1.Clinical application of diffusion tensor imaging in acute stroke

Objective To reveal the transmiting discipline of parameters and define time window for therapy of ischemia penumbra by continuous detection of diffusion tensor imaging(DTI), and evaluate the relations between cortical spinal tracts(CST) and myodynamia by diffusion tensor tract(DTT). Methods 71 patients of cerebral infarction with different timing were imaged with conventional MRI, DWI and DTI, measuring DCavg, FA, RA , 1-VR of different ROI and performing DTT in ten patients. Results DCavg of lesions in different phases were [(0.275?0.022),(0.349?0.019),(0.465?0.023),(0.538?0.035)]?10~ -3mm~2/s, respectively. 2.Comparing and Filtrating the Methods to Improve the Preoperative Earlier Diagnostic Accordance Rate of the Breast Cancer

Objective To explore the preoperative misdiagnosis in the breast cancer and the methods to improve the diagnostic accordance rate. Methods The preoperative diagnosis and results of 358 cases with breast cancer has been analysized retrospectively in the recent ten years. Results Total Cases with present diagnostic technique diagnosed befor operation was 176 only;the accordance rate was less than 50%.With aspiration biopsy and molybdenum palladium roentgenograph,the rate was 25%(79 cases).The actual preoperative accordance rate was less than 10%(32 cases).It is said approximately 90% cases were postoperatively diagnosed. Conclusion The present excision biopsy not only has made many patients lost the chance of chemotherapy preoperation, but also interferenced the application of new therapic methods in the clinical practice.Infrared spectroscopy and ultrasonography have been commonly used in the clinical practice.With the low diagnostic accordance rate,they were applied only for screening.With simplity and high diagnostic accor dance rate,aspiration biopsy and molybdenum pall adium roentgenograph should be enrolled extensively in the clinical practice. The puncture and specimen fixing technique also should be improved.
3.Experience of Sucting and Scraping Cold Separating Method in Laparoscopic Cholecystectomy.

Objective To find an effective operational approach that can mostly reduce heat and other injury to Calot's triangle when separating and anatomizing it at LC operation.Method We abandoned traditional hot separating method by electric coagulation hook which can cause hot injury easily and cold separating method simply by elastic separating plier which can cause errhysis and illegibility of operational field.We applied sucting and scraping cold separating method mainly by suction apparatus which can keep operational field clear and avoid hot injury.Result 52 cases with different cholecyst disease (including cholecystolithiasis,cholecyst polyp,acute cholecystitis with edema and acute cholecystitis with partly gangrene) were adopted by this method and achieved satisfacted effect.Conclusion Sucting and scraping cold separating method are easily operated and can mostly keep separated tube skeletonized.It can avoid hot and other injury in operation,keep operational field clear by less errhysis.To acute cholecystitis with edema it has more advantage and are worth to adopting.
4.Use of Drugs in Patients with Acute Cerebral Infarction During 2003~2006

OBJECTIVE: To optimize the therapeutic regimen for acute cerebral infarction for clinical reference. METHODS: Medical records of patients in who acute cerebral infarction occurred for the first time in a hospital during 2003~2006 were analyzed statistically in respect of drug use. RESULTS: In terms of DDDs, dehydrants, diuretics, platelet aggregation inhibitors, promoting cerebral circulation agents and supportive drugs took the lead in this hospital. The DDDs of drugs used for complications in descending order were antiinfectives, hypotensors, and drugs for heart diseases. CONCLUSION: The drugs used for inpatients with acute cerebral infarction varied in kinds; therefore, clinical choice of drugs and the rationality of drug use remain to be further studied.
5.The challenges to hepatitis C research in era of direct antiviral agents

