the field of cancer genomics. However, still, there have been no Python libraries capable to generate OncoPrint yet, a big hassle to plot OncoPrints within Python-based genetic variants analysis pipelines. This paper introduces a new Python package PyOncoPrint, which can be easily used to plot OncoPrints in Python. The package is based on the existing widely used scientific plotting library Matplotlib, the resulting plots are easy to be adjusted for various needs.

The ability of triadimefon (TDM), a triazolic fungicide, to alter the biochemical constituents and thereby minimizing the days required for sprouting in white yam (Dioscorea rotundata Poir.) tubers during storage under (30+/-2) degrees C in the dark, was studied. TDM at 20 mg/L was given to tubers by dipping the tubers in treatment solution containing 20 mg/L TDM on 10, 25 and 40 d after storage (DAS). Starch, sugars, protein, amino acid contents as well as protease and alpha-amylase activities were estimated on 15, 30 and 45 DAS from two physiological regions viz., apical and basal regions of the tubers. In normal conditions (control) sprouting occurred on 70 to 80 DAS. The starch content decreased, while protein, amino acid, sugar contents and protease and alpha-amylase activities were increased due to TDM treatment and led to early sprouting.

To determine the accuracy of thoracolumbar pedicle screw insertion with the routine use of three-dimensional (3D) intraoperative imaging and navigation over a large series of screws in an Asian population. The use of 3D intraoperative imaging and navigation in spinal surgery is aimed at improving the accuracy of pedicle screw insertion. This study analyzed 2,240 pedicle screws inserted with the routine use of intraoperative navigation. It is one of very few studies done on an Asian population with a large series of screws. Patients who had undergone thoracolumbar pedicle screws insertion using intraoperative imaging and navigation between 2009 and 2017 were retrospectively analyzed. Computed tomography (CT) images acquired after the insertion of pedicle screws were analyzed for breach of the pedicle wall. The pedicle screw breaches were graded according to the Gertzbein classification. The breach rate and revision rate were subsequently calculated. A total of 2,240 thoracolumbar pedicle screws inserted under the guidance of intraoperative navigation were analyzed, and the accuracy of the insertion was 97.41%. The overall breach rate was 2.59%, the major breach rate was 0.94%, and the intraoperative screw revision rate was 0.7%. There was no incidence of return to the operating theater for revision of screws. The routine use of 3D navigation and intraoperative CT imaging resulted in consistently accurate pedicle screw placement. This improved the safety of spinal instrumentation and helped in avoiding revision surgery for malpositioned screws.

Head and neck squamous cell carcinoma (HNSCC) is the most frequent type of head and neck cancer that usually arises from the mucosal surfaces of several organs including nasal cavity, paranasal sinuses, oral cavity, tongue, pharynx, and larynx. The Wnt signaling pathway is a crucial mechanism for cellular maintenance and development. It regulates cell cycle progression, apoptosis, proliferation, migration, and differentiation. Dysregulation of this pathway correlates with oncogenesis in various tissues including breast, colon, pancreatic as well as head and neck cancers. The present study aims to assess the gene alterations in the Wnt family of genes so as to derive an association with HNSCC. Computational approaches have been utilized for the identification of gene alterations in the Wnt family of genes. Several databases such as cBioportal, STRING, and UALCAN were used for the purpose. The frequency of alteration was high in case of Wnt family member 11 (5%). Gene amplification, deep deletions, missense and truncating mutations were observed in HNSCC patients. There was a marked difference in the gene expression profile of WNT11 between grades as well as normal samples. The survival probability measured using the Kaplan-Meier curve also presented with a significant difference among male and female subjects experiencing a low/medium level expression. The female patients showed less survival probability when compared to the male subjects. This provides the prognostic significance of the WNT11 gene in HNSCC. Taken together, the present study provides clues on the possible association of WNT11 gene alterations with HNSCC, which has to be further validated using experimental approaches.

10.Prevalence of peste des petits ruminants among sheep and goats in India.

This study measured the clinical prevalence of peste des petits ruminants (PPR) among sheep and goats in India between 2003 and 2009 by analyzing clinical samples from suspected cases of PPR that were submitted to the Rinderpest and Allied Disease Laboratory, Division of Virology, IVRI, Mukteswar for PPR diagnosis. PPR outbreaks were confirmed by detecting PPR virus (PPRV)-specific antigen in the clinical samples. Clinical samples (blood, nasal swabs, spleen, lymph node, kidney, liver, intestine, and pooled tissue materials) were taken from a total of 592 sheep and 912 goats in different states of India and screened for the presence of PPRV antigen using a monoclonal antibody-based sandwich ELISA kit. A total of 20, 38, and 11 laboratory-confirmed PPR outbreaks occurred among sheep, goat, and combined sheep and goat populations, respectively. Our findings provide evidence of widespread PPR endemicity in India. The underlying reasons could be variations in husbandry practices in different geographical regions, agro-climatic conditions, and livestock migration. Furthermore, decrease in the number of PPR outbreaks over time might be due to the effectiveness of current live PPR vaccines and timely vaccination of target species. Vaccination against PPR has been practiced in India since 2002 to control this disease.
