2.Clinical Observation of Pudilan Xiaoyan Oral Liquid Combined with Bronchial Dilatation Drugs in the Treat-ment of Moderate and Severe Stable Chronic Obstructive Pulmonary Disease

OBJECTIVE:To observe the clinical efficacy and safety of Pudilan xiaoyan oral liquid combined with bronchial dil-atation drugs in the treatment of moderate to severe stable chronic obstructive pulmonary disease(COPD). METHODS:According to admission time,102 patients with stable moderate to severe COPD were randomly divided into treatment group(n=53)and con-trol group(n=49). Control group received Budesonide and formoterol fumarate powder for inhalation and Tiotropium bromide pow-der for inhalation;based on control group,treatment group was additionally treated with Pudilan xiaoyan oral liquid,10 ml/time, 3 times a day, for 12 weeks. The lung function index, number of acute COPD attack, modified medical research council (mMRC),CAT and CCQ were performed and comparison in the two groups. RESULTS:After 12 weeks of treatment,the IC of treatment group was improved significantly,compared to before treatment and control group;there was statistical significance(P<0.05). There was no statistical significance in FEV1 and FEV1/FVC%of treatment group after treatment,compared to before treat-ment and control group(P>0.05);there was no statistical significance in the number of acute COPD attack between 2 groups(P>0.05). mMRC,CAT score and CCQ total score of treatment group were improved significantly,compared to before treatment and control group;there was statistical significance (P<0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Pudilan xiaoyan oral liquid combined with bronchial dilatation drugs for inhalation is better than powder for inhalation in the treatment of moderate to serve stable COPD,can improve pulmonary function significantly,exercise tolerance and quality of life.

4.Dual-phase18F-FDG coincidence detection SPECT/CT imaging for differential diagnosis of pulmonary lesions

Background and purpose:Although FDG tumor imaging has been applied in clinic widely, dual-phase imaging can provide much more information about the FDG uptaking of pulmonary lesions. The purpose of the study was to evaluate the usefulness of dual-phase18F-FDG coincidence detection SPECT/CT imaging in the differential diagnosis of the pulmonary lesions.Methods:There were 28 patients with pulmonary lesions which were detected by CT. All the patients undertook the SPECT/CT imaging at 2 time-phases respectively: early imaging at 40-60 min and delayed imaging at 2-3 h after the intravenous injection of FDG. Data processing: calculating the radio of T and N in early and delayed imaging respectively; T: The radioactive count of the lesions; N: The radioactive count of the normal tissue; and the change rate:ΔT/N. ROC was used to ifnd out the threshold of T1/N1, T2/N2及ΔT/N in the differential diagnosis between benign and malignant lesions. AUC was used to evaluate the diagnosis value of the dual-phase and single-phase imaging.Results:The threshold of T1/N1 in early imaging was 2.65, whereas AUC was 0.767. The sensitivity, speciifcity and accuracy were 83.3%, 30% and 64.3%, respectively. The threshold of T2/N2 in delayed imaging was 3.14, whereas AUC was 0.847. The sensitivity, speciifcity and accuracy were 94.4%, 60.0% and 82.1%, respectively. The threshold ofΔT/N in delayed imaging was 16.9%, whereas AUC is 0.950. The sensitivity, speciifcity and accuracy were 88.5%, 71.4% and 86.2%, respectively.Conclusion:Dual-phase18F-FDG coincidence detection SPECT/CT imaging has much higher accuracy and speciifcity. However it still has false positivity, and should be analyzed with CT and clinical history.
6.Nosocomial infection characteristics and control strategies of initial treatment acute leukemia patients

Objective To investigate the characteristics of co-infection in initial treatment acute leukemia induction chemotherapy.Methods The clinical features of 179 untreated acute leukemia patients with nosocomial infection were analyzed after combined chemotherapy.Results In the 179 patients,151cases achieved complete remission,the complete remission rate was 84.4 %,82 cases suffered from nosocomial infections,the incidence of infection was 45.8 %.The sites of infection were oral,anal,lung,as well as primary foci was not clear bacteremia.In 428 specimens,the isolated bacterial colony counted a total of 66,the number of fungal colonies was 9,the bacterial colony was G-bacteria-based.G-bacteria had different degrees of resistance to many antibiotics.Extended-spectrum β-lactamases strains had not been detected in these specimens.Conclusion Acute leukemia patients is easy to co-infection after chemotherapy.Control and prevention of nosocomial infections should run throughout the entire treatment process,application of laminar flow bed helps reduce the newly diagnosed acute leukemia patients with nosocomial infection incidence.
7.Survey on Secondary Research of Traditional Chinese Medicine Patent Literatures

