Objective To analyse the necessity and feasiblity about the diagnosis and classification of fibrocystic disease of breast, through using synthesize imaging methods.to increase the diseases diagnosis rate. Methods Using imaging checks (molybdenum photography, ultrasonography and infra - red scanner) for 600 breast's cases and contrast to pathogenesis if they were done operations. Results 143 cases were normal. 166 cases got other disease of breast,291 cases were fibrocystic disease of breast(48. 1%), 155 cases of them were performed operations and pathogenesis. the diagnosis similar rate between performed operations check and pathogenesis was 83. 2%. Conclusion The sensitivity of three kinds imaging check method is different each other, but one method can give some help each other. The synthesize imaging check can increase the diagnosis rate and avoid mis - diagnosis and doing needless operations.

Chinese Journal of Postgraduates of Medicine 2009;32(2):26-28
Objective To discuss the clinical characteristics of diagnosis and treatment of primary gastric malignant lymphoma (PGML). Method The data from 34 PGML medical records were reviewed retrospectively between January 2001 and May 2008. Results There were 23 cases of B-cell malignant lymphoma, 7 cases of T-cell malignant lymphoma, and 4 cases of non-B and non-T small cell lymphoma. Confirmed by pathology, 14 cases of them had metastasis to lymphoid nodes and 5 cases had invaded the organs nearby. All of these 34 cases had gastroendoscopy before operation. Conclusions PGML has no specific manifestation, and the definitely diagnostic rate of gastroendoscopy is low. The surgical resection is the major therapy.Operative pattern, the size of tumor, pathological stage can predicate the prognosis for PGML.

Objective To evaluate the effects of dexmedetomidine on the expression of phosphor-cAMP response element binding protein (p-CREB) in isoloated hippocampal neurons of fetal rats. Methods SpragueDawley rats on 16-18 days of gestation were sacrificed and the fetal rats were obtained. The hippocampi of fetal rats were isolated and hippocampal neurons were seeded in culture medium for 8 days. The cells were then divided into 4 groups (n =12 each) using a random number table:control group (group C),dexmedetomidine 0.001 μmol/L group (group D1),dexmedetomidine 0.010 μmol/L group (group D2),and dexmedetomidine 0.100μmol/L group (group D3).In D1.3 groups, dexmedetomidine with the final concentrations of 0.001 μmol/L,0.010 μmol/L,and 0.100 μmol/L was added to the culture medium, respectively, and then the cells were incubated for 3.5 h.The apoptosis in hippocampal neurons was detected by flow cytometry. The expression of p-CREB in hippocampal neurons was determined by RT-PCR and Western blot. Results Compared with group C,apoptosis rate was significantly decreased and the expression of p-CREB was up-regulated in D1.3 groups. Conclusion Dexmedetomidine inhibits apoptosis in isolated hippocampal neurons of fetal rats by up-regulating the expression of p-CREB.

Objective To study the meaning of breast cancer staging system by AJCC eighth edition to invasive lobular carcinoma and analysis the clinical pathological characteristics. Methods According to the eighth edition of the AJCC staging to evaluate the TNM stage and prognosis evaluation of invasive lobular carcinoma cancer patient in Peking University Shenzhen Hospital from 2011 to 2016,and compared with others in clinical pathological data. Results There were 21 cases of invasive lobular carcinoma, accounting for 2.7% of all invasive breast cancer. We found that invasive lobular carcinoma shows no significant difference (P > 0.05) in ages, menstrual status, molecular features and anatomic staging and prognosis staging with others;histological grade were significantly different (P < 0.05).There were significant differences in the prognosis and staging of invasive lobular carcinoma. Conclusions Eighth AJCC staging systemn provides a new reference for the clinical treatment of breast cancer, should be evaluated with anatomic stage.Histological grade is relatively good in invasive lobular carcinoma and the prognosis is good, needs more research to the individualized treatment of invasive lobular carcinoma.

Objective To introduce a method for fast establishment of pig models with secondary hypersplenism, and using this model to study the effect of radiofrequency ablation (RFA). Methods Congestive splenomegaly was created by CT- guided percutaneous embolization of splenic vein and its collateral branches in pigs. Then radiofrequency ablation was employed in the models to observe the changes of peripheral blood as well as the spleen changes in its morphology and histopathology. The blood cell count for peripheral venous blood was determined once a week, and the spleen size was measured by CT scanning. Results One week after splenic vein embolization, the counts of peripheral erythrocytes and platelets showed a reduction, which became more obvious in three weeks (P < 0.05). Erythropenia, thrombocytopenia and splenomegaly could last for over six weeks. At the second week after the procedure , pathologically the spleen assumed passive congestion appearance. The thermal energy produced by radiofrequency ablation could cause local splenic coagulation necrosis, splenic vessels occlusion and splenic thrombotic infarction due to diffuse micro-vascular thrombosis. Conclusion To establish an animal model with secondary hypersplenism, the method of CT-guided percutaneous embolism of splenic vein is simple and reliable. The animal models can be used for surgical or interventional researches. Radiofrequency ablation is an effective intervention means.

Objective To examine plasma inflammatory cytokines and assess stress response in different general anesthesia schemes in cesarean operation. Methods Sixty caesarean operation puerperas were randomly divided into two groups, including the group of sevoflurane (Group S) and the group of propofolum (Group P) in 30 cases of each group. At different time point, we recorded the mean arterial pressure and heart rate, text the blood concentration of inflammatory cytokines; The Apgar scores and arteria umbilicalis pH value after baby delivery were recorded. Results The blood pressure and heart rate of the P group after anesthesia induction of baby in delivery were lower than that before the anesthesia induction(P < 0.05). And there were also significant differences when compared with the S group (P < 0.05). The blood concentration of IL-6 and TNF-α of the P group afte operation were higher than that before the anesthesia induction(P < 0.05). There were also significant differences when compared with the S group(P < 0.05). The blood concentration of sTNFRⅡof the P group after operation and 24 hours after the surgery were higher than that before the anesthesia induction (P < 0.05). And there were also significant differences when compared with the S group (P < 0.05). Conclusion Administering Sevoflurane compound with Remifentanil to general anesthesia in Caesarean operation is safty and steady to inhibit stress reaction and prevent explicit memory from the operation.
