1.Effects of Group Exercise Program Tailored by Physical Fitness on Perceived Health Status, Physical Strength, Depression and Cognitive Function of the Elderly.

PURPOSE: The purpose of this study was to identify the effects of the tailored group exercise program on perceived health status, physical strength, depression and cognitive function in seniors. METHODS: The subjects were 42 seniors of the experimental group and 28 seniors of the control group. The exercise program tailored by physical fitness was carried out for 16 weeks. The dependent variables were measured by Perceived Health Status Scale, The Senior Fitness Test, MMSE-K, and GDS-K. The collected data were analyzed by Fisher's exact test, Chi-square test and t-test, and ANCOVA using the SAS program. RESULTS: In terms of physical strength and depression there was significant difference among groups. Perceived health status and cognitive function were not significant different among groups. CONCLUSION: In this study, the tailored exercise program was effective and safe for the elderly, and resulted in improving physical and psychological health status in the elderly. This demonstrated that the tailored group exercises program on the basis of the subject's fitness, played an essential role in maintaining and improving the health.

2.The Efficacy of Core Strengthening on the Trunk Balance in Patients with Subacute Stroke.

OBJECTIVE: To investigate the efficacy of core strengthening on trunk balance in patients with subacute stroke. METHOD: Fifty-nine subacute hemiparetic stroke in-patients participated in this study. All patients were randomly assigned to one of two groups: the control group and the experimental group. Both groups received the same conventional physical therapies for 4 weeks. The experimental group received an additional core strengthening program (CSP). Korean mini-mental state examination (K-MMSE) and Korean Modified Barthel Index (K-MBI) were performed on admission. CSP was performed for 30 minutes a day, 3 days per week for 4 weeks. We selected nine exercises available for stroke patients, and divided them as three steps according to the degree of difficulty. Because of the muscle weakness in stroke patients, a physical therapist often helped them during exercise. Outcome measurement included trunk control test (TCT), trunk impairment scale (TIS), and Berg balance scale (BBS) and we compared both groups before and after the period of training. RESULTS: There were no significant differences in baseline general characteristics and initial values between the two groups. After rehabilitation therapy, both groups showed significant improvements in BBS, TIS, and TCT (p<0.05). Following 4 weeks of therapy, changes of BBS, TIS score in the experimental group were significantly higher than those of the control group (p<0.05). However, the change of TCT score was not significantly different. CONCLUSION: Analysis of different clinical parameters showed that better outcomes in BBS, TIS scores were achieved from CSP. This study suggests that CSP conducted for 4 weeks may be beneficial as a therapeutic technique for use in improvement of trunk balance in patients with subacute hemiparetic stroke.

The benefits of exercises and physical activities have been proven for all-cause mortality, cardiovascular morbidity and many illnesses. To workout safely, medical examination and exercise testing should be done prior to exercise. Cardiac disease and musculoskeletal problems should be ruled out by medical examination and test. The result of exercise testing is the basic data for exercise prescription because of wide range of fitness on the same age and sex. Cardiopulmonary endurance, muscle strength and endurance, flexibility and balance ability are the basic components of fitness measurement. Appropriate method to assess fitness among various methods can be used. Moderate aerobic exercise is recommended to be done more than 30minutes a day, 5 times a week. Recently 150 minutes of moderate exercise a week is being recommended. Vigorous aerobic exercise more than 20 minutes a day 3 times a week or 75 minutes a week is recommended. Muscle strengthening and endurance exercise is recommended to be done 2~3 times a week

7.Establishment of comprehensive evaluation models of physical fitness of the elderly based on machine learning.

The present study aims to establish comprehensive evaluation models of physical fitness of the elderly based on machine learning, and provide an important basis to monitor the elderly's physique. Through stratified sampling, the elderly aged 60 years and above were selected from 10 communities in Nanchang City. The physical fitness of the elderly was measured by the comprehensive physical assessment scale based on our previous study. Fuzzy neural network (FNN), support vector machine (SVM) and random forest (RF) models for comprehensive physical evaluation of the elderly people in communities were constructed respectively. The accuracy, sensitivity and specificity of the comprehensive physical fitness evaluation models constructed by FNN, SVM and RF were above 0.85, 0.75 and 0.89, respectively, with the FNN model possessing the best prediction performance. FNN, RF and SVM models are valuable in the comprehensive evaluation and prediction of physical fitness, which can be used as tools to carry out physical evaluation of the elderly.

