1.Association between aquaporins expression in kidney tissue and edema of nephrotic syndrome patients

ObjectiveTo elucidate the association between aquaporins (AQPs) expression in kidney tissue and edema of nephrotic syndrome(NS) patients.MethodsNS patients were divided into edema group (14 cases) and non-edema group (8 cases).Ten patients without NS were used as control group.Expressions of AQP1,AQP2,and AQP4 in renal tissues of 3 groups were detectedbyimmunohistochemistrywithstandardtechniquesandsemi-quantitativeanalysis.Association between AQPs expression and edema was examined.ResultsThe positive index of AQP1 expression in proximal tubules in edema group was 0.0373±0.0110,which was significantly lower as compared to non-edema group (0.0510±0.0120) and control group 0.0574±0.0100),while the difference between non-edema and control groups was not significant.The positive index of AQP1 expression in glomerulus was 0.0106±0.0037 in edema group,which was significantly higher than that in non-edema group(0.0021±0.0013) and control group(0.0020±0.0012),while no significant difference was found between the last two groups.AQP2 mainly localized in the collecting duct system.The positive indexes of AQP2 expression were 0.0498±0.0081,0.0370± 0.0072 and 0.0255±0.0103 in edema group,non-edema group and control group,respectively.The differences were significant among 3 groups.AQP4 expression was not found in the renal cortex and collecting duct system.ConclusionsAQPs expression is different in renal tissues of NS patients.AQP2 may play an important role in the edema of NS patients,and AQP1 may involve in the occurrence of edema.
2.The primary study of postmortem interval estimation by the lipid in puparium cases of Diptera:Calliphoridae

Objective To explore the value of lipidic degradation in puparium cases of C.megacephala during weathering in postmortem interval estimation. Methods The puparium cases of reared C.megacephala were collected, and laid outdoor. They were taken back 5 days and 10 days later, and then be stored at -20℃. The samples of control group were cleaned and cryopreserved immediately. The spectra were collected and preprocessed. The peak position and peak height were read and performed statistical analysis by SPSS 19.0 software. Results Compared to control group, the positions of Vas CH3 band showed no shift, and the Vas CH2 band showed blue shift, meanwhile, the Vs CH2 band disappeared in both experimental groups, moreover, the Vs CH3 band showed red shift only in 10d group;except of the Vs CH3 band in 5d group, the intensities of both the other two lipidic bands reduced in both experimental groups. Compared with 5day group, the Vs CH3 band showed red shift in 10d group, meanwhile, the differences of all the lipidic bands had statistical signiifcance. Conclusion Detection of lipidic degradation in puparium cases of C. megacephala during weathering by micro-FTIR provides a novel way to estimate postmortem interval and performs its potential for forensic applications.
3.Comparisons of the prevalence and correlation factors of chronic kidney disease between the old urban and rural residents in MinHang district of Shanghai

