2.Recombinant human brain natriuretic peptide in acute decompensated heart failure

Objective To investigate the clinical efficacy and safety of recombinant human brain natriuretic peptide (rhBNP) in older patients with acute decompensated heart failure. Methods A total of 86 aged patients with acute decompensated heart failure who admitted to our hospital were enrolled. First, they were given recombinant human brain natriuretic peptide (rhBNP) loading doses 1. 5 μg·kg-1,3-5 min, intravenous injection, then continuous intravenous pump 0. 015 kg-1 for 3-5 days. The brain natriuretic peptide(BNP) ,left ventricular ejection fraction(LVEF) and blood urea nitrogen(BUN) ,serum creatinine(SCr) ,urine output/ intake ratio and blood pressure were compared befor and after treatment one week. Result Before and after treatment,BNP was(2387. 50 ± 1030. 99) pg·mL-1 and (783. 57 ± 476. 71) pg·mL-1; LVEF was(40. 85 ± 8. 76) % and(44. 35 ± 0. 94) %; the systolic blood pressure was (135. 21 ± 15. 58) mmHg and(127. 51 ± 16. 24) mmHg; the urine output/ intake ratio was (0. 81 ± 0. 50) and (1. 26 ± 0. 24). Statistically significant differences before and after treatment were found(all P < 0. 05). Conclusion The early application of rhBNP in older patients with acute decompensated heart failure is effective and safe. It can improve cardiac function,and has no adverse effect on renal function,and increases urine output.

4.Fire-needle therapy for deglutition disorders in post-stroke pseudobulbar palsy:a randomized controlled trial

Objective:To observe the clinical efficacy of fire-needle therapy in treating deglutition disorders due to pseudobulbar palsy in the remission stage of stroke.Methods:Sixty-two eligible subjects were divided into a fire-needle group and a rehabilitation group by a simple randomization method at a ratio of 1:1.The two groups received same basic intervention;in addition,the fire-needle group received fire-needle treatment,while the rehabilitation group received rehabilitation training.The two groups of subjects all received a 3-week treatment and were evaluated by the dysphagia severity rating scale (DSRS),modified Mann assessment of swallow ability (MMASA) and Kubota Toshio swallow test (KTST) before and after the intervention.The complications and adverse events occurred during the trial were recorded.The data were statistically analyzed.Results:At the third week,the DSRS,MMASA and KTST scores changed significantly compared with the baseline in both groups (P<0.05),and the changes in the fire-needle group were more significant than those in the rehabilitation group (P<0.05).The between-group comparison at the third week showed that the therapeutic efficacy in the fire-needle group was superior to that in the rehabilitation group (P<0.05).Conclusion:Fire-needle therapy can obviously change the DSRS,MMASA and KTST scores in pseudobulbar palsy in the remission stage of stroke,and significantly enhance the therapeutic efficacy of the treatment of deglutition disorders in this stage.
5.Internet addiction among Shanghai adolescents: prevalence and epidemiological features.

OBJECTIVETo study the prevalence of internet use and internet addiction in adolescents of Shanghai.
METHODFrom September to October 2007, a total of 5135 adolescents from 16 middle schools of 6 districts of Shanghai took part in the epidemiological study by a cross-sectional survey and 5123 adolescents finally enrolled in the study. A questionnaire of "DRM Adolescent Internet Use 52 Scale" was administrated to investigate the prevalence of internet use and internet addiction in adolescents of Shanghai. On this scale of 5-point rating, the scale includes seven parts of contents as tolerance, abstinence reaction, planning ability, self-control ability, whenness, sociability and hazard.
RESULTSThe rate of internet use among the surveyed adolescents was 94.32% (4673/5123), among which the incidence rate of internet addiction was 8.78% (450/5123). Boys, senior middle-school students were more susceptible to internet addiction than girls and junior middle-school students (male scores 122.42+/-43.46, female scores 116.92+/-38.10, F=23.22, P=0.00; senior middle-school students 125.24+/-39.02, junior middle-school students 110.30+/-42.35, F=164.68, P=0.00). Students from vocational senior middle-school were most susceptible (junior middle-school students 110.30+/-42.35, ordinary senior middle-school students 122.83+/-40.99, key senior middle-school students 120.05+/-37.87, vocational senior middle-school students 133.37+/-36.84, F=34.44, P=0.00). The 2nd grade students of senior middle-school have the highest risk for internet-addiction.
CONCLUSIONThe prevalence of internet use and internet addiction in adolescents of Shanghai should be comparatively high in China, which indicates that great attention should be paid to the prevention and control.

7.Effect of immunocyte therapy on benzene-induced bone marrow haemopoietic dysfunction.

OBJECTIVETo explore the effect of treatment with immunocyte therapy on benzene-induced haemopoietic dysfunction.
METHODSMono-nuclear cells (MNC) were separated from 40 - 50 ml peripheral blood in patients and mixed with interleukin-2 and granulocyte macrophage colony stimulating factor (GM-CSF) for six day cultivation. The new formed immunocytes were collected and transfused into the patients. Bone marrow aspiration and biopsy were taken before and after therapy for all patients with severe benzene poisoning. Blood samples were stained by flow cytometry for detecting CD(4) and CD(8) positive cells.
RESULTSOf 20 patients with chronic benzene poisoning, 9 were severe benzene poisoning. All examination including blood count, bone marrow biopsy and T cell subpopulation restored to normal after immunocyte therapy. Laboratory tests (liver and kidney function, and myocardial enzymes) were observed periodically and showed normal during therapy. Follow-up study (the longest time was more than 15 months) showed that bone marrow haemopietic function of all treated patients were in normal range.
CONCLUSIONBone marrow haemopoietic dysfunction caused by benzene poisoning may be closely related to disorder of immune function. Immunocyte therapy may significantly improve bone marrow haemopoietic dysfunction induced by benzene poisoning.

