Objective To evaluate the technique of Kirschner with a hole and tensile developed from the Kirschner tensile band wire.Methods Forty-two cases with fracture of patella and 23 cases with fracture of ulna olecranon were successfully operated with this apparatus.Average age was 42 years old(range,24～78 years),including 38 male patients and 27 females totally.Results Fracture healing rate was 100%,the rate of excellent and good result was 95% after 9 months to 3 years follow-up.No tenderness,skin penetrating caused by the bolt ends,and loosen fixations were found.Conclusion This apparatus can be considered as a rigid,safe and effective method and can meet the biomechanics principle of the Kirschner tensile band wire,complication of the Kirschner tensile band wire could be avoided by using this technique.

Objective To investigate the curative effect and the feasibility of treating femoral neck fractures with cannulated screw fixation.Methods Twenty-three patients with femoral neck fractures were treated with cannulated screw fixation,their average age was 51 years old(range,12～86 years),including 7 male patients and 16 females totally.Garden Ⅱ 2 cases,Garden Ⅱ 6 cases,Garden Ⅲ 12 cases,Garden Ⅳ 3 cases.A patient who suffered from fractures displacement needed traction for 3～7 days before surgical operation and the opening reduction was necessary if reduction was not satisfactory.Results During the follow-up 11～33 months the rate of nonunion was 9%(2 cases) and the rate of avascular necrosis was 13%(3 cases).Conclusion Trectin femoral neck fracture with cannulated screw fixating is very simple and the trauma is small.This may improve the cure rate of the femoral neck fractures and it is easy to be spreaded across the basic-level hospital.

Objective To investigate the infection of human papilloma virus(HPV) and expression of serum squamous cell antigen (SCCAg) in the follow - up of 120 cervical squamous carcinoma patients who had received operation or operation combined with radiotherapy or only radiotherapy. Methods 120 cases of therapical cervical squamous carcinoma patients were detected HPV - DNA by HC - Ⅱ and serum SCCAg using immunohistochemical method. Results The positive rate of HPV - DNA and serum SCCAg was 49.17% and 17.50% , with a significant difference between them( P 0.05 ). Conclusion HPV - DNA test with HC - Ⅱ for follow - up of cervical squamous carcinoma patients was feasible. It was more sensitive than serum SCCAg. But it suggested that high risk type HPV -DNA test combined with serum SCCAg may be the independent prognostic factors.

BACKGROUND: It is proposed that elevated serum homocysteine is an important independent risk factor for ischemic stroke (IS), and 5, 10-methylenetetrahydrofolate reductase (MTHFR) is the key enzyme for homocysteine metabolism. The relationship between genetic mutation of MTHFR and IS remains controversial.OBJECTIVE: To examine the association of hyperhomocysteinemia and two MTHFR gene polymorphisms with IS in Northwest Chinese population.DESIGN: Case-control study.SETTING: Department of Neurology, First Hospital Affiliated to Jilin University, and Department of Neurology, Xijing Hospital, Fourth Military Medical University of Chinese PLA.PARTICIPANTS: Ninety-seven consecutive patients with ischemic stroke (71 males and 26 females) treated between November 2001 and May 2002were recruited, who were diagnosed by CT scan or MRI in the Department of Neurology, Xijing Hospital, Fourth Military Medical University of Chinese PLA. The control group consisted of 94 subjects (58 males and 36 females) without history of ischemic stroke. All the subjects were free of intracranial hemorrhage, cancer, renal dysfunction, and none used multivitamins or estrogen.METHODS: Serum homocysteine was measured by fluorescence polarization immunoassay. Polymerase chain reaction-restriction length polymorphism (PCR-RFLP) method was employed to detect the genotype at the two sites of C677T and A1298C in MTHFR gene.MAIN OUTCOME MEASURES: Serum homocysteine levels and the genotypic frequency frequencies of the two mutations of MTHFR.RESULTS: The 677T allele frequency was 59.3% in IS patients and 44.7% in the controls, showing significant differences (P=0.006), but no difference in 1298C allele frequency was detected between the two groups (22.7% vs 19.7%, P ＞ 0.05). Homozygous 677TT genotype was closely associated with hyperhomocysteinemie (P ＜ 0.01). In multivariate logistic regression analysis,677T gene mutation and hyperhomocysteinemie were all associated with the IS, with an OR of 1.870 and 1.031 (P＜ 0.05), respectively.CONCLUSION: Hyperhomocysteinemie is a risk factor of IS, and C677T mutation significantly increases homocysteine levels, and serves also as an independent genetic risk factor of IS.

Syndrome can be divided into 4 types: qi deficiency with phlegm resistance, yin deficiency with toxic heat, qi stagnation with blood stasis, and lung and kidney deficiency. Clinical treatment should pay attention to the combination of disease and syndrome, which can achieve satisfactory efficacy.

Objective There is no enzyme multiplied immunoassay technique ( EMIT) reagent for therapeutic drug monito-ring ( TDM) in our country.The aim of this study was to compare the properties of self-developed kits and commercially imported kits in TDM, and to evaluate their feasibilities for routine TDM. Methods The sensitivities, accuracies and precisions of self-developed kits, Viva-E kit and AXSYM kits were evaluated by determining the quality control samples of carbamazepine, valproic acid and phe-nobarbital.Self-developed kits, Viva-E kit and AXSYM kits were employed to determine the concentrations of clinical serum samples of 83 cases of carbamazepine, 80 cases of valproic acid and 72 cases of phenobarbital separately, and the results were compared and analyzed. Results Recoveries of carbamazepine, valproic acid and phenobarbital were more than 90% for the three different kits. The recoveries of self-developed kits for the three drugs were in the range of 98.7%-103.9% and the limits of quantification of the self-developed kits for carbamazepine, valproic acid and phenobarbital were 2, 10, 5μg/mL, respectively.Precision was lower than 10%and its average relative deviation was less than 3%after single point correction.There were good correlations (R2>0.985) and no significant statistical difference between self-developed kits and AXSYM kits (P>0.05).Upper and lower 95%limits of agreement in Bland-Altman plots were (-1.32,1.26), (-15.24,15.17) and ( -3.69,3.00) for carbamazepine, valproic acid and phenobar-bital, respectively.About 5%of the points failed in the 95%confidence interval. Conclusion The self-developed kits showed good performance and are suitable for clinical use in TDM.
