1.Ultrastructural Study on Apoblema in Termination of Early Pregnancy by Pai Pei Tang and Mifepristone

To elucidate the mechanism of Pai Pei Tang in combination with mifepristone in termination of early pregnancy, samples of apoblema (5 each) were collected from the Pai Pei Tang group, mifepristone group, Pai Pei Tang and mifepristone group (N = 68) and routine artificial abortion group (N = 22). We found that atrophy of the decidua and chorion was the utmost in the Pai Pei Tang and mifepristone group under the electron microscope. The result suggested that Pai Pei Tang could induce degenerative and atrophic changes in the decidua and affect the chorion directly.

3.Differential diagnosis of hepatic tumors with dynamic grey-scale enhanced ultrasonography

Objective To evaluate the value of dynamic grey-scale enhanced ultrasonography in the differential diagnosis of hepatic tumors.Methods Twenty-four patients with 25 hepatic solid lesions were examined by dynamic enhanced ultrasonography with a bolus injection of Levovist (400 mg/ml). Results All lesions were confirmed by operation and pathology, including 16 hepatocellular carcinomas (HCC), 3 hepatic hemangiomas (HCH), 1 inflammatory pseudotumor of liver (IPL), 2 focal nodular hyperplasias (FNH) and 3 angiomyolipomas (AML). Twenty-five lesions were variously enhanced on grey-scale ultrasonogram after injection of Levovist except for IPL. The lesions with HCCs were enhanced early in the arterial phase and disappeared quickly in portal phase. The lesions with HCHs were enhanced slowly in the peripheral region of the lesion. The FNH and AML were also enhanced early in arterial phase, but disappeared slowly in delay phase. If the hepatic lesions showed the earlier enhancement in arterial phase and fast disappearance in portal phase on contrast enhanced ultrasonogram were regarded as a criterion of malignant tumor, the sensitivity, specificity and accuracy with contrast enhanced ultrasonography in the diagnosis of hepatic malignant tumor were 94%(15/16),89%(8/9)and 92%(23/25), respectively. Conclusions The dynamic grey-scale enhanced ultrasonography is a useful technique in the differential diagnosis of hepatic tumors.
4.Complications of femtosecond laser corneal small incision lenticule extraction

AIM:To investigate the safety and complications of femtosecond laser corneal small incision lenticule extraction(SMILE) procedure and discuss the prevention and treatment.METHODS: We retrospectively studied the complications of 403 patients(799 eyes) with myopia and myopic astigmatism treated by SMILE.RESULTS: All the patients underwent the operation successfully.Only 1 case(1 eye)suffered from dark spot and changed to femtosecond laser-assisted LASIK(FS-LASIK), 5 cases(5 eyes, 0.6%) suffered from the suction loss, 11 cases (17 eyes, 2.1%) developed opaque bubble layer.All patients gained perfect uncorrected visual acuity(UCVA) (20/20).The best corrected visual acuity(BCVA) did not decrease after operations.The incidence of haze and diffuse lamellar keratitis was low (0.3% and 0.4%, respectively)and no other complications were observed.There was 9 eyes in 6 patients (1.1%) found regression of refraction at 6mo after surgery, while the UCVA of rest patients reached 1.0 at 3mo after surgery.CONCLUSION: The SMILE procedure has high safety for myopia and myopic astigmatism.Effective prevention and management of the complications is the key to achieve the satisfactory visual acuity.
5.Clinical study of the clusters of diffuse lamellar keratitis after laser corneal refractive surgery

AIM: To investigate the potential causes and management of the clusters of diffuse lamellar keratitis (DLK) after laser corneal refractive surgery.METHODS: The study enrolled 98 eyes (53 patients) complicated with DLK after receiving laser in situ keratomileusis (LASIK), FS-LASIK or small-incision lenticule extraction (SMILE) in our center from February 10th,2016 to February 22th,2016.They were given clinical classification treatments according to corneal layer inflammatory extent and then followed up after 1, 3, 5, 7, 10d and 1mo.RESULTS: The clusters of DLK occurred 5 times in the study period.The incidence and degree of DLK significantly decreased after changed the sterilization, surgical equipments, temperature and humidity of the operating room.There were 80 eyes (82%) had stage 1 DLK, 11 eyes (11%) had stage 2, 4 eyes (4%) had stage 3 and 3 eyes (3%) had stage 4.The incidence of DLK after FS-LASIK was 40% (79 eyes in 42 patients), that after LASIK assistant by Hastome keratome was 45% (10 eyes in 5 patients), that after SMILE was 20% (9 eyes in 6 patients).After intensive treatment, as glucocorticoid treatment and flap lifting flushing, all cases recovered within 1mo.CONCLUSION: The outbreak of DLK may be associated with the disposable item, flushing liquor, temperature and humidity of the operating room.Early diagnosis, prevention and treatment are the key of decreasing the incidence of DLK.

8.Construction of a predicting model of uncertainty in illness for the postoperative patients with digestive system tumors undergoing chemotherapy

Objective Construction of a predicting model of uncertainty in illness based on Mishel's theories of uncertainty in illness in a sample of the postoperative patients with digestive system tumors undergoing chemotherapy.Methods By convenient sampling method,MUIS-A,MSAS,SSRS were used to evaluate 484 postoperative patients with digestive system cancer during chemotherapy period,and then AMOS 17.0 software was used to perform analysis.Results The score of uncertainty in illness of postoperative patients with the digestive system cancer undergoing chemotherapy was in the medium level (89.86+11.84) points; Structure provider (social support,economic income) could not only reduce the uncertainty directly,but also had an indirect effect on the uncertainty by stimuli frame (symptoms pattern,event familiarity).Conclusions Structure equation model can be applied to the uncertainty in illness model construction,so as to help medical staff manage uncertainty in illness pertinently.
9.Relationship between Changes of Umbilical Blood Biochemical Indexes and Fetal Distress

Objective To study the changes of umbilical blood lactate,pH, blood sugar(BS),bilirubin, electrolyte, osmotic pressure (OP) in the newboms with fetal distress.Methods Thirty-five newborns with fetal distress (distress group) and 40 healthy new-borns (control group) were studied. Distress group were divided into distress group Ⅰ and distress group Ⅱ respectively, based on without or with neonatal asphyxia. Concentration of umbilical blood lactate was determined with enzyme method, pH, BS,serum total bilirubin (BIL), serum electrolyte (Na+ ,K+ ,Ca2+ ) and OP were analyzed respectively. Results 1. The difference of incidence of newborn asphyxia between distress groups (29.03%) and control group (2.50%) was statistically significant. 2. Compared with the controls and distress group Ⅰ, the umbilical blood lactate concentration significantly increased in distress group Ⅱ (P 0.05).The incidence of hyperglycemia was significantly elevated in distress groups than that in the control group. 4. Lactate concentration in distress I and distress fl group showed negative correlation with pH. Conclusion The concentration of umbilical blood lactate can provide the proof for diagnosis and prognosis of fetal distress.
