1.Application of Mechanical ventilation in acute organophosphorous pesticide poisoning with respiratry failure

Objective To investigate the effect of mechanical ventilation in acute organophosphorous pesticide poisoning(AOPP) with respiratry failure. Methods Forty from forty-two AOPP patients with respiratry failure were treated with mechanical ventilation(MV) ,gastrolavage and other methods. Five minutes before mechanical ventilation and two hours after mechanical ventilation, patients' heart rate (HR), systolic blood pressure (SBP), arterial partial blood pH, oxygenation index, the pressure of carbon dioxide (PaCO2) were compared. Results Patients' HR was increased ( P<0. 05 ), SBP was increased ( P<0. 01 ), arterial partial blood pH was increased( P<0. 01 ), oxygenation index was increased(P<0. 01 ) ,PaCO2 was decreased(P<0.01 ). Thirty nine cases were cured,three cases dided. Conclusion Mechanical ventilation can effectively treat the AOPP patients with respiratry failure.
2. Molecular biological characteristics of bilateral primary breast cancer

Objective: To evaluate the molecular biological characteristics of bilateral primary breast cancer (BPBC). Methods: The clinicopathological characteristics of 325 patients with BPBC and 650 patients with unilateral breast cancer (UBC) were retrospectively analyzed. The evidence of molecular biology related to the occurrence of BPBC was evaluated by univariate analysis and multivariate Logistic regression analysis. Results: The early occurrence of first primary breast cancer, a family history of breast cancer, a maximum tumor diameter of more than 5 cm, and clinical stage III were more frequent in BPBC patients than in UBC patients (P < 0.05). The negative rates of estrogen receptor (ER) and progesterone receptor (PR), the positive rate of p53, and the incidence of high expression of Ki67 were higher in BPBC patients than those in UBC patients (P < 0.05), but there was no difference in the expression of human epidermal growth factor receptor 2 (Her-2) between BPBC patients and UBC patients (P > 0.05). The characteristics including a family history of breast cancer, a maximum tumor diameter of more than 5 cm and positive expression of p53 in BPBC were independent risk factors. The patients with positive ER or positive PR had a lower risk of BPBC. Conclusion: There are differences in biological characteristics between BPBC and UBC. The breast cancer patients with the characteristics including young woman, a family history of breast cancer, negative expression of hormone receptor, positive expression of p53, and high expression of Ki67 should pay attention to the occurrence of contralateral breast cancer. 3.The protective effects of erythropoietin/erythropoietin receptor system on optic nerve and retinal diseases

Erythropoietin(EPO) has an anti-apoptotic effect,and promotes the proliferation and differentiation of erythroid progenitor cells. Several studies have indicated that EPO can protect photoreceptor cells from the lightinduced retinal degeneration ;protect retinal neurons from ischemia-reperfusion injury and retinal ganglion cells after acute and chronic ocular hypertension; promote ganglion cell survival and axonal regeneration after optic nerve transaction; attenuate inflammation in multiple sclerosis optic neuritis; reduce the permeability of the retinal barrier and protect retinal neurons in diabetic retinopathy; enhance the stability of hypoxic retinal vessels in retinopathy of prematurity. Herein,we review the distribution of EPO and its receptor in retina,their expression in animal model of retinal diseases,and their effects and mechanisms in protection of retinal neurons and optic nerve.
4.Mediating effect of mental resilience between work stress and sleep quality of psychiatrists

Objective To analyze the mediating effect of mental resilience on the relationship between work stress and sleep
quality in psychiatrists. Methods A total of 221 front-line psychiatrists from four mental health centers in Shannxi Province
were selected as the study subjects using convenience sampling method. The questionnaires of Scale for Occupational Stressors
on Clinician,Chinese Version of Connor-Davidson Resilience Scale and Pittsburgh Sleep Quality Index Scale were used to
investigate the work stress,mental resilience and sleep quality of the psychiatrists. We conducted Bootstrap mediation test to
analyze the mediating effect of mental resilience using SPSS PROCESS V3.5 macro program. Results The total score of work
stress of psychiatrists was 97.0±17.5,and the medium(P25,P75)of mental resilience and sleep quality scores were 84.0(75.5,
94.0)and 6.0(5.0,9.0)respectively. The detection rate of sleep disorders among psychiatrists was 33.9%(75/221). The total
score of work stress of psychiatrists was negatively correlated with the total score of mental resilience [rank correlation
coefficient(rS
)=−0.34,P<0.01],and was positively correlated with the total score of sleep quality(rS
=0.48,P<0.01). The total
score of mental resilience was negatively correlated with that of sleep quality (rS
= − 0.39,P<0.01). The work stress of
psychiatrists had a positive predictive effect on sleep quality[standardized regression coefficient(β)=0.41.P<0.01],and a
negative predictive effect on mental resilience(β)=−0.38,P<0.01). Mental resilience had a negative predictive effect on sleep
quality(β)=−0.24,P<0.01. Mental resilience played a partial mediating role between work stress and sleep quality,and the
mediating effect accounted for 22.0% of the total effect. Conclusion Both work stress and mental resilience of psychiatrists can
directly affect their sleep quality,and the mental resilience has a partial mediating role in the effect of work stress on sleep
quality.
5.Changes in catecholamine, angiotensin converting enzyme and adenosine triphosphatase in ischemic preconditioning rat hearts

