Recently,with the development of the medical technique includes surgery,radiotherapy,chemotherapy and molecular targeted therapy,multi-subject combined modality therapy for esophageal carei noma has been become a tendency instead of surgery alone.Any single treatment is hard to improve the curative effect of treatment for esophageal carcinoma.Systemic individual comprehensive therapy for the different patients with esophageal carcinoma is likely to reduce recurrence rate,improve survival rates as well as to improve the quality of life.

Autoimmune pancreatitis (AIP)is a special type of chronic pancreatitis that originates from an autoimmune -mediated mecha-nism.AIP has unique radiological,serological,and histopathological features,often accompanied by peripancreatic lesions.AIP may be easily confused with pancreatic cancer and cholangiocarcinoma.It is necessary to diagnose AIP while integrating a variety of clinical indica-tors.Steroid therapy should be performed for patients diagnosed with AIP,and surgical treatment can be selected if necessary.
3.Application of high resolution magnetic resonance imaging in locally advanced rectal cancer

High resolution magnetic resonance imaging (HRMRI) has become an indispensable tool for multidisciplinary teams addressing locally advanced rectal cancer.Preoperative HRMRI is accurate in predicting tumor stage and poor prognostic features,such as circumferential resection margin and extramural venous invasion,et al.HRMRI is a precise diagnostic tool to select who may benefit from neo-adjuvant therapy and to avoid over treatment in those patients who can proceed directly to surgery,thus achieving better prognosis and quality of life for patients with locally advanced rectal cancer.
4.Comparision of Therapeutic Effects and Cost - effectiveness Analysis of Foscarnet and a - interferon for Chronic Hepatitis B

OBJECTIVE: To compare the clinical and economical effects of foscarnet and a - interferon in treating chronic hepatitis B.METHODS:90 cases were randomly divided into two groups.Group A,foscarnet in a dose of 2.4g,i. v. ,b.i .d . for 1 month . Group B, a - interferon in a dose of 3 million units, im, q . d . for 3 months . The clinical effects were observed and evaluated with pharmacoeconomic cost - effectiveness analysis .RESL LTS In improving symptoms and liver function, two therapeutic regimes presented similar effect.But lower expenses were costed in unit effect of decreasing ALT in group B.In clearance of virus index, group A showed better effects and costed lower expenses in unit effect of HBeAg negative conversion. CONCLUSION: Both foscarnet and a-interferon presented fairly good therapeutic effect for chronic hepatitis B.In improving liver functions, two preparations showed similar clinical effect but a - interferon presented better economical effect. In antiviral efficacy,foscarnet showed better clinical and economical effects.
5.Upper airway form and hyoid position of the adults with different vertical facial types

Objective:To study the difference of upper airway form and hyoid position of the adults with different vertical facial types.Methods:Cephalograms of 120 adults with different vertical facial types were retrospectively investigated by computerized cephalometric analysis.Upper airway depths and hyoid position were measured,and differences were compared between male and female,also compared among different vertical facial type groups.Results:The sagittal depth of upper airway in male was larger than that in female,but there was no statistical difference except V-LPW.Among different vertical facial types,the sagittal depths of upper airway in high angle group was smaller than that in average angle group and low angle group.There was statistical difference in hyoid position between genders,AH-SN,AH-FH and AH-C3 in male were larger than in female(P

7.Outpatients' Cognition on Adverse Drug Reactions

OBJECTIVE:To investigate outpatients' cognition(including knowledge,attitude and behavior)on adverse drug reactions(ADR),and to analyze the influential factors of ADR cognition.METHODS:An on-the-spot questionnaire survey was conducted in which a total of 488 outpatients sampled from our hospital from May to December in 2007 were investigated and the data analyzed statistically.RESULTS:The correct rate was 36.6% for the questions related to cognition on ADR.70.5% of the patients investigated had a poor cognition on ADR.The factors influencing patients' cognition on ADR included educational background,ADR experience,pharmacists' guidance on medication,source of medicine knowledge,occupation,doctors' guidance on medication,community ADR lecture,and gender.CONCLUSION:The outpatients have a poor cognition on ADR.Both doctors and pharmacists should offer guidance on medication and give lecture in community to inform patients the knowledge about rational administration and ADR.
8.Accuracy of the vascular ultrasound testing on detecting the arterial lesions in neck

