1.Comparison of the clinical effect of ultrasonic scalpel and electric knife in open thyroid surgery

Objective To investigate the clinical effect of the ultrasonic scalpel and electric knife in open thyroid surgery.Methods The clinical data of 182 patients underwent thyroid surgery were retrospectively analyzed.100 cases in the observation group were used the ultrasonic scalpel surgery and 82 patients in the control group were given electric knife surgery.The average surgical incision length,mean operative time,mean blood loss and postoperative complications were compared between the two groups.Results The average incision length of the observation group was (36.7 ± 6.1)mm,average time of thyroidectomy was (25.3 ± 5.1)min and blood loss was (14.2 ± 5.1)ml,which were significantly lower than those of the control group (P < 0.05).The incidence rate of adverse reaction between the two groups had no significant difference(P > 0.05).Conclusion Compared with electric knife,the use of ultrasonic scalpel in thyroid surgery can significantly reduce blood loss,shorten the operation time,the prognosis and clinical efficacy is good,it is worth to be further promoted in clinical.

3.Changes of thyroid weight and function after ~(131)I therapy for Graves′ disease

Objective To observe changes of thyroid weight and function after 131 I therapy in Graves′ diseaseandtooptimiseradiationdose for treatment of Graves′ disease. Methods From 1995 to 1998, 103 patients with Graves′ disease were selected (nodule was not found by palpation and nuclear scintigraphy). The mean age was 43 (range 27~64) years. Thyroid weight was estimated by nuclear scintigraph (SC) and ultrasound (US). Thyroid function was determined by serum TT 3, TT 4, FT 3, FT 4 and TSH levels. Results The oral dose of 131 I was (188.7?144.3)MBq. If the dose was larger than 555.0 MBq, it was divided into two doses orally. The thyroid weight of the patients before 131 I therapy was (54.8?32.3)g (SC) and (49.3?27.7)g (US), there was no significant difference between SC and US. The thyroid weight in 90 patients was significantly decreased from (51.5?23.4)g to (27.0?13.9)g by SC, and from (47.2?23.7)g to (20.6?8.4)g by US 6~7 months after 131 I treatment. Objective To construct eukaryctie expression vectors stably expressing Smad-7 and study its function as TGF-β1 inhibi- tor. Methods Smad-7 genes were obtained by PCR from fetal liver cDNA library and inserted in to eukaryotic expression plasmid pcD- NA3. 1/myc-his-B (-). The constructed plasmids containing right sequence of target genes were transfected into human mesangial cells by lipofectamine. The ceils were selected by G418, the expression of Smad-7 was detected by Western-blotting. Cell proliferation was detected by MTT method, and cell cycle was detected by Flow Cytometer to study its function as TGF-β1 inhibitor. Results We successfully con- struct Smad-7 expression plesmid. Smad-7 can inhibit the effects of TGF-[M induced GI cell cycle block and apoptosis. Conclusion The eukaryotic expression vector of Smad-7 has been successfully constructed. Over-expression of exogenous Smad-7 can inhibit the effects of TGF-β1 induced GI cell cycle block and apoptosis.
7.Symmetry study of temporomandibular joint in adults with mandible deviation

Objective:To comprehend morphology changes of temporomandibular joint in adults with mandible deviation,and the correlation in these changes.Methods:The mesofault radiographs of temporomandibular joint were taken in 21 adult patients with mandible deviation.The data described morphology of temporomandibular joint were analysed.Results:In adult patients with mandible deviation,the condyle process of opposite side was anterior and inferior when compared with deflected side.The height of condyle process,the upper height of condyle process,the gradient of prosobevel of condyle process and the gradient of back bevel of glenoid fossa were augmented when compared to deflected side.The gradient of prosobevel of condyle process showed positive correlation to the prosoblank of joint and the deep of glenoid fossa,and the height of condyle process showed positive correlation to the upper height of condyle process in both sides.The gradient of back bevel of condyle process showed positive correlation to the gradient of back bevel of glenoid fossa in deflected side.The gradient of back bevel of condyle process showed negative correlation to the supper blank of joint and the height of articular tubercle in opposite side.Conclusion:There are some differences in morphology of both temporomandibular joint in adults with mandible deviation,and there is some correlation between these changes.
8.Effect of Propofol Target-controlled Infusion on Stress Response during Nasoscopic Operation

Objective To observe the effect of propofol target-controlled infusion on stress response during nasoscopic procedures.Methods Totally 40 patients with ASA gradesⅠ-Ⅱ scheduled for the nasoscopic operation ware randomly divided into two groups:Group A(propofol continuously injection,2.5 mg?kg-1,n=20)and Group B(propofol target-controlled infusion,4 ?g?ml-1,n=20).The operations were all performed under general anesthesia.Venous blood samples were taken to measure cortisol and blood glucose at three time points:before operation,at 30 min after the operation started,and 60 min after the endotracheal catheter was withdrawn.Meanwhile,HR and MAP of the patients were recorded.Results At both 30 min after the operation started and and 60 min after the endotracheal catheter was withdrawn,Group A showed significantly higher MAP and serum levels of glucose and cortisol than Group B.At 30 min after the operation started:HR:(73?8)/min vs(65?13)/min,t=2.344,P=0.024;MAP:(74?7)mm Hg vs(68?7)mm Hg,t=2.711,P=0.010;blood glucose:(6.28?0.11)mmol/ml vs(5.31?0.15)mmol/ml,t=23.321,P=0.000;cortisol:(125.3?11.5)ng/ml vs(89.6?9.9)ng/ml,t=10.521,P=0.000.At 60 min after the endotracheal catheter was withdrawn:MAP:(79?6)mm Hg vs(73?8)mm Hg,t=2.683,P=0.011;blood glucose:(6.18?0.09)mmol/ml vs(5.62?0.16)mmol/ml,t=10.082,P=0.000;cortisol:(169.1?16.3)ng/ml vs(149.5?15.3)ng/ml,t=3.921,P=0.000.Conclusion Propofol target-controlled infusion can inhibit the stress response caused by nasoscopic operation.
9.Determination of Main Component and the Related Substances in Pirenoxinum Natricum Eye Drops by HPLC

OBJECTIVE:To establish a HPLC method for the determination of main component and the related substances in pirenoxinum natricum eye drops. METHODS: A Inertsil C8-3 column was used with pH2.2 perchloric acid-methanol (35∶65) as mobile phase at a flow of 1.0 mL?min-1. The detection wavelength was 230 nm and the sample size was 10 ?L. RESULTS: Pirenoxinum natricum and the related substances could be baseline separated. The calibration curve was linear in the range of 49.5~990 ng(r=0.999 9). The detection limits was 89.3 pg, and average recovery was 99.1%, RSD was 0.42%. CONCLUSIONS: This method is rapid, accurate sensitive and can be used for the quality control of pirenoxinum natricum eye drops.
