1.Tissue-engineered nerve conduits with internal structure in the repair of peripheral nerve defects

BACKGROUND: To enhance peripheral nerve regeneration, the focus on the development of nerve conduits has been shifted from conventional conduits without internal structure to novel conduits with internal stuucture.Therefore, a comprehensive understanding of the internal structure of nerve conduits is necessary.OBJECTIVE: To review the application of tissue-engineered nerve conduits with internal structure in nerve regeneration.METHODS: A computer-based research of CNKI, WanFang, SinoMed and PubMed databases was performed to retrieve relevant literatures about the tissue-engineered nerve conduits with internal structure published from January 2009 to December 2015, using the keywords of nerve conduit/conduits/scaffolds/scaffold/channel/channels, internal scaffolds/scaffold, nerve regeneration/repair, hydrogel, acellularized/decellularized nerve in Chinese and English,respectively. Forty-seven eligible literatures were included for overview.RESULTS AND CONCLUSION: The novel nerve conduits with internal structure have many advantages over conventional conduits without internal structure, which can support, guide, promote peripheral nerve regeneration and functional recovery, especially, when modified with seed cells, neurotrophic factors or extracellular matrix, can achieve more effective results comparable to autologous nerve grafts.
2.Effects of Electroacupuncture on Expression of Glutamate Receptor Interacting Proteins 1 and 2 in Hippocampus of Rats with Alzheimer's Disease

Objective To investigate the effect of electroacupuncture on the expression ofα-amino-3-hydroxyl-5-methyl-4-isoxazole propionate (AMPA) receptor associated protein glutamate receptor interacting protein (GRIP)1 and GRIP2 in hippocampal neurons of rats with Alzheimer's disease (AD) model induced by Aβ25-35. Methods 40 Male Sprague-Dawley rats were randomly divided into normal group, sham operation group, model group and electroacupuncture group with 10 rats in each group. The AD rat model was prepared by injecting Aβ25-35 in the hippocampus CA1 of rats, while the sham operation group was injected with equal amount of normal saline at the same loca-tion. On the second day after successful modeling, the electroacupuncture group received electroacupuncture at Baihui (DU20) and bilateral Shenshu (BL23) acupoints, once a day, 6 times a week for 2 weeks. The expression of GRIP1 and GRIP2 were detected with immunohisto-chemistry. Results There was no difference in the expression of GRIP2 and GRIP1 proteins in hippocampus between the normal group and sham operation group (t<1.7438, P>0.05), but was lower in the model group and the electroacupuncture group than in the sham operation group (t>9.5928, P<0.001), and was higher in the electroacupuncture group than in the model group (t>9.5326, P<0.05). Conclusion Elec-troacupuncture may increase the number of AMPA receptors on the postsynaptic membrane by increasing GRIP1 and GRIP2.
3.Effects of electroacupuncture on learning and memory in experimental Alzeheimer′s disease

Objective To observe the effects of electroacupuncture ( EA) on learning and memory ability and the expression of N-ethylmaleimide-sensitive fusion factor ( NSF) in experimental Alzheimer′s disease ( AD) and to explore the mechanism involved. Methods Sixty healthy, male Sprague-Dawley rats were randomly divided into a normal group, a sham operation group, a model group and an EA group, each of 15.The model group and the EA group were injected with Aβ25-35 bilaterally in the CA1 area of the hippocampus, while the sham operation group was injected with the same amount of normal saline in the same regions. From the 1stday after the AD model had been es-tablished successfully, EA was applied to the Shen Shu ( BL23) and Bai Hui ( GV20) acupoints in the EA group once a day,6 days a week for 2 weeks. No EA was given to the other 3 groups. After the treatment, Morris water maze tests were conducted to determine assess the rats′ learning and memory ability, and immunohistochemical methods were used to test the expression level of NSF in the CA1 areas of the rats′hippocampus. Results The average es-cape latent period of the EA group was significantly shorter than that of the model group and their average platform crossing time was significantly longer. The average expression of NSF in the EA group measured as integrated optical intensity was significantly higher than that in the model group. Conclusion EA can effectively improve learning and memory ability in AD, at least in rats. The mechanism may involve increasing the expression of NSF in the CA1 area of the hippocampus.
4.Therapeutic Observation of Acupuncture-moxibustion for Postsurgical Gastroparesis and Effect on Gastrin and Motilin

Objective To observe the clinical efficacy of acupuncture-moxibustion in treating postsurgical gastroparesis syndrome (PGS) and the effect on gastrin (GAS) and motilin (MTL).Method Fifty PGS patients were randomized into a treatment group and a control group, 25 cases in each group. The control group was intervened by conventional medication, including effective gastrointestinal decompression, nutrition support, and psychotherapy; the treatment group was by acupuncture-moxibustion therapy in addition to the intervention given to the control group. The time of restoring gastric motility, the distention and fullness grading score, and serum GAS and MTL levels were observed, and the clinical efficacies were also compared.Result The total effective rate was 100.0% in the treatment group versus 92.0% in the control group, and the between-group difference was statistically insignificant (P>0.05). According to statistical analysis, there was a significant difference in comparing the time of restoring gastric motility between the two groups (P<0.01). The distention and fullness grading scores, GAS and MTL levels were significantly changed after intervention in both groups (P<0.01). The distention and fullness grading score, GAS and MTL levels of the treatment group were significantly different from that of the control group after intervention (P<0.01).Conclusion Acupuncture- moxibustion plus medication is an effective approach in treating PGS, and it can up-regulate the GAS and MTL levels.
5.Clinical characteristics of retinal detachment after vitrectomy

