Objective:To investigate the effects of valproate sodium on P-CREB1 after hippocampal neuronal epileptiform discharge in rat.Methods:The neonate wistar rats were decapitated quickly to obtain the hippocampal neuron,Which were cultured in vitro,After the epileptiform discharge model of neuron was established,neurons were divided into control group,model group,low valproate dose group(50mg/L) and high dose group(100mg/L).Expression positions of P-CREB1 after epileptiform discharge were examined by immunofluorescence technique,and Western blot was used to examine the expression intensity of P-CREB1 in different group.Results:Through immunofluorescence,P-CREB1 was observed in the nucleus of each group,and the most intensive expression was found in model group.Through western blot,the expression tendency was found to be the same as the former Results,Moreover,after added valproate sodium,the expression of P-CREB1 decreased,and there was statistical significance of the difference between low valproate dose and high dose(P

BACKGROUND:The clinical treatment of thoracolumbar fractures and dislocation of the patients can be treated with pedicle screw fixation through adjacent vertebral segments. With the increased incidence of disease, many problems were found such as fixator loosening and breakage. OBJECTIVE:To compare the effectiveness of vertebral pedicle screw fixation versus cross-segment pedicle screw fixation for thoracolumbar vertebral fracture and dislocation. METHODS:100 patients with thoracolumbar vertebral fracture and dislocation were chosen in the 474 Hospital of Chinese PLA from February 2013 to May 2014. The patients were randomly assigned to two groups (n=50). Patients in the control group were treated with cross-segment pedicle screw fixation, and those in the observation group were treated with the vertebral pedicle screw fixation. Parameters of imaging were compared and observed. RESULTS AND CONCLUSION: Significant differences in the spinal canal area improvement value and internal fixation failure rate were detected between the two groups (P < 0.05), and the observation group was better than the control group. The height ratio of anterior border of the vertebral body was higher at 12 months after treatment than that pretreatment, but Cobb angle was lower than pretreatment (P < 0.05). These data indicate that compared with cross-segment pedicle screw fixation, vertebral pedicle screw short-segment fixation for thoracolumbar vertebral fracture and dislocation obtains more ideal outcomes and higher safety. It is more in line with biomechanics at the same time ensuring the fixing strength.

Optimal parameters of submerged fermentation for Antrodia camphorata were studied.According to the yield of Intracellular triter- pene,the optimal fermentation parameters were obtained as follows:40g/Lglucose,6 g/L soybean,1 g/L K_2HPO_4,0.5 g/L MgSO_4 and 100mg/L VB_1.The optimum cultural period was 6 d at 26℃in 250mL shake flasks at 100r/min.The best aeration ratio was 1:2.5(medi- um volume:flask volume).The inoculation volume was 20%.The intracellular triterpene of Antrodia camphorata was obtained(about 15.25mg/100mL) in the optimum culture.

Objective To investigate the relationship between nerve location and acupunture effect in patients with peripheral facial paralysis.Methods92 patients with peripheral facial paralysis, included simple facial paralysis (36 cases), Bell's paralysis (46 cases) and Hunt's paralysis (10 cases), were treated with acupuncture and medicine.ResultsEffect of the treatment was different from each groups. The curetive effect of simple facial paralysis was higher than Bell's paralysis and Hunt's paralysis, the Hunt's palsy had the lowest effect.ConclusionThe effect of acupuncture on peripheral facial palsy is depended on the location of injury.

OBJECTIVE To understand the factors of mycotic pharyngitis or stomatitis for patients after tracheotomy,especially related to antibiotic.METHODS Seventy patients after tracheotomy were divided into two groups.In each group,the clinical data,risk factors and culture results of oral cavity were analyzed.RESULTS The time,kind,dose,way,and combined use of antibiotic were all related to mycotic pharyngitis or stomatitis.CONCLUSIONS The longer time,the complex kind,the larger dose,and the combined use of antibiotic are all delayed patients recovering.