Objective To compare the prevalence and correlation factors of chronic kidney disease (CKD) in urban and rural areas in Minhang district of Shanghai through the social economic and clinical data of the elderly population.Methods Jiangchuan Street and Pujiang town were randomly selected to represent the urban and rural population in Minhang district of Shanghai,respectively.Based on the over-60-year old people health examination program,6151 objectives with complete clinical-epidemiological data and bio-chemical index were investigated.The prevalence of CKD in urban and rural areas was compared,and the correlation factors for the urban and rural CKD were evaluated by multiple logistic regression analysis.Results (1) The survey objectives with an average age of (69.57+7.04) years,including 4345 cases of the city residents and 1806 cases of rural residents,were enrolled.The age structures of urban and rural showed differences,population over 80 years old account for 13.1% of the rural total,significantly higher than 7.4% in the urban population (P < 0.001).(2) The prevalence rates of diabetes,hyperuricemia,hyperlipidemia and hyperlipidemia in urban residents were higher than those in rural residents,which were 26.4% vs 13.7%,9.9% vs 2.3%,53.7% vs 37.4%,51.4% vs 15.6% (all P< 0.01).The awareness rates of kidney disease and hyperlipidemia showed significant differences in urban and rural areas,which were 32.9% vs 44.2%,84.6% vs 62.8% (all P < 0.01).Compared with those in rural areas,the treatment rates of hypertension and high blood lipids in urban residents were increased (all P < 0.01).(3) The prevalence of CKD was 23.4%.Female CKD prevalence was higher than male,respectively 26.3% and 18.5% (P < 0.01).In urban CKD prevalence was 22.2%,lower than 25.2% in rural.The prevalence rate of hematuria in urban areas was lower than in rural areas,but the prevalence rate of decline in renal function was higher (all P < 0.05).With the increase of age,the prevalence rate of CKD was increased (P < 0.01).(4) Age (OR=1.072),smoking history (OR=1.543),previous history of kidney disease (OR=1.351),diabetes (OR=1.373),hyperuricemia (OR=2.498),obesity (OR=1.364),history of interventional therapy (OR=1.896) had positive correlation with CKD in city elderly population,while the higher education (OR=0.676,OR=0.604) and drinking (OR=0.585) had negative correlation (all P < 0.05).Age (OR=1.032),female (OR=1.860) had positive correlation with CKD in rural elderly population (all P < 0.05).Conclusions CKD has been a common chronic progressive disease of the aged in Minhang district.The prevalence of CKD is higher in urban areas than in rural.Age is a common factor for CKD in urban and rural.Previous smoking,history of kidney disease,diabetes,hyperuricemia,obesity,history of interventional therapy,education and drinking have correlation with urban CKD patients.Female has correlation with rural CKD population.
4.Study of Bupropion Hydrochloride Tablet and Fluoxetine in Treatment of Depression Multicenter Clinical Trial

It has similar effect and safety compared with fluoxetine in the treatment of depression.
5.Factors analysis on circadian rhythm of interdialytic blood pressure in maintenance hemodialysis patients

Objective To describe the circadian profiles of interdialytic blood pressure in maintenance hemodialysis patients and to investigate its related factors. Methods Ambulatory blood pressure monitoring (ABPM)was conducted in forty-four patients on regular hemodialysis during interdialytic days.Three groups were identified with their ambulatory blood pressure:non-hypertension group,controlled hypertension group and uncontrolled hypeaension group.Hemoglobin,creatinine,Kt/V,serum total calcium,serum phosphate,immunoreactive parathyroid hormone(iPTH),interdialytic weight gain and Morisky self-report scale etc.were assessed. Results (1)No significant difference was found in ABPM index ranged from decline in nocturnal systolic and diastolic blood pressure,AASI to dipper percentage in three groups.Nevertheless,24-h average pulse pressure in uncontrolled hypertension group was higher than the other groups[(80.06+13.41)vs(53.00±7.73),(57.85±21.97)mm Hg,all P<0.01].(2)All the three groups showed the blood pressure profile of double-peak and trough.In the uncontrolled hypertension group,however,the nocturnal blood pressure decrease was not notable.(3)Nocturnal systolic blood pressure decline Was negatively correlated to iPTH(r=-0.039,P<0.05).(4)In ten dippers out of all 44 patients,AASI was negatively correlated with nocturnal diastolic blood pressure decline (r=-0.748,P<0.05). Conclusion Nocturnal blood pressure decline is correlated to iPTH and arterial selerosis.
6.Control of hypertension in patients with chronic kidney disease and its associated factors