8.Levels of intracellular IL-6 and IFN-gamma in children with acute lymphoblastic leukemia.

OBJECTIVETo study the changes of intracellular interleukin-6 (IL-6) and interferon-gamma (IFN-gamma) expressions in children with acute lymphoblastic leukemia (ALL) at different stages, and to examine the correlation between IL-6 and IFN-gamma in ALL children.
METHODSThe levels of intracellular IL-6 and IFN-gamma in venous blood lymphocytes were detected by flow cytometry in 42 children with ALL at diagnosis and at remission stage. Twenty healthy children were used as the controls.
RESULTSThe intracellular IL-6 level in ALL children at diagnosis was 81.74+/-9.31, which was much higher than that in the Control group (5.67 +/- 0.96 ) (P < 0.01). The intracellular IFN-gamma level in ALL children (1.31 +/- 0.32) was significantly lower than that in the Control group (1.46 +/- 0.49) (P < 0.01). However, the intracellular IL-6 level (27.52 +/- 3.40) decreased remarkably in ALL patients at remission stage (P < 0.01), but was still higher than that in the Control group (P < 0.01). In contrast, the intracellular IFN-gamma level (1.97 +/- 0.72) increased noticeably in ALL patients at remission stage, which was higher than that at diagnosis and the Control group (P < 0.01). A negative correlation was found between the intracellular IL-6 and the IFN-gamma levels in ALL patients (r=-0.476, P < 0.05).
CONCLUSIONSIntracellular IL-6 and IFN-gamma levels may be used as the markers for monitoring the response to treatment in ALL patients. There is a negative correlation between intracellular IL-6 and IFN-gamma levels in ALL children.

9.An etiological survey on a foodborne disease epidemic outbreak caused by Salmonella enteritidis.

OBJECTIVETo conduct an etiological molecular epidemiological survey and laboratory test on a foodborne disease epidemic outbreak to make clear of the cause and implement effective prevention and control on it.
METHODSOn May 12th 2012, 135 kindergarten children were sent to Xuzhou City People's Hospital and Children's Hospital with gastrointestinal infection disease. A total of 34 anus swab samples and 4 vomit samples were collected from the patients. Real-time PCR rapid detection, strains separation and cultivation, phage lysis experiments, ATB automated identification system were used to make etiological detection and identification. The genomic DNA of salmonella enteritidis were typed with the pulsed-field gel electrophoresis (PFGE), cluster analysis were carried out together with the patterns of local Salmonella infections.
RESULTSChildren in 20 classes were suffered from the gastrointestinal infection among the 21 classes. There were no significant aggregation of class distribution. Among the 135 patients, 76 were boys (56.3%) and 59 were girls (43.7%). The main symptoms were fever (above 38°C), diarrhea and bellyache. Through real-time PCR detection and strains separation, 19 salmonella enteritidis were isolated from 34 anus swab samples of suspected cases and the detection rate was 56%. There were no strains detected from vomit samples. All of the 19 salmonella enteritidis showed the same serological subtype, biochemical reaction, drug sensitivity and phage lysis pattern. The salmonella enteritidis had the identical PFGE pattern (100% similarity), and were different from the pattern of local sporadic infection cases.
CONCLUSIONIt was confirmed that this was an epidemic outbreak of foodborne disease caused by homologous salmonella enteritidis by epidemiological survey, clinical information, lab etiological test and molecular typing.

10.Comparison of clinical and MRI features between dilated cardiomyopathy and left ventricular noncompaction

Objective To characterize the clinical and cardiac MRI features of dilated cardiomypathy ( DCM) and left ventricular noncompaction (LVNC). Methods Compared the clinical and MRI features between 25 patients with LVNC and 21 patients with DCM. The MRI derived diastolic left ventricular wall thickness and the number and degree of noncompaction (NC) were evaluated using the 17-segment model. Results Chest distress, shortness of breath and abnormal ECG were presented in all DCM patients, abnormal ECG was evidenced in 22 LVNC patients and 21 out of 25 LVNC patients presented similar clinical symptoms as DCM patients while the rest 4 LVNC patients were asymptomatic. Left atrial and ventricular dimensions were significantly smaller in LVNC patients compared to DCM patients. The degree of left ventricular (LV) spherical remodeling was significantly greater in patients with DCM (sphericity index, SI = 0. 81 ± 0. 06) than in patients with LVNC ( SI = 0. 74 ± 0. 11, P < 0. 05). The LV ejection fraction ( LVEF) was significantly higher in patients with LVNC (32. 7% ± 14. 2% ) than that in patients with DCM (15. 0% ±5. 1% ). The number of NC segments in LVNC patients (9 ± 1) was significantly higher than the number of hypertrabeculation segment in DCM patients (5 ±2). The left ventricular apex (the 17th segment) was unexceptionally involved in all LVNC patients, while hypertrabeculation was absent in the 17th segment of DCM patients. The NC was more common in the apical and mid segments (16th, 12th and 11th segments) than in basal and mid septal segments (2nd, 3rd, 8th and 9th segments) in both LVNC and DCM patients. The thickness of compacted myocardium of the segments associated with noncompaction appeared thin in two groups. The wall thickness of noncompaction myocardium segments was thicker in LVNC patients than in DCM patients. The end-diastolic NC/C ratio was, on average, higher in patients with LVNC (3. 3 ± 0. 6) than in patients with DCM (1.9 ± 0. 3 ) . Conclusions The clinical manifestation is similar while there are significant differences in the morphology and function of left atria and left ventricle between the LVNC and DCM patients. The different distribution and degree of NC were helpful to differentiate LVNC from DCM.