Objective To investigate the value of echocardiography and multi-slice spiral CT in the diagnosis of complex congenital heart disease(CCHD).Methods 73 children with CCHD were selected and diagnosed by echocardiography and multi-slice spiral CT in Children''s Hospital of Shanxi Province from April 2012 to April 2016.The distribution of disease types in 73 children with CCHD was recorded,and the diagnostic accuracy of multi-slice spiral CT was compared with echocardiography,multi-slice spiral CT and echocardiography.Results 73 children with CCHD had a total of 172 cardiac malformations confirmed by operation and pathology.By segment analysis,there were 30 cases of extracardiac vascular malformations,52 cases of abnormal heart and vascular connection,90 cases of atrial and atrioventricular junction deformity.Multislice spiral CT was performed to detect 155 cases of cardiac malformations,including 28 cases of extracardiac malformation,48 cases of maligant heart and great vessels,79 cases of intracardiac and atrioventricular malformation.Echocardiographic diagnosis of 158 cases of cardiac malformations,including 28 cases of extracardiac vascular malformations,49 cases of large vascular malformations,81 cases of intracardiac and atrioventricular junction deformity.Echocardiography combined with multi-slice spiral CT in the diagnosis of 167 cases of cardiac malformations,including 29 cases of extracardiac vascular malformations,50 cases of large vascular malformations,88 cases of intracardiac and atrioventricular malformation.The diagnostic accuracy between echocardiography [91.86％(158/172)] and multislice spiral CT [90.12％(155/172)] had no significant difference (P＞0.05).The diagnostic accuracy of echocardiography combined with multi-slice spiral CT was 97.09％(167/172),which was higher than 91.86％(158/172) of echocardiography and 90.12％(155/172) of multislice spiral CT,the difference was statistically significant(P＜0.05).Conclusion The echocardiography combined with multi-slice spiral CT in the diagnosis of CCHD has high accuracy,it can provide strong imaging support for surgical options and prognosis.

Objective To explore the effect of an in-hospital cardiac rehabilitation program on exercise capacity after cardiac surgery.Methods Forty-two patients with valvular heart disease or coronary heart disease were recruited and randomly divided into a rehabilitation group (n=22) and a control group (n=20).Both groups were given preoperative education and breathing guidance on the basis of conventional clinical treatment,but the rehabilitation group was also provided with in-hospital cardiac rehabilitation after the operation.The two-minute stepping test (2-MST) and the left ventricular ejection fraction test (LVEF) were performed before and after the intervention for both groups.Their hospital stays were also compared.One month after discharge,the exercise capacity of both groups was followed-up by telephone using the Veterans-specific Activity Questionnaire (VSAQ).Results After the intervention,there was no significant difference in average LVEF between the two groups (53.73±9.29％ and 56.25± 11.01％ respectively).The average 2-MST stepping frequency of the rehabilitation group was significantly greater than the control group (79.41± 10.99 times and 71.25± 14.21 times respectively,P＜0.05).One month after discharge,the exercise capacity of the rehabilitation group was also significantly higher than the control group for (5.77± 1.46 METs versus 4.75± 1.34 METs,P＜0.05).Stepping frequency at discharge and exercise capacity at one month after discharge were both positively correlated to 2-MST stepping frequency at admission(r =0.869,P＜0.01;r=0.752,P＜O.01).The average hospital stay of the rehabilitation group was significantly shorter (by about 3 days) than the control group (15.59±3.08 days versus 18.45±3.63 days,P＜0.01).Conclusions In-hospital cardiac rehabilitation can significantly improve exercise capacity after cardiac surgery and reduce the length of hospital stays.The 2-minute step test is a simple and practical way to assess the exercise capacity of patients early after cardiac surgery.

Objective To evaluate the clinical practicability of cefoxitin disk diffusion method in detecting methicillin-resistant Staphylococci.Methods 163 strains of clinically isolated Staphylococci were tested.The cefoxitin,oxacillin disk diffusion test and PCR amplification of mecA gene were performed simultaneously.The method of PCR was used as the golden standard.Results 163 strains of clinically isolated Staphylococci were tested by PCR.The positive ratio of mecA gene was 81.0%,The ocurrence rate of methicillin-resistant S.aureus(MRSA) and (methicillin-resistant coagulase-negative Staphylococci(MRCNS) was 74.1% and 83.8%,respectively.The cefoxitin,oxacillin disk diffusion and PCR were high consistency in detecting mecA gene.Both sensitivity and specificity were 100%,But cefoxitin disk diffusion was better for methicillin-resistant coagulase-negative Staphylococci.The sensitivity was 100% and 96.0%,the specificity was 94.3% and 88.0%,respectively.Conclusion Cefoxitin disk diffusion has high sensitivity in detecting methicillin-resistant Staphylococci,but for methicillin-resistant coagulase-negative Staphylococci which mecA gene is negative,5% strain may be reported as MRCNS by mistake.Since cefoxitin disk diffusion method is simple,and has higer sensitivity and specificity than oxacillin disk diffusion method in detecting MRS,the result has good correlation with PCR in detecting mecA gene.It may be applied in clinical microbiology laboratory.