?AIM:To investigate the causes and characteristics of retinal detachment after vitrectomy.?METHODS: Retrospective analysis of patients in our hospital in recent 4a who underwent primary, standard, three -port pars plana vitrectomy ( PPV ) . Retinal detachment occurred in 24 eyes ( 23 cases ) , 24-67 years old, average 49.0±12.0 years old.There were male in 17 cases, female in 6 cases, 6 right eyes, 18 left eyes.?RESULTS:Eighteen (75%) eyes without any symptoms were found during a routine check, 6 eyes ( 25%) with decreased visual acuity or in front of shadow.It took 1-223d from PPV to retinal detachment, average 40.7 ± 54.2d, of which retinal detachment occurred in 10 (42%) eyes within 1wk, 14 (58%) eyes within 1mo.The direct cause of retinal detachment was new retinal break ( a total of 27 holes ) in 19 eyes, proliferative vitreoretinopathy ( PVR) in 4 eyes, 1 eyes with primary hole not closed.All retinal detachment were ultimately completely reset.?CONCLUSION: The occurrence of new retinal breaks is the main cause of retinal detachment after vitrectomy, attention should be paid to the early postoperative peripheral retinal examination;after a timely surgery, the retina can be reset.
6.Effects of monoclonal antibody of insulin-like growth factor-Ⅰ receptor on proliferation and apoptosis of papillary thyroid carcinoma cells

Objective To investigate the role of monoclonal antibody of insulin-like growth factor Ⅰ receptor (IGF-ⅠR) in the proliferation and apoptosis of papillary thyroid carcinoma cell line CGTH W-3, and the effect of IGF-ⅠR monoclonal antibody on papillary thyroid carcinoma cells and normal follicular cells in primary culture. Methods MTT and TUNEL method were used to determine the proliferation and apoptosis of cells respectively after IGF-ⅠR monoclonal antibody intervention. Results The inhibitory effect on cell proliferation was enhanced with the increasing IGF-ⅠR monoclonal antibody concentration and duration of action. IGF-ⅠR monoclonal antibody induced apoptosis of CGTH W-3 cells remarkably even in the concentration of 50 nmol/L. Primary papillary thyroid carcinoma cells and normal follicular cells were inhibited by 100 nmol/L IGF-ⅠR monoclonal antibody, and the degree of inhibition was significantly higher in carcinoma cells than that in normal follicular cells (P
7.The outcomes and prognoses of in-hospital sudden cardiac death

Objective To explore the incidence,features and outcomes of in-hospital sudden cardiac death (SCD) in order to determine the predictors of survival. Methods The clinical data of 69 patients with cardiac arrest hospitalized from January 2008 through December 2010 were retrospectively analyzed.Information on genders,age,types of arrhythmia was collected and further analyzed to determine these factors associated with the occurrence and outcomes of in-hospital cardiac arrest. Results The overall incidence of SCD was 47.3 / 100 000 per year and 17.4% of them.survived at discharge.The occurrence rate was higher in male than that in female (66.7% vs.33.3%,P <0.01 ),whereas difference in gender did not affect the discharge rate ( P > 0.05 ). Survivors from in-hospital cardiac arrest were significantly younger than non-survivors (man:62.57 ± 12.83 years vs.75.56 ± 10.55 years; women:60.36 ± 13.24years vs.69.53 ± 11.72 years,P < O.01 ).From 62 ECG records of SCD patients,the incidence of nonshockable rhythms was higher than that of shockable rhythms.Compare to the non-shockable rhythms,the shockable rhythms brought a higher rate of restoration of spontaneous circulation (ROSC) (54.5% vs.24.5%,P <0.05),whereas survival rates at discharge between two groups were not statistically different ( 18.2% vs.18.4%,P > 0.05 ).Conclusions Non-shockable rhythms were more common in patients suffering from in-hospital cardiac arrest.Although defibrillation treatment contributed benefit to ROSC among patients with ventricular fibrillation or pulseless ventricular tachycardia,high-quality CPR and post-cardiac arrest care may play a more critical role in the outcomes of in-hospital sudden cardiac death.
8.Differentiation of amniotic fluid-derived fetal mesenchymal stem cells into osteoblasts

