1.Proposal of Program for Easier Access of Family Medicine Information through PC Communication.

BACKGROUND: It is expected that the demand for the information about the primary medical service in the forthcoming supersede age of communication will be on the increase. For that reason, family doctors who are in charge of the primary medical service have to play an important part in constructing a data-base for furnishing the medical information. This article presents a model which includes the roles of family doctors, constructing medical-related information site of family medicine in the communications net with PC to gat,her fundamental data in operating the system of medical information and elucidating the formation of medical-related information site and the effect of operating that system. METHODS: Since Aug. 20 in 1996, the site of family medicine, the program of medical information, has been set up in Unitel. The medical information of the site of family medicine is to be classified into two parts : for doctors and for the public. The former is composed of medicine documents, several kinds of medical data, a linkage with medicine site in Internet, movements of an academic society and information exchanges for friendship. The latter is mainly made up of some practical medical sense and medical counsel. RESULTS: From the opening the medical information site to Jul. 30 in 1997, 250 persons a day have connected with medical information site and this fact implied 15 hours connection a day in terms of the daily mean. The cases of health counsel averaged 6 cases connection a day and the annual statistics of those cases were about, 1980 cases. The major content of health counsel was related to common symptoms that could be settled in primary medical problem. According to the survey for connectors, most of connectors answered that the medical information through communications net was much more effective t.han that from other media in the educational aspects and very helpful in the prevention of diseases and in the medical expenditure. Above all, the medical information through communications net was very effective in correcting the mistaken practical medical sense. CONCLUSIONS: It is necessary for family doctors to give thought to the connection of both information communication and medicine and to have correct understanding of medical domain in the cyberspace. On the foundation of these necessities, all the family doctors have to make the best of their way to construct a database with rnedical data and to transfer these data into the computer sites.

2.An Experimental Study of the Effect of Kirschner-wire and Screw insertion upon the Epiphyseal Plate of the Distal Femur in the Growing Rabbit

The effect of surgical traumata upon the epiphyseal plate of the growing bone has been controversial among many authors. The resultant deformity subsequent to the epiphyseal plate injury-yields numerous different pattens and any of the explarations of the pathogenesis is also not accepted for all. Therefore, the author made an experiment to elucidate more clearly the histological changes of the epiphyseal plate of immature rabbits following insertion of Kirschner-wire and screw. Thirty rabbits weighing 700–1200gm were divided into 2 groups, as following snd the direction of longitudinal insertion from articular surface of the epiphysis of the distal femur. Group 1: lnsertion of Kirschner-wire Group 2: Insertion of screw The animals were sacrificed at 1, 2, 4, 8, and 10 weeks after the operative procedure and the histological examination of the injured epiphyseal plate of distal femur were made. The summary of the histological findings were follows: 1. The margin of the epiphyseal plate adjoining the Kirschner wire showed a new osseous tissue which formed a bony bridge running from the epiphysis to the metaphyseal bony trabeculae. 2. The margin of the damaged epiphyseal plate adjoining the screw revealed new bone formation and the amounts of the reactive new bone is less than Kirschner-wire. 3. Histological findings of the epiphyseal plate showed to be uniform and normal except in the adjacent area of the Kirschner-wire and screw.

4.A Case of Duodenal Leiomyosarcoma.

Leiomyosarcoma of small intestine is an uncommon tumor, comprising less than 20 percent of all primary malignant tumors of small intestine. Duodenal leiomyosarcoma is rare disease which amount to about 20 percent of all small bowel malignancy but potentially curable tumors often diagnosed at an advanced age, so its five-year survival following resection approximates 50% in reported series. Recently, several cases of leiomyosarcomas of duodenum were diagnosed by hypotonic duodenograhy in Korea. We experienced a woman with duodenal leiomyosarcoma that was diagnosed by endoscopic duodenal biopsy and received currative resection of the tumor(Whipple's operation). We report the case with review of the literatures.

5.Clinical Observation of Grenz Ray Therapy.

Supersoft roentgen rays called grenz rays, generated in a tube at low voltages and having wave lengths of 1 to 3 angstroms(H.V.L. 0. 01 to 0. 035mm. Al. ), are employed in the treatment of superficial derrnatoses patients (112 cases) in the Department of Dermatology, Cho Sun University hospital from March 1, 1972 to April 30, 1974. Results of these Series are as Follows: 1) Among the ll2 patients, including 28 patients of neurodermatitis, 14 patients of nummular eczema and. 10 patients of atopic dermatitis showed therapeutic success. 2) Grenz ray irradiations were remarkably effective for 6 cases of capillary hemangioma, 2 cases of basal celI epithelioma and 3 cases of verruca plana juvenilis, and less effective for psoriasis vulgaris, tinea pedis, acne vulgaris, nevus verrucosus and alopecia areata. 3) Complications included 21 cases of hyperpigmentation and burning sensations in neurodermatitis and nummular eczema.

6.Clinical Observation on the Non-calculous Obstruction at the Ureteropelvic Juncture.

