2.Clinical study of standing bed for the orthostatic hypotension after cervical spinal cord injury

ObjectiveTo observe the therapeutic effect of standing bed on orthostatic hypotension with cervical spinal cord injury. Methods36 patients with cervical spinal cord injury were divided randomly into the standing bed group and routine treatment group. The blood pressure of all patients were measured every day during rehabilitation treatment. The function of spinal cord was assessed before and after treatment.ResultsThe function of sensory and motor in the two groups were all improved, but there was no statistical difference between the two groups in the grades of spinal cord function. There were 27 patients who had orthostatic hypotension in the study. All the patients with complete spinal cord injury suffered the orthostatic hypotension. There was no improvement in blood pressure of complete spinal cord injury after treatment. In incomplete spinal cord injury patients, there was a significant improvement and the difference of blood presure between lying and standing was decreased in standing bed group after treatment (P<0.05). Compared with routine treatment group, the blood pressure in standing bed group was improved (P<0.05). Conclusions Cervical spinal cord injury could have orthostatic hypotension. Complete spinal cord injury would suffer orthostatic hypotension in different degree. The standing bed treatment could improve the orthostatic hypotension in incomplete spinal cord injury.
3.Expression of Growth Arrest and DNA Damage Inducible Gene 45 ? during Preadipocyte Differentiation and Regulative Role of Tumor Necrosis Factor-Alpha

Objective To investigate the changes of growth arrest and DNA damage inducible gene ?(GADD45?)expression during 3T3-L1 preadipocyte differentiation and analyze the regulative role of tumor necrosis factor-alpha(TNF-?) on GADD45? gene expression in matured 3T3-L1 adipocytes.Methods 3T3-L1 preadipocytes were cultured in vitro and differentiated into the matured adipocytes.TNF-? in different concentrations(0.1,1.0,10.0 ?g/L) was added into the culture medium of fully differentiated adipocytes(day 10) for various times(0.5,2,6,12,24 h).The levels of GADD45? gene mRNA expression were evaluated by reverse transcription polymerase chain reaction(RT-PCR).Results With the 3T3-L1 preadipocytes being differentiated into the matured adipocytes,the level of GADD45? gene mRNA expression was downregulated and reached the lower level in fully differentiated adipocytes.There was a significant difference between any 2 detected phases in the levels of GADD45? gene mRNA expression(P0.05).Treatment of day 10(3T3-L1) adipocytes with TNF-? of different concentrations resulted in a significant increase in the level of GADD45? gene mRNA expression.The induction effect of TNF? on GADD45? gene mRNA expression generally tended to be reinforced with the elongation of time course.Conclusions GADD45? gene may be involved in adipocyte differentiation and adipogenesis.TNF-? can upregulate the mRNA expression of GADD45? gene in matured adipocytes.The induction effect of TNF-? on GADD45? gene expression is generally(time-)correlated.

6.Effect of Nimodipine on hemorrheology and BAEP in the patients with vertebrobasilar insufficiency

ObjectiveTo observe the effect of Nimodipine on the hemorrheology and brainstem auditory evoked potentials (BAEP) in the patients with vertebrobasilar insufficiency (VBI) and to explore the mechanism.Methods50 cases with VBI were divided into Nimodipine group (25 cases) and routine therapy group (25 cases). The hemorrheology and BAEP were measured before the treatment and 1 month later.ResultsThe blood viscosity,including the whole blood viscosity shear value, plasma viscosity and blood fat of patients with VBI was increased. The total abnormity rate of BAEP was 76%. The main abnormity was brainstem type. The hemorrheology and the function of nerve conduction were improved distinctly (P<0.05) after treatment. Compared with the routine therapy group, the level of plasma viscosity was decreased markedly (P<0.05) in Nimodipine group, and peak latency of V wave, interpeak latency of III-V and I-V were also improved significantly (P<0.05).Conclusions Nimodipine can improve the hemorrheology and the function of nerve conduction in patients with VBI.
7.Variety of corneal endothelial cell in glaucoma by confocal microscope

ATM: To define the causes of corneal endothelial cell damage, to investigate the preventive methods, and to observe the variety of corneal endothelial cell in glaucoma using confocal microscope.
METHODS: Totally, 143 eyes of 97 patients with different types of glaucoma, and matched normal people were 20 cases, all 40 eyes. The cell density, cell area and cell variable coefficient were measured used confocal microscope. These indicatives of every kind of glaucoma were compared.
RESULTS: The corneal endothelial cell density of normal group was 2 893. 88±255. 026/mm2 , the group of acute angle-closure glaucoma ( AACG ) was 1 674. 11±683.95/mm2 , and the group of open angle glaucoma (OAG) was 2687. 22±391. 87/mm2, the group of chronic angle-closure glaucoma (CACG) was 2706. 97±351. 27/mm2. In all index the average cell density of corneal endothelial and the average area have statistical significance ( F =62.950, 8. 795;P=0. 000), especially the group of AACG.CONCLUSION: The index of corneal endothelial cell in AACG is lower than that of normal. All index in OAG and CACG is difference with that of normal, but the difference has no statistical significance. And the dominant factor of damaged corneal endothelial is the time of intraocular hypertension.
8.Clinical study of electro-acupuncture treatment with different intensities for functional constipation patients.

