1.Electrophysiology of peripheral nerve in children suffering from congenital clubfoot:report of 338 feet

Objective To investigate the etiology and provide data for clinical treatment and therapeutic assessment of congenital clubfoot(CCF) via neuroelectrophysiology on motor and sensory nerves and ultrastructure of sural nerve.Methods Totally 248 patients diagnosed with CCF(338 clubfeet) were studied.Motor nerve conduction velocity(MCV),sensory nerve conduction velocity(SCV) and wave amplitude were detected for any changes.An ultra-structural study of sural nerve ramification was also conducted in 36 patients(36 clubfeet) under electron microscope.The correlation between deformity degree,nerve conduction abnormality and therapeutic outcomes was analyzed.Results The motor nerve conduction in 291 clubfeet(86.1%) and the sensory nerve conduction in 252 clubfeet(74.6%) were found with different degrees of abnormalities such as prolonged latency,declined/disappeared wave amplitude,etc.The ultrastructural observation of sural nerve ramification demonstrated abnormalities in all the 36 cases,showing proliferation of myelin sheath in peripheral myelinated nerve fibers,formation of vacuoles,segmental demyelinating lesions and so on.Part of axial cord was squeezed by the proliferated myelin sheath,which resulted in disappearance of axoplasm.The correlation study showed that as deformity became more severe,the nerve conduction abnormality was more evident,requiring more complicated treatments,and the therapeutic outcomes were less satisfactory.Statistical analysis by Mantel-Haenszel method showed that:① the abnormality rate of motor nerve in different types of CCF is signifi-cantly different(?2=32.31,P
2.Establishment of 5-lipoxygenase inducible expression system

To develope a 5 lipoxygenase inducible expression system for further understanding the function and signal transduction pathway of 5 lipoxygenase, providing information for drug screening.The 2 kb 5 LO cDNA sequence containing HindⅢ site was generated by PCR using pEGFP 5LO as a template. Full length 5LO cDNA was subsequently cloned into the corresponding Pst Ⅰ and Sal Ⅰ sites of the plasmid pBI G to generate the mammalian expression vector pBI 5LO. HeLa Tet On cell line was co transfected with pBI 5LO and pTK Hyg plasmids. Inducible expression of target gene was detected using X gal histochemical staining of cell monolayers for ? galactosidase after incubating the transfected cell line with 1 ?g/ml doxycycline for 48 h.The HindⅢ restriction digest site was inserted into polyclonal site of pBI G and recombinant vector pBI 5LO was constructed. The desired recombinant plasmid pBI 5LO was identified by restriction analysis, and orientation and junctions were confirmed by sequencing. Induction of the target gene expression by doxcycline treatment was demonstrated by X gal histochemical staining and nuclei of the cells that have expressed the pBI 5LO expression vector were brilliant blue.The 5LO inducible expression system is established successfully and it works well in cell model.

4.Effects of Components of Medium for Photo-synthetic Bacteria on its Synthesizing Carotenoid

The medium of photo-synthetic bacteria synthesizing carotenoid was optimized with response surface methodology. The regression equation expressing the relationship between carotenoid yield and main components of culture medium was established by stepwise analysis. The R-squared in the model of regression equation was 0. 958, which meant the established equation could predict the carotenoid yield well at the range of factors in this design. It was indicated that the optimum medium components were: 0. 81% citric acid, 0. 35% NH4Cl and 0. 18% corn syrup. On those conditions, it was predicted that the highest production of carotenoid was 13. 34 mg/L, which was 2. 04 times higher than initial carotenoid yield before optimization.
5.Expression and clinical significance of p27 protein in stage Ⅱ colon cancer

Objective To investigate the expression and clinical significance of p27 protein in stage Ⅱ colon cancer.Methods The expression of p27 protein in 57 samples of stage Ⅱ colon cancer tissue from BeijingCancer Hospital was detected by immunohistochemistry and tissue microarray technique.Survival was analyzed by the Kaplan-Meier method,and the correlation between the expression of p27 protein and the prognosis of patients was analyzed by Pearson chi-square test.Multivariate analysis was done by using Cox proportional hazard regression model.Results The rate of high expression of p27 protein in stage Ⅱ colon cancer was 60%(34/57).The differentiation.location and distal metastasis of the tumor were correlated with the expression of p27 protein(X2=5.97,5.93,5.05,P<0.05);while age,tumor size,growth patterns of tumor were not correlated with the expression of p27 protein(X2=0.64,0.49,0.33,P>0.05).The 5-year survival rate of the patients with high expression of p27 protein was 94%(32/34),which was significantly higher than the 78%(18/23)of the patients with low expression of p27 protein(X2=3.86,P<0.05).Multivariate analysis showed that low expression of p27 protein and tumor penetrating serous membrane are the independent adverse prognostic factors for stage Ⅱ coloR cancer.Conclusions Low expression of p27 protein is one of the independent adverse prognostic factors for stage Ⅱ colon cancer,and the expression of p27 protein should be consulted when screening out the patients who are in the high risk group of stage Ⅱ colon cancer.
6.Supracricoid Partial Larynegectomy for Laryngeal Cancer

