5.Advances of neonatal hypoglycemia and hypoglycemic brain damage
International Journal of Pediatrics 2013;40(5):477-480
Hypoglycemia is a common and early metabolic disease of neonate.Serious and persistent hypoglycemia could result in brain damage. So far the pathogenesis of the disease is on research, but the definition of hypoglycemia, the glucose threshold of intervention, and diagnostic criteria of brain damage are still unclear. The pathogenesis of hypoglycemia is complicated.Brain damage may be related to the specific excitory amino acids receptors, reduced blood flow in occipital lobe, and cerebral hypoxia and ischemia.Diffusion weighted imaging of brain MRI and magnetic resonance spectroscopy are strongly recommended for the diagnosis of brain damage.Besides, clinical manifestation and cerebral function monitor are also included in the diagnosis. The threshold of intervention is suggested as serum glucose concentration with 2.2 ~ 2.6 mmol/L.
6.Imaging presentation of pancreatic cystic lesions
Chinese Journal of Digestive Surgery 2013;(2):151-155
Pancreatic cystic lesions are being recognized increasingly due to the development of computed tomography and magnetic resonance imaging.Differential diagnosis between pseudocysts and cystic tumors is the key point for pancreatic cystic lesions.Patients with pancreatic pseudocysts generally have a history of acute or chronic pancreatitis, whereas patients with cystic tumors did not have such a history. Some pancreatic cystic lesions did not have typical imaging presentations, and they were difficult to be diagnosed.Pancreatic cystic lesions could be divided into unilocular cysts, microcystic lesions, macrocystic lesions and cysts with a solid component according to the presentations under computed tomography and magnetic resonance imaging. This typing system is helpful for the diagnosis and differential diagnosis of pancreatic cystic tumors,as well as for the guidance of therapy.

AIM: To observe the effect of nonpenetrating trabeculectomy with implantation of amniotic membrane on the treatment of open-angle glaucoma.METHODS: Sixty-six cases (87 eyes) were randomly paired into 3 groups, treated with nonpenetrating trabeculectomy with implantation of amniotic membrane (22 cases, 29 eyes),compound trabeculectomy (22 cases, 29 eyes) and nonpenetrating trabeculectomy with implantation of SKGel (22 cases, 29 eyes), respectively.RESULTS: Compared with the other 2 groups, the group of nonpenetrating trabeculectomy with implantation of amniotic membrane got a better postoperative visual acuity (P<0.01),satisfactively-controlled intraocular pressure (IOP) (P<0.01)and fewer complications (P<0.01).CONCLUSION: Nonpenetrating trabeculectomy with implantation of amniotic membrane is an effective method for the treatment of open-angle glaucoma.
8.Comparison of clinical efficacy between multifocal and monofocal intraocular lens implantation for the treatment of cataract
Chinese Journal of Experimental Ophthalmology 2013;31(10):973-977
Background Multifocal intraocular lens (IOL) can produce two focus distance based on the principle of diffraction-refraction,and the full range of vision is better than monofocal IOL.So multifocal IOL can provide cataract patients better far, middle and near distant vision. Objective This study was to investigate and compare the clinical efficacy between multifocal IOL and monofocal IOL implantation in the patients after cataract surgery. Methods One hundred eyes of 100 patients with cataract enrolled in Gaomi People's Hospital from June 2010 to September 2012 and were divided into the monofocal IOL group and multifocal IOL group, with the matched demographic and medical baseline characteristics between the two groups. All the patients were aware of the trial and a written informed consent was obtained from each patient prior to the surgery.Multifocal IOL (Acrysof ReSTOR SA60D3) was implanted in the multifocal IOL group, and monofocal IOL was implanted in the monofocal IOL group after phacoemulsification.Uncorrected distant visual acuity (UCDVA),best corrected distant visual acuity (BCDVA),uncorrected near visual acuity (UCNVA),best corrected near visual acuity (BCNVA),uncorrected middle distance visual acuity (UCMVA) and distant-corrected near visual acuity (DCNVA) between the two groups were observed and compared, and near stereo acuity,IOL higher-order aberrations under the pupillary diameters of 3 mm and 5 mm,optimum reading distance and reading speed were compared 3 months after surgery. The satisfaction rates of patients for each visual function district were analyzed by a questionnaire survey. Results UCNVA and DCNVA in the multifocal IOL group were significantly better than those of