2.Clinical characteristics of lymph node metastasis in Siewert type II adenocarcinoma of the gastroesophageal junction.

OBJECTIVETo explore the pattern of lymph node metastasis (LNM) in advanced Siewert type II adenocarcinoma of the gastroesophageal junction (AGEJ) in order to properly guide lymphadenectomy.
METHODSFrom January 2009 to Jun 2011, a total of 86 patients with advanced Siewert type II AGEJ underwent radical esophagogastrectomy by thoracic-abdominal incision and two-field lymphadenectomy. The clinical characteristics, pathologic features, LNM, and influence factor of thoracic metastasis were retrospectively analyzed.
RESULTSLNM was observed in 65 of the 86 cases (75.6%). Simple abdominal lymph node metastasis was observed in 49 of the 86 cases (57.0%), simple thoracic lymph node metastasis was 2.3%, and thoracic-abdominal metastasis was 16.3%, with a significant difference between the abdominal and thoracic metastatic patterns. LNM frequency was found in the lymph node groups No. 3, No. 1, No. 7, No. 110, No. 2 and No. 9 (from the highest to the lowest). The incidences of those lymph node metastases were 46.5%, 41.9%, 17.4%, 14.0%, 10.5%, and 5.8%, respectively. Vascular tumor embolus was the only independent factor for thoracic lymph node metastasis.
CONCLUSIONSAbdominal lymph node metastases of advanced Siewert type II AGEJ mainly occur around the proximal stomach and the coeliac trunk. The metastatic rate of distal stomach and splenic perihilar lymph nodes is low, but metastatic rate of the group No.110 lymph nodes is high. The thoracic lymph node metastasis is only correlated with vascular tumor embolus.

3.An investigation of iodine content of food in six provinces of China

Objective To investigate the iodine content of food in six provinces of China,to add the results of this survey to the food iodine content database,and to provide a scientific basis for iodine supplementation in different parts of China.Methods A total of 8 categories and 39 species common food produced locally in the six provinces of Fujian,Chongqing,Shandong,Anhui,Gansu and Jilin were collected.Samples of cereals,beans and other dry samples were crushed into powder; samples of fresh fruits and vegetables were washed and dried to constant weight,and crushed into powder; poultry,meat and fish samples were washed and then their edible parts were crushed into meat paste,bake dried to constant weight,and crushed into powder.Iodine content in the above-mentioned food was determined by catalytic spectrophotometry,and the wavelength was 405 nm.Data processing and statistical analysis were carried out by using SPSS 13.0 statistical software.The results of total iodine content of the various types of food were expressed as median(P50) and interquartile range(P25 and P75).Results The iodine content of the cereal in Fujian,Chongqing,Shandong,Anhui,Gansu and Jilin were 11.9,12.0,48.0,95.1,13.0and 3.1 μg/kg,respectively; of the potato were 53.9,26.3,74.9,43.7,76.8 and 38.5 μg/kg,respectively; of the meat and the eggs were 56.0,30.4,78.6,124.6,47.7 and 34.8 μg/kg,respectively; of the aquatic products were 319.3,144.7,186.6,241.3,155.4 and 213.3 μg/kg,respectively; of the vegetables were 166.6,145.1,131.7,218.0,205.4 and 98.1 μg/kg,respectively; of the fruits were 105.5,17.8,80.9,1.7,76.7 and 10.3 μg/kg,respectively; of the kelp and laver were 36.0 × 103,1292.0 × 103,2810.0 × 103,48.0 × 103,75.0 × 103 and 120.0 × 103 μg/kg,respectively; of the Chinese pickled vegetables were 640.4,4163.5,3073.7,2635.3,1540.9 and 492.0 μg/kg,respectively.ConclusionsThe iodine content of different types of food,and same kind of food from different provinces are different.The results are a complement to the 2004 Chinese food composition database.
4.Screening of Poly-?-glutamic Acid High Productive Strain and Optimization of Fermentation Medium