This survey included 59 secondary researches of traditional pharmaceutical patent literatures. The analysis was made on statistical indicator of the amount of annual papers, authors, institutions, published journals and re-search contents. The survey revealed that in the field of traditional Chinese medicine (TCM), the number of sec-ondary research of patent literature was on the increase, and the degree of concern of TCM patent was far more than others. This survey not only assisted the understanding of current condition of TCM patent literatures, but also held its development trend in order to apply it in the studies.
8.Antibiotics prescribed to medical patients visiting polyclinic at 2008 Beijing Olympic Village

Objective To evaluate prescribing antibiotics in medical patients visiting the polyclinic at 2008 Beijing Olympic Village for its further reasonable use. Methods Clinical prescriptions for all the medical patients at the polyclinic were reviewed retrospectively. Consistency between antibiotics prescriptions and their indications formulated before 2008 Beijing Olympic Games was evaluated, as well as related factors for their overuse. Results In total, 1537 patients visited the polyclinic and 1233 recipes were prescribed,with 956 patients (62. 2% ) with infections, including 809 (52. 6% ) of respiratory infections. One hundred and forty-nine of 1537 patients (9. 7% ) were treated with antibiotics, accounting for 12. 1 percent of all prescriptions, and 43 cases (28.9%) were identified as antibiotics overuse, including 39 cases with respiratory infections. Prevalence of antibiotics overuse was significantly more for patients of domestic employees than that for non-employees (34. 6% vs. 14.3% , P <0. 01 ), with art independent association between them (OR 3.07, 95% CI 1.11 - 8. 50). Conclusions Although the polyclinic has successfully reduced unnecessary antibiotics use, their overuse remains to be reduced, and physicians' strict adhesion to indications for antibiotics use and patient education are essential for their rational use in primary care of China.
9.Relationship between Tei index and the myocardial enzymes in newborns after asphyxia LEI Xiao-yan,

Objective To study the relationship between Tei index and the myocardial enzymes in newborns with myocardial damage due to asphyxia. Methods Forty-four newborns with mild asphyxia,27 newborns with severe asphyxia and 20 healthy newborns hospitalized in our hospital from January 2005 to December 2008 were enrolled in this study.The peak E and peak A and E/A ratio at mitral valve,the ejection fraction,isovolumetric contraction time,isovolumetric relaxation time and Tei index were measured by conventional echocardiography,and aspartate aminotransferase,creatine kinase,lactate dehydrogenase,hydroxybutyrate dehydrogenase and creatine phosphate kinase isoenzyme,and cardiac troponin T were tested also.One-way ANOVA was used to analyze the difference between groups. Results Tei index,isovolumetric contraction time and isovolumetric relaxation time in the severe asphyxia newborns were 0.62±0.13,(47±7)ms and(52±8)ms,higher than those in the mild asphyxia newborns[0.51±0.14,(41±6)ms and(43±6)ms],those in the mild asphyxia newborns were higher than in the healthy newborns[0.39±0.12,(34±6)ms and (37±6)ms] (all P<0.01).Aspartate aminotransferase,lactate dehydrogenase,hydroxybutyrate dehydrogenase,creatine kinase and its isoenzymes and cardiac troponin T in the severe asphyxia newborns were higher than in the mild asphyxia newborns and healthy group(P<0.01).Tei index,isovolumetric contraction time and isovolumetric relaxation time were positively related to the myocardial enzymes.The positive relationships were more statistically significant in Tel index,creatine kinase,its isoenzymes and cardiac troponin T(r=0.762,0.821,0.778,P<0.01).Conclusions Combination of Tei index and myocardial enzymes can not only be applied to assess the cardiac function in newborns with myocardial damage due to asphyxia,but also to evaluate the degree of cardiac dysfunction accurately and dynamically.
