1.APPLICATION OF HYPNOTIC SUGGESTION TO PHYSICAL EDUCATION

In order to develop skill of students in the physical education, the authors investigated the relation between the intensity of stimulus and the amplitude of evoked potential and how the amplitude is affected by the verbal suggestion of stimulus intensity when the subject is in the hypnotic state.
1. There is an approximately parallel relation among the intensity level (db) of auditory stimulus, the light area of optical stimulus, and the amplitude of evoked potential.
2. When the subject is in the hypnotic state, the amplitude of vertex potential is larger if he is told that the stimulus is loud, and the amplitude is smaller if he is told that the stimulus is small.
3. When the subject is in the hypnotic state, the verbal suggestion that he feels no pain at all produces a big positive change in the amplitude of vertex potential, while the suggestion that he feels very much pain produces a negative change and in some cases, an after-discharge can be observed. But no change was seen in the evoked potential of the central area.
4. When the subject is hypnotized, the amplitude of evoked potential in the occipital area increases and decreases according to the verbal suggestion that the light looks larger and smaller respectively.
Hypnotic suggestion may be applied to the guidane in physical education.

Acute encephalopathy is classified into multiple syndromes, such as acute encephalopathy with biphasic
seizures and late reduced diffusion (AESD), clinically mild encephalitis/encephalopathy with a reversible
splenial lesion (MERS) and acute necrotizing encephalopathy (ANE), characterized radiologically
by lesions in the cerebral subcortical white matter, splenium of the corpus callosum and bilateral
thalami, respectively. We described a previously healthy 8-year-old boy who had febrile and biphasic
seizures, and encephalopathy. MRI showed abnormal signal intensity in the corpus callosum on day
2 and cerebral subcortical white matter and bilateral thalamic lesions on day 8. This is the first case
of acute encephalopathy in which callosal, subcortical and thalamic lesions co-existed. The clinical
course of this case was typical for AESD, atypical for MERS, and different from that of ANE.

The 82 nd and 83 rd National Examination for Physicians' License, which were held in 1988 and 1989 respectively, were evaluated from question to question as well as in all the questions as a whole to set minimum pass scores and analysis “relevance” and “difficulty” in a matrix utilizing a modified Ebel's method.
The evaluators were teachers in different disciplines in nationwide medical schools and teaching hospitals and clinical trainees who had taken and passed the immediate past examinations.
Following data processing, the questionable and difficult questions were on the decrease compared with the preceding year, and it was tentatively concluded that the National Examinations have gradually improved year by year.

Based on two years working of “the enlarged working group for the curriculum of chemistry for medical education”, a provisional plan of the curriculum of chemistry for medical students is proposed. The article is consist of four sections; namely A: general problems, B: “chemistry” as a general education, C: purpose and specific problems of the general education for medical students, D: a provisional plan of the curriculum of chemistry for medical students.
The main part D is consist of three subsections: namely (1) a plan of the curriculum of physical and inorganic chemistry, (2) a plan of the curriculum of organic chemistry, (3) a plan of the curriculum of experiments.
6.A Case of Coronary Artery Bypass Grafting for a Patient with Eosinophilic Granulomatosis and Polyangiitis

We present herein the case of a 45-year-old man with a coronary artery aneurysm (diameter 19 mm) in the proximal part of the left anterior descending branch associated with eosinophilic granulomatosis with polyangiitis (EGPA). As coronary angiography showed #6 : 100% and #12-2 : 90%, and Tc-99 m myocardial scintigraphy showed exertional ischemia in the anterior septum, revascularization was considered to be indicated. Prednisolone and mepolizumab were administered preoperatively to suppress the activity of vasculitis due to eosinophilia, and surgery was performed when the eosinophil count normalized. The patient underwent off-pump coronary artery bypass grafting (LITA-LAD, SVG-OM2). The patient was discharged, and the postoperative course was uneventful. In coronary artery bypass grafting for EGPA, eosinophils may infiltrate the internal thoracic artery and result in vasculitis, which may affect the patency rate. Perioperative management of vasculitis may thus be important.

8.1.The Survey of Trends on Japanese Post-marketing Study after Enforcement of Revised Good Post-marketing Study Practice (GPSP)

In Japan, a re-examination system was introduced in the 1970s, and Post-marketing surveillance (PMS) has been conducted in many cases after the launch of new drugs or the approval of new indications for existing drugs. The revised Good Post-marketing Study Practice (r-GPSP) was enacted in 1 April 2018, and more scientific approach has been required for PMS. The survey aims to identify what kinds of change occurred in the PMS planned after r-GPSP enacted. As for the study method, after confirming the review report of each product, Risk Management Plan (RMP), and the package insert, and extracting necessary items, tabulation and analysis were performed. As a result of analysis, Drug Use Investigations (DUI) still accounted for more than 60% of the total in PMS. Regarding the target number of DUI, Fifty-eight out of 90 had a sample size of 500 or fewer, only 3 studies exceeded 3,000. Regarding the newly introduced Post-marketing database surveillance (DB study) after r-GPSP enacted, 13 products and 18 surveillances were confirmed, although the number was not as large as the number of DUI. The most used database was Medical Data Vision Co., Ltd. (MDV), which were used in twelve DB studies, and cardiovascular disease and interstitial pneumonia were set as survey targets for safety considerations in multiple studies. There was no Drug Use Comparative Investigation. Furthermore, there were two products which were approved on the condition with re-examination period but did not required additional Pharmacovigilance activities. In terms of sample size calculation, 88 out of 135 studies mentioned rationales for setting the number of samples. Of which, 58 studies had statistical rationales. In many studies, the sample size calculation method that was commonly used in conventional PMSs, called ‘Rule of three’ was described as the statistical rational. After r-GPSP enacted, newly introduced Post-marketing surveillance, including DB surveys, and the scientific approach recommended by PMDA have been adopted in companies gradually. In the future, in order to further promote the scientific approach, it is necessary to further improve the capability of human resources of companies involved in PMS.