Functional constipation (FC) is a common functional bowel disorder disease that affects life quality of a large number of people. This study aimed to explore the impact of different intensities of electro-acupuncture (EA) treatment for FC patients. Totally, 111 patients with FC meeting the Rome III criteria were randomly assigned to different intensities of EA groups (low and high intensity of EA groups) and medicine-controlled (MC) group. In EA groups, patients were treated with EA at quchi (LI11) and shangjuxu (ST37) bilaterally for 4 weeks, 5 times/week in the first 2 weeks, and 3 times/week in the last 2 weeks. In MC group, 5 mg mosapride citrate was administered orally 3 times/day for 4 weeks. Spontaneous bowel movement frequency each day was recorded using a constipation diary. Self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used to assess the patients' psychological state. Cortisol (CORT), substance P (SP), and vasoactive intestinal polypeptide (VIP) were evaluated at baseline and at the end of 4 weeks after treatment. As compared with the baseline, there was statistically significant increase in stool frequency every week (P<0.01), but there was no statistically significant difference among the three groups. As compared with the baseline, after 4 weeks of EA therapy, the scores of SDS and serum levels of CORT were decreased significantly in low intensity of EA group (P<0.01), and the serum levels of SP and VIP were increased significantly (P<0.05); the scores of SAS and SDS and serum levels of CORT were decreased significantly in high intensity of EA group (P<0.05), and the serum levels of SP and VIP were increased significantly (P<0.05); the serum levels of CORT and VIP were increased significantly in MC group (P<0.05). As compared with MC group, after 4 weeks of treatment, the serum levels of SP were signifcicantly increased in low intensity of EA group (P<0.01). Low and high intensities of EA could increase the stool frequency, improve the FC patient's anxiety and depression, reduce the serum levels of CORT, and increase the serum levels of SP and VIP effectively. It is concluded that both low and high intensities of EA are effective for FC patients, but there is no significant difference between the low and high intensities of EA.
9.The MRP1 expression in childhood acute lymphoblastic leukemia and its clinical significance

Objective Multidrug resistance-associated protein 1 (MRP1) has been reported with a close correlation with tumor multi-drug resistance. Real-time quantitative PCR (QRT-PCR) was performed to detect the MRP1 gene expression in childhood acute lymphoblastic leukemia (ALL) and its clinical signiifcance was analyzed. Methods Sixty-seven denovo ALL patients and 10 healthier children as bone marrow donor were studied. The chemotherapy was given according to CCLG-2008 protocol. SPSS software was employed to analyze the data and p-value below 0.05 was regarded as statistic signiifcance. Results MRP1 expression level showed a close correlation with ALL risk, the median of MRP1 expression was 4.28 (2.75~6.12), 5.62 (4.99~8.60) and 7.56 (3.66~11.13) for standard-risk group (SR), intermediate-risk group (IR) and high-risk group (HR), respectively. MRP1 mRNA expression in T-ALL group was 7.71 (6.49~14.35), which is higher than that of B-ALL (5.18(3.89~8.46)) (P<0.01). The rate of leukemia cells’ sensitivity to prednisone on 7th day was 70.6%in high expression group (n=34), which was signiifcantly lower than that in low expression group (n=33, 90.9%, P=0.035). The complete remission rateon 33th day was 64.7%in high expression group, and 87.9%in low expression group, which showed a signiifcant difference between them (P=0.026). Conclusions In children ALL, the expression of MRP1 is closely related with immunophenotyping, treatment response, hazard level and disease relapse.
10.Effects of interventional therapy with norcantharidin microsphere on hepatoma in rats and its mechanism

OBJECTIVE: To investigate the effects of interventional therapy with norcantharidin-alginic acid/poly acid anhydride microspheres (N-MS) infusion via hepatic artery on hepatoma in rats. METHODS: N-MS was prepared by emulsion-chemical crosslink technique. Eighty-nine hepatoma-bearing rats were randomly divided into five groups, which were normal saline group, norcantharidin (NCTD) group, blank microsphere (B-MS) group, NCTD-lipiodol group and N-MS group. Normal saline, NCTD, B-MS, NCTD-lipiodol and N-MS were injected via hepatic artery accordingly. After the interventional therapy, eight rats from each group were observed for survival time, and the rest rats were killed on the 8th day after intervention to measure the tumor volume and necrostic degree. The apoptotic index of liver tumor cells was detected by TUNEL staining, and the expression of ki-67 was assayed by immuno-histochemical streptavidin-biotin peroxidase method. RESULTS: The survival time of the rats in the N-MS group was prolonged as compared with those in the other four groups, and the tumor volume of the rats in the N-MS group was smaller than those in the other four groups. The tumor growth rate and the expression level of ki-67 in the N-MS group were both significantly lower than those in the other four groups. The tumor necrotic degree and the apoptotic index in the N-MS group were significantly higher than those in the other four groups. CONCLUSION: Interventional therapy with N-MS could yield preferable therapeutic effects on hepatomas in rats. This anti-tumor efficacy may be associated with microvessel embolization in liver tumor and the sustained releasing of NCTD. Its inhibiting effect on tumor cell proliferation maybe result from decreasing the expression of Ki-67 and inducing the tumor cell apoptosis.
