1.Microsurgical Reimplantation of a Completely Amputated Penis in a Child.

Penile amputation is quite rare and requires emergency reimplantation by using microvascular surgery. Herein we report a case of an 8-year-old boy who accidentally has his penis amputated during circumcision. Emergency reimplantation was performed by using microvascular surgery. Thereafter partial necrosis of glans penis and distal penile skin was noted So penoplasty by using scrotal skin was done to restore normal shape penis. After 6 months later, normal erection and sensation were restored.

2.Metanephric Adenoma of the Kidney.

Metanephric adenoma is a rare renal epithelial tumor. Its light microscopic features are very characteristic, and immunohistochemical and electron microscopic studies are not critical to the diagnosis. The literature indicate that, to date, the tumor has behaved in a benign fashion, and predominantly but not exclusively occurred in middle-aged women. It occurs in a wide range up to 11 cm and is usually an incidental finding but may be symptomatic with hematuria or flank pain. Recently, we have experienced a case of renal tumor showing distinctive adenomatous features, which is incidentally found in a 52-year-old female. This tumor is confined to the renal cortex and is well-circumscribed with a characteristic uniform and orderly proliferation of compact well-differentiated small tubules lined by bland oval cells with a very low level of mitotic activity. The term metanephric adenoma is appropriate for this tumor because it accurately describes its bland proliferation of tubules and reflects the embryonic architectural and cytological appearance of this proliferation. The pattern of the tumor, with its occasional papillary glomeruloid- like bodies and foci of elongated tubules, is reminiscent of the fetal metanephric kidney.

3.Postoperative Suppression of Nausea and Vomiting by Preoperative Administ ration of 5-HT3 Receptor Antagonist Granisetronin Strabismus Surgery Under Topical Anesthesia.

Traction of the extraocular muscles during strabismus surgery could cause nausea and vomiting in postoperative period. Although the extent of these symptoms would vary in wide range, sometimes they are so severe that patients may feel very uncomfortable and have difficulties with adjusting back to their routine life in immediate postoperative period. Authors have investigated whether the 5-hydroxytryptamin-3 [5HT3] receptor antagonist, which is now used as an effective antiememtic agent after general anesthesia and anticancer chemotherapy, can control the nausea and vomiting after strabismus surgery or not. As the occurrence of the oculocardiac reflex during strabismus surgery is closely related with postoperative emesis, we also examined if 5HT3 receptor antagonist can suppress the oculocardiac reflex as well. We performed strabismus surgery in 80 patients under topical anesthesia. In experimental group[N=40], 3 milligramof Granistron in 50 milliliter of normal saline was administered intravenously and in control group[N=40], the same amount of normal saline was administered one hour before the surgery. To evaluate the degree of nausea and vomiting, authors used the Rhodes 'Nausea and Vomiting Estimation Index[NVEI]and monitored heart rate for oculo-car-diac reflex. The NVEI in experimental group was 12.10 +/-5.60 compared with 16.85 +/-8.88 in control[P=0.005]. But there was no difference in frequency of oculocardiac reflex and the degree of heart rate decrement between the two groups [P=0.152, P=0.345]. In conclusion, Granisetron, 5HT3 receptor antagonist, is still effective in controlling the nausea and vomiting after strabismus surgery, but it has no suppressive effect on the oculocardiac reflex.

5.Death Anxiety and Terminal Care Performance of Nurses at Long Term Care Hospitals.

PURPOSE: The purpose of this study was to identify the relationships between death anxiety and terminal care performance of nurses at long term care hospitals. METHODS: Data were collected from 148 nurses working at 12 long term care hospitals in Busan from March 16, 2016 through May 16, 2016. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Scheffé test and Pearson's correlation coefficient with SPSS 18.0 for Windows. RESULTS: Terminal care performance was positively correlated with anxiety about other people's death (r=0.310, P≤0.001) and that of their own death (r=0.250, P=0.002). CONCLUSION: It appears necessary to develop a systematical educational program for terminal care nurses of to reduce their death anxiety and improve their terminal care performance.

6.Treatment of Exposed Hydroxyapatite Orbital Implant Using Acellular Dermal Allograft.

PURPOSE: To investigate the results of acellular dermal allograft (SureDerm(R), Hans Biomed Co., Korea) as a new covering material in the exposure of hydroxyapatite orbital implant. METHODS: SureDerm graft was performed in 5 patients with exposed hydroxyapatite orbital implant. Under local anesthesia, the anterior surface of the exposed implant was trimmed and the designed SureDerm was sutured at the margin of the sclera with 6-0 Vicryl. Finally, conjunctival suture was done. The patients were followed up regularly until postoperative 10 months. RESULTS: The average diameter of exposed implant was 9.6 X 9.0 mm. The grafted SureDerm had survived in all patients at postoperative 10 months and exposure of implant was treated successfully in all cases. CONCLUSIONS: We consider that acellular dermal allograft will be a good replacing material when preserved sclera is not available. It also features the advantage of avoiding the intricate harvesting procedure and the scarring of the donor site in dermo-fat graft.

