Journal of Kunming Medical University 2014;(2):86-89
Objective To investigate the incidence of abnormal thyroid function in pregnant women and explore the significance of thyroid function screening in early pregnancy. Methods We randomly selected 630 cases of pregnant women who received routine inspection in the obstetric clinic in our hospital, and gave them five items of thyroid function screening besides routine check at the same time. Then we statistically analyzed the test results, and followed up 61 cases of pregnant women with hypothyroidism or subclinical thyroid function (hypothyroidism) to learn the treatment and adverse pregnancy outcomes,so as tto explore the correlation between hypothyroidism, subclinical hypothyroidism and adverse pregnancy outcomes. Results In 630 cases of pregnant women, there were 8 cases of hyperthyroidism (1.3%),61 cases of hypothyroidism or subclinical hypothyroidism (9.7%),including 5 cases of hypothyroidism (0.8%), 56 cases of subclinical hypothyroidism (8.9%) .There was statistically significant difference in TSH and FT4 between hypothyroidism group and normal control group ( <0.01) .There was statistically significant difference in TSH,FT4 and TT4 between subclinical hypothyroidism group and normal control group ( <0.05) . In 61 cases of hypothyroidism and subclinical hypothyroidism, 39 patients received levothyroxine therapy,22 received no treatment. The incidence of adverse pregnancy outcomes in the treatment group was significantly lower than the untreated group ( <0.05) . Conclusion Thyroid function testing in 6-20 weeks in early pregnancy has important significance in timely founding gestational thyroid dysfunction, so it should be as routine examination project.

AlM:To compare the results of lOL Master, contact and immersion A-scan ultrasound measurements for anterior chamber depth ( ACD ) , and evaluate the difference and consistency.METHODS:Fifty-eight cases (98 eyes) with age-related cataract during July to October in 2013 did the A-scan ultrasound with contact and immersion measurements and lOL Master to get the results of ACD. Difference in measurements between methods was assessed using the variance analysisi. Consistency was assessed using Bland-Altman.RESULTS:The ACD measured by lOL Master was 2. 31~3. 90mm, the mean was 3. 03 ± 0. 38mm. The ACD measured by contact A- scan ultrasound was 1. 51 ~4. 06mm, the mean was 2. 88 ± 0. 56mm. The ACD measured by immersion A-scan ultrasound was 1. 99 ~4. 17mm, the mean was 3. 17±0. 38mm. The results of lOL Master and contact A - scan ultrasound had statistical differences (P=0. 022<0. 05). The results of lOL Master and immersion A - scan ultrasound had statistical differences (P=0. 031<0. 05). The results of contact A-scan ultrasound and immersion A-scan ultrasound had statistical differences (P=0. 000<0. 05). The consistency between three methods was poor. CONCLUSlON: The rank of ACD of patients with cataract is immersion A-scan ultrasound, lOL Master and contact A-scan ultrasound. The consistency is poor, and the three methods can’t be interchanged clinically.

Journal of Leukemia & Lymphoma 2021;30(1):31-37
Objective:To investigate the association of human leukocyte antigen (HLA) class Ⅰ (A, B and C), class Ⅱ (DRB1, DQB1 and DPB1) allelic and haplotypic polymorphisms with acute lymphoblastic leukemia (ALL), acute myeloid leukemia(AML) and chronic myeloid leukemia (CML) in Han nationality of southern China. Methods:The peripheral blood samples of 845 leukemia patients (323 cases of ALL, 350 cases of AML and 172 cases of CML) and 745 healthy blood donors from Han nationality of southern China in Shenzhen Blood Center were collected. The HLA-A, -B, -C, -DRB1, -DQB1 and -DPB1 genes were genotyped by using polymerase chain reaction-reverse sequence specific oligonucleotide (PCR-rSSO) and polymerase chain reaction-sequence based typing (PCR-SBT) methods to identify the first 4 digits of HLA alleles. The Arlequin 3.5 software was used to analyze HLA haplotypes. The correlations between HLA allelic and haplotypic polymorphisms and three types of leukemia were statistically analyzed at HLA low-resolution level (the first 2 digits of alleles) and high-resolution level (the first 4 digits of alleles), respectively.Results:P-values were adjusted by Bonferroni correction. In ALL group, the frequencies of A*02 (36.22% vs. 28.26%, χ 2 = 13.41, PC < 0.01) and its haplotype A*02-B*46-C*01 (15.35% vs. 10.23%, χ 2 = 10.90, PC = 0.02), DRB1*12 (15.79% vs. 11.10%, χ 2 = 9.02, PC = 0.03), A*02:03 (9.75% vs. 5.32%, χ 2 = 14.25, PC = 0.002) and its haplotype A*02:03-B*38:02-C*07:02 (3.80% vs. 1.51%, χ 2 = 10.41, PC = 0.02) were higher than those in healthy controls, which implied that these factors could confer risk effect in ALL. In AML group, the frequency of A*11-B*15-C*08-DRB1*15-DQB1*06-DPB1*02 (1.34% vs. 0.07%, χ 2 = 12.54, PC = 0.003) was significantly higher than that in healthy controls, suggesting that the risk effect might be conferred by this haplotype. In CML group, the frequencies of A*02 (36.63% vs. 28.26%, χ 2 = 9.33, PC = 0.02) and its haplotype A*02-B*15-C*04 (2.17% vs. 0.29%, χ 2 = 11.74, PC = 0.02), and DRB1*03:01-DQB1*02:01-DPB1*02:01 (1.86% vs. 0.14%, χ 2 = 13.10, PC = 0.01) were higher than those in healthy controls, which implied that these factors could confer risk effect in CML, whereas the frequency of DRB1*13 (1.45% vs. 5.25%, χ 2 = 9.29, PC = 0.03) was lower than that in healthy controls, suggesting that it was a CML antagonistic gene. Conclusion:Leukemia susceptible or antagonistic HLA alleles and haplotypes are found at low-resolution and high-resolution levels of HLA, which might provide reference for investigating the pathogenesis of leukemia and guiding formulation of effective treatment strategies in Han nationality of southern China.