AIM: To investigate the role of catecholamine, angiotensin converting enzyme(ACE) and adenosine triphosphatase in ischemic preconditioning in isolated rat hearts. METHODS: Isolated perfused rat heart was subjected to global ischemia for 40 min followed by reperfusion for 10 min (I/R). Preconditioning (PC) was induced by 5 min of ischemia and 10 min of reperfusion. The tissue concentrations in NE, and ACE, ATPase activities were determined in left ventricle in the PC and I/R groups by fluorometry and spectrophotometry. RESULTS: There were no significant difference in NE and ACE between PC and I/R groups. PC hearts showed improved recovery of the contractile function after 40 min ischemia/10 min reperfusion, but activities of the myocardial total ATPase, Mg2+-ATPase, Na+K+-ATPase were inhibited markedly compared with I/R group. CONCLUSION: The inhibited myocardial ATPase may be involved in the mechanism of ischemic preconditioning protection in the isolated rat heart. Endogenous myocardial norepinephrine and ACE activation are not essential for ischemic preconditioning in the isolated rat heart.
6.Analysis of risk factors related to ventilator-associated tracheobronchitis

A total of 178 patients with the time of mechanical ventilation beyond 48 hours in the second affiliated hospital of Wenzhou medical college from January 2010 to December 2013 were enrolled in this study,and there were fifty-six patients with ventilator-associated tracheobronchitis (VAT).The associated factors included age,sex,blood pressure,blood glucose,BMI,the time of mechanical ventilation,tracheal intubation methods,raise head to 30-45°,proton pump inhibitors (PPI),prophylactic antibiotic treatment,glasgow coma scale (GCS),acute physiology and chronic health evaluation (APECHE) Ⅱ score.The related factors of VAT were evaluated by using univariate logistical regression analysis,and the statistical significant variables were analyzed by using multivariate logistical regression analysis.By using univariate logistical regression analysis age,blood glucose,the time of mechanical ventilation,raise head to 30-45°,prophylactic antibiotic treatment,GCS and APECHE Ⅱ score were the important factors of VAT (P < 0.05),but sex,blood pressure,BMI,tracheal intubation methods and PPI were insignificant related to VAT(P > 0.05).By using multivariate logistical regression analysis the time of mechanical ventilation (OR =4.072,95% CI 2.036-8.146),GCS[2.198(1.155-4.184)],age[2.128 (1.119-4.046)],APECHE Ⅱ score [2.109 (1.084-4.104)] and raise head to 30-45 ° [0.488 (0.243-0.979)] were associated independently with the VAT.The time of mechanical ventilation,GCS,age over 60 years,APECHE Ⅱ score and raise head to 30-45°were the independent factors associated with VAT.
7.Histological observation on the effects of TGF-β3 and dental pulp stem cells in repair of rabbit facial nerve injury

Objective To investigate the feasibility of combing the application of transforming growth factor-β3 (TGF-β3) with concentration of 100 ng/μl and dental pulp stem cells (DPSCs) for recovering rabbit facial nerve transverse trauma.Methods Thirty-six healthy adult Zelanian rabbits of clean-grade were selected and randomly divided into group DPSCs+TGF-β3 (experimental group), group TGF-β3 (control group 1) and group PBS (control group 2) with 12 rabbits in each group.The operations for all three groups were applied at rabbit's left cheek.A model of traumatic transection was set on upper buccal branch, then 100 ng/μl TGF-β3 solution and 0.1 ml of 1 ×108/L DPSCs suspension were added into regeneration chamber for the experimental group, while the same amount of 100 ng/μl TGF-β3 solution was added for group TGF-β3 and the same amount of PBS for group PBS.The recovery of facial nerve regeneration with the prepared animal's specimen was evaluated in the 1st, 4th and 12th week after the operation on sacrificed rabbits.Results The effects on nerve regeneration recovery for the experimental group was superior to that of control groups 1 and 2 with all the 36 models included in the result analysis, and that of control group 1 was superior to that of control group 2, which was getting better with the extension of time.Conclusions The combined application of TGF-β3 and DPSCs can effectively promote the facial nerve regeneration, which is better than that of single application of TGF-β3.Meanwhile, the effect with TGFβ3 application is better than that with application of PBS.
8.Biological Characteristics of Marine Bacterium Strain E18 and the Stability of its Indigo Pigment

