Promoted vigorously by state and government , it is a strategic emphasis of a new round of medical reform that public hospitals combine with social capital .In order to expect more attention to the hot issue from wheth-er theoretical or practical views , this article draws lessons from the experiences that foreign hospitals have managed to combine with social capital , to analyze the necessity of the new medical mode under the background of the medical re -form policies implementation and the enlarged health industry flourishing development , and meanwhile to discuss the value of the new medical mode for the present medical situation .

Polyglandular autoimmune syndromes (PAS) comprises a wide spectrum of autoimmune disorders for autoimmune inflammatory to invade.According to age of presentation,characteristic patterns of disease combinations,and different modes of inheritance,PAS are classified into four subtypes.Actual diagnosis of PAS involves clinical syndromes,serological measurement of organ-specific autoantibodies and subsequent functional testing.Principles of treatment included comprehensive assessment,hormone replacement,general consideration.

The origin of northern Artemisia argyi recorded in Compendium of Materia Medica(Bencao gangmu) is Fudao(Chinese characters) in Tangyin county, While there is only Fudao(Chinese characters) instead of Fudao(Chinese characters). Whether indeed Fudao(Chinese characters) is Fudao(Chinese characters)? By reviewing the genuine evolution of A. argyi, doing textual research on Fudao(Chinese characters) and combing with field survey data of national census of Chinese Materia Medica resources, this paper concluded that the word Fudao(Chinese characters) firstly emerged in Figure Canon of Chinese Materia Medica(Bencao tujing) of Susong in Song dynasty and was applied in later generations, but the implication was not clear, then emerged both Tangyin and Fudao(Chinese characters) in Compendium of Materia Medica(Bencao gangmu). The place Fudao(Chinese characters) is one of the graves of Bianque, that existed from Shang and Zhou dynasty and never changed until now, the A. argyi of Tangyin was famous from the grave of Bianque in Fudao(Chinese characters), which could infer that Lishizhen considered Fudao (Chinese characters) was Fudao(Chinese characters) indeed, and the origin of northern A. argyi was Fudao(Chinese characters) in Tangyin county.

Objective To evaluate CT findings and dynamic changes of post-traumatic intrapulmonary hematoma.Methods CT data of 43 patients with traumatic intrapulmonary hematomas were analyzed retrospectively.Results A total of 52 lesions were found in 43 patients with intrapulmonary hematomas.The majority of hematomas were located in the peripheral lung fields close to the pleura,single or multiple in number.CT showed nodular,mass,fusiform,ribbon and irregular shadows with uniform and high density in lung.There are two types of evolution: (1) The hematomas gradually reduced in size and finally disappeared;(2)The hematomas evolved into gas-liquid cyst cavity or gas cyst cavity, and then was absorbed gradually.Conclusion CT examination is helpful in the early diagnosis of post-traumatic intrapulmonary hematomas, and can monitor the morphology changes over time.

Objective To compare the therapeutic efficacy of two different administration routes of insulin administra?tion on juvenile type 1 diabetes mellitus (T1DM) complicated with diabetic ketoacidosis(DKA). Methods A total of 223 cases of juvenile T1DM was included in this study, among which 98 were complicated with DKA. Insulin was delivered through either continuous subcutaneous insulin infusion(CSII) by insulin pump or via multiple subcutaneous insulin injec?tion (MSII). Recovery period of blood glucose, insulin doses that were adminstrated, the urinary ketone bodies clearance time, the recovering time from DKA and the frequency of hypoglycemia incidence were all compared between these two routes. Results Both CSII and MSII routes reversed blood glucose and DKA effectively. However the recovering time of blood glucose and DKA, insulin dosage,the urinary ketone bodies clearance time and the frequency of hypoglycemia inci?dence all improved better or quicker in CSII than in MSII. Conclusion CSII by insulin pump is safer and more effective than MSII in the treatment of junvenile T1DM with metabolic disturbance and diabetic ketoacidosis.

Objective To investigate metformin combined with insulin aspart 30 injection (NovoMix 30) in the treatment of type 1 diabetes mellitus (T1DM) in children and adolescents. Methods A total of 126 T1DM children over 10 years of age were randomly divided into insulin group (A) and insulin + metformin group (B). A group (n=60) was given insulin aspart 30 injection (insulin aspart 30), and B group (n=66) was given the metformin and insulin aspart 30 injection (NovoMix 30). Results The two groups can effectively control blood glucose, but the B group in the blood glucose control time, insulin dosage, the incidence of hypoglycemia, fasting blood glucose and hospitalization time were better than those of A group. There was no significant difference in liver and kidney function before and after oral administration of metformin in B group (P>0.05). Conclusion Metformin combined with insulin is effective and safe in the treatment of children with T1DM.

