Objective To investigate the relationship between serum leptin(LP) level and glycometabolism in pregnancy with abnormal glucose challenge test(GCT) and the corresponding outcome of pregnancy. Methods Serum LP level, fasting serum insulin(FINS) and fasting blood glucose(FBG) levels were measured by radioimmunoassay in 23 patients with abnormal GCT but normal oral glucose tolerance test(OGTT),20 with impaired glucose tolerance(IGT),20 with gestational diabetes mellitus(GDM)(studied groups above) and 30 normal pregnant women(control group).LP,FINS,FBG, insulin resistance index(HOMA-IR) and ?-cell function index(HOMA-IS) were compared among the groups and the outcomes of pregnancy were observed. Results ①There were significant differences in LP,FINS,FBG,HOMA-IR and HOMA-IS between the studied groups and the control group(P0.05).③There were signi-ficant differences in the gestational weeks of delivery, cesarean section rate and neonatal weight between the studied groups and the control group(P0.05). Conclusion Both insulin secretion and insulin resistance are elevated in pregnancy with abnormal glycometabolism, and are closely correlated to the increased serum level of LP.There existed abnormal glycometabolism in patients with abnormal GCT but normal OGTT,which is correlated to the outcome of pregnancy.

Objective To study the effects of cyclooxygenase-2(COX-2) selective inhibitor celecoxib on proliferation and apoptosis of human endometrial adenocarcinoma HEC-1B cell line. Methods Methabenzthiazuron(MTT) assay was used to examine the effects of different concentrations of celecoxib on proliferation of HEC-1B cells. When HEC-1B cells were treated with different concentrations of celecoxib for 24 h,cell cycle and apoptosis were detected by flow cytometry. The expression of COX-2 mRNA was analyzed by RT-PCR. Results MTT results indicated that celecoxib could inhibit HEC-1B cell proliferation in a time-and dose-dependent manner. When HEC-1B cells were treated with different concentrations of celecoxib for 24 h,flow cytometry results showed that the cell percent of(G_(0)/G_(1)) phase increased,S and G_(2)/M phase decreased, and cell apoptosis rate also increased, which was significantly different from that of the control group(P

Objective To investigate static closure functional characteristics of lower urinary tract in continence in late pregnancy of primigravida through perineal ultrasound and urodynamieal measurement. Methods From January of 2003 to December of 2005,83 primigravida undergoing antenatal health care in Shanghai Jiaotong University Affiliated Shanghai Sixth People's Hospital were randomly enrolled in the study, while 28 nulliparous women were recruited as control. The perineal ultrasound and urodynamical measurement were performed at the gestation age of 36-40 weeks,in both groups. We analysed the parameters of lower urinary tract function in late pregnancy compared with nullipara, such as maximal urethral closure pressure(MUCP),functional urethral length(FUL),maximal urethral pressure(MUP), valsalva leak point pressure(VLPP),postvoid residual bladder volume(PVRBV)and volume of first desire to voiding(VFDV).Results The average weight of 83 neonates was(3504?404)g.In all of 83 primigravida PVRBV was less than l0 ml.MUP(110 + 22)cm H_2O(1 cm H_2O=0.098 kPa),MUCP (96?22)cm H_2O and FUL(44?9)mm were significantly increased in the third trimester compared with nullipara(P0.05). There were 33 pregnant women suffering from urinary leakage throughout the terms. The data showed that MUCP was significantly lower in the pregnant women with symptoms of leaking than without leaking. Conclusion The static closure function increased significantly compared with nulliparous women. The VLPP was decreased and had relationship with symptoms of leakage. The results suggest that the function of lower urinary tract in continence in late pregnancy of primigravida is complex.

OBJECTIVETo get a complete cDNA sequence of B2 and evaluate the correlation on structure and expression between B2 and insulin like growth factor binding protein related protein 1 (IGFBP-rP1) in colorectal carcinomas, paired normal tissues, adenomas, tissues adjacent to the tumor, and colorectal carcinoma cell lines.
METHODS5'RACE (rapid amplification of cDNA end) was applied to get the sequence of the 5' end of B2. Semi-quantitative RT-PCR and immunohistochemistry were used to detect the expression of B2 in colorectal cancer tissues and cell lines (SW480, SW1116, SW620, HCT8, CoLo205 and LoVo).
RESULTSA sequence of 1,125 bp was obtained by combining the sequence from 5'RACE product and the known sequence of B2. It shared 1,122/1,125 identities with IGFBP-rP1. At the level of mRNA, the expression of B2/IGFBP-rP1 was high in colorectal carcinomas, moderate in adenomas and tissues adjacent to tumor, low in normal tissues (P < 0.05). Five cell lines except SW480 showed no expression of B2/IGFBP-rP1. A significant difference was obtained in the immunoreactivity of B2/IGFBP-rP1 between normal tissue and cancer (P < 0.05). In 28.9% (22/76) samples, cancer cells locating at the invasive front of cancer nest had a stronger staining of B2/IGFBP-rP1 than those surrounding the lumen. These samples had also an increased frequency of lymph node metastases, increased depth of invasion and a stronger staining of B2/IGFBP-rP1 than in other samples (P < 0.05).
CONCLUSIONSB2 is the same gene as IGFBP-rP1. Overexpression of B2/IGFBP-rP1 may play an important role in the initiation and promotion of colorectal cancer. Its overexpression in invading tumor cells may be linking with an increased potential of invasion.

