1.Predicting Burnout And Psychological Distress Risks Of Hospital Healthcare Workers

Burnout and psychological distress were reported at higher rates among hospital healthcare workers. Despite this, there is a paucity of research examining the associated risk factors among workers across specialties and occupations in Asia. This paper aimed to examine the risk factors associated with burnout and psychological distress among Malaysian hospital healthcare workers from diverse medical specialties and occupations. A total of 368 doctors, nurses, assistant medical officers and hospital attendants were recruited from major medical and surgery departments in an urban general hospital. The participants were self-administered a questionnaire consisting of demographic information, Maslach Burnout Inventory-Human Services Survey and Depression Anxiety and Stress Scale. In the fully adjusted multivariate analyses, doctors were about four to five times more likely to be emotionally exhausted (aOR [adjusted Odds Ratio], 4.826; 95% CI [Confidence Interval]: 1.492-15.604, p<0.01), depressed (aOR, 5.221; 95% CI: 1.995-13.661, p<0.01) and stressed (aOR, 3.990; 95% CI: 1.473-10.809, p<0.01). Paediatric workers demonstrated three to five times higher risks of depression (aOR, 3.105, 95% CI: 1.043-9.243, p<0.05), anxiety (aOR, 3.517, 95% CI: 1.194- 10.356, p<0.05) and stress (aOR, 5.404, 95% CI: .1.628-17.942, p<0.01). Emotional exhaustion (aOR, 1.046, 95% CI: .1.013-1.079, p<0.01) and depersonalization (aOR, 1.078, 95% CI: .1.015-1.145, p<0.05) led to higher risks of psychological distress, while stress predicted higher risks of burnout (aOR, 1.153, 95% CI: 1.062-1.251, p<0.01). There were occupational and departmental differences in susceptibility to burnout and psychological distress, requiring further investigation into the unique working environments and roles of hospital healthcare workers.

2.A cross-sectional study on self-determined motivation towards physical activity among healthcare professionals at a tertiary hospital in Makati.

BACKGROUND
According to the World Health Organization, in 2016, there were more than 1.9 billion adults who were overweight. Of these, over 650 million were obese. Physical inactivity is one of the major risk factors for several non-communicable diseases. Healthcare workers who have direct contact with patients often influence their behaviors. However, health care workers who educate their patients but they themselves do not practice what they recommend, may be one of the barriers that can affect patient education and influence.
OBJECTIVEThis research paper aimed to investigate the motivation to participate in physical activity among healthcare workers and to determine the association between the profile of respondents with the different types of motivation.
METHODSThere was a total of 250 randomly selected respondents who were included in the study. Data were gathered through a self-administered questionnaire utilizing the International Physical Activity Questionnaire-Short Form (IPAQ-SF) and the Behavioral Regulation in Exercise Questionnaire (BREQ-2). Descriptive statistics was used to summarize sociodemographic information, physical activity levels and BREQ-2 profiles. T-test was used to analyze differences in gender while analysis of variance (ANOVA) was used for levels of physical activity and professional category.
RESULTSFemale healthcare professionals have a higher average in amotivation (mean=0.56) while males have a higher average in intrinsic (mean=2.82) motivation. Nurses and ancillary services have higher average amotivation scores than physicians.
CONCLUSIONHealth care workers who have high classification in physical activity have the highest average scores in terms of relative autonomy index, introjected, identified, and intrinsic scores. This demonstrates a positive association between motivation from internal regulation and increased physical activity.

3.The peripandemic mental well-being of selected healthcare workers in Chong Hua Hospital: Implications for institutional mental health program.

In the realm of mental health promotion and public mental health, a consistent acknowledgment has prevailed, recognizing the imperative to enhance mental well-being while concurrently preventing mental illnesses. Such recognition extends to the pivotal role that healthcare providers’ overall health plays in their capacity to deliver comprehensive and quality patient care. This study embarked on a journey to illuminate the mental well-being landscape of healthcare workers within the ambit of Chong Hua Hospital, spanning the tumultuous peripandemic period and beyond. Employing a quantitative research design, the standardized Warwick-Edinburgh Mental Well-Being Scale (WEMWBS) was methodically administered to discern the intricate threads of mental well-being. The study’s findings stand poised to serve as the cornerstone for the crafting of an institutional mental health program, meticulously tailored to the diverse needs of healthcare workers within the hospital. Results revealed that resident doctors and the PGIs while generally reporting a high level of mental well-being in the peripandemic period, a sizeable proportion of these medical healthcare workers still succumbed to manifesting poor mental well-being. The paper also proved that married and those who contracted the infection were a notable cohort who recorded a significantly poorer mental well-being. The need to prioritize well-being immediately and after the pandemic so that services are sustained cannot be overemphasized hence the myriad of recommendations for a comprehensive and sustainable institutional mental health program was proposed in this paper.

4.The effectiveness of mindfulness meditation on burnout among healthcare workers: A systematic review and meta-analysis.