Bacillus subtilis B6-1 was used as an original strain for mutagenic treatment and a defined medium was used as the selective medium. A mutant named B. subtilis W003 was isolated after three serial ultraviolet (UV) irradiations and one diethyl sulfate (DES) treatment. The ?-PGA yield on a rotary shaker was enhanced from 10.9 g/L in parental strain to 20.5 g/L in the mutant. It was illustrated by single factor experiments that the optimal carbon and nitrogen sources were glucose and (NH4)2SO4 respectively. The optimal fermentation medium was achieved by orthogonal test. In the optimal medium, a ?-PGA yield of 45.3 g/L was obtained after 36 h cultivation.
5.Expressions of nucleostemin and Ki-67 in pituitary adenomas and their clinical significance

Objective To study the expressions of nucleostemin gene mRNA and Ki-67antigen in pituitary adenomas and investigate the role of nucleostemin gene in the tumorigenesis and development of pituitary adenomas. Methods Seventy-one samples of pituitary adenomas were collected. Semi-quantitative reverse transcriptase-polymerase chain reaction (RT-PCR) was used to detect the expression of nucleostemin gene mRNA in samples. Immunohistochemistry technique was applied to examine Ki-67 antigen expression in the paraffin sections of samples. At the same time,coherent clinical data were collected. Results Nucleostemin gene mRNA was detectable in all samples of pituitary adenomas. Between invasive and noninvasive pituitary adenomas, the difference of nucleostemin expression was extremely significant (P<0.01), and Ki-67 labeling index (LI) was also significantly different (P<0.05). The difference of Ki-67LI between recurrent and non-recurrent groups was significant (P<0.05). There was a positive correlation between nucleostemin gene and Ki-67LIlevels (r=0.237, P<0.05). Conclusions Nucleostemin gene plays an important roles in the invasion of human pituitary adenoma. Expression of nucleostemin gene is positively related to Ki-67 antigen expression, and both may be the valid clinical detection markers for assessing proliferation, invasion and recurrence of pituitary adenomas.

Conclusion APBSCT may improve the disease activity and the immunological markers in SLE.It is a valid therapy for refractory SLE,but the long-term effects need to be observed.

Thyroglobulin(Tg)is a glycoprotein with large molecular weight,which is synthesized and secreted into the bloodstream by thyroid follicular cells.The concentration level of Tg in blood is one of the important biomarkers for diagnosis of thyroid diseases such as differentiated thyroid cancer(DTC),subacute thyroiditis,etc..Radioimmunoassay(RIA),immunoradiometric assay(IRMA),and immunochemiluminescence assay(ICMA)are the main clinical methods for detecting Tg.Recently,for meeting the requirement of detecting low concentration of Tg in blood after thyroid clearance surgery,researchers have developed various high-performance analysis methods for detecting Tg concentration in blood samples,providing new assays for thyroid disease screening and efficacy evaluation.This review summarized the analysis methods of Tg,especially the new progresses in the biosensors for monitoring low concentration of Tg in blood during the past five years.The current technical challenges of these methods in clinical applications were briefly discussed,which might provide useful information for developing new liquid biopsy methods of DTC.
8.Application of urinary proteomics in early diagnosis of bladder urothelial carcinoma.

OBJECTIVETo investigate the difference in urinary proteome between patients with bladder urothelial carcinoma (BUC) and healthy volunteers and to provide a basis for the early diagnosis of BUC.
METHODSThe urine samples from BUC patients and healthy volunteers (controls) were treated by 25% ethanol precipitation and two-dimensional gel electrophoresis (2-DE), and the obtained urinary proteins were subjected to Coomassie brilliant blue staining and analysis by PDQuest 8.0 (2-DE image analysis software); the differentially expressed proteins were sequenced by matrix-assisted laser desorption/ionization time-of-flight/time-of-flight mass spectrometry and identified using the Swiss-Prot database; the differential expression of these proteins was verified by western blot.
RESULTSHigh-resolution and high-reproducibility 2-DE images were obtained from the urine samples of BUC patients and controls, with 789 ± 18 and 762 ± 14 protein spots, respectively. Compared with the control group, the BUC grouP had significantly decreased expression of 6 protein spots and significantly increased expression of 11 protein spots. The mass spectrometry revealed five proteins with increased expression in the BUC group, including fibrinogen, lactate dehydrogenase B, apolipoprotein A1, clusterin, and haptoglobin, and the results were confirmed by western blot.
CONCLUSIONThere is significant difference in urinary proteome between BUC patients and healthy volunteers; the identification of differentially expressed proteins in urine lays the foundation for identifying potential molecular markers in early diagnosis of BUC.

