The global prevalence of diabetes by 2030 estimated an increase in number of people with diabetes. People who are 64 years of age will be 82 million in developing countries and 48 million in developed countries. The study aimed to assess the health care needs of older persons' physical, cognitive, functional and self-care abilities and determine relationship of these to their profiles. These assessments are the bases in designing modules for Diabetes Self-Management Education training module for Diabetes Educators. A descriptive correlational design was utilized to assess the health care needs of 100 young older persons. The Filipino Activities of Daily Living (Fil-ADL) and the Filipino Instrumental Activities of Daily living were utilized to assess the functional status of the older persons. The Montreal Cognitive Assessment (MOCA-P) and cognitive function exams were used to assess cognitive status while Mini- Nutritional Assessment (MNA) was used to assess nutritional status of the older persons. The Self-Care Inventory Revised (Sci-R) was used to assess self-care behaviors of older persons to diabetes management. Frequency and percentage were used to describe the health care needs of older persons, while mean and standard deviation were used in assessing knowledge and self-care abilities. Chisquare test for Association was utilized to determine relationship of the health care needs and the profile of the respondents. Study shows that most of the older persons are female, 66-70 years old, married and had primary education. The older persons' knowledge in diabetes is satisfactory, had a normal nutritional status, was functional in daily living activities (ADL), was functional in their instrumental activities of daily living (IADL), was normal of their cognitive status, and sometimes perform self-care abilities to diabetes management. There is significant relationship between knowledge, civil status and history of diabetes. Nutrition and cognitive status had significant relationship to their civil status, education and history of diabetes. The functional status of the older persons has significant relationship with history of diabetes. It is necessary to include the culture-based health care needs of older persons and to consider demography such as education and history of diabetes in designing the training module for diabetes educators.

OBJECTIVE: Since cataract and glaucoma often coexist, and there is no agreement as to their optimal management, a review of the surgical strategies for coexisting I cataract and glaucoma is necessary. The latest evidence-based findings from various studies are presented. METHODS: A literature search of the latest full articles (up to September 2006) was conducted on the surgical management of coexisting cataract and glaucoma. The results of the 2001 Johns Hopkins milestone study were also included for analysis and comparison. RESULTS: Evidence is strong that trabeculectomy is associated with increased risk of postoperative cataract. Though cataract surgery alone may be appropriate for some glaucoma patients, combined cataract and glaucoma surgery lower long-term intraocular pressure (IOP) more than cataract extraction alone. Use of intraoperative mitomycin-C is beneficial in combined surgery. Limbu and fornix-based conjunctival flaps are equally effective for lowering IOP combined surgery. Trabeculectomy alone lowers long-term IOP more tha combined extracapsular cataract extraction (ECCE) and trabeculectomy. Evidence is weak that combined surgery with phacoemulsification rather than ECCE results in lower long-term IOP, as does two-site compared to single-site combined surgery. CONCLUSION: The literature does not point to an optimal strategy for controlling in patients with coexisting cataract and glaucoma needing surgery. There is a continued need for high-quality studies of longer duration and more information on the optic nerve and visual field.

Proximal femoral focal deficiency is a rare birth defect that affects the hip bone and the proximal femur. The incidence is one case per 50,000 to 200,000 population. The disorder may be unilateral or bilateral, with the hip being deformed and the leg shortened. The goal of treatment is to provide optimal function during standing and ambulation. A 15-year-old male diagnosed with left proximal femoral focal deficiency was admitted for prosthetic rehabilitation. He presented with a very short left lower extremity, 38 cm leg length discrepancy, flail left hip and knee joints, and normal range of motion at the left ankle, and with muscles graded at 4/5. The patient was independent in transfer activities and ambulated with bilateral axillary crutches. A combination of orthosis and prosthesis (henceforth "prosthosis") was designed for the patient with a mechanical hinge joint to equalize the leg length and to improve lower extremity function during standing and ambulation. Upon discharge, the patient was independent in donning and doffing the prosthesis, was ambulatory using the prosthesis without gait aid but with minimal listing during the stance phase on the prosthesis side. During the patient's two-year follow-up, adjustment of the prosthesis was done to accomodate growth; checking of the prosthesis for mechanical breakdown and anticipatory management of potential musculoskeletal complications and psychosocial concerns on the use of the prosthesis were also done.
