1.Analysis on blood test result of voluntary blood donors in Fuling District during the past seven years

Objective To get the information about the infection condition of blood in voluntary donors in Fuling District from 2001 to 2007, so as to take measures to improve blood quality and ensure blood safety.Methods Serum ALT was detected with Reitman-Frankel assay, and HBsAg, anti-HCV, anti-HIV and anti-TP were detected with ELISA.Results A total of 72 868 serum samples from blood donors were detected, in which 5 512 (7.56%) were positive. The positive rates of HBsAg, anti-HCV, anti-HIV and anti-TP were 3.09%, 0.48%, 0.07% and 1.27% respectively, and the abnormality rate of ALT was 2.73%. There was statistical difference of positive rate among various years (from 2001 to 2007).Conclusion The positive rate of blood in voluntary donors decreases year by year. HBsAg was the highest (1.16%), followed by ALT (2.73%), anti-TP (1.27%), anti-HCV and anti-HIV. It suggests that blood quality monitoring should be strengthed to assure blood safety.
2.Systematic evaluation of clinical application of Dingkun Dan.

Dingkun Dan is a representative of the classic gynecological medicine. With Tonifying the liver and kidney, supplementing Qi and nourishing blood, regulating menstruation Shuyu, promoting the role of pain. Used in treatment of liver and kidney deficiency, deficiency of both qi and blood, Qi stagnation and blood stasis caused by irregular menstruation, menstrual pain, uterine bleeding, leukorrhea with reddish discharge, bruise blood removal, infertility, and various postpartum deficiency and bone steaming hot flashes of gynecological common disease. In recent years, Dingkun Dan's new uses have been reported, the clinical application value is worth further digging. In this paper, clinical application and research progress of Dingkun Dan since the founding of new China were briefly discussed, and summarize for the randomized controlled trials.

3.Discussion on strategy of treatment of perimenopausal syndrome with Chinese and Western Medicine.

Perimenopausal syndrome refers to a series of physical and mental symptoms ,caused by the fluctuation of the sex hormones in the menopause, which is one of the common diseases in gynecology. With the acceleration of the aging population ir the world, it has caused the world's attention. Modern medicine has developed rapidly in the field of basic theory and clinical research, but there are still some problems in the treatment. Clinical treatment is mainly in hormone replacement therapy, but there are some potential risks, adverse reactions and many contraindications, thereby reducing patients' compliance and tolerance. Traditional Chinese medicine under the guidance of the concept of wholism cure perimenopausal syndrome based on syndrome differentiation, which has unique advantages, such as a variety of ways, less side effect, curative effect, which was patients more and more welcome. It is very important for the study to explore the perimenopausal syndrome.

5.Clinical study on auricular point sticking plus Western medicine for moderate gastric cancer pain

Objective: To explore the clinical efficacy and mechanism of auricular point sticking plus Western medicine for moderate gastric cancer pain. Methods: A total of 80 patients were selected and divided into an observation group and a control group according to the random number table method, with 40 cases in each group. Patients in the control group received Western medicine treatment, while patients in the observation group received additional auricular point sticking. Both groups were treated for 2 weeks. Numeric rating scale (NRS) and Karnofsky performance status (KPS) were adopted before and after treatment. The total time and times of flare-up pain in 24 h were recorded. The cyclooxygenase-2 (COX-2) and tumor necrosis factor-α (TNF-α) levels were detected. The clinical efficacy of both groups was evaluated after treatment. Results: The total effective rate of the observation group was significantly higher than that of the control group (P<0.05); after treatment, NRS scores of both groups were significantly lower than those before treatment (both P<0.05), and the score of the observation group was lower than that of the control group (P<0.05); KPS scores of both groups were significantly higher than those before treatment (both P<0.05), and the score of the observation group was higher than that of the control group (P<0.05). After treatment, the total time and flare-up times of pain during 24 h of both groups were significantly reduced (all P<0.05), and those of the observation group were significantly less than those of the control group (both P<0.05). After treatment, the COX-2 and TNF-α levels of both groups were significantly reduced (all P<0.05), and were lower in the observation group than in the control group (all P<0.05). Conclusion: The clinical efficacy of auricular point sticking plus Western medicine for moderate gastric cancer pain is valid. This combined treatment can alleviate cancer pain and improve patients' quality of life, which may be related to its ability to reduce COX-2 and TNF-α levels.
6.Application of clinical pathway in uterectomy patients

Objective Through application of clinical pathway in hospitalization patients undergoing uterectomy, we aimed to study the efficacy of clinical pathway in uterectomy patients. Methods 120 patients randomly sampled into the experimental group and the control group with 60 in each. The experimental group received diagnosis, treatment, nursing and rehabilitation according to the clinical pathway table, patients in the control group received routine nursing interventions. The average hospitalization days, medical costs, patient satisfaction degree, disease cognition, first exercise time after operation and medical disputes were compared between the two groups. Results The average hospitalization days and hospitalization costs of the experimental group were obviously lower than those in the control group, patients' satisfaction degree, knowledge of health and first functional exercise time in the experimental group were better than those of the control group, while the medical disputes in the experimental group were obviously de -creased in the experimental group. Conclusions Application of clinical pathway in nursing care for uterectomy patients can effectively decrease hospitalization days, medical costs and disputes. It is also helpful for patients to cooperate with doctors in diagnosis, treatment, nursing, thus increase the satisfaction degree of patients, ensure the reasonable use of medical resources and promote the sustainable improvement of nursing quality.
7.Experience of diagnosis and treatment of iatrogenic injury by ureteroscopic surgery

