2.The design of high-efficiency exhalation disinfection device for the patient infected with SARS

High-efficiency exhalation disinfection device for the patient infected with SARS can also meet the disinfection requirements of the patients with severe respiratory communicable diseases. Connected with the exhaust vent of the breathing mask or the respirator by corrugated tubes, this device gathers exhalation containing a large amount of causative agents through the completely leakproof ductwork and then expels purified air after multiplicate disinfection by high temperature, high pressure and chemical disinfectant. The device can prevent the patient with severe respiratory communicable disease from polluting their surrounding air and thus cut off the route of transmission, reduce the infection rate and the death rate of the person and medical personnel who has made intimmate contact with the patient.
3.Protective Effects of Bosentan on Kidney in Rats with Unilateral Ureteral Obstruction

Objective To research the effects of bosentan in on unilateral ureteral obstruction (UUO) rats. Methods Wistar rats were randomly divided into three groups. UUO group was subjected to complete ligation of left ureter, sham operation group with sham operation was used as control. Bosenten group was subjected to complete ligation of left ureter and to stomach with bosentan suspension. Three groups were killed at 5,10,15,20 d. We examined renal pathological changes and the expressions of transforming growth factor-?1 (TGF-?1) in kidney of all rats using immunohistochemistry analisis method. The plasma and ureteral obstructive kidney urinary concentration of endothelin - 1 (ET - 1) was determined by radioimmunoassay. Results In Bos and UUO groups, the urinary concentration of ET - 1 increased substantially and the expression of TGF - ?1 were much higher than those in the S group. At the same time.tubulointerstitial fibrosis was present in the kidney of rats in Bos and UUO group. But the expression of TGF- ?1 and tubu-lointerstitial fibrosis in Bos group were lower than those of UUO group. Conclusion Bosentan can effectively amelirate tubulointerstitial fibrosis in rats with UUO by ET and TGF - ?1 machanism.
4.Expression of Endothelin in Kidneys of Rats with Unilateral Ureteral Obstruction

Objective To determine the expression of endothelin in kidneys of rats with unilateral ureteral obstruction (UUO). Methods Rats were evenly divided into two groups. Some were subjected to complete ligation of left ureter and the others with sham operation were used as control group.Four weeks later, we examined the mRNA expressions of ET-1 in kidneys of all these rats using reverse transcription-polymerase chain reaction (RT-PCR) method.The immunohistochemistry analysis was carried out to investigate the protein level of ET-1. The urinary concentration of ET-1 was determined by radioimmunoassay.Results In UUO group, the urinary concentration of ET-1 increased substantially;both gene expression and protein level of ET-1 were much higher than those in control group. At the same time, tubulointerstitial fibrosis was present in the kidneys of rats with UUO. Conclusion ET-1 is involved in the pathogenesis of the injury of UUO.
5.The changes of main upper gastrointestinal diseases and Helicobacter pylori infection status in the past thirty five years

Objective To study the spectrum of upper gastrointestinal diseases and infection rate of Helicobacter pylori(Hp) in our hospital during the past 35 years.Methods Patients who were 16 or older with duodenal ulcer,gastric ulcer,reflux esophagitis,gastric cancer and esophageal cancer diagnosed by gastroscopy and pathology were retrospectively enrolled in our study from January 1980 to December 2014.Patients with chronic superficial gastritis,chronic atrophic gastritis or Hp infection from January 1989to December 2014 were also included in our study according to the same diagnostic criteria.The incidences of diseases and the infection rates of Hp were studied.Results A total of 213 495 patients underwent gastroscopy in our department during the past 35 years.The overall diagnostic rates of duodenal ulcer,gastric ulcer,reflux esophagitis,gastric cancer and esophageal cancer were 9.87%,3.79%,6.66%,1.59% and 0.66% respectively.There were 183 426 patients receiving gastroscopy in our department from January 1989to December 2014.The overall endoscopic diagnosis rates of chronic superficial gastritis and chronic atrophic gastritis were 49.83% and 22.43% respectively.The overall infection rate of Hp was 36.18%,which had a declining trend consistent with peptic ulcer (all P =0.000).Yet,the prevalence of reflux esophagitis,chronic superficial gastritis and chronic atrophic gastritis were increasing (all P =0.000).The diagnostic rates of gastric cancer and esophageal cancer were persistent (P =0.266,P =0.156).Conclusions The Hp infection during years has been decreasing,consistent with the declining tendency of peptic ulcer.On the other hand,reflux esophagitis,chronic superficial gastritis and chronic atrophic gastritis show an ascendant trend.The proportion of patients with gastric cancer and esophageal cancer is relatively stable.
6.A comparative study using the transabdominal route versus the transoral route in establishing nasobiliary drainage in laparoscopic surgery

