4.Efficacy and safety of Bevacizumab versus MMC in augmenting trabeculectomy:a Meta-analysis

AIM: To compare the efficacy and safety of bevacizumab with mitomycin ( MMC ) in augmenting trabeculectomy for glaucoma.
METHODS: Databases including PubMed, CBM, CNKI, VIP and WanFang Data were electronically searched for all randomized controlled trials ( RCTs) about comparing the efficacy and safety between bevacizumab and MMC in augmenting trabeculectomy for glaucoma before the date of Jun. 2016. Two reviewers independently screened literatures according to the inclusion and exclusion criteria, and evaluated the included studies. Then, Meta-analysis was performed.
RESULTS: A total 4 RCT involving 286 eyes ( 143 for bevacizumab group, 143 for MMC group) were included. The results of Meta-analysis showed that there was no significant difference between bevacizumab and MMC in the last follow-up after surgery in IOP (WMD=2. 21, 95%CI: -0.17 to 4.58, P=0.07), complete success rate (OR=0. 69, 95%CI:0. 26 to 1. 81, P=0. 45) and the numbers of anti-glaucoma medicine ( OR= 0. 12, 95%CI: -0. 15 to 0.39,P=0. 39). And there was no significant difference between bevacizumab and MMC in postoperative complications:hypotony (OR=0.7, 95%CI:0.12 to 4.05, P=0.69), bleb leak (OR=1, 95%CI: 0. 21 to 4. 74,P=1), encapsulated bleb (OR=1. 15, 95%CI: 0. 38 to 3. 44, P=0.81), choroidal detachment (OR=1. 22, 95%CI: 0. 29 to 5.22, P=0. 78) and cataract (OR=1. 15, 95%CI: 0. 38 to 3.44, P=0. 81).
CONCLUSION: Bevacizumab and MMC in augmenting trabeculectomy for glaucoma have similar efficacy and safety. Bevacizumab can't result in better outcome in term of IOP reduction. Clinicians should choose suitable solution according to disease characteristics.
5.Effects of natural moxibustion on cytokines and specific transcription factors in asthma rats.

OBJECTIVETo explore the mechanism of natural moxibustion on regulating immune imbalance in asthma rats.
METHODSSeventy SD male rats were divided into a normal group, a placebo group, a dexamethasone group, a big-cake for long-course moxibustion group, a big-cake for short-course moxibustion group, a small-cake for long-course moxibustion group and a small-cake for short-course moxibustion group, ten rats in each one. The rat model of asthma was established by egg albumen sensitization and stimulation in all the groups except the normal group. The natural moxibustion was used in all moxibustion groups, in which big cake of 1 cmX 1 cm size was used in the big-cake groups and small cake of 0.5 cmX 0. 5 cm size was used in the small cake groups. According to relevant acupoints, the natural moxibustion was performed, 5 h per time, once a day. Four times of treatment was considered one course, and three courses were required in the long-course groups and one course was required in the short-course groups. Intraperitoneal injection of dexamethasone was applied in the dexamethasone group, which had the same course as long-course moxibustion group. After the treatment, changes of EOS in peripheral blood of asthma rats were observed; enzyme linked immunosorbent assay (ELISA) was applied to test the contents of IgE, IL-4 and IFN-gamma in the lung tissue; real-time Q-PCR method was adopted to measure the expression level of transcription factor T-bet and GATA binding protein 3 (GATA-3) in the lung tissue.
RESULTSCompared with the normal group, the EOS in whole blood as well as IL-4 and IgE in plasma were all increased in the placebo group (all P< 0. 01), IFN-gamma in plasma was obviously decreased (P<0. 01); while the levels of EOS, IgE and IL-4 were significantly reduced (all P<0. 01), the content of IFN-gamma was increased (P<0. 01) in all moxibustion groups and dexametnasone group. Compared with the normal group, the expression of T-bet mRNA in the placebo group was significantly reduced (P<0. 01). Each treatment group could significantly increase the expression of T-bet mRNA and reduce that of GATA-3 mRNA (P<0. 01). Compared with the short-course moxibustion groups, the expression of T-bet mRNA was obviously increased in the long-course moxibustion group and dexamethasone group (both P<0.01), and that of GATA-3 mRNA was reduced (P<0.01). There was no significant difference between long-course moxibustion group and dexamethasone group (P> 0. 05), and also no significant difference could be seen between big-cake moxibustion group and small-cake moxibustion group (P>0. 05).
CONCLUSIONThe natural moxibustion could obviously reduce airway inflammation in asthma rats. With time passing, the efficacy is enhanced, indicating evident timeliness, which has no apparent relationship with the size of moxibustion cake.

6.Correlation between integrated pharmacokinetics and pharmacodynamics of bile processed Rhizoma Coptidis in febrile rats.

