1.Enriched Environment and Stroke Rehabilitation (review)

Abstract: An enriched environment refers to conditions that facilitate sensory, cognitive, motor, and social stimulation. Behavioural,cellular and molecular studies have revealed significant effects of enriched environments on rodents and other species. Post-ischemic housing in an enriched environment improves functional outcome, modifies gene activation, induces alterations in the expression of N-methyl-D-aspartic acid (NMDA) and α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor subunits, increases dendrite branching and number of dendritic spine in pyramidal neurons in layers II/III in the contra-lateral cortex, induces structural changes in synaptic junctions. Furthermore, it alters lesion-induced progenitor cell differentiation and interacts with necrotic grafting. The idea of environmental enrichment has been applied to clinical stroke rehabilitation and is getting more and more attention. This review will summarize experimental effects of enriched environment on post-ischemic rehabilitation and the clinical application of enriched environment in stroke rehabilitation.
2.Effect of high-expression HOXB4 on human cartilage endplate stem cells and its significance

Objective To observe the effect and significance of high-expression HOXB4 in controlling proliferation and cycle of human cartilage endplate stem cells (CESCs).Methods CESCs were divided into adenovirus-mediated HOXB4 delivery group (Group A),empty virus delivery group (Group B) and blank control group.Gene and protein expressions of HOXB4 in Group A were detected by PCR and Western blot respectively; cell proliferation among those groups were determined using cell counting kit 8 (CCK8) technique; cell cycle among those groups was measured by propidium iodide (PI) assay and flow cytometry.Results (1) Over-expressed HOXB4 virus was transferred to CESCs successfully; (2) Real-time quantitative PCR results showed 3.6 times higher expression of HOXB4 in Group A than in blank control group.Western blotting indicated HOXB4 protein in Group A was 3 times the level in control group; (3) HOXB4 promoted CESCs proliferation (P < 0.05) and blocked the cells at phase S.Cells at phase S in Group A was increased from 29.27 to 30.28 (P < 0.05).Conclusion Over-expressed HOXB4 accelerates proliferation of CESCs and increases cell population at phase S,indicating that HOXB4 hindering CESCs degeneration may be an approach to treat lumbar intervertebral disc protrusion.
3.Clinical isolates of Acinetobacter baumannii from bloodstream infections: PFGE characterization and virulence-associated factors

Objective The epidemiology of Acinetobacter baumannii isolates from bloodstream infections,their antibiotic resistance profiles and virulence-associated factors were studied.Methods A total of 90 isolates from 17 hospitals were collected from the patients with bloodstream infections during July 2013 and July 2014.Vitek-2 Compact system was used for identification of the strains and antibiotic susceptibility testing.The epidemiology was studied by pulsed-field gelectrophoresis(PFGE).Drug-resistant genes and associated virulence genes were amplified by PCR.Results According to antimicrobial susceptibility testing,75 isolates are multi-drug resistant Acinetobacter baumannii.PFGE results showed that 75 multi-drug resistant Acinetobacter baumannii isolates belonged to eight clone types(A to H),with the A (n=51)and B (n=14)clone being the dominant PFGE clone types.Different clone isolates spread in different hospitals.Most of the hospitals were given priority to with clone A.Clone A only maintaining high sensitive rate to cefoperazone/sulbactam、amikacin and tigecycline.Virulence gene abaI,cusE,ompA,bap,bfms detection rates are 93.3% (84/90),92.2% (83/90),100.0% (90/90),84.4% (76/90),92.2% (83/90),respectively.There were 7 mucoid isolates,which are all multi-drug resistant Acinetobacter baumannii,all belong to clone B and all associated virulence genes can be detected.Conclusions The dominant clone type of multi-drug resistant Acinetobacter baumannii from bloodstream infections is clone A.The abaI-,bap-and bfms-positive strains are associated with a higher incidence of antibiotic resistance in most types of antimicrobials.The acquisition of mucous type may indicate the emergence of virulent strains,which should be paid attention to during clinical treatment.
4.The post -operation rehabilitation nursing of re-establishing anterior cruciate ligament of the knee by using allogeneic tendon

