OBJECTIVETo explore a new method to reconstruct the defect of the distal foot.
METHODSA distally based dorsum pedis island flap pedicled with the first dorsal metatarsal artery was designed and transferred to the defect of the distal foot.
RESULTSFive patients were treated with this flap, which ranged from 2 cm x 4 cm to 6 cm x 7 cm in size. Four flaps survived completely, one flap had marginal necrosis and healed after conservative therapy.
CONCLUSIONSThe reverse first dorsal metatarsal artery flap, with good blood circulation and easy manipulation, is a good option for the defects of the distal foot.

OBJECTIVETo improve the survival of mesenchymal stem cells (MSCs) for prefabrication of an osteo-musculo-cutaneous flap.
METHODSIn a mouse model, the compound of MSCs and p(3HB-co-3HH) were embeded in the latissimus dorsi muscle as an experimental group and the muscle pocket of the buttock as the control. The examinations of the HE staining, hybridization in situ of osteonectin mRNA and Von kossa staining were used to evaluate the results.
RESULTSThe expression of osteonectin mRNA and the Von kossa staining showed that the latissimus dorsi muscle group was superior to the control in 2 weeks and 4 weeks after the surgery in vivo.
CONCLUSIONThe results indicate that the above-mentioned technique may be a good alternative for the prefabrication of the osteo-musculo-cutaneous flap.

OBJECTIVETo establish a method for analyzing solid phase extraction-high performance liquid chromatography-electrospray ionization tandem mass spectrometry (SPE-HPLC-ESI-MS(n)), in order to recognize and identify the main chemical constituents in Qinghuo Zhimai tablets.
METHODThe possible structures of the compounds were determined according to the structure information of compounds observed from molecular ion peaks and fragment ions in HPLC-ESI-MS(n) negative ion mode and by comparing with literature data or control samples.
RESULTThrough the comparative analysis on Qinghuo Zhimai Tablets and components of its formula, 39 chemical constituents were identified, including 7 caffeoylquinic acids, 7 iridoids, 6 diterpenoid lactones, 5 homoisoflavonoids, 13 steroidal saponins and 1 flavone glycoside.
CONCLUSIONThis study provides a simple and rapid method for identifying chemical components in Qinghuo Zhimai tablets.

Objective To investigate the feasibility of self-made capacitation liquid for in vitro fertilization of Mongolian gerbils, and to provide a reference for gerbil embryo cryopreservation. Methods In vitro fertilization of Mongo?lian gerbil was performed with the self?prepared capacitation solution and semen, and the 2?cell embryos of Mongolian ger?bils were cultured in vitro using an improved KSOM culture medium. Results The in vitro fertilization rate of gerbils was over 60%, and some gerbil 2?cell embryos could develop further in vitro. Conclusions An in vitro fertilization and embry?o development system of Mongolian gerbil has been established, but it needs further optimization.
8.Expression of CD44, TRIM24, TAGLN-2, ER and PR in breast invasive ductal carcinoma and their clinicopathologic significance

Purpose To study the expression of biomolecules of PI3K-AKT signaling pathway including CD44,TRIM24,TAGLN-2,ER and PR in breast invasive ductal carcinoma,and to explore their clinicopathologic significance.Methods The expression of CD44,TRIM24,TAGLN-2,ER and PR in 73 cases of breast invasive ductal carcinoma were detected by immunohistochemist~ (IHC) technology.And the relationship between the expression of these biomarkers and the age of patients,tumor size,histological grade,lymph node metastasis was analyzed.Besides,the influence of these biomarkers on prognosis and the relationship between the expression of these biomarkers were also analyzed.Results There was no significant relationship between the expression of these biomarkers and the age of patients,tumor size,histological grade,lymph node metastasis.CD44 expression was positively conelated with TAGLN-2 expression (r =0.311,P =0.007),TRIM24 expression was positively correlated with TAGLN-2 expression (r =0.421,P =0.000).CD44 expression was negatively correlated with ER expression (r =-0.285,P =0.015).ER expression was positively correlated with PR expression (r =0.598,P =0.000).The postoperative 5-year cumulative survival-rate of CD44 positive group was lower than those of CD44 negative group (P =0.002),in contrast,the postoperative 5-year survival rate of ER positive group was higher than those of ER negative group (P =0.026).Conclusion Through PI3K-AKT signaling pathway,CD44 and TRIM24 may up-regulate TAGLN-2 expression,however,CD44 may down-regulate ER and PR expression.CD44 and ER may act an useful predictor for the prognosis of breast invasive ductal carcinoma.Combined detection of CD44,ER and PR may provide additional therapeutic information which may be useful in breast cancer treatment.