This study was designed to validate the correlation between integrated pharmacokinetic and therapeutic effects of alkaloids using bile processed Rhizoma Coptidis (BRC). Rats were divided into three groups: normal, disease model, model+BRC. Rats were induced to have an excessive heat syndrome. Rectal temperatures were collected at 0, 3, 6 and 9 h after single oral administration of the drugs. The plasma concentrations of three alkaloids were quantified at different times by UPLC-MS/MS after the administration of BRC. An approach of self-defined weighting coefficiency was created to the holistic pharmacokinetic profiles of alkaloids in BRC. The classified and integrated synthetic concentrations were obtained, and then the pharmacokinetic parameters of alkaloids were calculated from non-compartmental model analysis. The potential relationship between the integrated mean concentration of alkaloids and the antifebrile efficacy was investigated. The holistic t(max) of alkaloids was 1.11 h, the antifebrile effect of BRC at 3 h was improved over the model group. Double peaking appeared in the integrated blood concentration-time curve, the second t(max) of alkaloids was 4.82 h. The antifebrile effects of BRC at 3-6 h were significant, and the antifebrile effects at 6-9 h was decreased significantly. Dynamic variation of alkaloids of BRC in the body exhibited the similarity to the pattern of its antifebrile effect.

7.MRI features of urethral sphincter changes in female with stress urinary incontinence

Objective To investigate the MRI features of urethral sphincter in female with stress urinary incontinence(SUI) by the study of the indexes of the thickness of middle urethral sphincter, the function urethral length and bladder neck funnel . Methods Forty patients of female patients diagnosed as SUI by clinical were retrospectively as SUI group, and 40 asymptomatic female volunteers were recruited as reference group. All of the subjects were tested by pelvic MRI scanning both in static status and in the condition of Valsalva. The thickness of middle urinary sphincter was measured in the horizontal axis plane. In the midsagittal plane, the function urethral length was measured from the internal orifice to perineal fascia. The bladder neck funnel whether exist was observed from the midsagittal plane. For the comparison of the urinary sphincter thickness and urethral length of the two groups, independent sample t test was adopted. For urethral length in static status and under the condition of Valsalva, paired t test was done. For comparison of positive rate of the bladder neck funnel, Chi?squared test has been done. Results The value of the middle urinary sphincter thickness in the groups of SUI and reference were (2.23±0.68) mm and (2.69± 0.75) mm, respectively. Thus the differences of the two groups had statistical significance (t=-2.839, P<0.01 ). In the static status, the urethral length in SUI and reference group were (2.72±0.51) cm and (2.94± 0.34) cm respectively, wheras in Valsalva condition, the value were (2.33 ± 0.49) cm and (2.43 ± 0.43) cm respectively. The differences of the two groups had statistical significance in static status (t=-2.322, P=0.023), wheras there had no statistical significance in the two groups in Valsalva condition (t=-1.049,P=0.297). For SUI and reference group, both in static status and Valsalva condition, the difference of function urethral length had statistical significance (P< 0.01). The positive rate of bladder neck funnel for SUI was 72.5% (29/40), and for reference was 22.5% (9/40), the difference of the two groups had statistical significance(χ2=20.050, P<0.01). Conclusion For female SUI patients, urinary sphincter muscle is much thinner, function urethral length is much shorter and has higher positive rate of bladder neck funnel.
8.Preparation Optimization for TCM Pastes (Tonic Semifluid Extract of Ten Ingredients) by Response Sur-face Method

Objective:To screen the best decocting technology by multi-index comprehensive score combined with response sur-face. Methods:The content of paeoniflroin and the ratio of dry extraction were used as the indicators, and response surface Central Composite with two factors and five levels was used in the experimental design. The effects of the ratio of solvent to solid, decocting du-ration and decocting times on the preparation process of tonic semifluid extract of ten ingredients were studied. Results:Design Expert 8. 0. 6b software was used for the data analysis. A quadratic mathematical model between the factors and the comprehensive score of paeoniflroin content and the ratio of dry extraction was established. Combined with the actual production, the best decocting conditions were as following:the ratio of solvent to solid was 10. 6 ml·g-1 , the decocting duration was 96 min, and decocted 3 times. Conclu-sion:The optimal decocting technology is scientific, reasonable and stable.
9.Occupational Burnout and Related Factors in Rehabilitation Therapists

Objective To investigate the occupational burnout status of rehabilitation therapists in medical institutions and analyze relat-ed factors. Methods From December, 2015 to February, 2016, a total of 264 rehabilitation therapists, selected with random cluster sampling method, were investigated with Maslach Burnout Inventory-General Survey (MBI-GS) in Bengbu, China. Results The detection rate of ther-apist burnout was 81.1%, and the average score was (3.13±1.07). There were significant differences in the score of occupational burnout and its three dimensions (P<0.05), except emotional exhaustion dimension among different ages (P>0.05). Multiple regression analysis showed that the gender, age, marital status, educational background, professional title, shift work, job demand, job autonomy, social support, de-mand-autonomy ratio and income levels were the factors related with occupational burnout. Conclusion Rehabilitation therapists generally feel occupational burnout. It is different with demographic characteristics and working conditions.