Objective To approach rehabilitation nursing measures in using the allogeneic ten-don to re-establish the anterior cruciate ligament of knee through the tibia twin tunnel under arthroscopy.Methods Mental nursing and direct functional exercises was given to 30 patients who received this kind of operation from November,2006 to July,2007,and functional exercises of flection and extention of knee joint by steps,functional exercises of muscle force of quadficeps muscle and back muscle group of legs and the correct using of artificial brace of knee joint.Results Range of joint motion could reach 95°after 2 weeks and 125°after 4 weeks postoperation.Score of joint function(Lysholm)could reach(74.8±5.7)after 6 weeks,(80.0±2.3)after 9 weeks,(91.8±3.4)after 12 weeks, and(94.5±2.2)after 1 year.Conclusion Selective perioperative rehabilitation nursing instruction is the important guarantee of function recovery.
5.Imaging manifestation of metastatic tumors in nasal cavity and paranasal sinuses

Objective To investigate the imaging manifestations of metastasis in the nasal cavity and paranasal sinuses. Methods Twelve lesions of 10 patients with pathologically proved metastasis were retrospectively reviewed, including 6 renal clear cell carcinoma, 2 pulmonary adenocarcinoma, 1 colorectal carcinoma and 1 hepatic carcinoma. All the patients underwent CT and MRI scan.Among them,9 patients had contrast enhanced MRI scan and 5 patients had dynamic contrast-enhenced MRI as well. The location, bone changes, shape,margin,density,signal intensity, and enhancement characteristics of the lesions were observed. Results The metastasis involved spheno-ethmoid area(n=3), ethmoid sinus (n=2), maxillary sinus (n=2), nasal cavity(n=2), fronto-ethmoid area(n=2) and sphenoid sinus (n=1). All 12 metastatic lesions demonstrated bone destruction with tumor bone formation in 1 lesion. The CT displayed 10 metastases showed equal density, one mixed density was high, another form of low-density mixed. MR imaging showed equal signal in 9 lesions, while low-mixed signal in 3 lesions on T1WI compared with grey matter were found. On T2WI, 11 lesions showed high mixed signal and homogeneous low signal in 1 lesion. All 12 lesions demonstrated markedly heterogenous enhancement after intravenous administration of contrast media. Of 5 cases with dynamic contrast-enhanced MRI scanning, 4 lesions demonstrated wash-out time-signal intensity curve (TIC) pattern and 1 lesion demonstrated plateau pattern. Conclusions Renal carcinoma is the most common primary tumor for nasal cavity and paranasal sinuses metastasis. A typical imaging finding is located in the ethmoid sinuses, the soft tissue mass surrounded with rich blood supply multiple sinuses and significant bone destruction.
6.Curative Effect of Underatured Lactalbumin Treatment in Children with Nephrotic Syndrome

Objective To investigate the clinical efficacy of nephrotic syndrome in children adjunctively treated by underatured lactalbumin.Methods Thirty-six cases of nephrotic syndrome were studied for about 1 year.The changes in red blood cells of reduced glutathione(GSH) were analyzed in 18 cases of underatured lactalbumin treatment group and 18 cases of control group.The clinical presentations were observed in both groups.Results GSH levels increased in underatured lactalbumin treatment group when compared with control group.The frequency of relapse and upper respiratory tract infection in underatured lactalbumin treatment group were significantly reduced compared with control group.Conclusion Underatured lactalbumin can provide support for the potential use of an antioxidant therapy in these patients.
7.Relationship of microcirculation pattern and histopathologic factors in choroidal melanoma

