AIM: To observe the effect of tranilast on experimental COPD rats in terms of airway remodeling. METHODS: Forty eight SD rats were divided into two groups in random: untreated model group, tranilast-treated group. Another eight rats were selected as control group. The COPD rat model was established by passive smoking and intratracheal instillation of lipopolysaccharide (LPS) and then treated with sterile saline or tranilast (400 mg?kg~ -1 ?d~ -1 ) respectively. Eight rats in each group were killed in 7th, 14th, 28th day after the beginning of proceeding. Bronchoalveolar lavage fluid (BALF) was collected, and the total and differential cells were counted. The distribution and the ratio of type I to type III collagen in the lung tissue were determined using a sirius red polarizing microscopy morphometry method. Lung tissues were observed by hemotoxylin and eosin stain, then the image analysis were made. Objective To investigate the therapeutic effects of traditional Chinese medicinal herbs(TCMH) on benign prostate hyperplasia(BPH) and their influence on proliferation and apoptosis of prostate cells in experimental mice. Methods A total of 120 male Kunming mice(weight,30 to 40 g)were randomly divided into 5 groups:①normal control group;②hyperplastic group;③normal saline(NS)(negative control)group;④TCMH group;⑤finasteride (positive control) group.Each group included 24 mice.The mice in normal control group were killed and their prostates were weighed.The BPH model was induced by implanting the urogenital sinus in the mice in other 4 groups.In the 61st day,the prostate weight of the mice in hyperplastic group was examined after they were killed;and the mice in the remaining groups were fed with NS,TCMH and finasteride,respectively.In the 91st day,the mice in these 3 groups were killed and their prostates were weighed.The proliferation and apoptosis of the prostate cells of mice in the 3 groups were determined by flow cytometry(FCM). Results The prostates in hyperplastic group [(149.30?8.46)mg] were heavier than those in TCMH group [(85.60?17.97)mg] ( P 0.05). Conclusions The TCMH has a significant therapeutic effects on experimental mice with BPH.The mechanisms may be as follows:TCMH can regulate the relevant genes of the prostate cells so that they can promote the apoptosis of the prostate cells and as a result,the volume and weight of the prostate of the mice are reduced.

[Objective]To verify whether or not the modified open-door laminoplasy preserving semispinalis cervicis insert into axis is effective in preventing postoperative axial symptom and maintaining postoperative cervical sagittal alignment.[Method]Since March 2002 to December 2003,forty-eight patients underwent modified open-door laminoplasy preserving semispinalis cervicis insert into Axis,among them,thirty-two patients who were followed up for more than 2 years were included in this study.Preoperative and postoperative JOA score,degree of axial symptom,ranges of neck motion,cervical curvature index were recorded and compared.[Result]Preoperative and postoperative JOA score was(9.04?2.9)and(12.5?3.1)respectively,with recovery rate as(44.9?26.9).There was significant difference between preoperative evident axil symptom rate(46.8%)and postoperative evident axil symptom rate(18.7%),while no significant difference between preoperative and postoperative ranges of neck motion,cervical curvature index was seen.[Conclusion]Modified open-door laminoplasy is less invasive to the posterior extensor mechanism especially the semispinalis cervicis than conventional open-door laminoplasy.This new procedure is effective in preventing postoperative morbidities often seen after conventional laminoplasy with adequate decompression of the spinal cord.

Objective To verify whether the modified open-door laminoplasty with C3 laminectomy is effectve in preventing postoperative axial symptom. Methods Since March 2001 to March 2005, 44 patients who underwent modified open-door laminoplasty with semispinalis cervicis insertion intact were included in this study(group A). 50 patients who underwent conventional open-door laminoplasty in the authors' department during the same period were served as control(group B). Preoperative and postoperative Japanese Orthopedic Association(JOA) scores, the degree of axial symptom, ranges of neck motion, cervical curvature index were recorded and compared. Results The average JOA recovery rate was 59.2%?11.3% for group A and 60.1%?19.5% for group B. There was no significant difference in JOA recovery rate between two groups. [Objective]To verify the clinical outcome of cervical arthroplasty in the treatment of spondylotlc myelopathy exclusively.[Method]From January 2004 to August 2003,twelve spondylotic myelopathy patients who underwent cervical arthroplasty were included in this study.Preoperative and final follow-up Nuriek grade,Oswestry neck disability index(ONDI)score,neck and arm pain VAS score of patients were recorded and compared.Surgical outcome was also measured using odom' s criteria.[Result]100% of patients had good or excellent outcomes using Odom' s criteria,while the final follow-up Nurick grade,ONDI score,neck and arm pain VAS score were improved significantly compared with the preoperative valule.[Conclusion]Cervical arthroplasty is effective in the treatment of spinal cord compression secondary to spondylotic disease with interveterbral movement being preserved.

Bile acid can not only emulsify fat and facilitate the absorption of fat soluble substances,but also it is an important signaling molecules that plays an important physiological role by activating specific bile acid receptors.Takeda G proteincoupled receptor 5 (TGRS) is a membrane receptor of the bile acid,it can activate G-protein effectors and promote the expression of the second messenger and activate a variety of signal transduction pathways to exert physiological functions.The obstructive jaundice bile acid can cause a series of pathophysiological reactions through activating TGR5,which plays a significant role in the obstructive jaundice.In this article,the latest researches about the function and molecular mechanism of TGR5 in obstructive jaundice were reviewed in order to provide a new idea for exploring the new method and strategy to cure the disease.

From the aspects of advantages, barriers, opportunities and challenges, this paper analyzed the environmental factors of Shanghai medical resources serving people in the Pan-Yangtze River Delta. As envisaged by the author, such services will provide medical resources information to patients from outside Shanghai, probe into feasible ways to medical insurance based on experiences drawn from serving such patients, help with the resources integration of medical systems in Shanghai, and probe into the potentials of telemedicine and distant medical e-learning, as well as improve hospital management levels and service functionality.
