1.A Comparison of the Physical Activity of Hematopoietic Stem Cell Transplantation Patients and Healthy People.

PURPOSE: The study aimed to compare the physical activity of hematopoietic stem cell transplantation (HSCT) patients with healthy people (HP). METHODS: The research design of this study was cross sectional and, descriptive study. It was conducted from January 26th to May 22nd, 2013. We matched HSCT patients to HP based on age and gender, and measured physical activity using a pedometer. Patient's clinical information were gathered by the medical record reviews. RESULTS: The number of steps was significantly less in HSCT patients (HSCT: 214 (7~3,373) vs HP: 7,921 (3,180~17,539), p<.001). Physical activity time was significantly less in HSCT patients (HSCT: 840 (680~1,320)min vs HP: 990 (540~1,090)min, p<.001). Step count showed a significant relationship with platelet count (r=.40, p=.004). CONCLUSION: HSCT patients perform very low intensity of physical activities. The study shows the necessity to apply programs to promote physical activities in isolation rooms for patients undergoing HSCT.

2.Nutrient Composition and Content of Vitamin and Mineral Supplements and Their Appropriateness for Pregnant and Lactating Women in Korea

OBJECTIVES: Use of dietary supplements containing vitamins and minerals is growing in Korean adults, especially in pregnant and lactating women. Vitamin and mineral supplements are available in different composition and in a wide range of contents. The purposes of the study were to examine nutrient composition and content of vitamin and mineral supplements for pregnant and lactating women and assess their appropriateness as dietary supplements. METHODS: Information on the name, manufacturer, nutrient composition, and usage of vitamin and mineral supplements for pregnant and lactating women were obtained from the homepage of the Food Safety Information Portal managed by the Ministry of Food and Drug Safety, and Korean Index of Medical Specialties. A total of 264 products were identified. RESULTS: Among 264 products, 26.1% were single nutrient products, and 73.9% were multinutrient products. The most commonly included nutrient was iron (70.1%), folic acid (66.3%), vitamin B12 (45.8%), vitamin C (38.6%), and vitamin B6 (38.6%). Although more than 50% of products contained nutrients less than 150% of Recommended Nutrient Intakes or Adequate Intakes for daily use, some products contained inappropriately high amounts of nutrients. When a maximum daily dose of supplements was taken as described on the label, iron in 73 products (39.5%), folic acid in 14 products (8.0%) were likely to be consumed in amounts greater than Tolerable Upper Intake Levels. Most products were assessed as inappropriate for pregnant women due to the possibility of excessive intake of vitamins or minerals when compared with Dietary Reference Intakes. CONCLUSIONS: Pregnant and lactating women need to carefully select dietary supplements containing adequate amounts of vitamins and minerals. Nutritionists should provide guidelines regarding selection of appropriate vitamin and mineral supplements for pregnant and lactating women.

4.A Case of Focal Segmental Glomerulosclerosis in Turner Syndrome.

Turner syndrome is usually accompanied with various anomalies. Congenital urological and renal abnormalities are often associated with this syndrome. The occurrence of glomerulonephritis is uncommon. An 18-year-old woman showed fatigue and profound proteinuria. She had been diagnosed with Turner syndrome in her age of 15. The kidney biopsy specimen examined by light microscopy, immunofluorescence and electron microscopic examination revealed focal segmental glomerulosclerosis. This is the first case report of focal segmental glomerulosclerosis in turner syndrome in South Korea.

5.In vivo CTL Activity Induced by Prime-boost Vaccination using Recombinant Vaccinia Virus and DNA Vaccine Expressing Epitope Specific for CD8+ T Cells.