BackgroundBiological behavior of choroidal melanoma is closely related with angiogenesis.The microcirculation pattern in which tumor cells may be involved is different from neovascularization.ObjectiveThis study was to observe the microcirculation pattern of the choroidal melanoma tissue and analyze its relationship with the clinical pathology factor and the degree of cellular proliferation.Methods Forty-eight specimen of choroidal melanoma tissue were collected at Beijing Tongren Eye Centre from November 1997 through March 2006.Periodate acid Schiff staining was used to determine the microcirculation pattern of choroidal melanoma.The morphology of the tumor cells and the distribution of microcirculation pattern in the tumor were observed under a fluorescence microscopy with 544 nm wavelength,and the Ki-67 expression in the tumor was detected by immunochemistry.The relationships between the choroidal melanoma clinical pathology factor with Ki-67 expression and microcirculation pattern were evaluated by multiple stepwise regression model.ResultsA total of 9 kinds of microcirculation patterns were found by periodate acid Schiff staining.The occurring rate of loop- or network-like vascular pattern showed a significant elevation in the tumor tissue with epithelial cells in comparison with the tumor with spindle-like cells (66.7% vs.30.6% ) ( P=0.027 ).The expression rates of Ki-67 were 18.961 ± 10.995 and 19.481 ± 12.167 in the tumor tissue with loop- or network-like microvascular pattern,and those in the tumor tissue without loop- or network-like microvascular pattern were 10.261 ±5.669 and 12.021 ± 6.802,presenting significant differences between them ( P =0.000,P =0.010).Loop-like microvascular pattern was determined as the risk factor of the proliferation and metastasis of choroidal melanoma by multiple stepwise regression analysis (P=0.002).ConclusionsAmong the nine microcirculation patterns in choroidal melanoma,networks pattern is the most fashion,and Ki-67 expression is more strong in the tumor with epitheliod cells,suggesting that this microcirculation pattern is associated to the malignant degree and extent of proliferation in choroidal melanoma.
8.A prospective study on nerve-sparing radical hysterectomy in patients with cervical cancer

Objective To assess the nerve-sparing radical hysterectomy (NSRH) technique and its impact on postoperative voiding function. Methods Forty-fonr patients with International Federation of Gynecology and Obstetrics(FIGO) stage Ⅰ b1 - Ⅱ a cervical cancer were enrolled and randomized into NSRH group ( study group, n = 22) and conventional radical hysterectomy (CRH) group ( control group, n = 22). The pelvic autonomic nerve pathway (including hypogastric nerve, pelvic splanchnic nerve, inferior hypogastric plexus and bladder branch) was completely preserved in the NSRH group. Related parameters were compared between the two groups. Results The estimated blood loss in NSRH group and CRH group were (550±241) ml and (475±284) ml, respectively, with no significant difference (P >0. 05). The mean operation time in NSRH group and CRH group were (329±43) min and (272±56) min, respectively, with a significant difference (P < 0. 01). More patients in NSRH group had post-void residual urine volume (PVR) < 100 ml than that in CRH group on day 8 after surgery (68% vs. 18%, P <0. 01). The median duration of postoperative catheterization was significantly shorter in NRSH group (8 - 23 days, median 8 days) than that in CRH group ( 8 - 32 days, median 20 days; P < 0. 01 ). Neither surgery-related injury nor pathologically positive margin was reported in either of the groups. Conclusions NSRH is a feasible and safe technique for preserving bladder function. Larger prospective studies are needed to confirm the efficacy of this technique.
9.Designation and evaluation of antisense oligodeoxynucleotides targeted to glial glutamate transporter-1a.

OBJECTIVEThe present study was undertaken to design antisense oligodeoxynucleotides (AS-ODNs) of glial glutamate transporter-la (GLT-1a) and to evaluate the effectiveness of the designed AS-ODNs on the expression of GLT-1a.
METHODSFive sequences of GLT-1a AS-ODNs were designed according to the C terminus specific sequences of GLT-1a mRNA using antisense design software of IDT Com- pany. Western blot analysis was used to evaluate the inhibition effects of the five GLT-1a AS-ODNs on the expression of GLT-la.
RESULTSThe sequence of GLT-1a AS-ODNs with sequence of 5'-GGTTCTTCCTCAACACTGCA-3' could specifically inhibit the expression of GLT-1a in the hippocampal CA1 subfield of rats, while it had no effect on the expression of GLT-1b. This sequence showed similar inhibition on the expression of GLT-la in sham and ceftriaxone (Cef)-treated rats. It could also significantly inhibit the cerebral ischemic preconditioning (CIP)-induced up-regulation in the expression of GLT-1a. The magnitude of the inhibition in sham, Cef- or CIP-treated rats was similar by more than 60%.
CONCLUSIONFrom the designed five sequences of GLT-1a AS-ODNs, we obtained an effective sequence which can specifically inhibit the expression of GLT-1a.
