1.Effect on early tear film function of different adjustable suture in glaucoma trabeculectomy

Abstract?AlM:To compare the effects of different adjustable suture in glaucoma trabeculectomy on early tear film function.?METHODS:Sixty-eight cases of primary glaucoma ( 76 eyes) during January 2012 to June 2014 in our hospital were selected and divided into exposure conjunctival suture group ( 34 cases, 36 eyes ) and embedded conjunctival suture group ( 34 cases, 40 eyes ) according to treatment. Adjustable suture exposed conjunctival suture and embedding conjunctival suture was given to two groups, respectively. lntraocular pressure ( lOP ) before and after treatment 7, 14, 30d were observed and Schirmer test, tear break-up time, No Hikaru sensitivity and the occurrence of adverse reactions after treatment 1, 30d were recorded.?RESULTS: After the treatment, the mean lOP of two groups were decreased significantly ( P < 0. 05 ). The average lOP after treatment of 1d in the two groups were not statistically different (P>0. 05), after treatment 7, 14, 30d embedded conjunctival suture group was significantly higher than that of exposure conjunctival suture group ( P<0. 05). After 1d of treatment, Schirmer test, tear break-up time, No Hikaru sensitivity of two groups compared no significant difference (P<0. 05). After treatment 7, 14, 30d embedded conjunctival suture group Schirmer test, tear break- up time, was significantly superior to expose conjunctiva suture group (P<0. 05). The 30d package after treatment of conjunctival suture group buried adverse reaction rate was significantly lower than that of exposed conjunctival suture group (P<0. 05).? CONCLUSlON: Trabeculectomy operation with adjustable thread embedding conjunctival suture has few effects on the tear film function in patients with early postoperative complications, lower, and operation effect is better than that of exposed conjunctival suture.

Institute for International Medical Education(IIME) develops the concept of global minimum essential requirements,in which the knowledge and skills of"group health"are ranked as the important parts of training the qualified medical students. But in China,the medi-cal colleges almost run the schools according to the traditional teaching system. Whether teach-ing the knowledge of group health is consistent with the current personnel training requirements deserves our attention. Based on the investigation of teaching status about the medical students "group health"knowledge,the author finds the problems in the field of education with regard to the knowledge and skills and puts forward specific measures to improve it.
3.Spatial-temporal clustering analysis of soil-transmitted helminth transmission risk in Jiangxi, 2016-2019

Abstract: Objective To analyze the epidemic characteristics of soil-transmitted helminth (STH) by the spatial epidemiological method in Jiangxi Province. Methods Surveillance data of STH in Jiangxi Province from 2016 to 2019 were collected for descriptive epidemiological analysis. The space-time permutation distribution of STH infection areas was analyzed with spatial autocorrelation, hotspot analysis methods and retrospective space-time permutation scan statistics. Results The infection rates of STH were 1.32% (947/71 766), 1.14% (803/70 547), 0.87% (604/69 507) and 0.90% (628/69 620), respectively, and the differences were statistically significant in different years (P<0.01). The rates of hookworm infection were 0.65% (466/71 766), 0.64% (450/70 547), 0.48% (336/69 507) and 0.52% (362/69 620), respectively, and the differences were statistically significant (P<0.01). The infection rates of Ascaris lumbricoides were 0.55% (393/71 766), 0.41% (287/70 547), 0.32% (221/69 507) and 0.32% (225/69 620), respectively, and the differences were statistically significant in different years (P<0.01). The infection rates of Trichuris trichiura were 0.17% (125/71 766), 0.10% (74/70 547), 0.08% (57/69 507) and 0.07% (49/69 620), respectively, and the differences were statistically significant among different years (P<0.01). The results of global spatial autocorrelation analysis showed that there was a spatial autocorrelation index Moran's I value of hookworm infection rate in 2016, Trichuris trichiura infection rate in 2016, 2018 and 2019, with statistical significance (P<0.05). The results of hotspot analysis, SaTScan and FleXScan spatial and temporal scanning analysis showed that there were high-value aggregation regions for each species in each year. Conclusion The infection rates of STH, hookworm, Ascaris lumbricoides and Trichuris trichiura showed a decreasing trend and space-time clustering from 2016 to 2019. It should be taken into account in the formulation of control strategies.
4.Evaluation of the effectivenesss of noninvasive positive pressure ventilation as a weaning strategy in patients with acute respiratory failure