Objective To compare using the transabdominal route versus the transoral route in establishing naso-biliary drainage in laparoscopic surgery.Methods The combined use of laparoscopy with choledochoscopy and duodenoscopy to establish naso-biliary drainage was carried out in 204 patients with gallbladder and common bile duct calculi.In 162 patients,the naso-biliary drainage was established transabdominally and in 42 patients it was established transorally.The success and the complication rates in the two groups were compared.Results Of 162 patients using the transabdominal route,4 patients failed.There were 6 patients (3.7%) who had no output from the nasobiliary drain.There were 3 patients (1.8%) who had only intestinal juice outflow from the nasobiliary drain.Primary closure failed in 3 patients (1.8%),all resulting in bile leak.Pancreatitis occurred in 2 patients (1.2%) after the operation.There was 1 patient (0.6%) whose nasobiliary drain was wrongly ligated.Of 42 patients with nasobiliary drainage using the transoral route,6 patients failed.There was 1 patient (2.4%) who had no output from the nasobiliary drain.There was 1 patient (2.4%) who had intestinal juice output from the nasobiliary drain.Primary closure failed in 1 patient (2.4%) with resultant bile leakage.Pancreatitis occurred in 4 patients (9.5%) after the operation.The success rate of establishing a nasobiliary drainage in the transabdominal group was significantly higher than that in the transoral group,but the complications were less.Conclusions Nasobiliary drainage established through the transabdominal route in laparoscopy surgery for patients with gallbladder and common bile duct calculi was technically easier and had a high success rate.It had less complications.

Objective To study the feasibility of TNF-α promoting migration of rat mesenchymal stem cells (MSCs) to local damaged tissues. Methods The MSCs was exposed to TNF-α at different concentrations and the expression rate of surface adheslon molecules and specific markers as well as their adhesion to endothelial cells were detected.Based on the above steps,the MSCs stimulated with the optimal concentration of TNF-α were obtained and were injected intravenously to the rats whose hindlimbs experienced ischemia damage.The rats were executed for achieving the muscle samples in the ischemic area,which were made into frozen section to count the number of MSCs. Results ( 1 ) Twenty-four hours after the TNF-o stimulation,the expression of adhesion molecule (VCAM-1) of MSCs increased in a concentration-dependent manner,while the expression of adhesion molecules (ICAM-1,L-Selectin and VLA4) of MSCs showed no significant changes.Besides,the expression rate of specific markers of MSCs was also obscure.(2) Exposed to 10 ng/ml TNF-o,MSCs presented an obviously increased ability in adhesion to the endothelial cells.(3) MSCs stimulated with 10 ng/ml TNF-α showed a larger number in the ischemia-damaged tissue of rat hindlimbs than that in the control group. Conclusion TNF-α at concentration of 10 ng/ml is effective within a short term in increasing VCAM-1 expression in rat MSCs and promoting the adhesion of MSCs to endothelial cells without affecting their character.
9. Relationship between protein-energy wasting and mineral metabolism disorders in maintenance hemodialysis patients

Protein-energy wasting(PEW)and chronic kidney disease-mineral and bone disorder(CKD-MBD)are common complications in maintenance hemodialysis(MHD)patients, and they have notable correlation with the mortality of MHD patients. PEW and CKD-MBD have a common pathophysiological basis and are mutually reinforcing factors. At the same time, their therapy strategies also affect each other. Uremia status, hemodialysis treatment, chronic inflammatory, metabolic acidosis, nutrient intake and musculoskeletal pain promote the development of PEW and MBD. Improvement in the hemodialysis quality, keeping balanced diet of protein and phosphorus intake, and scientific rational physical exercise may improve both PEW and MBD, thereby reducing mortality and improving life quality in MHD patients.

A novel method was established for the qualitative and quantitative determination of fatty acids in Channel Catfish muscle by gas chromatography-electron ionization-mass spectrometry (GC-EI-MS) after supercritical carbon dioxide fluid extraction (SFE-CO_2). The extraction parameters for the methodology were optimized). The optimal conditions were extraction pressure of 25 MPa at 45 ℃ and extraction time of 100 min at the rate of carbon dioxide 30 L/h. The fatty acids in the muscle oil were derived by boron-trifluoride method). The saponification time was 10 min, and the esterication time was 20 min. The obtained fatty acid methylesters were separated by gas chromatography using a HP-Innowax capillary column, and were detected by electron) ionization) mass spectrometry. Full scan mode and SIM mode were used for the qualitative and quantitative analysis), respectively. In the SIM mode, saturated fatty acids were determined with m/z 74, mono-unsaturated) fatty acids were determined with m/z 55, double-unsaturated fatty acids were determined with m/z 67, and polyunsaturated fatty acids were determined with m/z 79. The detection limits of 14 fatty acids were 2.2-20.0 μg/L(S/N=3)), and the quantitative limits were 7.39-59.85 μg/L(S/N=10). The recoveries fell in the range from 90.0% to 111.2%(n=4), and the relative standard deviation was between) 2.0% and 5.9%. This effective, sensitive and reproducible method can be used for the determination of fatty acids in Channel Catfish muscle sample.
