1.Diagnosis and treatment of primary colonic malignant lymphoma

Objective To investigate the diagnosis and treatment procedures of primary colonic malignant lymphoma(PCML). Methods Data of clinical and pathological findings, surgical treatment and prognosis of 12 cases with PCML verified by pathology were retrospectively analyzed, workups of barium clysma,colon-scopic examination and multislice spiral computed tomography(MSCT) were compared to evaluate their role in diagnosis. Results The classic presentation of PCML included abdominal pain, abdominal bulge, bowel obstruction, gastrointestinal bleeding, and serum levels of tumor markers were within the normal ran-ges. Six cases underwent procedure of barium clysma, and anomaly appeared in 2 cases, however, there were no indications of lymphoma, 6 cases underwent colon-scopic examination, anomaly in 3 cases, of which 1 revealed possibility of lymphoma, 10 cases underwent MSCT; 9 cases demonstrated tumor origin by CT scan of which diagnosis was established in 5 cases. All the patients underwent surgery. No incidence of peri-opreative death happened. Non-Hodgkin' s lymphoma was confirmed in all of cases. For all of the cases a T-cell origin was in 1 case and a B-cell origin in other cases. Ten cases were administrated postoperative chem-otherapy. 11 cases were followed up, and the overall 1-year and 3-year survival rates were 81.9% (9/11) and 54.5% (6/11), respectively. Conclusions Typical presentation is still lacking for PCML, workups of barium clysma and colon-seopic examination are little sensitive and specific to make the diagnosis. However, MSCT is useful in diagnosing PCML with its characteristic information as well as clinical findings. Surgical management and postoperative adjuvant-chemotherapy would be likely the optimal therapeutic maneuver of this disease in early period.
3.Study on Transformation of Complete Atrioventricular Block in Children

Objective To observe the transformation features of children with complete atrioventricular block(CAVB), and to improve the diagnosis and treatment of this disease.Methods Seventeen children with primary diagnosis of CAVB were reviewed by retrospective and follow-up study and the clinical characteristics, treatment schemes and prognosis were evaluated.Results Four cases of congenital CAVB lived normally without obvious symptoms in the tracing period from 6 months to 9 years;1 case had the onset of Adams-Stokes syndrome induced by diarrhea;3 cases of the CAVB caused by virus myocarditis turned into sinus rhythm after comprehensive therapy;2 cases persistently presented with the CAVB Complicated with enlarged heart;1 case gave up treatment after deterioration, and one case died.Three cases of temporary CAVB after the open heart operation turned to sinus rhythm in transitory time, while other two cases presented with permanent CAVB and treated with epicardium pacemaker,among whom one had the pacemaker replaced for one time in the 8-year follow-up,and the follow-up of other cases were intermitted.Conclusions The congenital CAVB in the study group with normal QRS interphase and no obvious symptom might not require treatment but follow-up is needed. While infants with heart malformation and wide QRS wave could not endure the low ventricular rhythm are in high risk. Virus myocarditis induced CAVB children tend to present the Adams-Stokes syndrome, and require effective treatments. Partial cases of the CAVB caused by open heart operation may turn to normal sinus rhythm in 2-4 weeks after surgery. cases has persistent CAVB for over 4 weeks, or Adams-Stokes syndrome onset after the surgery demand epicardium pacemaker treatment.

Objective To explore the clinical significance of the brain natriuretic peptide(BNP) in evaluating the cardiotoxicity caused by daunorubicin(DNR) through studying the changes of the plasma BNP levels in children with acute leukemia who accepted the chemotherapy with DNR.Methods Thirty-one children with acute lymphoblastic leukemia(ALL) admitted in the year of 2002-2004 underwent the chemotherapy in DVLP project.The plasma level of BNP was measured by enzyme linked immunosorbent assay(ELISA) and the left ventricular end diastolic diameter(LVEDD) by color Doppler respectively before and after the administration of DNR.Simultaneously,electrocardiography(ECG) and cardiac muscle enzymes(LDH1,CK-MB) were measured as routine.Results The plasma level of BNP increased from(3.97?2.41) ng/L to(18.25?7.63) ng/L(P
5.Common gram-negative bacilli lower respiratory tract infection in PICU: analysis of pathogens and drug resistant pattern from 2004 to 2009

