1.The relationship between the expression of inflammatory factors and joint function in patients with traumatic arthritis and the expression of inflammatory factors in joint fluid

Objective To study the relationship between the expression of inflammatory factors and joint function in patients with traumatic arthritis and the expression of inflammatory factors in joint fluid.Methods 90 cases of knee joint trauma in our hospital for the treatment of traumatic arthritis patients from September 2015 to September 2016 as the object of study,90 healthy volunteers as control group,two groups were detected in the synovial fluid of patients with inflammatory factors,to explore the level of inflammatory factors in synovial fluid (TNF-α,IL-6,IL-8) in patients with X-ray classification and joint function.Results TNF-α,IL-6 and IL-8 levels in the synovial fluid of patients with traumatic arthritis were significantly higher than that in healthy adults(P<0.05),radiographic evidence of traumatic osteoarthritis in patients with grade Ⅰ 37 cases,Ⅱ 23 cases,Ⅲ grade 21 cases,9 cases of grade Ⅳ,with bone damage aggravated statistically significant TNF-α,IL-6,IL-8 level is high and the differences between groups,patients with HSS score and KSS score in patients with joint X-ray classification,TNF-α and IL-6 in synovial fluid and IL-8 level is negatively related to(P<0.05).Conclusion The expression of inflammatory factors and joint function in patients with traumatic arthritis were closely related to the expression of inflammatory factors.

3.Emergency treatment and nursing care of patients with traumatic diaphragmatic rupture

Objective:To summarize the experience in nursing diaphragmatic laceration patients after traumatic rupture. Methods:A retrospective analysis was performed for 28 patients admitted to our department between 1995 and 2007,the treatment,nursing care and complications were reviewed and summarized. Results:All the patients recovered uneventfully and no complications occurred. Conclusion:Early diagnosis,active treatment and meticulous nursing care play a key role in the successful treatment of patients with traumatic diaphragmatic rupture.
4.Endostatins combined radiotherapy and chemotherapy in the treatment of advanced non-small cell lung cancer

Objective To preliminary study clinical efficacy and toxicity of endostatin (ES) combined radiotherapy and chemotherapy in advanced non-small cell lung cancer (NSCLC),and evaluate its effectiveness and safety.Methods We retrospectively reviewed 64 patients with Ⅲ B-Ⅳ stage NSCLC of Harbin Medical University Cancer Hospital from February 2009 to February 2012.The patients were divided into two groups:chemoradiotherapy group,39 patients and ES add chemoradiotherapy group,25 patients.The short-term effect,the total efficiency,median survival time,progression-free survival time and disease-free survival time were compared.Results The total effective rate of chemoradiotherapy group was 76.9%,while the total effective rate of ES add chemotherapy group was 84.0% (x2 =0.47,P =0.492).Chemoradiotherapy group,compared to ES add chemotherapy group,the median survival time,median progression-free survival time,median disease-free survival time were 11.52 months vs 16.51 months (x2 =3.74,P =0.042),7.32 months vs 10.37 months (x2 =5.32,P =0.025) and 5.21 months vs 7.57 months (x2 =4.56,P =0.035) respectively.The mainly adverse drug reactions were hematologic toxicity and gastrointestinal reactions,but there were no significant differences between the two groups; radiotherapy side effects mainly showed the grade 1 to 2 radiationinduced lung injury and radiation esophagitis (chemotherapy group had one case of grade 3 radiation-induced lung injury),but also had no significant differences between the two groups.Conclusion ES combined chemoradiotherapy can achieve a better short-term clinical efficacy without increasing adverse effects of radiotherapy or chemotherapy in advanced non-small cell lung cancer.
5.Clinical observation on the treatment effect for pruritus in uremic patients by different dialysis modalities

Objective To study the effect of different dialysis modalities on pruritus in uremic patients .Methods Patients with maintenance hemodialysis who were suffered with severe cutaneous pruritus were randomly divided into hemodialysis group (HD group) ,hemodialysis combined with hemodiafiltration group (HD+ HDF group) ,hemodialysis and hemodiafiltration combined with hemoperfusion group (HD+ HDF+ HP group) .Plasma P3+ ,parathyroid hormone (PTH) and β2-microglobulin (β2-MG) were measured at pre-dialysis ,1 weeks ,4 weeks and 12 weeks after dialysis ,cutaneous pruritus was scored too .Results Compared with pre-dialysis ,the level of plasma P3+ ,PTH ,β2-MG and the scores of cutaneous pruritus were significantly lower at 4 weeks and 12 weeks after dialysis in HD+ HDF+ HP group (P<0 .05) .The were statistically significant difference in P3+ ,PTH ,β2-MG and cu-taneous pruritus scores among 3 groups after 12 weeks (P<0 .05) .Conclusion HD+ HDF+ HP is superior to HD+ HDF in effi-ciently clear P3+ ,PT H andβ2-M G ,and relief cutaneous pruritus and itching .
6.The current study on treatment of avascular necrosis of the femoral head with cytokines

