3.Effects of 1,25-dihydroxyvitamin D3 on the expression of urinary nephrin in diabetic nephropathy patients

Objective To investigate the effects of 1 ,25-dihydroxyvitamin D3 on the expression of urinary Nephrin in patients with diabetic nephropathy .Methods According to albumin excretion rate ,40 type-2 diabetic nephropathy patients were divided into 2 groups ,10 healthy volunteers served as control .The treatment groups received 1 ,25-dihydroxyvitamin D3 0 .25 μg/d for 12 weeks .The levels of blood calcium ,phosphorus ,serum creatinine ,24-hour urinary protein excretion were detected .Enzyme-linked immunosorbent assay was used to detect urinary Nephrin in different groups .Results The expressions of urinary Nephrin were sta-tistically different among NC group ,DN1 group and DN2 group .1 ,25-dihydroxyvitamin D3 treatment significantly decreased the ex-pression of urinary Nephrin in DN1 patients ,whereas no statistically changes were observed in DN2 group .Conclusion In diabetic nephropathy patients ,urinary Nephrin is supposed to be effective an marker of disease progression .1 ,25-dihydroxyvitamin D3 may be effective to prevent podocyte injury in early-stage diabetic nephropathy .
4.Relationship Between Erythrocyte Sedimentation Rate and Myocardial Infarction Occurrence in Patients With Rheumatoid Arthritis Combining Coronary Artery Disease

Objective: To explore the relationship between erythrocyte sedimentation rate (ESR) and myocardial infarction (MI) occurrence in patients with rheumatoid arthritis (RA) combining coronary artery disease (CAD).
Methods: A total of 106 consecutive patients with RA combining CAD were studied. There were 46 male and 60 female patients and divided into 2 groups:RA with MI group, n=46 and RA without MI group, n=60. The base line condition was compared between 2 groups, and multivariate regression analysis was conducted to explore the risk factors for MI occurrence in relevant patients.
Results: Compared with RA without MI group, RA with MI group had the lower level of cholesterol and higher levels of inlfammatory indexes of ESR, high sensitivity C-reactive protein (hs-CRP) and CRP, P<0.05. The base line condition was similar between 2 group, P>0.05. Multivariate regression analysis indicated that ESR was the independent risk factor of MI occurrence, OR=1.024, 95%CI 1.024 (1.007-1.043), P=0.007.
Conclusion: ESR was independently related to MI occurrence in patients with RA combining CAD.
5.Feasibility of Leucovorin Rescue Guided with Methotrexate Plasma Concentration

Objective To evaluate the feasibility of leucovorin(LCV) rescue protocol defined by us,we compared the plasma concentrations,toxicity,LCV doses of different high doses methotrexate(HD-MTX).Methods Seventeen children with acute lymphoblastic leukemia and 1 children with non-Hodgkin′s lymphoma were randomly treated with total 43 courses of HD-MTX.MTX plasma concentrations were measured by fluorescence polarization immuno-assay.Different LCV rescue protocols were prospectively defined for 3 kinds of HD-MTX protocols.Adjusting LCV dose by plasma MTX concentrations.Results No irreversible MTX-related toxicity was observed in all patients.Significant differences of mean steady-state plasma concentrations(Cpss) and total rescue doses were found between 3 groups(P

Objective To observe effect of corticosteroid and long-acting ?2-agonist seretide to treat patients with moderate asthma who were uncontrolled on existing corticosteroid therapy.Methods From Oct.2005 to Feb.2007,85 asthmatic patients were randomly divided into two groups,treatment group were treated with seretide twice daily and the control group were treated with pulmicort(budesonide)twice daily.The clinical effect index and peak expiratory flow(PEF)were analyzed statistically.Results There were 61 cases in seretide group and 24 cases in pulmicort group who were follow-up examined and evaluated effectively the clinical effect.The treatment period was 1 year,the efficacy were 100% and 75% respectively,there was significant difference between two groups(P
9.Laparoscopic Tubal Reversal After Tubal Sterilization:Report of 31 Cases

Objective To study the feasibility of tubal reversal after sterilization by laparoscopy.Methods From May 2002 to December 2005,31 patients with tubal ligation received laparoscopic tubal reversal by one-core or two-dot apposition and demixing suture of the serosa.Results The laparoscopic tubal reversal was successful in all the 31 cases.Thirty of the patients were followed up for 8-24 months(mean,16 months);27 of them had intrauterine gestation,3 were non-pregnant;none of them had ectopic gestation.Conclusions Laparoscopic tubal reversal after sterilization can satisfy with the desire and principle of microsurgery.It is feasible for clinical application and has good outcomes.
10.Effect of Injection of Botulinum Toxin Type A on Spasticity of Upper Extremity with Different Functional Status after Stroke

Objective To observe the effect of botulinum toxin A (BTX-A) on spasticity of upper extremity with different functional status after stroke.Methods 32 post-stroke patents with upper extremity spasticity were divided into the good function group (n=13) and poor function group (n=19) according to the function of upper extremity. All patients in two groups were treated with injection of domestic BTX-A at the biceps brachii and rehabilitation therapy. The efficacy was assessed with rang of motion, Modified Ashowrth Scale (MAS), Fugl-Meyer Assessment (FMA) and Mot-FIM upper extremity before and 8-week after treatment.Results After treatment, the rang of motion and FMA scores of patients in two groups improved ( P<0.05), but the good function group was superior to the poor function group; the scores of MAS of two groups were not different ( P>0.05); the scores of Mot-FIM upper extremity of the patients in two groups increased ( P<0.05), and there was no significant difference between two groups ( P>0.05).Conclusion The effect of BTX-A injection at the biceps brachii on post-stroke patients with good function of upper extremity is superior to the patients with poor function of upper extremity.