8.Lower plasma selenoprotein P levels in regularly exercising young adults

Objective:
Physical exercise can provide many health benefits in humans. Exercise-induced reactive oxygen species (ROS) formation and its downstream signaling cascades are reported to induce mitochondrial biogenesis in exercising tissues. Selenoprotein P (SELENOP) is the antioxidant hepatokine whose hypersecretion is associated with various metabolic diseases. It was reported to impair exercise-induced reactive oxygen species signaling and inhibit subsequent mitochondrial biogenesis in mice. However, the relationship between selenoprotein P and mitochondrial dynamics in humans has not yet been reported. While reduction of plasma selenoprotein P becomes an attractive therapeutic target for metabolic diseases, the role of regular exercise in this regard is still unknown. This study aimed to analyze the influence of regular habitual exercise on plasma selenoprotein P levels and its association with leucocyte mitochondrial DNA copy number in healthy young adults.
Methodology:
Plasma selenoprotein P levels and leucocyte mitochondrial DNA copy numbers were compared in 44 regularly exercising subjects and 44 non-exercising controls, and the correlation between the two parameters was
analyzed. Plasma selenoprotein P levels were measured by Enzyme-linked Immunosorbent Assay, and leucocyte
mitochondrial DNA copy numbers were measured using the qPCR method.
Results:
The regular-exercise group had lower plasma selenoprotein P levels with higher leucocyte mitochondrial DNA copy numbers than the non-exercise group. There was a tendency of negative correlation between the two variables in our studied population.
Conclusion
Regular habitual exercise has a beneficial effect on reducing plasma selenoprotein P levels while raising mitochondrial DNA copy numbers.

9.Association of Angiotensin Converting Enzyme I/D and alpha-actinin-3 R577X Genotypes with Growth Factors and Physical Fitness in Korean Children.

This study analyzed the differences in aerobic and anaerobic exercise ability and growth-related indicators, depending on the polymorphism of the ACE and the ACTN3 genes, to understand the genetic influence of exercise ability in the growth process of children. The subjects of the study consisted of elementary school students (n=856, age 10.32+/-0.07 yr). The anthropometric parameters, physical fitness and growth factors were compared among groups of the ACE I/D or the ACTN3 R577X polymorphisms. There were no significant differences between the anthropometric parameters, physical fitness and growth factors for the ACE gene ID or the ACTN3 gene R577X polymorphism. However, the DD type of ACE gene was highest in the side step test (p<0.05), and the DD type was significantly higher than the II+ID type (p<0.05) in the early bone age. The combined group of the ACE gene II+ID and the ACTN3 gene XX type significantly showed lower early bone age (p< 0.05). This study did not find any individual or compounding effects of the polymorphism in the ACE I/D or the ACTN3 R577X polymorphisms on the anthropometric parameters, physical fitness and growth factors of Korean children. However, the exercise experience and the DD type of the ACE gene may affect the early maturity of the bones.

10.Electrocardiographic T-wave inversion among young, asymptomatic, pre-employment, "normal" males: A report of 7 cases.

OBJECTIVES: 1. To report cases of "normal", asymptomatic, young, pre-employment males without significant cardiovascular risk factors but with electrocar diographic (ECG) T-wave inversion in multiple leads; and 2. To emphasize the importance of overall clinical correlation in interpretation of the ECG and in overall diagnostic and management decisions.
SETTING: Outpatient urban pre-employment clinic
CASE SUMMARY: Seven young (44 years of age), pre-employment, asymptomatic, male cases with resting ECG findings of T-wave inversion in multiple leads but with normal physical, radiographic, and cardiosonographic cardiac anatomy are herein reported. All cases did not have significant cardiovascular risk factors and were able to uneventfully hurdle at least 9 METs of monitored treadmill exercise. All cases were eventually cleared for employment and presumably enjoying a productive life; nobody among them was repatriated.
CONCLUSION: Clinical presentation is the more important arbiter of cardiovascular/physical fitness than any isolated ECG finding.