OBJECTIVETo investigate the association of artemin and GFRalpha3 expressions with perineural invasion and metastasis of pancreatic carcinoma.
METHODSSemi-quantitative reverse transcription polymerase chain reaction (RT-PCR) and immunohistochemistry were used to detect the expression of artemin and GFRalpha3 in pancreatic carcinoma tissues, adjacent tissues and normal pancreas tissues, and the relevance of artemin and GFRalpha3 expressions to the perineural invasion and metastasis of pancreatic carcinoma were analyzed.
RESULTSThe positivity rates of artemin and GFRalpha3 expressions were 72.09% and 67.44% in pancreatic carcinoma, respectively, significantly higher than those in the adjacent tissue (18.19% and 22.73%). The positivity rates of artemin and GFRalpha3 expressions were significantly higher in patients with perineural invasion than in those without perineural invasion (chi(2)=11.11 and 11.78, respectively, P<0.01). Significantly higher expression of artemin mRNA was noted in pancreatic carcinoma (0.741-/+0.014) than in the normal pancreas tissue (0.101-/+0.031, P<0.05), and patients with perineural invasion showed significantly higher positivity rates of artemin mRNA expression (0.843-/+0.012) than those without perineural invasion (0.512-/+0.017, P<0.05).
CONCLUSIONArtemin and GFRalpha3 expressions may play an important role in perineural invasion of pancreatic carcinoma and can be used a useful indicators for evaluating the biological behavior of pancreatic carcinomas.

OBJECTIVETo study the clinical characteristics and the treatment of periorbital injuries.
METHODS61 cases were treated, including 30 cases orbitozygomatic fracture, 6 cases of frontal-orbital fracture, 8 cases of naso-ethmoid-orbital fracture, 7 cases of blow -out fracture and 10 cases of complicated fracture. The patients were diagnosed after physical examination and other examination, like CT. Through bicoronal or local mini incision at the end of eyebrow, combined with subciliary incision and local wound approach, the fractured sites were exposed completely. Then the fractured fragments were repositioned and fixed rigidly. The orbital wall was reconstructed with titanium net and Medpor.
RESULTSThe wounds healed primarily. Good cosmetic and functional results achieved in most of the patients. 4 cases underwent second-stage ophthalmectomy. 2 patients had diplopia after operation, but improved gradually. 3 cases of blepharoptosis needed further treatment.
CONCLUSIONSEarly diagnosis and treatment is very important for periorbital injuries. Fracture reposition and orbital wall reconstruction should he performed at early period.

OBJECTIVETo review the results for minimally invasive aortic valve replacement (AVR) through a 5 cm right anterolateral thoracotomy.
METHODSFrom July 2009 to September 2011, 101 consecutive patients with isolated aortic valve disease (degenerative in 37 patients, rheumatic in 21 patients, congenital in 37 patients, endocarditic in 3 patients and aorta-arteritis in 1 patients) underwent AVR through the right anterolateral thoracotomy approach in the third intercostal space with a groin incision for femoral connection of cardiopulmonary bypass. The mean age was 45.7 years (ranging from 17 to 71 years). Sixty patients were male.
RESULTSOperations were successfully performed in all but 1 patient (1.0%) who required intraoperative conversion to full sternotomy. Mean duration of cardiopulmonary bypass time and aortic cross-clamp time was (88 ± 24) minutes and (55 ± 18) minutes, respectively. Thirty-day mortality was 1.0% (1/101), this patient was found difficult in weaning off cardiopulmonary bypass and exhibited severe coronary artery plaque, although bypass graft was carried out immediately, the patient died of severe low cardiac output syndrome finally. No blood products were needed in 83.2% patients. Follow-up was performed in all patients at an average of (16 ± 7) months postoperatively. A good recovery was obtained in all patients except one who died of multiple organ failure caused by massive cerebral infarction 38 days after surgery.
CONCLUSIONSMinimally invasive aortic valve replacement though the right anterolateral thoracotomy approach is safe and feasible, with good cosmetic results and rapid postoperative recovery. It is worthy of clinical elective application.
