Objective To study the changes of cerebral haemodynamics in neonates with hypoxic-ischemic encephalopathy(HIE) treated with Shengmai injection compared with citicoline, and explore the clinical interventions of Shengmai injection on HIE.Methods Fifty patients with HIE were randomized into 2 groups: shengmai group treated with Shengmai injection(n=30),and citicoline group treated with citicoline(n=20).The haemodynamic parameters included peak systolic flow velocity(PSFV),end-diastolic flow velocity(EDFV),pulsation index(PI) and resistance index(RI) of anterior and middle cerebral artery were detected by color Doppler ultrasound before treatment and on 7~(th) day after treatment respectively.Control group(n=50) compared with the two groups. Results PSFV,PDFV and RI of HIE patients were significantly lower than those of control group(P

Aldose reductase ( AR ) belonging to nicotinamide-adenine dinucleotide phosphate ( NADPH ) -dependent aldehyde-keto reductase superfamily, is the key rate-limiting enzyme in the polyol pathway which plays an important role in the body’s high-sugar metabolism. AR is widely present in the kidneys, blood vessels, lens, retina, heart, skeletal muscle and other tissues and organs, converts glucose to sorbitol which easy permeability of cell membranes, cause cell swelling, degeneration, necrosis, and have a close relationship with the development of chronic complications of diabetes mellitus. Diabetic retinopathy ( DR ) is a multifactorial disease, the exact cause is currently unknown, but polyol pathway has been demonstrated to play an important role in the pathogenesis of DR. Clinical risk factors such as blood sugar control, blood pressure and other treatments for DR only play a part effect of remission or invalid, if we can find out DR genes associated with the disease, this will contribute to a better understanding of the pathological mechanisms and contribute to the development of new treatments and drugs. The current research progress of AR, AR gene polymorphism, Aldose reductase inhibitors to DR was reviewed in this article.

Objective To obtain the recombinant protein LytA (rLytA) of Streptococcus pneumoniae strain (ATCC49619) through prokaryotic expression system and to investigate their diagnostic value for patients with community acquired pneumonia (CAP).Methods The specific primers were designed according to LytA gene sequence of Streptococcus pneumoniae M66 strain recorded in Genbank. The recombinant plasmid pET32a(+)/LytA was constructed and transformed into BL21(DE3) to express LytA.The expressed protein LytA was purified by electroeluting of bag filter.Serum IgM of anti-LytA accordingly of patients with CAP were detected by ELISA.The results were evaluated by Chi-square test. Results The recombinant protein LytA was expressed and purified successfully with a relative molecular weight of 56 000.The IgM antibodies level of anti-LytA was significantly higher than the healthy control group (P=0.000).Diagnostic sensibility and specificity of LytA-IgM were 27.8% and 100.0%,while sensibility and specificity of sputum culture were 19.4% and 72.2%,respectively.The sensibility of LytA-IgM was equal to sputum culture(χ2=0.693,P=0.405),but the specificity was higher than it(χ2=14.316 P=0.000).Conclusions A rLytA-ELISA assay maybe has clinical value for diagnosis of pneumococcal infections. It is more rapid and objective than the culture method.

Basic & Clinical Medicine 2006;0(04):-
Objective To study the mechanism of dermal fibroblasts as a feeder layer to support the growth of human keratinocytes. Methods Human dermis fibroblasts were isolated and cultured and then treated with mitomycin-C. The expression of type Ⅰand type Ⅲ precollagen mRNA and relevant protein in feeder layer were examined by RT-PCR and Immunohistochemistry. KCs were cultured both on FB and NIH3T3 feed layer as control, the adhering numbers and the time of fusion were recorded. Results RT-PCR showed an increase of type Ⅰprecollagen mRNA in FB feeder layer as compared with that of normal fibroblasts (P

AIM: To evaluate the effect of intravitreal bevacizumab (IVB) injection 1 week before pars plana vitrectomy (PPV) in proliferative diabetic retinopathy (PDR) patients. METHODS: A retrospective research was done on 46 PDR patients who were divided into PPV group (n=28) and IVB group (n=18, PPV with preoperative IVB). Bevacizumab was injected 1 week before PPV. Main outcome measures were visual acuity, incidence of iatrogenic retinal breaks, intraoperative and postoperative bleeding. RESULTS: At 1 month after surgery, visual acuity in PPV (82.1%) and IVB group (88.9%) improved significantly (P<0.01) and the difference between the two groups was not significant. Iatrogenic retinal breaks were reported in 18 cases (64.3%) in PPV group and 4 cases (22.2%) in IVB group (P<0.05). Intraoperative bleeding was encountered in all cases in PPV group and 7 cases (39%) in IVB group (P<0.01). Postoperative bleeding was reported in 9 cases (32.1%) in PPV group and none in IVB group (P<0.01). CONCLUSION: IVB injection before PPV is helpful in reducing iatrogenic retinal breaks, intraoperative and postoperative bleeding in PDR patients.

By participating in the national medical colleges and universities students' clinical skill competition and summarizing the experiences after the competition,we reflected on problems and weakness in the past clinical teaching including weak sterile concept and lack of clinical thinking, humanities, communication skills, teamwork awareness, etc. We should take methods to future improve medical students' clinical training capabilities including strengthening the concept of sterile and clinical skills, promoting training of comprehensive clinical thinking ability and problem-solving ability, emphasizing on humane care and communication between doctors and patients; cultivating teamwork awareness thus to comprehensively enhance the overall quality of medical students.
10.Tongue acupuncture.
Chinese Acupuncture & Moxibustion 2010;30(4):347-348
Tongue acupuncture is originated from the theory of TCM through scientific research. Forty points on the tongue are discovered that correspond to organs and certain parts of the body. The findings indicate that the patient with encephalopathy is accompanied by the pathological swelling sublingual fold. The sublingual fold is correlated with spinal and limbs lesions. Tongue acupuncture provides new approaches to treat many diseases. This paper introduces the determined points, manipulation skills and the theory of tongue acupuncture. The applications of PET, fMRI and VEP in tongue acupuncture are covered in this paper.