Hepatitis C virus (HCV)is an important human pathogen that causes chronic viral hepatitis worldwide.HCV infection,often persistent and asymptomatic,could lead to liver fibrosis/cirrhosis and hepatocellular carcinoma.The interferon/ribavirin therapy has been widely used for treating chronic hepatitis C,but it is not always effective and has strong side effects.The development of small-molecule di-rect antiviral agents (DAA)has greatly improved the curing efficiency of chronic hepatitis C therapeutics and will likely replace the interfer-on-based therapy in the future.However,these developments do not necessarily suggest that the problems of chronic hepatitis C have been completely solved.The HCV research is still facing many challenges in this new DAA era.
6.EVALUATION FOR EFFECTS OF LOPID AND FENOFIBRATE ON LIPIDS METABOLISM

Effects of Lopid and Fenofibrate on serum lipids, lipoproteins, apolipoproteins were studied in 22 diabetic patients with dyslipidaemia. The levels of FBG, HBA1, lipids, lipoproteins and apolipoproteins were determined before treatment, 3 weeks and 6 weeks after treatment. The results showed: 1). Lopid lowered the levels of TC, HDL-TG,TG,apoB100 and raised the levels of HDL-C, apoA-I significantly.2). Fenofibrate lowered the levels of TG, TG, HDL-TG, apoB100 and raised the level of HDL-C, but no effect on apoA-I. Comparing the efficacy of these two drugs on lipids metabolism, i seems that Lopid is better than Fenofibrate. Both of the drugs can be well tolerated by most diabetic patients.
7.Two Conformations of Native Pseudomonas aeruginosa Apoazurin in Solution

The mechanism of complex unfolding process of Pseudomonas aeruginosa apoazurin is an arguing problem. Recent published results indicated that this problem could be resolved by hypothesizing two native conformations coexisting in solution.Urea-induced unfolding of apoazurin was investigated further using intrinsinc fluorescence and CD spectra. Equilibrium unfolding curves in urea could be depicted with an apparent two-state transition, but a biphase kinetic process. The fast unfolding process is finished within a few seconds as monitored by stopped-flow fluorescence intensity, whereas the slow process requires several hours for as well as the circular dichroism spectra just after manual mixing of protein and denaturant could be simulated by superposition of the spectra of the native and fully unfolded protein with the same coefficient of 0.37. This strongly suggests the three-state mechanism with a partially unfolded intermediate on its pathway is inadequate. The hypothesis of two native conformations coexistence is a reasonable selection.
8.Diagnostic value of screening autism children by Chinese edition infant and early children social development screening test

Objective:To screen autism children by Chinese edition infant and early children social development screening test and to evaluate the diagnostic value.Methods:From October 2006 to May 2007,843 normal children from 3 children health care clinics(N=564) and 9 kindergartens(N=331) were selected and assessed with the Chinese Edition Infant and Early Children Social Development Screening Test.The 95% reference range was made.From February to June 2007,in the third affiliation hospital of Sun yet-san university,170 children who were diagnosed as autism and 391 normal children were selected to draw ROC curve to evaluate the checklist's screening ability.Results:(1) The total scores of the scale were increased with the age of subjects(F=643.83,P<0.001).(2) The areas below ROC curve of whole checklist corresponding three age period were 0.969,0.988 and 0.994.The scores of whole checklist were 52.5,54.5 and 63.5,when the screening ability was best. Conclusion:The diagnostic value of the checklist for autism children is so good that it is useful for clinical evaluation on the social cognitive development of young children between 0.5~3.5 years.
9.Impact of self-management of hypertension on self efficacy among community residents

Objective To evaluate the impact of hypertension self-management on self efficacy of community residents.Methods November 2011 to February 2012,twenty-eight medium-sized and better adherence to community was selected from fourteen districts and counties in Beijing.In this community-based randomized controlled trial,323 hypertensive patients were randomly assigned to the treatment group (n =159,receiving hypertension self-management) and the control group (n=164,receiving standard hypertension management).A questionary survey was completed at baseline and 6 weeks.Rank sum test or Chi-square test was used for data analysis.Results The average age of the treatment group or the control group was (61.9 ±9.1) vs (61.8 ±9.0) years old,respectively.No significant difference in mean age,gender and education level was found between the two groups (F =0.164,x2 values were 0.782 and 2.093,respectively ; all P > 0.05).In the treatment group,the scores of fatigue,headache and shortness of breath were declined (when compared with the controlled females,Z =-5.198,-3.873,-2.781 ; P < 0.05).Self-efficacy score of the treatment group was increased after the intervention.Women's symptom management self-efficacy and common disease management self-efficacy were significantly higher (Z=-2.958,-2.582; P < 0.05).In comparison with the control group,before and after self-management,the added scores of communication ability with doctors were increased.But the difference was not statistically significant (P > 0.05).Conclusion Six-week self-management program may improve self efficacy among hypertensive patients.
10.Study on the correlation between preoperative albumin-globulin ratio and prognosis of patients with low rectal cancer surgery