Objective To evaluate the effectiveness of noninvasive positive pressure ventilation(NPPV) as a weaning strategy in patients with acute respiratory failure after failure to wean from invasive positive pressure ventilation(IPPV). Method A prospective randomized and controlled clinical trial of weaning of IPPV was carried out in patients mechanically ventilated in mode of IPPV for more than 48 hours with failure in a spontaneous breathing trial(SBT: PSV 6 cmH_2O). Patients with contraindications to NPPV were excluded. After failure the SBT, patients were randomly divided(random number) in two groups. Patients in NPPV group were extubated after being ventilated with high pressure support for 30 minutes and then placed on NPPV. Patients in IPPV group were weaned following conventional procedure. Arterial blood gases, maximal inspiratory pressure, respiratory rate,tidal volume, rapid shallow breathing index, heart rate, arterial blood pressure, and peripheral oxygen saturation were measured before and after failing the SBT. The rate of complications, including pneumonia and tracheotomy duration mechanical ventilation, days of hospital stay and outcome were observed. Findings of the two groups were vompared using the Student t test and the chi-square test. Results The percentage of complications in the NPPV group was lower(22.9% versus 72.2%, P <0.01) ,with lower incidences of pneumonia(6.1%,36.1%; P <0.01) and tracheotomy. Compared between the two groups, days of ICU stay( 14.16(3.45) d vs. 22.57( 7.71 ) d; P <0.01) and total days of mechanical ventilation(14.88±3.76 days vs. 20.68± 2.79 days, P <0.01) of NPPV group are shorter than IPPV group. Conclusions NPPV is a good alternative to the mechanically venti-lated patients who fail in initial weaning attempts. The key to successful NPPV weaning is the proper selection of weaning candidates and using NPPV as soon as possible after extubation.
5.The effects of transcutaneous electrical nerve stimulation on pulmonary function, airflow, dyspnea, exercise capacity and TNF-α levels in patients with pulmonary obstruction

Objective To observe any effect of transcutaneous electrical nerve stimulation (TENS) on pul monary function,airflow obstruction,dyspnea,exercise capacity and levels of tumor necrosis factor-α (TNF-α) in the sputum of patients with chronic obstructive pulmonary disease (COPD).Methods Thirty patients with stage Ⅰ or Ⅱ COPD were randomly divided into a treatment group and a control group with 15 cases in each.TENS applied at the Feishu acupuncture point (BL13) was used in the treatment group,while sham stimulation of the same point without current output was used in the control group.The treatment was administered 40 minutes once daily,5 days a week for a total of 4 weeks.The outcome measures were measured before and after 4 weeks of treatment.Results FVC,FEV1,FEV1%,and peak expiratory flow rate (PEFR) all improved significantly in the treatment group after 4 weeks of treatment.After treatment,the average TNF-α level in induced sputum and exercise capacity as indicated by the average BODE index had both improved significantly in the treatment group.Compared with the control group,pulmonary function,TNF-α and the BODE index in the treatment group were all significantly better.There was no significant difference in pulmonary function,BODE index or TNF-α before and after treatment in the control group.Conclusion TENS at the Feishu acupoint can reduce airway inflammation,improve pulmonary function and exercise capacity,and decrease TNF-α levels in patients with stage Ⅰ or Ⅱ COPD.This treatment should be very useful for enhancing their ability in the activities of daily living.
6.Analysis on Cost-Effect of Three TCM Injections for the Treatment of Stroke

