1.A Clinical Study on Pyogenic Osteomyelitis of 725 Cases During Past 21 Years

Pyogenic osteomyelitis is a difficult and complex disease. The wide spectrum of manifestation of disease related to the site of involvement, the infecting organism, the initiating event and the acute or chronic course of the illness are important in determining the success or failure of treatment. Seven hundred and twenty-five cases of pyogenic osteomyelitis were studied in the Department of Orthopedic Surgery, Seoul National University Hospital for 21 years from December 1956 to December 1977. The results were as follows: 1. During past 21 years, total number of admitted patients were 10,456. Among them, number of patients of pyogenic osteomyelitis were 725. 2. Femur and tibia were frequently involved (64%), and humerus, spine, pelvis, in that orders. 3. Sex ratio was 2.4: 1. Age distribution showed the group between 6 and 15 years old was 58% in acute course, and the group between 16 and 25 years old was 34% in chronic course. 4. Duration of disease was average 7.4 days in acute and 46.8 months in chronic osteomyelitis. 5. Most of acute osteomyelitis had pyrexia (av. 38.3C), but in chronic osteomyelitis body temperature was mostly within normal limit. 6. Erythrocyte sedimentation rate was definitely increased in 81% of male, in 58% of female. 7. The causative organism was mostly coaulase (+) staphylococcus aureus (73%), but there was tendency to increase Gram (−) bacilli infection. 8. One kundred and six patients had been complicated with draining sinus, joint stiffness, and pathologic fracture. 9. Various treatments had been carried out case by case. Author considered the best method of treatment was ealry decompression and antibiotics in acute osteomyelitis, and saucerization with closed irrigation in chronic osteomyelitis.

5.A Case of Malignant Duodenocolic Fistula Diagnosed by Endoscopy.

The occurrence of a fistula between the duodenum and the colon is very rare, although these two structures commonly lie in intimate relationship to each other. It is not uncommon for tumor of the right colon and proximal transverse colon to involve the duodenum by continuity, but fistulous connection between the duodenum and colon are infrequent In most of the reported malignant cases, the fistulas follow carcinomas of the transverse colon or the hepatic flexure of the colon. We report a rare case of malignant duodenocolic fistula which was first suspected by endoscopic examination and confirmed by operation.

6.Positivity of Hepatitis B Virus(HBV) Markers and Changes of HBV Markers After Vaccination Among Healthy Works.

Hepatitis B Virus(t-IBV) infection is one of the major health problems in Korea and HBsAg positive rate was known to be as 6-10% in general population. This study was carried out to determine HBV infection rate of workers who worked in one company and to assess the conversion rate of HBV markers(HBsAg: by RPHA method, Anti-HBs: PHA method) from 1990 to 1992. A total number of study subject was 1,972 male workers who were interviewed during health check up at 1992. Healthy workers were defined those who have none of the past histories of transfusion, hepatitis, abnormal liver function. The results were as follows: 1. HBsAg positive rate among healthy workers in 1990 was 2.7% and it was highest in age group thirties as 3.0% with no statistically significance by age group. 2. HBsAg positive conversion rate was 3.0% and negative conversion rate was 7.7% from 1990 to 1992. 3. The rate of Anti-HBs formation was 32.5% after 3 doeses hepatitis B vaccination.

7.Clinial Study of Nevocellular Nevus in Korea.

Nevocellular nevi are benign tumors composed of nevus cells, which are peculiar to man and almost everyone has a few nevi somewhere on his body. The dermatologist has a unique opportunity to study the most common neoplaam of man, the nevocellular nevus, becasue of its ready accessibility to visual observation and its simplicity obtaining for microscopic specimen. Much has been leamed last two decades about the nature, origin, and development of the nevocellular nevus. However, most of the these knowledge obtained by investigation of Caucasians. The investigation of colored subjects are scarcely reported. This paper is an attempt to clarify 1) the clinical features of nevocellular nevi developed on Korean, colored race. 2) the incidence of pigmented nevi developed on normal individuals and vitiligo patients. During the last l0 month(1975. 3. - 1975. 12. ) 525 normal individuals and 67 vitiligo patients were randomly selected and were investigated about incidence and clinical morphology(according to Levers clinical classification) of pigmented nevi developed on different body areas. The results were summarized as follows. 1) There was average 20 nevocellular nevi in each Korean individuals. Average 23 nevi was found in male and 15 nevi in female. 2) The number of nevus and its morphology were closely related to the age of host and body areas. 3) The flat type nevi was the most common type nevi and face was the most common site among the body areas. 4) In comparison with Caucasian, it seemed no racial difference in clinical features of the pigmented nevus. 5) There was no difference between the incidence of nevocellular nevus developed on normal individual and that of vitiligo patient.