Objective:To analyze the preoperative albumin-globulin ratio (AGR) of patients with low rectal cancer, and explore its correlation with the clinical outcome of patients after surgery.Methods:One hundred and fifty-two patients who underwent radical resection of low rectal cancer at the General Surgery Department of the First Affiliated Hospital of Xinjiang Medical University from June 2013 to June 2015 were selected as the research objects. According to the clinical outcome of the patients, the patients were divided into survival group (128 cases) and death group (24 cases). The general clinical data and preoperative and postoperative serum ALB, GLB, and AGR levels of the two groups of patients were compared; multivariate logistic regression analysis of factors affecting postoperative death of patients; a nomogram prediction model was constructed and verified based on the independent risk factors selected ; ROC curve analyzes the predictive value of AGR for patients′ clinical outcomes, and obtains the best cut-off value; analyzes the correlation between AGR and postoperative clinical outcomes of patients; divides patients into group A and group B according to the cut-off value, and draws by Kaplan-Meier method Survival curve, comparing the survival rate of the two groups of patients during the 5-year follow-up period.Results:In the survival group, the number of tumor stages was Ⅰ, Ⅱ, and Ⅲ, the number of cells with high/moderate differentiation, the depth of tumor invasion was T1/T2, the number, the number of lymph node metastasis, the number of chemotherapy, the preoperative ALB, GLB, and AGR levels were 68 cases, respectively, 42 cases, 18 cases, 99 cases, 95 cases, 108 cases, 60 cases, (43.52±6.73) g/L, (23.75±4.38) g/L, 1.90±0.34, the death group was 6 cases and 11 cases respectively, 7 cases, 15 cases, 17 cases, 18 cases, 8 cases, (39.42±4.28) g/L, (26.58±4.12) g/L, 1.62±0.45, the difference was statistically significant ( t/ χ2=18.695, 4.455, 5.120, 3.742, 2.137, 2.873, 2.931, 3.506, all P<005), all of which were independent risk factors leading to death. ALB, GLB, and AGR in the survival group were (38.72±5.69) g/L and (24.13±4.56), respectively g/L, 1.60±0.35, ALB and AGR were significantly lower than before operation ( t=6.162, 6.956, both P<005), and the death group were (34.18±4.37) g/L and (26.92±4.25) g/L, respectively, 1.27±0.39, ALB and AGR were significantly lower than before operation ( t=4.197, 2.879, both P<005), ALB and AGR of the death group were significantly lower than those of the survival group, and GLB was significantly higher than that of the survival group, the difference was statistically significant ( t=3.705, 2.779, 4.162, all P<005); the best cut-off value of AGR predicting patient death was 1.73; AGR≤1.73 was closely related to postoperative recurrence and death ( OR=1.18, 1.23, 95% CI: 1.02-1.59, 1.08-1.64, both P<005); the number of survivors of patients with AGR>1.73 was 98, which was significantly higher than that of patients with AGR≤1.73 (30 cases). The difference is statistically significant ( χ2=24.869, P<0.05). Conclusion:The preoperative serum AGR level is an independent risk factor affecting the postoperative clinical outcome of patients with low rectal cancer. Patients with a low level of AGR are at greater risk of postoperative death. The preoperative serum AGR level has a higher predictive value for the patient′s clinical outcome.