OBJECTIVE:
To observe the occurrence time of neuralgia and the expression of purinergic ligand-gated ion channel 7 receptor (P2X7R) in the dorsal horn of the spinal cord after intraperitoneal injection of streptozotocin (STZ) in diabetic rats, and to explore the effect of electroacupuncture (EA) and pretreatment of EA on the heat pain threshold and expression of P2X7R in the spinal dorsal horn in rats with diabetic neuropathic pain (DNP), and to explore the possible mechanism of EA for DNP.
METHODS:
PartⅠ: Thirty male SD rats were randomly selected from 64 male SD rats as the control group; the remaining rats were given intraperitoneal injection of STZ (10 mg/mL) at a dose of 65 mg/kg to establish the diabetes model, and 30 rats were successfully modeled as the model group. The control group and the model group were divided into three subgroups respectively at 7, 14 and 21 days, with 10 rats in each subgroup. Body mass, fasting blood glucose (FBG) and thermal pain threshold were recorded at 7, 14 and 21 days after injection; the expression of P2X7R in spinal dorsal horn was detected by Western blot. PartⅡ: Eight SD rats were randomly selected from 35 male SD rats as the blank group, and the remaining 27 rats were given intraperitoneal injection of STZ (10 mg/mL) at a dose of 65 mg/kg to establish the diabetes model. The 24 rats with successful diabetes model were randomly divided into a DNP group, an EA group and a pre-EA group, 8 rats in each group. Fifteen to 21 days after STZ injection, the EA group received EA at "Zusanli" (ST 36) and "Kunlun" (BL 60), continuous wave, frequency of 2 Hz, 30 min each time, once a day; the intervention method in the pre-EA group was the same as that in the EA group. The intervention time was 8 to 14 days after STZ injection. The body mass, FBG and thermal pain threshold were recorded before STZ injection and 7, 14 and 21 days after STZ injection; the expression of P2X7R in spinal dorsal horn was detected by Western blot 21 days after injection.
RESULTS:
PartⅠ: Compared with the control group, in the model group, the body mass was decreased and FBG was increased 7, 14 and 21 days after STZ injection (P<0.01), and the thermal pain threshold was decreased 14 and 21 days after STZ injection (P<0.05), and the expression of P2X7R in spinal dorsal horn was increased 7, 14 and 21 days after STZ injection (P<0.05, P<0.01). PartⅡ: Compared with the blank group, in the DNP group, the body mass was decreased and fasting blood glucose were increased 7, 14 and 21 days after STZ injection (P<0.01). Compared with the DNP group, in the pre-EA group, the heat pain threshold was increased 14 and 21 days after STZ injection (P<0.05), while in the EA group, the heat pain threshold was increased 21 days after STZ injection (P<0.01), and the expression of P2X7R in the dorsal horn in the EA group and the pre-EA group was decreased (P<0.01).
CONCLUSION
The diabetic neuropathic pain is observed 14 days after STZ injection. EA could not only treat but also prevent the occurrence of DNP, and its mechanism may be related to down-regulation of P2X7R expression in the dorsal horn of the spinal cord.

BACKGROUND:
Shenyankangfu Tablet (SYKFT) is a Chinese patent medicine that has been used widely to decrease proteinuria and the progression of chronic kidney disease.
OBJECTIVE:
This trial compared the efficacy and safety of SYKFT, for the control of proteinuria in primary glomerulonephritis patients, against the standard drug, losartan potassium.
DESIGN, SETTING, PARTICIPANTS AND INTERVENTION:
This was a multicenter, double-blind, randomized, controlled clinical trial. Primary glomerulonephritis patients, aged 18-70 years, with blood pressure ≤ 140/90 mmHg, estimated glomerular filtration rate (eGFR) ≥ 45 mL/min per 1.73 m
MAIN OUTCOME MEASURES:
The primary outcome was change in the 24-hour proteinuria level, after 48 weeks of treatment.
RESULTS:
A total of 735 participants were enrolled. The percent decline of urine protein quantification in the SYKFT group after 48 weeks was 8.78% ± 2.56% (P = 0.006) more than that in the losartan 50 mg group, which was 0.51% ± 2.54% (P = 1.000) less than that in the losartan 100 mg group. Compared with the losartan potassium 50 mg group, the SYKFT plus losartan potassium 50 mg group had a 13.39% ± 2.49% (P < 0.001) greater reduction in urine protein level. Compared with the losartan potassium 100 mg group, the SYKFT plus losartan potassium 100 mg group had a 9.77% ± 2.52% (P = 0.001) greater reduction in urine protein. With a superiority threshold of 15%, neither was statistically significant. eGFR, serum creatinine and serum albumin from the baseline did not change statistically significant. The average change in TCM syndrome score between the patients who took SYKFT (-3.00 [-6.00, -2.00]) and who did not take SYKFT (-2.00 [-5.00, 0]) was statistically significant (P = 0.003). No obvious adverse reactions were observed in any group.
CONCLUSION:
SYKFT decreased the proteinuria and improved the TCM syndrome scores of primary glomerulonephritis patients, with no change in the rate of decrease in the eGFR. SYKFT plus losartan potassium therapy decreased proteinuria more than losartan potassium therapy alone.
