Objective To assess the necessary excision of normal parenchyma tissue in nephron sparing surgery for renal cell carcinoma. Methods 102 specimens from radical nephrectomy for RCC were step sectioned at 3 mm intervals and examined. The tumor and field 20 mm beyond pseudocapsule were continuously sectioned and examined for completeness of pseudocapsule, the presence of micro multifocal carcinoma and for vascular and parenchyma invasion beyond pseudocapsule. Results 49 (48.0%) of the 102 specimens were void of intact pseudocapsule. The presence of extra pseudocapsule cancerous lesions was within 0~8 mm [(1.2?1.9) mm] beyone the capsule, with 30.4% of them within the field of 1~8 mm.with the statistic method of one side analysis of frequency, the percentile value of P97.5 and P100.0 was 7.4 and 8.0 mm respectively. Conclusions These data denote that when nephron sparing surgery is done for renal cell carcinoma,at least 10 mm of normal parenchyma tissue beyond the pseudocapsule should be excised with the tumor. The nucleation techniquc should not be encouraged.

0.4).ConclusionsThese data denote that biological behaviors and malignant features of multifocal tumors are very similar to those of primary ones.In addition, considering the significant relationship of multicentricity with vascular invasion and incompleteness of pseudocapsule of RCC,it is suggested that the secondary tumors might be more likely the result of intrarenal metastasis of the primary tumor rather than the results of multifocal genesis.

Objective To evaluate the efficacy of endoscopic injection hemostasis and sequential application of sandostatin in treatment of Dieulafoy's lesion. Methods After diagnosis of Dieulafoy's lesion confirmed, endoscopic injection hemostasis(EIH) together with mixture hypertonic saline and epinephrine(HSE) was performed, and then sandostatin was given consecutively for 7 days. The control group included the patients who were not given sandostatin or given just for 1 to 2 days. Results The rates of recurrent bleeding and transfer into surgery in sandostatin group were significantly lower than those in control group,(12.5% and 6.3% versus 53.8% and 46.2% respectively, P

Medical Journal of Chinese People's Liberation Army 1982;0(03):-
Objective To elucidate the clinical features and treatment of hemangiopericytomas (HPCs) in different parts of the body, especially in CNS. Methods Clinical data of 41 cases with HPC which were treated at 301 Hospital from 1993 to 2004 were retrospectively analyzed with a review of relevant literature. Follow-up periods ranged from 4 to 150 months after surgery (mean 34 months). Results Among 41 patients of HPC, 21 tumors (51.2%) were found in CNS, and the rest 20 (48.8%) in other systems. Thirteen tumors were diagnosed as malignant HPC. Surgery for tumor removal was performed in all 41 patients. Among the 62 operations, total removal of tumor was achieved in 49 cases and subtotal removal in 13 cases. Recurrence in original site of the tumor occurred 21 times (33.9%) in 14 patients. Five patients (12.1%) presented one or more distant metastasis, and 5 patients died. HPCs in CNS and in other systems displayed similar clinical characteristics and pathological morphology. Conclusions HPC is a rare angioneoplasm that may arise in any part of the body. It is a great challenge to neurosurgeons as well as other surgeons to surgically manage the tumor. HPC has an unfavorable prognosis because its high rate of recurrence and metastasis. HPC in CNS and in other parts of the body should be recognized as one disease. At the same time, HPC in CNS has its distinct features.