9.Effects of fenvalerate exposure on the semen quality of occupational workers.

OBJECTIVESTo study the effects of fenvalerate exposure on the semen quality of occupational workers in order to find out the early effective biomarkers.
METHODSThirty-two male workers who exposed to fenvalerate and 46 male administrators in the office in a pesticide factory were selected as the exposure group and internal control group, respectively, and 22 male administrators in a center for disease control were served as the external control group. In order to evaluate the exposed levels, the concentration of fenvalerate, toluene and xylene in the ambient air of working place in these three groups were monitored simultaneously for three consecutive days. After the semen were collected according to the standard method, the workers' semen qualities were analysed with University of California at Davis (UCDavis) method and the sperm motility were evaluated using computer assisted sperm analysis(CASA).
RESULTSIn the exposure group, the concentrations of fenvalerate were significantly higher than those in the internal and external control group (P < 0.01), while no significant difference of the concentration on toluene or xylene was found (P > 0.05). Sperm motion parameters in the exposure group such as linearity(LIN), straightness(STR), and the sperm count were decreased significantly, and the abnormality rate of viscidity, coagulation and sperm count were increased significantly as compared with the internal and the external control groups(P < 0.05). Furthermore, the sperm progression and beat cross frequency (BCF) in the exposure group were also lower significantly than those in the external control group(P < 0.05), while the abnormality rate of sperm progression was increased significantly.
CONCLUSIONSIn such a low concentration, occupational exposure to fenvalerate can affect workers' semen quality, especially the sperm count and sperm movement ability.

10.Study on the effects of different risk stratifications in patients with diabetes mellitus: a clinical endpoint event

Objective To estimate the relationship between the risk stratification of patients with diabetes and their clinical endpoint events.Methods In this cohort study,we prospectively followed 8302 individuals under the following situations: contents of fasting plasma glucose ≥7.0 mmol/L,being diagnosed as diabetes or having used hypoglycemic drugs from Kailuan study in which 101510 employees (81110 males,20400 females,who were being employed and those retired from the company were included) from the Kailuan Company,were screened.During the 38-53(48.01 ± 3.14) months of follow-up period,a new heart or cerebrovascular events were ascertained every six months.The impacts of different risk stratification in diabetic population on the incidence rates of cardiovascular and cerebrovascular events were estimated.Results Using the definitions of “people with ischemic cardiovascular disease incidence of 10-year risk assessment methods”developed by the Chinese Academy of Medical Sciences,Institute of Cardiovascular Disease,the study cohort was divided into four groups,namely,very low-risk,low risk,medium risk and high risk.(1) Along with the increasing risk of the disease,the incidence rates of total cardiovascular and cerebrovascular events,myocardial infarction,stroke,cardiovascular death and all-cause death rate also gradually increased and the differences were statistically significant (P<0.01).However,the difference on incidence rate of sudden death was not significantly different (P>0.05).(2)Compared to the very low-risk group,the age and sex adjusted relative risk for cardiovascular and cerebrovascular events were 1.42 (95%CI: 1.02-1.96,P<0.05),2.26 (95%CI: 1.67-3.04,P<0.01 ) for those with medium and high risk groups,respectively.Conclusion In diabetic patients,those risk factors as age,hypertension,body mass index,total cholesterol and smoking having been used on ischemic cardiovascular disease,could also be used to predict the occurrence of cardiovascular events.Along with the increasing risk factors,the risk of cardiovascular events incidence also increased.