Objective To investigate the management and control of hypertension in patients with chronic kidney disease(CKD) and its associated factors.Methods Data of 726in-patients with CKD and hypertension who hospitalized in our hospital from March 2009 to April 2010 were studied.Results 91.74% of patients was treated with antihypertensive medications,and 21.21%, 22.59%, 19.56%, 28.37% of patients received 1, 2, 3, ≥4 antihypertensive drugs,respectively.42.4% of patients with CKD and hypertension could be controlled up to the standard,and the mean blood pressure was(137.86±20.75)/(76.30±11.35) mm Hg.There was significant difference among stage 1 plus 2, 3, 4 plus 5 (non-dialysis), 5 (dialysis) kidney diseases, with the hypertension control rate being 50.8%, 46.7%, 42.0%, 33.5%, respectively.The hypertension control rate of non-dialysis patients was significantly higher than that of dialysis (44.9% vs 33.5%,P<0.05).There was no significant difference between blood dialysis group and peritoneal dialysis group(32.3% vs 38.7%, P>0.05).Multivariate Logistic regression analysis showed that female (OR=1.787, 95%CI 1.045-3.056)and ACEI application (OR=4.378, 95%CI1.830-10.472) were positively associated with hypertension control.Whereas, diabetes (OR=0.415, 95%CI 0.188-0.919)and pulse pressure (OR =0.847, 95% CI 0.811-0.885) were associated with inadequate blood pressure control.ConclusionsDespite almost universal hypertension treatment is used in patients with CKD and high blood pressure, the hypertension control rate is still suboptimal.Female and ACEI are positively associated with adequate hypertension control, whereas diabetes and pulse pressure are negatively associated with the standard.
7.Epidemiological and molecular characterization of seasonal influenza A/H3N2 viruses circulating in Shenzhen, 2005 - 2007.

OBJECTIVETo determine the epidemiological characteristics of seasonal influenza in Shenzhen from 2005 to 2007 and the molecular variation of HA1 domain of influenza H3N2 viruses.
METHODSThe consultation rate for influenza-like illness (ILI) were calculated weekly for indicating the influenza activities (the Shenzhen Influenza Surveillance System mainly consisted of 16 institutions with 9 hospitals, 6 districts and one municipal centers of disease control and prevention). Pharyngeal swabs from the cases of ILI, which were collected during 2005 to 2007 from the city-wide and quality-controlled surveillance network, were used to propagate the viruses. The HA1 region of the influenza A/H3N2 viruses were detected by RT-PCR and sequenced subsequently. The analyses of pairwise amino acid variations, genetic clustering and phylogenetics was performed.
RESULTSThe activity levels of influenza showed certain changes during each year from 2005 to 2007, and there were summer peaks from May to July in 2006 and 2007. The positive rates of influenza virus were 4.78% (114/2385), 5.77% (212/3674) and 12.12% (343/2831) from 2005 to 2007 respectively. The weekly isolating rates changed accordingly with the trend of the percentages of ILI. The proportions of influenza H3N2 virus were 25.46% (28/114) and 2.83% (6/212) in 2005 and in 2006 respectively, but the proportion increased to 62.68% (215/343), which indicated that H3N2 virus became the predominant strain in 2007. Phylogenetic clustering analysis of influenza H3N2 virus revealed that there were 5 clades. The viruses which were isolated in 2005 contained in the clade I and II, the viruses in 2006 were comprised in clade III, and clade IV and V included the viruses isolated in 2007. Although the stem of cladogram developed with one accord of the time isolated viruses, the viruses which were similar to vaccine strains had circulated in Shenzhen before a given strain was determined as vaccine strain by WHO. It was also noticed that more amino acid changes at antigenic sites, especially at sites A and B in the H3N2 viruses, were found in 2007 than that in 2005 and in 2006. But the sequences at the receptor-binding sites and disulphide bond sites were conserved and no new circulating strain for genetic reassortment had been found in the period.
CONCLUSIONShenzhen might be one of areas where the ongoing genetic drift of influenza H3N2 viruses appeared earlier in China. The changes of influenza H3N2 virus showed the active status in the population. The results suggested that monitoring seasonal influenza viruses by sequence analysis could provide important and timely information on the appearance of strains with epidemiologic significance.