the monofocal IOL group 3 months after surgery, and the differences were statistically significant (t=15.48,22.44,both at P<0.05).UCMVA at the distance of 25,30,40,50,60 cm in the multifocal IOL group were better than ones of the single IOL group (t=12.60,16.75,23.18,14.04,7.60,all at P<0.05).Uncorrected near stereo acuity in the multifocal IOL group was (49.1 ±15.2),showing a significant reduction in comparison with (85.3 ± 18.6) of the multifocal IOL group (t=10.66,P<0.05).The total higher-order aberrations, spherical aberration and coma under the pupil diameter of 3 mm and 5 mm were not significantly different between the two groups (all at P>0.05).The optimal reading distance in the multifocal IOL group was (33.5 ± 4.1) cm,and was significantly shorter than (46.3 ± 5.2) cm of the monofocal IOL (t=13.67,P<0.05).The reading speed in the multifocal IOL group was (172.5 ± 52.3) words/minute,and that in the monofocal IOL group was (108.3 ± 31.4) words/minute,with the significant difference between them (t =7.44,P<0.05).The patients who were satisfied with the functional areas of near vision and middle-distance vision were more in the multifocal IOL group than those of the monofocal IOL group (x2=56.59,40.85,both at P<0.05),but those in the distance vision zone between the two groups was no significant different (x2=3.09,P>0.05).Conclusions Comparison with the monofocal IOL,multifocal IOL implantation for the treatment of cataract can obtain clear imaging both at the far-distance and middle-and near-distance.Multifocal IOL implantation can improve the life quality of cataract patients after operation.
9.Speech Evaluation for Apraxia of Speech with Chinese
Chinese Journal of Rehabilitation Theory and Practice 2013;19(1):70-71
Objective To explore the evaluation for apraxia of speech (AOS) with Chinese. Methods 20 Chinese cases with AOS were assessed with Chinese material referred from Motor Speech Evaluation (MSE) using. Results The subtest of multiple repetitions of multisyllabic words was the most difficult and single repetitions of monosyllabic words was the easiest for AOS patients (P<0.05). The score of the short sentences negatively correlated with both the scores of repetition ability (r=-0.865) and the fluency of speech (r=-0.614) (P<0.001).Conclusion Chinese material referred from MSE can be used for evaluation of AOS with Chinese.
10.A morphological study of the pelvic floor muscle fibers of rats with spinal cord injury
Chinese Journal of Physical Medicine and Rehabilitation 2021;43(4):295-300
Objective:To seek better treatments for abnormal pelvic floor muscle tension and pelvic floor dysfunction after spinal cord injury. Methods:The morphology of pelvic floor muscle fibers of rats with spinal cord injury at different levels was observed under the electron microscope. Thirty female adult Sprague-Dawley rats were randomly divided into a suprasacral (SS) cord injury group, a group with spinal cord injury at or below the sacral level (SC) and a normal group (NG), each of 10. The relevant spinal cord injury models were established in the SS and SC groups through spinal cord disconnection. Four weeks later, the pixel area of ATPase-positive fibers was used to quantify the content of type I fibers in the pubococcygeus muscle of each rat through observation under the electron microscope after hematoxylin and eosin staining. Results:The average content of type I muscle fibers in both the SS and SC group was significantly lower than in the normal group. The SC group′s average level was significantly lower than that of the SS group. Under the microscope the stained myofibers were tortuous, deformed in appearance and with proliferated nuclei. Capillary dilation could be seen locally in the SS group 4 weeks after the injury. In the SC group at 4 weeks after the injury the pubococcal fibers were seriously "dissolved" , or disordered, with spherical nuclei and mild hyperplasia. Under the electron microscope, the sarcomeres of the SC group were obviously dissolved, atrophied and broken, though the basic structure persisted, with mild mitochondrial proliferation. The sarcomeres of the SC group were extremely dissolved and broken, completely losing basic structure, with abundant connective tissue proliferation but without obvious mitochondrial proliferation. Conclusions:After suprasacral cord injury, the content of type I muscle fibers in the pubococcygeus muscle of the pelvic floor decreases somewhat, with the basic structure of the muscle fibers remaining intact. However, after spinal cord injury at or below the sacral level, type I muscle fibers decrease significantly in the pubococcygeus muscle of the pelvic floor, and the basic structure is seriously damaged.