DNA vaccine approaches have been applied to generate the protective immunity against various pathogens. However, the strength of immune responses induced by DNA vaccine is weak compared with conventional vaccines. The primeboost vaccination using DNA vaccine and other viral vector has been suggested as one way to circumvent this limitation. In the present study, we used in vivo CTL activity assay to determine CD8+ T cell-mediated immunity induced by prime-boost vaccination with a DNA vaccine (gB498-505 DNA) and recombinant vaccinia virus (VVgB498-505) expressing gB498-505 epitope peptide (SSIEFARL) of herpes simplex virus type 1 (HSV-1) glycoprotein B (gB). The most potent in vivo CTL activity was induced in mice received VVgB498-505 when both gB498-505 and VVgB498-505 were used at priming step and boosted with the alternative vaccine vector expressing whole antigen protein (gBw). Priming with vaccine vector expressing gBw followed by the use of VVgB498-505 at boosting step also induced strong in vivo CTL activity. We also examined in vivo CTL activity after immunization of mice with epitope-expressing vaccine vector at both priming and boosting step. Curiously, in vivo CTL activity mediated by CD8+ T cells was strongly elicited at memory stage when animals were primed with VVgB498-505 and subsequently boosted with gB498-505 DNA. Because the use of VVgB498-505 at priming followed by boosting with gB498-505 DNA induced most optimal immunity, these results suggest that the order of vaccine type should be carefully considered when used vaccine type expressing only epitope for prime-boost vaccination.

6.Serum IGF-I and IGFBP-3 Concentrations in Children with Cancer: Comparisons with Normal Children.

PURPOSE: Since insulin-like growth factor-I (IGF-I) and insulin-like growth factor binding protein-3 (IGFBP-3) are often used as markers for growth assessment, we compared the serum IGF-I and IGFBP-3 levels in children diagnosed with cancer with those in the healthy children. METHODS: Forty-nine children who were diagnosed with cancer and treated with radiotherapy or chemotherapy were enrolled in the study. Sixty-four healthy children without any medical problems were enrolled as controls. Height, weight, body mass index (BMI), height standard deviation score (HTSDS), serum IGF-I, serum IGFBP-3 and molar ratio of IGF-I/IGFBP-3 were compared between the two groups. RESULTS: The mean age of children in the control group was 9.3+/-2.9 years and that of children in the cancer group was 8.8+/-3.3 years. There were no significant differences in mean height, weight, BMI and HT SDS between in the two group. Serum IGF-I levels increased with age in both groups, and the mean level of the control group was significantly higher than that of the cancer group. The mean serum level of IGFBP-3 in the control group was higher than that of the cancer group. IGF-I/IGFBP-3 molar ratio also increased with age in both groups and the mean level of molar ratio of the control group was higher than that of the cancer group. IGF-I, IGFBP-3, and IGF-I/IGFBP-3 molar ratio in the leukemia group and the solid tumor group were not significantly different. Mean age, IGF-I and IGFBP-3 concentrations in the chemotherapy group were higher than those in the chemotherapy and radiation therapy group. CONCLUSION: The serum levels of IGF-I and IGFBP-3 in children diagnosed with cancer, treated with radiotherapy or chemotherapy, were lower than those of the control group. In conclusion, the serum levels of IGF-I and IGFBP-3 can possibly be used as early markers of growth assessment in children with cancer treatment.

7.Monocentric Derivative Y Chromosome with Duplication of the SRY Gene in an Azoospermic Male.

Structural abnormalities of the Y chromosome affect normal testicular differentiation and spermatogenesis. The present case showed a rare monocentric derivative Y chromosome with partial duplication of Yp including the SRY gene and deletion of Yq12 heterochromatin. The karyotype was 46,X,der(Y) (pter-->q11.23::p11.2-->pter).ish der(Y)(DYZ3+,DYZ1-,SRY++), confirmed through a FISH study. Even though the patient possessed an abnormal Y chromosome, testicular biopsy showed normal testicular volumes in the proband, with gonadal hormonal levels in the normal range but bilateral varicocele and hypospermatogenesis. We speculate that the abnormal Y chromosome arose from sister chromatids during Y chromosome recombination or intra chromosomal NAHR (non-allelic homologous recombination) during meiosis in the patient's father or in the very early stages of embryogenesis. The derivative Y chromosome might interfere in the meiotic stage of spermatogenesis, leading to the developmental arrest of germ cells. The present case illustrates that the infertility phenotype can have various causes. Also, it emphasizes the importance of accurate and various genetic analyses and could aid in male infertility treatment.