8.Study on molecular epidemiological characteristics of influenza H1N1 viruses circulating in Shenzhen,China from 2005 to 2007

Objective To study the genetic and epidemiological characteristics of HA1 of influenza H1N1 viruses circulating in Shenzhen from 2005 to 2007. Methods The HA1 region was analyzed by RT-PCR and subsequently sequenced to analyze the HA1 genetic evolution. Phylogenetic analysis was confirmed on the homology of nucleitide comparing with the reference viruses of vaccines recommended by WHO and representative virus confirmed by China CDC. Relationship between isolation rates and genetic evolutions was explored. Results The average isolation rate from 2005 to 2007 was 7.16%. Of the isolates, the proportions of influenza H1N1 viruses in 2005, 2006 and 2007 were 56.14%, 66.03%,3.61% ,respectively. Data from HA1 phylogenetic analysis showed that there were at least three clades circulated in Shenzhen. Different viruses isolated during January to April were clustered with A/New Caledonia/20/1999 viruses isolated in the latter months of 2005 clustered with A/Solomon Island/3/2006 and viruses from 2006 to 2007 were in the same clade with A/GDLH/219/2006. Results showed that most viruses had a deletion of lysine at position 130. Compared with A/New Caledonia/20/1999, the virus isolated after May of 2005 occurred T82K, Y94H, R146K, R209K, T267N amino acid substitution, while some virus isolated after May 2006 took place the amino acid substitutions of A190T, H193Y,E195D (located at antigenic site B) and R146K(antigenic site A). The sequences at the receptor-binding sites and glycosylation sites were conserved. Compared with referring viruses, A/SZ/68/2007 had 50 amino acid substitutions in the HA1 region.Of these,eleven and six were located at antigenic sites and receptor-binding sites,respectively.Four amino acid substitution resulted in the deletion of glycosylation site.Conclusion Three different genetic lineages of influenza H1N1 virus were circulated in the population in Shenzhen during 2005-2007.The special virus named A/SZ/68/2007 should be paid further attention on its antigenic and epidemiological characteristics.

OBJECTIVETo investigate the relationship between two polymorphisms (Intronic VNTR and 5-HTTLPR) of the serotonin transporter gene and schizophrenia.
METHODSA set of 314 schizophrenic trio samples collected from Shanghai, Xi'an and Jilin regions of China independently was subjected to analysis of the polymorphisms by transmission/disequilibrium test(TDT).
RESULTSNo significantly preferential transmission of any allele was detected from both polymorphisms investigated.
CONCLUSIONThe results suggest that the serotonin transporter gene is unlikely to have a major contribution to susceptibility to schizophrenia in Han Chinese population.

10.Laboratory diagnosis and molecular characterization analysis of the H5N1 influenza virus isolated from the first human case in Shenzhen, China.

The tracheal aspirates and serum samples of a suspected human case of high-pathogenic avian influenza (firstly found in Shenzhen, China) were collected and tested by a series of assays. The results showed that the RNA extracted from the tracheal aspirate specimens of the patient was confirmed positive for H5N1 avian influenza virus by Real-time PCR. The H5N1 avian influenza virus was isolated from patient's tracheal aspirates on MDCK cell and was named A/Guangdong/2/06(H5N1). The viral load of tracheal aspirates collected at different time points were detected by Real-time PCR. The virus microneutralization and the antigenic ratio of human H5N1 isolated were also assayed. It was found that when the virus load decreased gradually after the disease onset, the serum neutralizing antibody titer in the patient increased to 1 : 160 and subsequently decreased gradually. By molecular analysis, the eight gene segments of A/Guangdong/2/06 revealed to be similar to that of H5N1 avian influenza viruses isolated from south China in 2005-2006. However, there were obvious differences in the gene sequence of the detected H5N1 viral RNA as compared with that of the strains isolated from Vietnam, Thailand and Indonesia.