Objective To assess the effectiveness of supracricoid partial laryngectomy in the treatment of laryngeal cancer. Methods This study infiuded 22 patients operated on from 1993 to 2000 using this surgical procedure. 22 were males with mean age of 63 years (ranging from 43 to 74 years). 21 were glottic cancers (3 T1aNoMo, 4 T1bNoMo, 11 T2NoMo, 3 T3NoMo) and 1 supraglottic cancer (T2N1Mo) according to the 1997 UICC system. Supracrieoid partial laryngectomy was performed, with the epiglottis preserved and reconstructed with cricohyoidoepiglottopexy (CHEP). Results The overall 3-year and S-year survival rates were 88.24% and 70%, respectively. All patients were decannulated. The average time for decannulation was 25 days (ranging from 14 to 60 days). Speech was good in all cases. Conclusion CHEP not only excises the neoplasms completely and safely but also preserves the laryngeal physiologic function well.

8.Comparative study of arthroscopic reconstruction of anterior crucial ligament with autograft and aliograft of bone-patellar tendon-bone

Objective To compare the effect of allograft and autograft of bone-patellar tendon- bone (B-PT-B) under arthroscopy in reconstruction of anterior cruciate ligament (ACL).Methods The injured ACL was reconstructed under arthroscopy by using autograft of B-PT-B in 30 cases and al- lograft of B-PT-B in 28 cases from January 2000 to August 2003.After over twelve months follow up,a comparison was done on rehabilitation of quadriceps femoris,knee stability,the Lysholm's score of knee and the enlargement of tunnel in both groups.Results Both groups obtained marked improvement of knee stability and symptoms after operation.But in the early time,the rehabilitation of quadriceps femoris in the ailograft group is faster than that in the autograft group.The rehabilitation of quadriceps femoris, knee stability and the Lysholm's score of knee were similar at 12th month (P>0.05).There could be seen enlargement of tunnel in both groups,with more in the allograft group (P>0.05).Conclusion For arthroscopic reconstruction of anterior crucial ligament (ACL),allograft of B-PT-B is good substitute, for it is characterized by effectiveness,convenience and minor side effect.
9.The effect of intramedullary nails and locking compression plate in treatment of femur shaft fracture

Objective To compare the clinical effects of intramedullary nails and locking compression plate in the treatment of femur shaft fracture.Methods The clinical data and follow-up records of 43 patients with femur shaft fracture treated by intramedullary nails and locking compression plate were retrospectively reviewed.Including 31 males and 12 females with an average age of 40.07 years(25 to73).According to AO classification:5 cases were type A,24 cases were type B,14 cases were type C.26 patients in group A treated by intramedullary nails and 17 patients in group B treated by locking compression plate.Operation time,fracture union time,blood loss and complications were compared between the two groups.The Karlstrom and Olerud result were compared and analyzed.Results All patients were followed up at an average of 15 months(range 5-24) after surgery.According to Karlstrom and Olerud score system,the excellent and good rate of group A was 88.4%,higher than the group B 82.3%.But there was no significant difference between the two groups.In group A:the operation time (113.77 ±20.14)min,the blood loss in the operation (386.74 ± 65.16) ml,clinical bone healing (15.52 ± 1.77) weeks.All these outcomes were better than group B (P < 0.01).One case in group A appeared screw loose,but X-ray showed the nail stable,the bone wasn't displacement.One case in group B appeared broken plate 1 month after operation,and received twice operation,the clinical outcome was good as well.Another 1 case with fracture delayed union in group B,after internal fixation removed,we check the X-ray showed bone union.Conclusion Two methods of treatment femur shaft fracture can all reach the above requirement.Fixation of intramedullary interlocking nail is the preferred surgery than locking compression plate.
