1.PDCA Applied in Special Rectification of Medical Instrument Clinical Trial.

PDCA cycle was applied in special rectification activities for medical instrument clinical trial, with quality criteria of implementation made. Completed medical instrument clinical trial from January 2011 to December 2012 was believed as control group, from January 2013 to December 2014 as PDCA group, the scores of clinical trial and the score rate of items were compared and analyzed. Results show quality scores of clinical trial in PDCA group are higher than that in control group (51 vs. 81, P < 0.001), score rate of items increased except adverse events (P < 0.001). The special rectification activities with PDCA applied in our department are feasible and effective. It significantly improves implement quality of medical instrument clinical trial.

3.A study of serum endotoxin and its gene expression in a model of cerebrogenic multiple organ dysfunction syndrome

Objective To study changes of serum endotoxin and its receptor CD14 gene expression in lung, liver, intestines, kidney tissues in model of acute forebrain ischemia complicated by multiple organ dysfunction syndrome(MODS), and to investigate the pathogenesis of cerebrogenic MODS. Methods 54 Wistar rats were randomly divided into seven groups: normal control group(n=6)、sham-operative group(n=8) and 5 ischemia groups(n=40)including 12 h, 24 h, 36 h, 48 h, 72 h five time points. Model of cerebral infarction was established. The content of endotoxin in plasma was evaluated with a test kit. The area density and optical density of positive staining of CD14mRNA expressing were analyzed for the relative content of CD14mRNA using in-situ hybridization and CMIA medical imaging analysis system. Results Plasma endotoxin level were markedly high at 12 h (0.184?0.055)Eu/L after acute forebrain ischemia, peaking at 24 h (0.639?0.064)Eu/L and it was somewhat decreased at 72 (0.117?0.024)Eu/L. The CD14mRNA expression in lung, liver, intestines, kidney tissues increased after brain ischemia, reaching the peak at the 24~36 h, and decreased after 48 h. 4.Effect of nuclear factor κB antisense oligodeoxynucleotides on transforming growth factor β1-induced epithelial mesenchymal transition in human renal tubular epithelial cells

Objective To investigate the effects of nuclear factor κB(NF-κB) antisense oligodeoxynucleotides (AS-ODNs) on transforming growth factor β1 (TGF-β1)-induced epithelial mesenchymal transition (EMT) in human renal tubular epithelial cells. Methods NF-κB AS-ODNs were transferred into the human renal tubular epithelial cells (HK-2), and the cells were stimulated by 10 μg/L TGF-β1 for 24 hours. The expression of NF-κB mRNA and α-SMA mRNA were measured by RT-PCR. α-SMA protein expression was assessed by fluorescence spectrum.Results TGF-β1 significantly up-regulated the expression of NF-κB mRNA, which was 8 folds of blank control (P<0.01). TGF-β1-indueed epithelial mesenchymal transition was inhibited by NF-kB AS-ODN and the NF-KB mRNA expression of AS-ODNs was decreased by 75%(P<0.05).The expression of α-SMA mRNA and protein was also down-regulated obviously (P<0.05).Conclusion NF-κB AS-ODN can inhibit the expression of NF-κB and the epithelial-mesenchymal transition, which may be a new therapeutic strategy against tubulointerstitial fibrosis.
5.Causes and treatment of childhood vulvovaginitis

Objective To find out and clarify the causes and the pathogens of vulvovagmitis in childhood girls, to determine the clinical treatment. Methods There were 345 cases of childhood girls .ranged from 18 days to 12 years, with symptoms of vulvovagini tis, and their vaginal secretions were examined by routine smear for cleaning degree, trichomonas , Candida and clue cells, gram-stain for neisseria gonorrhoeae, culture for bacteria, mycoplasma urealytium, mycoplasma hominis and chlamydia trachomatis. Results One hundred and nine pathogens were detected(31.6% of the whole cases ), including specific pathogen 70 cases(20. 3% )and non-specif ic pathogen 32 cases(9.3%). Gardnerella was detected most frequently in specific pathogen while enteric bacilli was detected most frequently in non-specific pathogen. Conclusions Non-specific vulvovaginitis is the most frequent cause in childhood vulvovagimtis, and bacterial vaginosis is the mast frequent in specific infection. Symptomatic treatment is effective, but antibiotic treatment should be based on pathogenic findings of vaginal secretions.
6.Clinicopathological features of 26 cases of Reed nevi

Objective:To analyze clinical and pathological features of Reed nevi (pigmented spindle cell nevi) .Methods:The clinical and pathological data were collected from 26 patients with Reed nevi, who visited Department of Dermatology of Xijing Hospital from January 2015 to June 2019, and analyzed retrospectively.Results:Among the 26 patients, 15 were males, and 11 were females. Their average age of onset was 17.35 years, and median age of onset was 13.85 years. Reed nevi occurred predominantly on the lower limbs (12 cases) , followed by the face (6 cases) , upper limbs (5 cases) and trunk (3 cases) . All the skin lesions were black in color, and 7 cases presented with maculae, 19 with flat papules, 22 with quasi-circular skin lesions, and 4 with irregular skin lesions. The diameter of skin lesions ranged from 2 to 10 mm, and 23 cases had skin lesions with a diameter of ≤ 5 mm. As histopathological examination showed, pathological subtypes included junctional nevus (15 cases) and compound nevus (11 cases) ; melanocytes were spindle-shaped with obvious cytochromes in 26 cases; nevus cell nests merged with the surrounding epidermis in 4 cases, and obvious fissures were formed between nevus cell nests and the surrounding epidermis in 22 cases; no obvious cell atypia or mitotic figure was observed in any of the 26 cases, and Kamino bodies were observed in 6 cases. All the skin lesions were resected by surgery, and no recurrence was observed during the follow-up of 1 - 5 years.Conclusion:Reed nevi can manifest as irregular lesions, pagetoid distribution of spindle cells of various sizes can be observed histopathologically, and they need to be differentiated from melanoma.
7.Effect of NADPH oxidases inhibition on the development of interstitial fibrosis in unilateral ureter obstruction rats

