3.Effect of Local Delivery of Paclitaxel on Anastomotic Wound Healing of the Common Bile Duct in Guinea Pig

Objective To investigate the effect of local delivery of paclitaxel on bile duct scar formation and biliary stricture in guinea pig.Methods Models of end-to-end anastomosis of the common bile duct were established in 42 guinea pigs.The animals were randomly divided into paclitaxel and control groups.In the paclitaxel group,the drug solution(1000 ?mol/L,0.05 ml) was smeared onto the anastomotic wound before closing the abdomen.The animals were killed in 3 days,2 weeks,and 1 month after the operation(7 in each group at each time point).The sepcimens of the common bile duct were obtained for histological examination.Transmission electron microscopy was employed to observe the ultrastructure of paclitaxel-induced apoptotic cells.Results Histological examination showed inflammatory reaction and hyperplasia of the bile duct wall.The bile duct wall of the paclitaxel group was significantly thiner than that of the control group in 3 days and 1 month after the operation [3 days:(574.41?24.53)?m vs(802.98?24.42) ?m,t=-6.604,P=0.000;1 month:(1383.36?36.64) ?m vs(1518.56?34.89)?m,t=-2.672,P=0.020].Paclitaxel-induced cell apoptosis and injured cell organs were found by transmission electron microscopy.Conclusion Single local delivery of paclitaxel can inhibit biliary stricture in one month.

7.Clinical Observation of Chinese Medicine Fumigation-washing Therapy for Treatment of Talipes Equinus Denervated Ulcer Induced by Tethered Cord Syndrome

Objective To investigate the clinical effectiveness and safety of Chinese medicine fumigation-washing therapy in treating talipes equinus denervated ulcer induced by tethered cord syndrome(TCS) . Methods A total of 21 qualified patients were randomized into fumigation-washing group (N=12) and antibiotics group (N=9). Patients of fumigation-washing group were given fumigation-washing therapy with the self-prescribed Chinese medicine, and antibiotics group was given intravenous dripping of sensitive antibiotics. The treatment lasted for 7 continuous days. The clinical efficiency of the two groups was evaluated after treatment, and the period of preparing for surgery was recorded. The changes of ulcer area and ulcer surface exudate volume in the two groups were observed before and after treatment. And the changes in blood routine examination, hepatic and renal function, and immune function of the fumigation-washing group were monitored. Results(1) The total effective rate of fumigation-washing group was 83.3%, and that of the antibiotics group was 77.8%, the difference being insignificant (P>0.05). (2) The ulcer area and ulcer surface exudate volume in both groups were significantly decreased after treatment (P<0.05 compared with those before treatment), and the decrease in the fumigation-washing group was superior to that of the antibiotics group(P < 0.05).(3) The period of preparing for surgery in the fumigation-washing group was(7.75 ± 1.71)d, shorter than(11.56 ± 4.39) d in the antibiotics group, the difference being significant(P < 0.05).(4) There were no statistically significant differences in the hepatic and renal function, and immunoglobulin and complement levels in the fumigation-washing group before and after treatment (P > 0.05). (5) The average daily hospitalization fee in the fumigation-washing group was (47.98 ± 5.17) yuan, less than (227.88 ± 43.24) yuan in the antibiotics group, and the difference was significant between the two groups (P < 0.05). Conclusion Chinese medicine fumigation-washing therapy is effective, safe and practical in treating talipes equinus denervated ulcer induced by TCS, and the therapy has the advantages of decreasing exudation, promoting ulcer healing, reducing hospitalization fee, shortening the period of preparing for surgery, and decreasing the incidence of infection.

Recently,study on cencer stem cells has been a focus of study.Cancer stem cell is a small population of cencer cells possessing the properties of stem cells:self-renewal,differentiation and proliferation.To date,the existence of cancer stem cells has been proven in acute and chronic myeloid leukemia,breast cancer,brain tumors,liver cancer and colon cancer,etc..In this article we reviews the current progress on cancer stem cells,including the defination,existing evidence,research methods, and challenges in clinical application.
9.Effects of total flavonoids of Herba Epimedii on the metabolism of typeⅠcollagen and the expression of cathepsin K in the bone of ovariectomized rats

Objective To investigate the effects of total flavonoids of Herba Epimedii(HEF)on the metabolism of typeⅠcollagen and the expression of cathepsin K in the bone of ovariectomized(OVX)rats. Methods Fifty-four female SD rats were allocated into 6 groups;OVX group,sham operation group,OVX rats followed by three doses of HEF(40,80 and 160 mg?kg~(-1)?d~(-1))and nilestriol(0.1 mg?kg~(-1)?d~(-1))for 12 weeks respectively.Bone mineral density(BMD)of whole body was determined by dual-energy X-ray absoptiometry.The level of cross-linked N-telopeptide of typeⅠcollagen(NTx)in the urine were determined by ELISA.The amounts of typeⅠcollagen protein and cathepsin K protein in bone tissue were detected by immunohistochemical method and Western blotting.Results Compared with OVX group,the total BMD values in the HEF treated groups were increased(all P<0.05),and the expression levels of typeⅠcollagen in three HEF treated groups rose significantly in a dose-dependent manner after 12 week,and simultaneously,both the expression of cathepsin K in bone and the level of NTx/Cr were reduced markedly(P<0.05),being most significant(P<0.01)in the group treated with the highest dose of HEF(160 mg?kg~(-1)?d~(-1)).Conclusion HEF seems to be able to elevate BMD and improve bone quality of rats via promoting synthesis and inhibiting proteolysis and absorption of typeⅠcollagen in the bone.
10. Clinical observation on effets of high frequency oscillating ventilation on patients with early acute respiratory distress syndrome

Objective To investigate the effect of high frequency oscillating ventilation (HFOV) on patients with early acute respiratory distress syndrome (ARDS) and its mechanism. Methods Through self controlled study, in 25 patients who met criteria of ARDS, their PaO2/FiO2, Paw, arterial blood pH, PaCO2, Cst, cadiac index (CI), heart rate (HR), central venous pressure (CVP), percentage pulmonary shunt (QS/QT), oxygen index (OI) and incidence rate of ventilator associated lung injury (VALI), ventilator associated pneumonia (VAP) were recorded 1, 3, 5 days before and after HFOV. Results Five days later, X ray examination showed that all of the patients got better. Seven days later, 3 patients died because of original diseases, but their symptoms of ARDS improved. Three days after HFOV treatment, the value of PaO2/FiO2 (172.5±69.8) and Cst (63.4±10.5) increased compared with baseline (95.5±29.5, 31.5±4.5, P<0.05, P<0.01), while OI(12.8±8.1) and QS/QT (0.28±0.11) decreased compared with baseline (25.4±13.5, 0.46±0.12, P<0.05). There were not significant difference in Paw, PaCO2, arterial blood pH, CVP, HR, CI and related parameters at different time point after treatment (P>0.05). Conclusion HFOV can improve oxygenation index and pulmonary complications of patients with early ARDS.