One strain of Pseudoalteromonas sp. E18 was isolated from the sea mud of Ningbo, Zhejiang. It can produce indigo pigment. The morphological, cultural and biochemical characteristics of the bacterium were studied. The indigo pigment was also extracted. The results showed that the maximum absorption peak of the pigment was at 579nm. The pigment was stable to UV, Na_2SO_3,It was also stable at pH 3~9. The pigment was unstable to the sunlight and high concentration H_2O_2. Temperature of 60℃~80℃ could increase the hue while temperature higher than 90℃ could reduce the hue.
9.Protective effect of astaxanthin on human retinal pigment epithelial cells injured by hydrogen peroxide

AlM:To investigate the protective effect of astaxanthin (AST) on human retinal pigment epithelial (RPE) cells against oxidative damage induced by hydrogen peroxide (H2O2).METHODS:Human RPE cells were subcultured, cell activity was detected by MTT, rate of apoptosis was detected by flow cytometry and cell ultrastructure changes were observed under transmission electron microscope. RESULTS: MTT results showed that cell activity elevated to ( 53. 66%± 3. 25% and 70. 43%± 2. 38% after 10-8 mol/L and 10-4 mol/L AST treated. The difference had statistically significant (P<0. 05) compared with oxidative injury group (38. 76%± 3. 74%). Flow cytometry results showed that the apoptosis rate of RPE cells decreased to 30. 23%± 1. 91% and 12. 58%± 2. 12% in AST pretreated group, the difference was significant (P<0. 05) compared with oxidative injury group ( 42. 50%± 1. 94%); Electron microscopy showed that the morphology of cells gradually improved accompanied with the concentration of AST elevated.CONCLUSlON:AST may inhibit hydrogen peroxide-induced apoptosis of RPE cells, it can provide reliable evidence for pursue effective medicine to prevent and treat retina injury.
10.Effects of TGF-β3 combined with dental pulp stem cells on the repair of rabbit facial nerve injury

ObjectiveTo investigate the effects of using transforming growth factor-β3 (TGF-β3) combined with dental pulp stem cells (DPSCs) on the regeneration of rabbit facial nerve.MethodsSixteen New Zealand adult rabbits were selected randomly.These rabbits were divided into three groups (TGF-β3 and DPSCs treated group,TGF-β3 control group and PBS control group.treatment group (silicone guidance channel with collagen protein sponge were filled with TGF-β3 and DPSCs),TGF-β3 control group (silicone guidance channel with collagen protein sponge were filled with TGF-β3) and PBS control group(silicone guidance channel with collagen protein sponge were filled with PBS).After 3 months,a series of examinations were performed,including gross morphology,histological staining,neuroelectrophysiological tset.ResultsIn the experimental group,the diameter of the regenerating nerve and near distal nerve stem were almost the same.There were no formation of neuroma.The adventitial angiogenesis was rich and with tough texture.3 months after operation,in the experimental group,the facial nerve membrane integrity,nerve fibers arranged in neat rows,form a more complete,myelin swelling.The total number of regeneration of nerve fibers in the experimental group were more than of control groups,statistical analysis was significant (P<0.05).Diameter of regenerating nerve fibers in the experimental group were greater than that of control groups,statistical analysis was significant (P<0.05).The ultra-thin section showed that the regenerated fibers in the treatment group were mainly myelinated never fiber.The layer structure of myelin sheath was clear,and there were rich organells axoplasma.The neuroelectrophysiological examinations revealed that the latency of nerve and muscle action conduction in the treatment group was shorter than that of the control groups,the treatment group:(1.96±0.32) ms,the TGF-β3 control group:(2.35±0.41) ms,the PBS control group:(3.42±0.55) ms.The wave amplitude of nerve and muscle action conduction in the treatment group was obviously higher than that of control groups,the treatment group:(11.06±3.25) mV,the TGF-β3 control group:(8.40± 1.68) mV,the PBS control group:(4.62±0.77) mV.ConclusionThe combination of TGF-β3 and DPSCs can improve the effects on the repair of facial nerve injury.