Objective To study common Gram negative bacterial pathogens and their drug resistant pattern in sputum isolates from patients in pediatric intensive care unit (PICU) of Chengdu children' s hospital. Methods Comparing to the period of 2004-2006 with 2007-2009, we retrospectively reviewed eligible sputum isolates of common Gram negative bacilli and their drug resistance pattern. All specimens were processed according to standard microbiological methods for both quantitative and qualitative cultures. Pathogen was the single predominant growth after two consecutive cultures. Strict quality control was used in all test agents and paper disk for drug resistance analysis. Results Total of 598 Gram negative bacterial isolates were identified in patients over 6 years. The first 5 bacteria respectively were Escherichia coli 167 samples(27.93%), Klebsiella pneumoniae 109 samples ( 18.23% ), Enterobacter cloacae 93 samples ( 15.55% ),Acinetobacter baumannii 64 samples ( 10. 7% ), Pseudomonas aeroginosa 43 samples (7. 19% ). They were mostly sensitive to Imipenam, Amikacin, Piperacillin/Tazobactam. Most drug resistance rate was found in Amoxicillin, Ampicillin, Cephalosporin. Apart from Pseudomonas aeroginosa and Acinetobacter Baumannii,no Gram negative bacteria was found to be resistant to Imipenam. Klebsiella pneumoniae and ESBL secreting Escherichia coli isolates were obviously increasing when compared to 3 years ago. Conclusion Comparing the period of 2004-2006, the Gram negative bacterial isolates were increasing in our PICU patients during 2007 to 2009. Close monitoring of drug resistance pattern changes in PICU would guide rational use of antibiotics in these critically ill children.
6.Suppression effect of expressing parvovirus H-1 nonstructural protein 1 gene on human gastric cancer cells

Objective To investigate the suppression effect of expressing parvovirus H-1 nonstructural protein 1 (NS1) gene on human gastric cancer cells and the possible mechanisms.Methods A recombinant enhanced green fluorescent protein (eGFP) labeled NS1 of parvovirus H-1 plasmid was constructed.Human gastric cancer cell line SGC7901 was transfected with recombinant plasmid (experiment group) or blank vector (negative control group) and blank control group was treated with equal amount of phosphate buffered saline (blank control group).After transfection,the distribution of fluorescent signal was observed under fluorescent microscope.The expression of NS1 at gene and protein level was measured.Cell growth curve of each group was drawn.The expression of cell senescence-associated β-galactosidase (SA-β-Gal) was tested.The changes of cell cycle were investigated by flowcytometry.Two groups' comparision was performed by t-test.Results After transfection,NS1 was expressed in SGC7901 cells at gene and protein level.Compared with negative control group,the fluorescent signal accumulated in cell nucleus in experiment group.The percentage of SA-β-Gal positive cell in experiment group ((30.5 ± 1.4) %) was higher than that of negative control group ((4.4± 1.1) %) and the difference was statistically significant (t =-12.931,P < 0.01).The growth inhibition rate of SGC7901 cells from the first day to the fourth day was 45%,62%,73% and 77%,respectively.The cell cycle of eGFP-NS1 expressed SGC7901 cells was arrested at G0/G1 phase.Conclusion Parvovirus H-1 NS1 play the role in cell nucleus of gastric cancer cell line SGC7901 and could make cell cycle arrested at G0/G1 phase,which effectively inhibited the proliferation SGC7901 cell.

9.Determination of Paracetamol in Ganmaoqing by HPLC

Objective: To study the feasibility of the determination paracetamol in Gangmaoqing by HPLC. Methods: The paracetamol contents of the different samples of Ganmaoqing Tablets were determined by the dead stop titration and HPLC, respectively. Results: The dead stop titration was complex and the error was larger. Whereas HPLC was simple, quick with a good seperation and small error.Conclusion: HPLC is accurate and reliable and reproducible for the determination of paracetamol in Ganmaoqing Tablets.
10.Large-scale expansion of clinical-grade human adipose-derived stem cells using the extracellular matrix

BACKGROUND:Large-scale expansion of undifferentiated and multipotential adipose-derived stem cells using serum-free culture system is a difficult issue to be resolved. OBJECTIVE:To establish an in vitro culture system combined with the extracellular matrix in order to investigate the efficiency, effectiveness and security of extracellular matrix on expanding adipose-derived stem cells. METHODS:In vitro isolated adipose-derived stem cells were seeded in traditional two-dimensional plastic plates and extracellular matrix-coated plates supplemented with serum-free medium respectively. After in vitro expansion, total cellnumber, expression of cellsurface markers, cellsenescence degree and multipotent differentiation ability (adipogenic, osteoblastic and chondrogenic differentiation) of adipose-derived stem cells cultured under both conditions were detected and compared. Moreover, the clinical safety of adipose-derived stem cells expanded in extracellular matrix-coated plates was investigated. RESULTS AND CONCLUSION:Total cellnumber of passage 5 adipose-derived stem cells cultured in extracellular matrix-coated plates was 10 times more than that in traditional two-dimensional plastic plates. Flow-cytometric analysis showed that adipose-derived stem cells cultured with extracellular matrix expressed stem cellsurface markers. cellular senescence examination showed that almost al of passage 15 adipose-derived stem cells cultured with extracellular matrix showed no aging, while most passage 5 adipose-derived stem cells cultured by the two-dimensional system aged and lost their proliferation ability. Multidirectional induction of adipose-derived stem cells showed that passage 15 adipose-derived stem cells cultured with extracellular matrix could stil differentiate into adipocytes, osteoblasts and chondrocytes as passage 5 adipose-derived stem cells did, which performed much better than the induced differentiations of passage 5 adipose-derived stem cells cultured by the two-dimensional system. Karyotype analysis and in vivo invasion experiment insured the clinical safety of adipose-derived stem cells expanded with extracellular matrix. Al above results suggest a safe and more efficient expansion system of extracellular matrix for clinical application using the serum-free culture system combined with extracellular matrix.