Objective To evaluate the effect of gabapentin on Cav3.2 channels in the dorsal root ganglia ( DRG) of rats with neuropathic pain. Methods Thirty male Sprague?Dawley rats, aged 6-7 weeks, weighing 225-275 g, were divided into 3 groups ( n=10 each) using a random number table:sham operation group ( S group) , neuropathic pain group ( NP group) and gabapentin group ( G group) . In NP and G groups, neuropathic pain was induced by chronic constriction injury to the left sciatic nerve. Starting from 7th day after operation, gabapentin 100 mg∕kg in 1 ml of normal saline was injected intraper?itoneally twice a day for 7 consecutive days in group G, and the equal volume of normal saline was given twice a day for 7 consecutive days in S and NP groups. The mechanical paw withdrawal threshold ( MWT) and thermal paw withdrawal latency ( TWL) were measured on day 3 before operation and on postoperative days 3, 7 and 14. After the last measurement of pain thresholds on the postoperative day 14, 4 rats were sacrificed for determination of Cav3. 2 mRNA expression ( by real?time polymerase chain reaction) and Cav3.2 protein expression (by Western blot) in the DRG. Results Compared with S group, the MWT was significantly decreased, and TWL was significantly shortened on postoperative days 3, 7 and 14, and the expression of Cav3.2 protein and mRNA in the DRG was significantly up?regulated in group NP, and the MWT was significantly decreased, and TWL was significantly shortened on postoperative days 3 and 7 ( P<0.05), and no significant change was found in the expression of Cav3.2 protein and mRNA in the DRG in group G ( P>0.05) . Compared with NP group, the MWT was significantly increased, and TWL was signif?icantly prolonged on the postoperative day 14, and the expression of Cav3.2 protein and mRNA in the DRG was significantly down?regulated in group G ( P<0.05) . Conclusion The mechanism by which gabapentin attenuates neuropathic pain may be related to inhibition of the function of Cav3.2 channels in the DRG of rats.

Purpose:To investigate the therapeutic effect a nd toxicity of radiotherapy plus ultrasound hyperthermia for treatment of malign ant tumors. Methods:41 cases of locally advanced, recurrent or radioresista nt cancer treated from January 1997 to January 2000 were given radiotherapy plus Sonotherm 1000 ultrasound hyperthermia. The mean radiation dose was 61?12 Gy;h yperthermia was administered within 30 minutes following radiotherapy with mean heating time of (52.4?6) minutes at 1-2 times/week. 3-year local control and survival were estimated by Kaplan-Meier method and Log rank test, factors affec ting prognosis were analyzed by Cox regression. The significance of differences between temperatures was tested by one-way ANOVA. Results:There was significant difference between Tmin and Tmax on tumor local response. Tumor complete response was 58.5%, the 3-year local co ntrol and survival were 13.6%% and 32.69% respectively. Objective To evaluate the clinical application of serum pyridinoline cross-linked carboxy-terminal telopeptide of type Ⅰ collagen(ⅠCTP),anti-cyclic citrullinated peptide(CCP)antibody,rheumatoid factor(RF)and C-reative protein(CRP)detection to the diagnosis and disease monitor of rheumatoid arthritis.Methods Serum ⅠCTP and anti-CCP antibody were measured by ELISA,RF and CRP by immunonephelometry assay in 57 patients with RA and 25 with non-RA as well as 30 normal controls.Results Serum ⅠCTP concentration was positively correlated with that of anti-CCP antibody,RF and CRP(rs=0.474,0.366,0.645,P0.05).Serum ⅠCTP in the patients with active RA was significantly higher than in those with stable RA and normal controls(P0.05).The combined sensitivity of RF and anti-CCP antibody was 87.5%.Conclusion Serum ⅠCTP is a sensitive serum marker to monitor the early damage of arthrosis.Missed diagnosis rate of RA is reduced by detecting anti-CCP antibody and RF together.Combinational monitoring the variance of serum ⅠCTP,anti-CCP antibody,RF and CRP could effectively monitor the disease development in time,which exhibits great sense on impede arthrosis damage.
