1.Two Cases of Pseudocyst of Auricle.

We present two cases of pseudocyst of auricle which occured in 59-year-old man and 21-year-old man. Each showed bean sized non-inflammatory cystic swelling of upper part of right auricle with slight tenderness and the cystic content was serosanguinous fluid. Cystic wall was composed of fibrovascular tissue replacing degenerated cartilage without lining epithelium.

2.A Case of Xeroderma Pigmentosum.

The role of the immune system in pathogenesis of skin cancer has been attracted increasing attention in recent years. Some limited studies and observations suggest that altered immune status may be a factor in the initiation and growth of cutaneous malignancies. Authors experienced a typical case of xeroderma pigmentosum in a 6 year-old girl, who has been suffered from hyper and hypopigmented macules on sun-exposed area, photophobia and also noted eroded lesion on the lower lip. The biopsy finding of the hyperpigmented macule showed that of freckle and the finding of actinic cheilitis was observed from the specimen of eroded 1esion on the lower lip. We performed immunologic test and obtained some evidences of immunologic defect; such as decreased numbers of total T cells, T-suppressor cell and B cell in our patient.

5.Results of Primary Endoscopic Urethral Realignment as a Treatment of Urethral Injury According to the Injury Site.

PURPOSE: There are a variety of open surgical and endoscopic methods to the treatment of urethral injuries. The objective of our study is to evaluate the efficacy of primary endoscopic urethral realignment according to the injury site. MATERIALS AND METHODS: Twenty eight patients with urethral injuries(23 anterior and 5 posterior) were treated by primary endoscopic urethral realignment from March 1990 to August 1997. According to the injury site, age distribution, etiology of injury, associated injuries, time to operation, operating time, duration of urethral Foley catheterization, maximal flow rate, postoperative complications and treatment of post-realignment stricture were reviewed. RESULTS: The age range of our patients was from 20 to 86 years(mean 45.0). Among the 28 patients, 23 were anterior and 5 were posterior urethral injuries. Pelvic bone fracture was associated in 1 patient(20.0%) in the anterior urethral injury group, while 5 patients(100%) in the posterior urethral injury group. The mean time after injury to realignment was 1.9 days (range 0 to 9) and the mean operating time was 53.9 minutes in the anterior urethral injury group(range 20-190) and 79.0 minutes in the posterior urethral injury group(range 25-170). The mean duration of urethral Foley catheterization was 24.5 days in the anterior urethral injury group and 61.4 days in the posterior urethral injury group. The mean maximal flow rate after catheter removal was 31.4ml/sec in the anterior urethral injury group and 24.6ml/sec in the posterior urethral injury group. Of the 23 patients, 9 patients(39.1%) had post-realignment strictures in the anterior urethral injury group and 8 were treated with visual urethrotomy, and only one patient was required open urethroplasty. Of the 5 patients, 4 patients(80.0%) had post-realignment strictures in the posterior urethral injury group and treated with visual urethrotomy. CONCLUSIONS: Primary endoscopic urethral realignment is a safe and simple technique with minimal mobidity regardless of injury site. The stricture formation, impotence and incontinence rates of this technique are comparable to those reported for open surgical methods. Finally, most post-realignment strictures can be treated successfully with visual internal urethrotomy with or without occasional sound dilation.

7.Clinical Study on Torsades de Pointes.

Torsades de pointes is characterized by paroxysms of ventricular tachycardia at rates typically greater than 200 beats/min in which QRS morphology shows alternating polarity in an undulating pattern so that the complexes appear to be twisting about the beseline;this arrhythmia is virtually always associated with prolongation of the QT interval. Its importance lies not in its unusual structure but in the potentially fatal outcome if conventional treatment is administred. Torsades de pointes was diagnosed in 4 patients;the first with hypokalemia, the second with congenital QT prolongation syndrome, the third with amiodarone, the fourth with organophosphorous and hypokalemia. Treatment of these patients consisted of potassium supply, isoproterenol, lidocaine, phenobarbital, tenormin, phenytoin, cardioversion, atropine. Three patients improved successfully, but one patient died, as a direct result of the ensuing ventricular fibrillation and cardiac arrest on one hour after admission.

8.A Case of Erythema Elevatum Diutinum Developed on the Unusual Sites.

We report a case of 77-year old male with erythema elevatum diutinum, associated with pancytopenia, which developed on the unusual sites. Erythematous or brownish nodules developed on the face, neck, trunk, upper extremities and tongue. Histopathological examination of skin and tongue lesions showed perivascular infiltration of neutrophils, lymphocytes and nuclear dusts, and deposition of fibrinoid material within and around the vessel walls. His skin lesions improved slightly with systemic corticosteroid therapy.

10.A Case of Pseudoxanthomatous Mastocytosis.

We report a case of pseudoxanthomatous mastocytosis that is an uncommon variant of urticaria pigmentosa. A 3 month-old boy visited our hospital with yellowish cutaneous nodules At the age of 2 months, small yellowish macule developed on the posterior neek, and during the following months, lesions spread to trunk, upper extremities, face, and scalp, most of them became gradually to the nodules. Darier's sign could be elicited easily. A biopsy of one. of the nodules or the back showed a very dense oval or cuboidal cells infiltrate in the dermis. Special stain with toluiddine blue showed this to he composed of mast cells. Electron microscopic examination revealed crowded mast. cells which was showing numerous cytoplasmic granules and long, thin villi. The patient: was managed symptomaticvlly and recommended to avoid mechanical or chemical irritation to the lesions.