A clinical observation was made on the obstruction at the ureteropelvic Juncture of the inpatient in the Department of Urology. Seoul National University Hospital during the period September, 1961 through August, 1968 and the following results were observed. 1. Of 1824 cases hospitalized, 33 cases werehydronephrosis of non-calculous origin. Among the 33 cages 10 were obstruction ofthe ureteropelvic juncture giving a rate of 30.3%. Age distribution ranged from 4to 48 years with average of 26 years. 2. As to the origin of the obstruction, 5 cases were due to aberrant vessel one congenital stricture one fibrous band and one fibrosis, respectively. The causes were unknown in 2 cases. 3. The most common complaints were pain on the involved site in 8 cases. Palpable mass, gastrointestinal trouble and general weakness were found in 6 cases. Loss of weight and pollakisuria were noted in 3 cases. Hypertension. hematuria, fever andedema were found respectively in one case. 4. All cases including two which showed non-function on IVP showed markedly dilated pelvis and marked narrowing ofthe ureteropelvic juncture. 5. Nephrectomy was performed in 5 cases, removal of aberrant vessel in 2, ureteroplasty in one and ureteral dilation with contralateral nephrectomy in 2.

7.A Study on 1 , 302 Cases of Skin Tumor.

A study was made on 1,302 cases of skin tumors which were obtained mainly from surgical pathology specimen during the period of 14 years from 1973 to 1986 at Department of Dermatology in Korea University Hospital. The results were summarized as follows : 1. Of the 1,302 cases of skin tumors, 1,143 cases(87.8%) were benigh tumor, 159 cases(12.2%) of malignant tumor and male to female ratio was 1.1: l. 2. Among the 1,143 cases of benign tumors, 301 cases of epidermal cyst, 183 cases of lipoma, 173 cases of pigmented nevi and 88 cases of hemangioma were noted. 3. Among the 159 cases of malignant tumors, 46 cases of squamous cell carcinoma, 27 cases of basal cell carcinoma, 26 cases of malignant melanoma, and 17 cases of metastatic carcinoma were observed. 4. The sites of predilection of the epidermal cysts were trunk (21.9%), face (20.9%), and neck (13.9%) and lipoma was trunk (48.6%). The sites of predilection of the suamous cell carcinomas were penis (11.1%), sclap (12.9%), face (18.5%), and leg (11.1%), basal cell carcinoma was face (55.6%) and malignant melanoma was sole (26.9%). 5. Among the 17 cases of cutaneous metastatic cancer, primary sites were breast (4 cases), pancreas (1 cases), lung (2 cases), stomach (1 cases) and unkown sites (9 cases).

8.Epidemic Pneumonia Caused by Mycoplasma Pneumoniae.

BACKGROUND: Most studies of Mycoplasma pneumonia involve a group of admitted patients in hospital, usually with major medical illness. So we investigated the epidemiologic and radiologic features during the course of outbreak of pneumonia in Chunnam coastal area. METHODS: We retrospectively studied the epidemiologic and clinical feature of 105 patients with serologically proven Mycoplasma pneumonia treated at Kwang-Yang Hospital during a epidemic period of Jun. 1993 to Dec. 1993. All cases of pneumonia developed in this period were also reviewed and compared with serologically proven group. RESULTS: 1) There were 63 males and 42 females. 2) More than half(57%) of cases belonged to 5-9 years of age group, and mean age was 6.5 years old. Mean age was steadily decreased as prevalence of Mycoplasma pneumonia had been subsided. 3) A major determinant of the outbreak seemed to .to the population density rather than the population size. 4) The common radiologic features were interstitial in type, and 67 cases was restricted to one lobe. Lobar types are more common in late childhood, and interstitial or diffuse types in early childhood. CONCLUSION: These epidemiologic and radiographic characteristics would contribute to the diagnosis of Mycoplasma pneumonia.

9.Histopathologic findings of necrotizing lymphadenitis.

Necrotizing lymphadenitis was first recognised as a self-limiting lymphadenitis by Japanese workers in 1972. It is a distinct clinicopathologic entity, but can be mistaken as malignant lymphoma. We have studied clinicopathologic features in 15 cases of necrotizing lymphadenitis. This disease occurs predominantly in young adult. Male-female ratio is 2 : 1. The commonest presentation is lateral cervical lymphadenopathy. Pain, tenderness, and fever can be seen. Biopsy of the lymph nodes from all patients demonstrates the characteristic histologic features : multifocal, relatively circumscribed nodules in the cortex and/or paracortex, consisting of a mixture of activated large lymphoid cells, histiocytes and Small lymphocytes. Numerous karyorrhetic debris are present. Neutrophils and plasma cells are strikingly absent.

10.A case of hypereosinophilic dermatitis.

Hypereosinophilir. dermatitis, which was first described in 1981, is a clinically distinet disease within the spectrum of the hypereosinophilic syndrome. It is characterized by a generalized polymorphous pruritic eruptions and blood eosinophilia with minimal or no extracutaneous involvement known cause hypereosinophilia, namely parasites, allergies and drug association are absent in these patients. We present a patient with hypereosinophilic dermatitis who did not show evidence of systemic disease. The patient was a 18-year old male who had pruritic erythematous brownish elevated plaque on both lower extremities for 5 months. A)though the patients skin lesions improved after treatment with steroids and dapsone, after discontinuing medication, hypereosinophilia recurred.