Objective To clarify whether the NADPH oxidases (NOXs) family contributed to the reactive oxygen species (ROS) production and subsequent interstitial fibrosis in unilateral ureter obstruction (UUO) rats.Methods Male Wistar rats were randomly divided into sham operation group (n =8),sham operation + apocynin treatment group (n =8),UUO operation group (n =8) and UUO operation+apocynin treatment group (n =8).Either vehicle or apocynin (100 mg/kg per day) were given by gavage for 7 days after surgery.Rats were sacrificed at 7th day.ELISA was used to detect the activity of superoxide dismutase (SOD) and catalase (CAT),and the level of 8-iso-prostaglandin F2alpha (8-isoPGF2α) in renal tissue.Western blotting was used to detect the protein expressions of NADPH oxidase subunit NOX2 and NOX4,α-smooth muscle actin(α-SMA),collagen Ⅰ (COL-Ⅰ) and the level of ERK1/ 2 phosphorylation (p-ERK1/2).Results UUO rats with vehicle displayed increased oxidative stress,as measured by renal tissue 8-iso-PGF2α,accompanied with increased renal expression of NADPH oxidases (NOX2,1.5-fold and NOX4,1.7-fold,respectively),compared with sham-operated rats (P <0.05).Furthermore,vehicle treated UUO rats showed increased renal COL-Ⅰ and α-SMA levels,compared with sham-operated rats (P < 0.05).ERK1/2 was also activated as detected by p-ERK1/2 expression in UUO rats with vehicle (P < 0.05).Apocynin treatment significantly decreased renal tissue 8-iso-PGF2α level and expressions of NOX2 (-28.7%) and NOX4 (-31.0%) in UUO rats,respectively,compared with vehicle treated rats (P < 0.05).And significant decrease of COL-Ⅰ (-26.4%) and α-SMA expression (-80.0%) were also observed (P < 0.05).The activation of ERK1/2 in UUO rats was greatly inhibited by apocynin treatment (P < 0.01).Despite the pronounced dysregulation of pro-oxidative NOXs family,no compensatory increase of antioxidative enzyme activities occurred.Conclusion The NOXs family contributes largely to the production of ROS and subsequent interstitial fibrosis after ureter ligation,and inhibition of the NOXs family may be a choice for preventing interstitial fibrosis.

9.Immunohistochemical study on interstitial cells of cajal in Hirschsprung’s disease

Objective:To investigate the role of interstitial cells of cajal(ICC) in the pathogenesis of HD through observing the distribution of the cells in normal colon and HD's different segments.Methods:The aganglionic、transitional and dilated segments were collected in operations from 25 children with Hirschsprung’s disease.Another 6 normal segmengts were also collected.Each sections was stained by c-Kit immunohistochemistry.The distribution of the interstitial cells of Cajal was observed by light microscope.Results:The ICC in aganglionic segments of colon from Hirchsprung’s disease were decreased significantly and statistically than that in dilated and normal colon.Moreover,whole mount preparations showed that,not only the number of ICC-IM was decreased down,but also the branches of ICC-IM and the cellular network were also broken down.Conclusion:The pathogenesis of HD is related to the abnormal distribution of ICC,which lead to or aggravate the bowel spasm in HD.
10.Observation on Treatment of Advanced Esophageal Carcinoma with Three-Dimensional Conformal Radiotherapy Combined with Chemotherapy and Traditional Chinese Medicine

Objective To observe the clinical effect of treating advanced esophageal carcinoma with three-dimensional conformal radiotherapy combined with chemotherapy and traditional Chinese medicine.Methods 36 cases with advanced esophageal carcinoma were randomly divided into a eontrol group and a treatment group.The control group was treated with conventional radiotherapy combined with chemotherapy,and the treatment group was treated with three-dimensional conforrnal radiotherapy combined with chemotherapy and traditional Chinese medicine.The short-term effect,toxicity and long-term effect were observed,and cause of death was analyzed.Results The complete remission rate in the treatment group was significantly higher than that of the control group(P<0.05).The toxicity of the treatment group was markedly decreased.One and three years'survival rate between the two groups had a significant difierent(P<0.05),but five years'survival rate between the two groups had no significant difference(P>0.05).Local uncontrolled rate and recurrence rate in the treatment group were significantly lower than those in the control group(p<0.05).Conclusion The treatment of three-dimensional conformal radiotherapy combined with chemotherapy and traditional Chinese medicine on advanced esophageal carcinoma was better than conventional radiotherapy combined with chemotherapy.