Objective To summarize the experience of diagnosis and treatment of iatrogenic injury by ureteroscopic surgery.Methods Retrospective analysis of 13 cases with iatrogenic injury by ureteroscopy from December 2008 to December 2011,including 8 men and 3 women,aged 15 to 75 years.Among the 13 cases (Holmium laser lithotripsy under ureteroscope),there were 5 cases of ureterostoma severe disruption,4 cases of submucosa injury,2 cases of perforation,1 case of disruption,and 1 case of sleeve exfoliation of mucosa.Results Among these 13 cases with iatrogenic injury by ureteroscopy,10 cases underwent double J drainage (drainage duration:60 days),and 3 cases underwent open surgery immediately.There was no hydronephrosis when examined by IVU after six months to two years follow-up.Conclusions The skills and techniques of surgical operation should be improved when performing ureteroscopic operation,and it is essential to be familiar with ureteric dissection and alignment,which can avoid ureteric injury.Indwelling D-J tube is very important in dealing with mild ureteral injury secondary to ureteroscopes.Surgical intervention should be given to severe cases of ureteric injury in time.
8.Comparison of different operation methods for the treatment of localized and high risk prostate cancer

Objective To compare clinical efficacy and complications of three operation methods for treating the localized and high risk prostate cancer.Methods From July 2006 to July 2010,102 cases with localized and high risk prostate cancer were included in the study.Among them,51 cases received open radical prostatectomy (Group A).Their characters included aged (63.7±7.9),PSA (31.8±6.6) μg/L,Gleason scores (8.5±0.5).In this group,T2c stage was diagnosed in 38 cases and T3a stage in 13 cases.32 cases received laparoscopic radical prostatectomy(Group B).Their characters included aged (64.4± 8.3),PSA (29.9±5.2) μg/L,Glcason scores (8.7±0.4).In this group,T2c stage was diagnosed in 21 cases and T3a stage in 11 cases.19 cases received 125I implantation (Group C).Their characters included aged (61.4± 7.4),PSA (30.6±.5.7)μg/L,Gleason scores (8.6±0.6).T2v stage was found in 8 cases and T3a stage in 11 cases.Operation time,amount of bleeding,hospitalization time,drainage time,gastrointestinal function recovery time,medical expenses and survival and recurrence rate in 1 year,3 years,5 years were compared within those groups.Results Operation time in three group were (232.6±38.4) min,(186.3±31.4) min,(35.4±14.6) min,respectively.Amount of bleeding in three group were (413.6±132.4) ml,(273.9± 77.4) ml,(19.4±4.4) ml,respectively.Hospitalization time in three group were (20.9±3.7) d,(15.6± 2.2) d,(6.4±2.6) d respectively.Drainage time in three group were (8.3±1.8) d,(7.5±0.9) d,(3.2± 0.8) d,respectively.Gastrointestinal function recovery time in three group were (4.1 ±0.6) d,(3.2± 0.4) d,(0.4±0.1) d,respectively.Medical expenses in three group were (23±4) thousand yuan,(32±3) thousand yuan,(45t3) thousand yuan respectively.All those items exhibited the significantly statistical difference (P<0.05).The survival and recurrence rates had no significant difference during the follow-up in three groups.Conciusions Compared to the open radical prostectomy and laparoscopic radical prostectomy,particle implantation for prostate cancer had advantages in minimally invasive,less bleeding,short operation time,fast recovery and protection for Intestinal function.
9.Testicular cancer in cryptorchids

Objective To study the diagnosis and treatment of testicular cancer in cryptorchids. Methods The data of 10 patients with cryptorchid tumor treated at our hospital between 1986 and 2003 were retrospectively analyzed.Their average age was 40 years (range,32~55 years).Of these patients,1 had the testis in the inguinal area;3,in the scrotum (with a history of inguinal cryptorchidism) and 6,in the abdomen.Cryptorchidism occurred on the left in 2 cases,on the right in 5 and bilaterally in 3.Three patients presented with stage Ⅰ A disease,5 with stage Ⅰ B,and 2 with stage Ⅱ D. Results All the 10 patients received surgical treatment including radical tumor resection,radical tumor resection with retroperitoneal lymph node dissection (RPLND),and palliative operation.Pathology showed that 9 cases had seminoma and 1 had seminoma accompanied by embryonic carcinoma.Radiotherapy was performed in 8 cases,and radiotherapy plus chemotherapy,in 2 (including 1 with embryonic carcinoma).Postoperatively,6 patients were followed up for 6 months to 14 years with a mean of 5.2 years;these patients showed no evidence of tumor recurrence and metastasis. Conclusions Cryptorchid testes have a greater risk of malignant change than do normally descended testes,and early intervention is advocated to prevent malignant canceration.Even treated with orchidopexy or orchiectomy,the patient should be followed up lifelong.Most of cryptorchid tumors are seminomas, and the ideal treatment strategies are tumor resection and radiotherapy.