7.Anaphylactic reaction after topical Lidocaine anesthesia during bronchoscopy.

Fiberoptic bronchoscopy is a valuable diagnostic and therapeutic procedure in many clinical situations and is relatively simple to perform with proper technique. Local anesthetic technique is often preferable to general anesthesia for bronchoscopies since these examinations are mostly undertaken as outpatient procedures. Inhaled topical lidocaine, used to produce anesthesia of the repiratory tract prior to bronchoscopy, may cause anaphylactoid reaction in patients. However lidocaine hypersensitivity reaction is uncommon. We report the case of death due to hypersensitivity to topical lidocaine spray given during routine premedication for this procedure. The possibility of bronchospasm secondary to an adverse reaction to premedication or anesthesia must also be considered.

8.Relationship between Nausea, Vomiting and Oculocardiac Reflex Developing after Strabismus Surg e ry under Topical Anesthesia.

Stimulation of trigeminal nerve by traction of extraocular muscle during strabismus surgery induces various vagal symptoms such as oculocardiac reflex, nausea and vomiting. We have investigated the incidence and severity of nausea and vomiting in the patients who underwent strabismus surgery under topical anesthesia and relationship with oculocardiac reflex. Among 40 patients, 16 patients(40%) complained of moderate to very severe degree of nausea and vomiting during or after surgery, and the severity of emesis increased in proportion to frequency of oculolocardiac reflex. These results suggest that so many patients would feel discomfort after strabismus surgery under topical anesthesia, and that estimation of oculocardiac reflex will be helpful in prediction of such symptoms and in taking preventive measure in advance.

9.Clinical usefulness of rapid antigen test to detect respiratory syncytial virus infection.

PURPOSE: Respiratory syncytial virus (RSV) is the most frequent cause of lower respiratory infections in infants and young children. Early detection allows quarantining of infected inpatients to prevent nosocomial transmission and to choose a treatment. To achieve rapid reporting, to facilitate prompt antiviral therapy, and to avoid unnecessary use of antibiotics, an easy, rapid diagnostic method for RSV is needed. We evaluated a lateral flow immunochromatography (RSV Respi-Strip test) and EIA (Enzyme immuno assay) compared to RT-PCR. METHODS: From April 2007 to March 2008, 112 consecutive respiratory specimens (nasopharyngeal aspirates, throat swabs, tracheal aspirates, sputum) from patients who were suffering from the clinical signs and symptoms of respiratory tract infection were enrolled in Busan. A total of 112 patients were tested with RSV Respi-Strip (Corio-BioConcept, Belgium), EIA, and RT-PCR at the same time. RESULTS: Of the 112 specimens tested, the number of children who showed positive results at RT-PCR and Respi-Strip were 45 and 42, respectively. The Respi-Strip rapid antigen test had a sensitivity of 88% and a specificity of 94%. The positive and negative predictive values were 90% and 92%, respectively. The agreement was 83%. CONCLUSION: In our study, the rapid antigen test had as much sensitivity as any method for detection of RSV. The test has many advantages such as easy performance, simple interpretation, and rapid results. If the rapid antigen test is widely applied in the clinical setting, the may be useful for diagnostic and epidemiological studies of RSV infection.

10.Outcomes after Surgery for Spinal Metastasis of Colorectal Origin: Case Series.

STUDY DESIGN: Retrospective study. PURPOSE: The aim of this study was to evaluate the clinical management and outcomes of patients who underwent surgical intervention for metastatic colorectal adenocarcinoma of the spine. OVERVIEW OF LITERATURE: Gastrointestinal (GI) cancer metastasis to the spine are relatively rare and represent later manifestations of the disease. Studies and reports on the outcomes of patients who undergo surgery for spinal metastasis of GI origin are scarce. METHODS: A retrospective chart review of all patients who underwent surgery for spinal metastasis of colorectal origin was performed. Four patients were identified. Patient characteristics, outcomes, and survival were analyzed. RESULTS: Two patients experienced improvement in pain or myelopathic symptoms. Although the mean survival was 15.3 months, this average included a patient still living at 57.1 months. The mean survival was just 1.3 months for the 3 patients who expired. CONCLUSIONS: In certain cases, symptomatic improvement with prolonged survival is possible after surgery for metastatic spinal lesions of colorectal origin; however, survival is poor in the majority of cases.