ObjectiveTo evaluate the accuracy of the vascular ultrasound testing technique on detecting large arterial lesions in neck. MethodsVascular ultrasound testing and digital substraction angiography(DSA) were performed on 75 arteries in 53 consecutive patients with chronic cerebral ischemia, transient ischemic attack(TIA) or cerebral infarction.The sensitivity, specificity, and accuracy of vascular ultrasound testing on moderate stenosis(50%—69%), severe stenosis(70%—99%),or occlusion of arterial lesions in neck were analysed by means of cross tabulation, using DSA as the standard of reference. ResultsThe sensitivity, specificity and accuracy of vascular ultrasound testing on arterial lesions of moderate stenosis, severe stenosis and occlusion were 85.7%,78.9%,84.0%;90.2%,88.2%,89.3%;93.3%,95.0%,94.6%,respectively.The complete corresponding rate between the two type of tests was 73.3%. ConclusionThe vascular ultrasound testing technique shows high accuracy on detecting large arterial lesions, especially on severe stenosis or occlusion in neck. It can be applied extensively in prevention and treatment of cerebrovascular diseases.
9.Measurement and analysis of staffing standards of family doctor interdisciplinary team

Objective To measure and analyze the staffing standards of family doctor interdisciplinary team by means of the WHO workload indicator of staffing need (WISN)method,for reference of the government in building family doctor interdisciplinary teams.Methods 150 community health centers in 16 provinces were selected.The related data from 150 centers were collected to analyze the population served by each family doctor,and the staffing standards of family doctor interdisciplinary team by means of the WHO workload indicator of staffing need(WISN)method.Results There were 10 721 community health professionals in the sample centers which provide 132.14 million standard equivalents of service to the public.1 9 6 1 6 community health professionals were needed to ensure the quality of service and no extra workload after work for professionals.Averagely,each family doctor can serve 1 558 residents,who needs 1.3~1.5 nurses or public health workers.Conclusions A huge gap was found for community health professionals.The population served by each family doctor should be lowered compared to the governmental requirement and more nurses should be introduced to the family doctor interdisciplinary team.
10.The visual cortex plasticity of adult rat after binocular form deprivation by pattern visual evoked potential

Background The visual evoked potential (VEP) is an important functional index to assess visual cortex plasticity.Monocular form deprivation(MD) of rat is a classic model to study visual cortex plasticity change.Utilizing VEP technique record the shift of ocular dominance in adult rats model is of an important significance for explore the treating opportunities of amblyopia.Objective The present study was to observe the pattern VEP changes in different ages of Long-Evens rats and the adult rats after binocular form deprivation.Methods Thirty-six healthy SPF Long-Evens rats were divided into PW3,PW4,PW5,PW6 groups according to the postnatal weeks,and PW7 rats included 36 SPF Long-Evens rats.The left eye lids of the rats were sutured in PW3,PW6 and PW7 rats for 3,5,7 days respectively,and PVEP in both left and right eye were recorded to assess the rat age that visual cortex plasticity ended.Bilateral PVEP were recorded in PW7 rats,and the bilateral eyelids were sutured to establish the bilateral form deprivation models.The right eyes were opened in 7,10,14 days,and the left eyes were opened in the next 3,5,7 days respectively and the PVEPs were recorded again to find the shift of ocular dominance and whether binocular form deprivation induce the visual cortex plasticity in adult rats.Results The P100amplitudes of the left eyes were gradually declined and those of the right eyes were raised from 3 to 7 days after MD of the left eyes in rats of PW3 group in comparison with before MD ( P<0.01 ).In PW6 groups,no significant changes in P100amplitudes of both the right and left eyes were found in the third day after MD,but significant raise in the right eyes and lowing in the left eyes were seen in the 5 and 7 days after MD in comparison with before MD(P<0.01 ).No any P100changes in both eyes were found in 3-7 days after MD compared with before MD in PW7 rats (P>0.05).The C/I ratio (contralateral VEP amplitude to ipsilateral VEP amplitude in occluded eyes) in the rats of PW3,PW4 and PW5 groups were 1.07±0.15,1.16±0.16 and 1.14±0.15 respectively in 3 days after MD,showing the significant lowing in comparison with before MD (2.69±0.45,2.58±0.4,2.62±0.32) (P<0.01),but those of PW6 and PW7 were unchanged (2.80±0.48 vs 2.90±0.46,2.59 ±0.36 vs 2.90±0.46,P>0.05 ),indicating the absence of ocular dominance plasticity in the adult rats.In rats of PW7 with binocular form deprivation for 14 days,a significant decrease was observed in the C/I ratio in rats with next MD for 3 days,demonstrating that the visual cortex plasticity was reactivated after 14 days of binocular form deprivation in PW7 rats( 1.33±0.18 vs 2.70±0.45,P<0.01 ).Conclusions PVEP can be recorded in Long-Evens rats.It is a major index for identifying the shift of ocular dominance in the Long-Evens rats.Binocular form deprivation can reflect the visual cortex plasticity in the adult rats.