BACKGROUND: Amniotic fluid cells have been widely used in antenatal diagnosis for gene mutation-related diseases. However,there are few reports concerning isolation, culture, surface character identification, differentiation and application perspective of fetal mesenchymal stem cells (MSCs) derived from amniotic fluid.OBJECTIVE: To investigate the differentiation of fetal mesenchymal stem cells (MSCs) derived from second-trimester amniotic fluid into osteoblasts in vitro.DESIGN, TIME AND SETTING: The cytological/n v/tro study was conducted at the Experimental Center of Xinhua Hospital from August 2005 to May 2006.MATERIALS: Ten amniotic fluid samples were obtained from pregnant women (18-22 weeks after conception) or aborted women,The informed consents were obtained from pregnant women.METHODS: Fetal MSCs were separated mechanically from amniocyte culture system and expanded in medium in vitro. At passage 3, fetal MSCs were induced in 100 nmol/L dexamethasone, 10 mmol/L β-glycerophosphoric acid and 50 mg/L vitamin C for 14 days.MAIN OUTCOME MEASURES: Collagen Ⅰ and alkaline phosphatase expression was detected by immunohistochemistry.Collagen Ⅰ protein expression was determined by Western Blot analysis. Calcium tuberoses were measured by Von Kossa staining. The cytoskeletal protein was detected by laser confocal microscopy.RESULTS: The isolated fetal MSCs were uniformly positive for CD44 and HLA-ABC, negative for CD34, CD45 and HLA-DR. After being induced with osteogenic medium for 14 days, 91% cells were positive for alkaline phosphatase, and 87% cells for collagen Ⅰ.Cells expressed collagen Ⅰ protein. Number of calcium tuberoses was increased and became big over time. Cytoplasm microfilament presented green fluorescence and the microfilament surrounding cells formed dense bundle.CONCLUSION: Fetal MSCs derived from amniotic fluid could be induced into osteeblasta and displayed a typical osteoblastic morphology and biological characteristic.
9.Analysis of therapeutic effect of hyperbaric oxygen therapy on neuropsychological disorders in senile patients with type 2 diabetes mellitus combined with cerebral infarction

Objective To explore the therapeutic effect of hyperbaric oxygen therapy (HBO) in the treatment of neuropsychological disorders in senile patients with type 2 diabetes mellitus combined with cerebral infarction. Methods A total of 70 senile patients with type 2 diabetes mellitus combined with cerebral infarction were randomly divided into two groups: treatment group receiving HBO plus routine medicine treatment and control group receiving routing medicine treatment only. A suit of senile neuropsychological tests was administered pre and post treatment by an experienced psychometrician who was blinded to treatment group assignments. Results Neuropsychological disorders were improved partly in control group while the efficacy in treatment group was markedly improved(P<0.01). Spearman rank correlation analysis revealed that the improvement degree of neuropsychological disorders was positively correlated with hyperbaric oxygen therapy (HBO) (r= 0.502,P<0.05), the time of test before therapy (r=0.260, P<0.05) and the time of test after therapy (r=0.386, P<0.05), and was negatively correlated with hyperlipoidemia (r=-0.261, P< 0.05), glycosylated hemoglobin(GHbAlc) (r=-0.321,P<0.05) and body mass index(BMI) (r= 0.360,P<0.05). Multiple stepwise regression analysis showed that HBO and hyperlipoidemia were the independent predictors for the improvement degree of neuropsychological disorders. Conclusions HBO is an efficacious way to treat neuropsychological disorders in senile patients with type 2 diabetes mellitus combined with cerebral infarction.
10.The diagnostic value of axially loaded magnetic resonance imaging in patients with degenerative disorders of lumbar spine

Objeetive To evaluate the diagnostic value of axially loaded MR imaging with supine position in patients with degenerative disorders of lumbar spine.Methods Thirty asymptomatic volunteers and 89 patients were examined in psoas-relaxed position(PRP)and axially compressed supine position(ACE)of the lumbar spine.Sixty-one patients with low back pain,19 with sciatica and 9 with neurogenic claudication were included in the symptomatic study group.The disc levels from L3 to S1 were examined.Results In 30 asymptomatie volunteers,a significant decrease in dural sac cross-sectional area(DSCA)was found at 14 disc levels(15.6%)in 10 individuals(33.3%)during ACE(>15 mm2).In 89 patients.a significant decrease in DSCA was found at 55 disc levels(20.6%)in 38 patients(42.7%)during ACE(>15 mm2),and the mean decrease was 28 mm2.During ACE,32 disc levels with an increasing severity of disc herniation were noted in 26 patients.16 disc levels with neural foramen stenosis were found in 12 patients,11 disc levels with ligamentum flavum thickening were observed in 10 patients,3 cases facet dislocation and 3 cases lumbar spondylolisthesis were also seen.In 22 of the 89 patients(24.7%).additional valuable information(AVI)was found during ACE,including 7 patients(7/9)with neurogenic claudication,8 patients(8/19)with sciatica,and 11.5%(7/61)of the patients with low back pain.Conclusion As compared with conventional imaging methods,axially loaded imaging provides AVI,and more occult lesions can be found during ACE.ACE MRI is a valuable tool in diagnosing degenerative disorders of lumbar spine.