Bone regeneration and repair after avascular necrosis of the femoral head are a complex process involving interactions of numerous kinds of cells. In the past decade when more and more cytokines have been identified and characterized, basic and clinical researches have found the efficacy of various cytokines in healing avascular necrosis of the femoral head. This article summarizes findings about some cytokines associated with treatment of avascular necrosis of the femoral head in order to deepen our understanding. The article also discusses the functions, current problems and future applications of cytokines in this field.
7.Clinical application of transdermal fentanyl for 30 patients with moderate or severe cancer pain

Purpose:To study analgesia effect ,adverse reactions and life quality changes of transdermal fentanyl in patients with cancer pain.Methods:30 patients with moderate or severe cancer pain were treated by transdermal fentanyl .The pain intensity, quality of life and adverse reactions were observed before and after the treatment.Results:After using transdermal fentanyl ,all the patients obtained moderate to good remission.The excellent,marked and moderate rates were 43.33%,53.33% and 3.33% respectively. The adverse reactions included nausea, vomiting,constipation,dizziness and lethargy.The rate of the adverse reactions was less.The quality of life improved markedly. Conclusions:Transdermal fentanyl is a safe,effective and simple way to relieve cancer pain in patients with moderate or severe pain.It can improve the quality of life markedly.Its adverse reactions are mild.

9.Correlation Analysis of Apoptosis and Clinical Indicators in Rheumatoid Arthritis Patients.

OBJECTIVETo observe the expression of peripheral blood CD4+ T lymphocyte apoptosis gene in rheumatoid arthritis (RA) patients with cold dampness type (CDT), and to explore its correlation with clinical indicators of RA.
METHODSSixteen RA patients with CDT (as the RA group) and 16 healthy subjects (as the normal control group) were recruited. CD4 T cell apoptosis rate was detected in the RA group and the normal control group using FCM. mRNA expressions of fas, fasL, caspase-3, caspase-8, bcl-2, and bax were detected using RT-PCR. Correlations between the expression of apoptosis gene and clinical activity indicators of RA (ESR, CRP, RF, CCP, integrals for Chinese medial symptoms, morning stiffness time, joint tenderness number, joint swelling number, DAS28-3) were analyzed.
RESULTSThe apoptosis rate of CD4+ T was significantly lower in the RA group than in the control group [(2. 6 +/- 0.9) % vs. (7.7 +/- 1.3) %, P < 0.01]. mRNA expression levels of fas, fasL, caspase-8, caspase-3, and bax mRNA of CD4+ T significantly decreased, but bcl-2 mRNA expression increased in the RA group (P < 0.01). The apoptosis rate of CD4+ T was negatively correlated with ESR (P < 0.05). The mRNA expression of caspase-8 was negatively correlated with joint swelling number (P < 0.05). The mRNA expression of bcl-2 was negatively correlated with integrals for Chinese medial-symptoms and joint function classification (P < 0.01, P < 0.05).
CONCLUSIONApoptosis obstacle exists in peripheral blood CD4 +T lymphocyte of RA patients, and is closely related to disease activity.

10. Influence of granulocyte-colony stimulating factor combined with adrenocortical steroid and/or immunosuppressants on immune system of donor mice during hematopoietic stem cell mobilization

Objective: To explore the influence of granulocyte-colony stimulating factor (G-CSF) combined with adrenocortical steroid and/or immunosuppressants on immune system of donor mice during hematopoietic stem cell mobilization. Methods: The donor C57BL/6 mice were divided into 4 groups according to the mobilization strategies, namely, the G-CSF group (250 μg/kg for 4 days), G-CSF (250 μg/kg for 4 days) + methylprednisolone group (100 mg/ kg on day 3), G-CSF (250 μg/kg for 4 days) + methylprednisolone group (100 mg/kg on day 3) + cyclosporine A (100 mg/kg on day 3), and PBS control group. The post-mobilization T lymphocytes of C57BL/6 mice and the splenocytes of BALB/c mice were subjected to mixed lymphocyte reactions (MLR). The levels of IFN-γ, IL-4, and TGF-β 1 were determined in the supernatants of donor mice splenocytes by ELISA method and the mRNA levels of IFN-γ and IL-4 were determined by RT-PCR. Results: The results showed that the MLR-inhibitory activities in the 3 G-CSF mobilized groups were 42 %, 21 %, and 39 %, respectively; all lower than that in PBS control group (68%). IFN-γ levels in the 3 G-CSF mobilized groups were lower than that in PBS control group, but he levels of IL-4 and TGF-β 1 were significantly higher than those in PBS group(P<0.01), with no significant difference found between the 3 G-CSF mobilized groups. RT-PCR analysis found that the expression of IFN-y mRNA was obviously decreased and the expression of IL-4 was obviously increased (P<0.01). Conclusion: G-CSF combined with adrenocortical steroid and/or immunosuppressants can decrease the allogenic reaction of T lymphocytes and change the types of secreted cytokines in donor mice during hematopoietic stem cell mobilization, which may induce immune resistance in the recipients.