AIM:To compare the efficiency of fibrin glue to suture technique in pterygium surgery performed with limbal autograft under different methods of anesthesia.
METHODS: A prospective randomised clinical trial was carried out in 60 eyes of 55 patients operated for primary nasal pterygium, which were divided into two groups randomly: experimental group ( 30 eyes in 27 patients ) was under surface anesthesia ( oxybuprocaine ) and control group ( 30 eyes in 28 patients ) was under local anesthesia ( 20g/L lidocaine ). Autologous limbal graft taken from the superotemporal limbus was used to cover the sclera by a fibrin tissue adhesive after pterygium excision. Patients were followed up at least for 6mo. Time of operation, matching degree of graft and VAS score were mainly observed and recorded.
RESULTS: In experimental group the average surgery time was shorter (P=0. 008) and matching degree of graft ( 93%) was better than control group ( 83%) , the differences had statistical significance(P<0.05).
CONCLUSION: The surface anesthesia is enough when using fibrin glue for graft fixation in pterygium surgery, which will shorten surgery time and get better matching degree of graft.

[Summary] Drug naive, newly diagnosed type 2 diabetic subjects were randomized to Saxagliptin/Metformin / Rosiglitazone(Triple Therapy, n=23) or insulin 70 30 mix group(Intensive Insulin Therapy) (n=21) for 24 weeks. How did the 2 therapies influence fasting blood glucose, fasting insulin, C-peptide, and glucagon levels and the change of body weight were compared. This study was aimed to explore the comparative glycemic efficacy and impact on α/ β-cell function of two different antidiabetic therapies, triple combination therapy using saxagliptin/metformin/ rosiglitazone and intensive insulin therapy, for newly diagnosed type 2 diabetes mellitus. The results indicated that fasting blood glucose, HbA1C , insulin resistance index 2(HOMA 2-IR), glucagon and body mass index level were significantly decreased, and insulin secretion index 2 ( HOMA 2-% β) was increased significantly( P <0. 05) in triple therapy group, and the decreasing extent of HOMA 2-IR, glucagon, and body mass index were significantly greater than that in the intensive insulin group(P<0. 05). Triple therapy group has a stronger effect of reducing insulin resistance, as well as on inhibiting glucagon and promoting weight loss.

Teaching rounds is an important part of clinical teaching but it is difficult to implement in place for a variety of reasons. To endocrine and metabolic section as an example, Lack of enthusiasm, inadequate preparation and other issues result that teaching rounds is difficult to achieve the desired effect in the teaching rounds, especially in the English teaching rounds. In view of the above situation, based on the concept of PBL to carry out teaching rounds in English is not only an important method to cultivate the clinical thinking of medical students, but also an important measure to improve teachers' and students' English proficiency, as well as to cultivate medical students' clinical thinking. The last but not the least, carrying out teaching rounds in English is an important measure for medical education reform and improve-ment. Facts have proved that carrying out teaching rounds in English has a high practicality and feasibility in clinical practice.