Objective To compare the clinical efficacy and carry out analysis on pharmacoeconomic cost-effect of three therapeutic schemes in the treatment for stroke. Methods Through a retrospective survey method, 115 stroke patients, based on different treatment methods, were divided into 3 groups:Xuesaitong group (A), carthamin yellow group (B), and Xueshuantong group (C). An analysis on pharmacoeconomic cost-effect was conducted. Results The costs of three therapeutic schemes were 1030.4 yuan, 1876 yuan, and 1545.6 yuan, respectively. The total effective rates of stroke patients in groups A, B and C were 85.37%, 88.57% and 90.04%, respectively. The cost-effect ratios of groups A, B and C were 12.07, 21.18, and 17.17. The added cost-effect ratios of groups B and C compared with group A were 264.25 and 110.32, respectively. Conclusion Xueshuantong Injection has more pharmacoeconomic advantage than Xuesaitong and carthamin yellow Injections in treating stroke.
7.Asymmetrical Perception of Facial Recognition on Race and Gender Judgment.

Objective To study face recognition rule of Chinese subjects during judging the differences of facial recognition of race,gender and their joint property.Methods Eye tracking technique and moving window experimental method were used.Results The present results showed that,in facial race recognition,gender recognition and joint property recognition,the key area of face recognition was eyebrow and eye.In face recognition process,the dominant area in face of information acquired was right facial area.The key brain area of facial cognition was right brain area.Conclusion The recognition performance of face racial recognition is the best than face gender recognition and joint property recognition.Asymmetrical perception effect in human face area is appeared in facial recognition.
8.Etiology of Community-acquired Lower Respiratory Tract Infections in Elderly in Guangzhou Area

OBJECTIVE To investigate the pathogenic causes of lower respiratory tract infections(LRTIs) in the elderly in Guangzhou area.METHODS Pathogens obtained from 107 patients with LRTIs were performed by multiple diagnostic tools that including bacterial culture,PCR and specific immunological assays.RESULTS A bacterial cause was established in 42(68.5%) and an atypical pathogen cause in 25(31.6%) of the 107 patients.Mycoplasma pneumoniae and Haemophilus influenzae remained the most important pathogens for LRTIs.CONCLUSIONS In the prescription of antibiotics in the elderly with LRTIs,not only bacteria but also atypical pathogens should be taken into account.
9.Determination of Related Substances in Lovastatin Tablet by HPLC

OBJECTIVE:To establish a method for the determination of related substances in Lovastatin tablet. METHODS:HPLC was performed on the column of Waters XTerra? MS C18 with mobile phase A of 0.01%Phosphoric acid solution and B of acetonitrile(gradient elution)at a flow rate 1.0 ml/min,column temperature was 40 ℃,the detection wavelength was 238 nm,and the injection volume was 10 μl. RESULTS:The impurity components were well separated in principal components;the linear range of lovastatin was 17.5-700 μg/ml(r=0.9999);RSDs of precision,stability and reproducibility tests were lower than 1%;recov-ery was 99.30%-100.67%(RSD=0.4%,n=9). CONCLUSIONS:The method is reproducible with good durability and high preci-sion,and can be used for the quality control of Lovastatin tablet.

This study is to investigate the activation effect of butyl-p-hydroxybenzoate (Bpb) on cAMP-dependent cystic fibrosis transmembrane conductance regulator (CFTR) chloride channel gating. A stably transfected Fischer rat thyroid (FRT) epithelial cell lines co-expressing human CFTR and a green fluorescent protein mutant with ultra-high halide sensitivity (EYFP) were used to measure CFTR-mediated iodide influx rates. Bpb was identified as an effective activator of wild-type CFTR chloride channel, it can correct delta F508-CFTR gating defects but not processing defect. Bpb can't potentiate G551D-CFTR channel gating. The activity was reversible and dose-dependent. The study also provided clues that Bpb activates CFTR chloride channel through a direct binding mechanism. Our study identified Bpb as a novel structure CFTR activator. Bpb may be useful for probing CFTR channel gating mechanisms and as a lead compound to develop pharmacological therapy for CFTR-related disease.
