2.A rare case of an appendiceal mass masquerading as a pelvic tumour and causing bilateral hydronephrosis

Diagnosing acute appendicitis in children can be difficult due to atypical presenting symptoms. While there are reported cases of acute appendicitis or appendiceal masses causing unilateral hydronephrosis, bilateral hydronephrosis as a complication of appendiceal mass is very rare. We report a case of a child who presented with cardinal symptomatology associated with the urogenital tract. Ultrasound (US) investigation showed a pelvic mass causing bilateral hydronephrosis. An initial diagnosis of a pelvic teratoma was made based on the US and computed tomography (CT) scan findings. The final diagnosis of an appendiceal mass causing bilateral hydronephrosis was established intraoperatively.

Signet ring cell adenocarcinomas may be encountered in various parts of gastrointestinal tract but are almost unheard
of in the region of ampulla of vater. In the region of ampulla of vater even if we come across an adenocarcinoma, it
is usually a well differentiated variant. A 56-year-old female with features of obstructive jaundice on evaluation was
diagnosed to have a peri-ampullary carcinoma.The patient was subjected to a laparotomy. On exploration patient was
found to have a malrotation of gut. Patient was subjected to a pylorus preserving pancreatico-duodenectomy.
Histopathology of the resected specimen revealed a signet ring morphology which is a rare variant in periampullary
region.

4.The Role of Multislice Computed Tomography (MSCT) in the Detection of Blunt Traumatic Intra Abdominal Injury: Our Experience in Hospital Tengku Ampuan Afzan (HTAA),Kuantan, Pahang

Multislice computed tomography (MSCT) is the imaging
modality of choice in assessing clinically stable patients with blunt abdominal trauma. This study assessed the role of MSCT in the detection of intra abdominal injury caused by blunt trauma in our centre within a two-year-period (2008-2009). A total of 151 patients had MSCT abdomen for blunt abdominal trauma within this study period. Positive scan were seen in 126 patients (83.4%). Out of these positive scans, liver, spleen and renal injuries were seen in 42.1% (n=53), 34.9% (n=44) and 30.0% (n=34) of cases respectively.
Laparotomies were performed in 45 patients. Out of these 45
laparotomies, 10 patients had surgically significant injuries that were missed on CT scan findings. The injuries were bowel perforation (n=4), serosal tear of bowel (n=1), mesenteric injuries with active haemorrhage (n=3), spleen injury (n=1) and liver injury (n=1).
5.Cubitus Valgus with Tardy Ulnar Nerve Palsy - Is Anterior Transposition of the Ulnar Nerve Necessary?: A Case Report

Tardy ulnar nerve palsy is a known complication of cubitus valgus. The options for treating the ulnar neuropathy include anterior nerve transposition or neurolysis. We report on an 11-year-old boy who had a tardy ulnar nerve palsy due to cubitus valgus resulting from a non-union of a lateral condyle fracture of the humerus. Anterior transposition of the ulnar nerve was not done after the closing wedge osteotomy of the distal humerus. The close wedge osteotomy relieved the tension on the nerve and not transposing the ulnar nerve anteriorly prevented an iatrogenic nerve injury. The patient had no restriction with activities of daily living at the six years follow-up although neurological recovery was incomplete.
6.Anxiety Reaction in Children During Cast Removal using Oscillating Saw versus Cast Shear - A Randomised, Prospective Trial

Introduction: To compare the anxiety levels demonstrated
by children during cast removal procedure between
oscillating saw vs cast shear methods.
Material and methods: A randomised prospective study of
102 children (mean age 8.3 ± 3.5 years) with fractures
involving upper or lower limbs. Children undergoing
removal of cast were divided into 2 groups; either by an
oscillating saw or a cast cutting shear. The level of anxiety
was assessed by recording the heart rate with a portable
fingertip pulse oximeter before, during and after removal of
the cast. Objective assessment was performed by
documenting the fear level on Children’s Fear Scale (CFS).
Results: There was a significant increase in the heart rate of
children during cast removal while using the oscillating saw
compared to cast shear (p<0.05). The noise level produced
by the saw exceeded 80 dB (mean 103.3 dB). The fear level
was significantly lower in the cast shear group (p<0.05).
Conclusion: The noise produced by the oscillating saw was
associated with an increased anxiety level in children
undergoing cast removal. Cast shear is a simple and
inexpensive instrument that can be used for cast removal in
overly anxious children.