INTRODUCTION
Burnout is becoming more common among healthcare professionals, notably during the COVID-19 pandemic. It can result in lower performance and effectiveness at work as well as employment withdrawal, all of which affects the standard of healthcare services provided.
OBJECTIVEIn order to ascertain the effectiveness of mindfulness meditation-based interventions (MMBIs) in reducing burnout among healthcare workers, a systematic review and meta-analysis was done.
METHODSTwo investigators searched records in CENTRAL, PubMed/MEDLINE, Google Scholar, Preprints, Grey Literature, and cross-referencing to acquire articles using search terms related to “mindfulness meditation”, “healthcare workers”, and “burnout”. Inclusion criteria included randomized controlled trials (RCTs) and nonrandomized controlled trials (NRTs) that assessed the effectiveness of MMBIs on burnout as measured by the Maslach Burnout Inventory (MBI) among healthcare workers in the hospital setting. Study selection, data extraction, risk of bias assessment were done by the investigators independently. Analysis was done using RevMan 5 software, forest plots were generated, and subgroup analyses were done.
RESULTSOf 25,453 identified records, 28 studies were included. The studies were rated with low to unclear selection bias and high risk of performance bias. MMBIs were associated with significant reduction on the emotional exhaustion, depersonalization and personal accomplishment subscales with pooled mean differences of -2.60 (95% CI = -3.64, -1.55), -0.51 (95% CI = -0.77, -0.26), and 0.82 (95% CI = 0.24, 1.39), respectively. On subgroup analyses, the types of MMBI implemented had no influence in the intervention effect noted on all subscales among RCTs but had significant influence among NRTs. Reduction of burnout was noted to be higher in nurses compared with physicians and mixed healthcare workers. Overall quality of evidence for RCTs was low to moderate and very low to low for NRTs.
CONCLUSIONThe results suggest that MMBIs can reduce the burnout symptoms of healthcare workers. To address the high risk of bias of included studies and improve quality of evidence, future research should be done with high-quality RCTs.

5.Factors affecting willingness to report to work during COVID-19 pandemic among health care workers in a tertiary government hospital.

BACKGROUND
Healthcare workers’ (HCWs) willingness to report to work despite personal risk is a requisite for an effective pandemic response. At present, there are no local studies that have examined the factors affecting willingness to report to work during the COVID-19 pandemic.
OBJECTIVETo determine the factors associated with willingness to report to work during COVID-19 pandemic among healthcare workers in a tertiary government hospital.
METHODSThis was a cross sectional study among the nursing staff (nursing attendants, nurses), doctors (residents, fellows), medical technologists, radiologic technologists, and respiratory technicians in a tertiary government hospital, who were employed from January 2021 to January 2022. Data was collected through an online questionnaire and was analyzed using SPSS.
RESULTSA total of 311 participants included in the study. The median age of the respondents was 34 (29-46) years old. More than a third of the workers were nurses (37%) followed by residents and fellows (34%), nursing attendants (19%), radiologic technologists, medical technologists, and respiratory technicians (10%). Over 4 out of 5 were assigned in a non-COVID area while 11% were assigned in the COVID area. The odds of willingness to report to work is 60% lower among males compared to females. On the other hand, the odds of willingness to report to work was 78% lower among nurses and 84% lower among residents and fellows compared to medical technologist, radiologic technologists, and respiratory therapists. The median rating of the staff on willingness to report to work was 80% (60-90), and 73% of respondents were willing to report to work during the entire COVID-19 pandemic.
CONCLUSIONFactors that were associated with willingness to report to work were female gender and occupation (radiologic technologists, medical technologists, respiratory technicians).

6.Teachers as health workers in the Philippines.

OBJECTIVES
While the chronically overburdened state of public school teachers in the Philippines is well-established, little is known about how they specifically provide ‘care’ and attend to their students’ health in the workplace. This article addresses that knowledge gap by illustrating the many forms of ‘health work’ undertaken by public school teachers on a daily basis, and analyzing the concrete challenges they face in doing such work. In so doing, this article provides a qualitative construction of school teachers as ‘health workers’ in the country.
METHODSThis article draws from two focus group discussions conducted in Southern Luzon and Eastern Visayas between November 2018 to May 2019, as part of a multi-sited study on the health-related challenges faced by lowand middle-income Filipinos. A total of 19 teachers participated in those two discussions. We used the principles of thematic analysis to code and analyze the discussion transcripts.
RESULTSTeachers regularly fulfill various tasks that can be considered health work, including measuring students’ anthropometrics; supervising and administering government programs like deworming, feeding programs, and vaccinations; providing first-aid and various forms of immediate medical attention; addressing students’ mental and psychological health concerns; and working with students’ families in ways that resemble social work. However, the study participants largely considered themselves unqualified to do health work, often sacrificed their own personal health and interpersonal relationships to take on the additional burden of health work, and felt they received insufficient institutional support.
CONCLUSIONDoing health work not only takes away from actual teaching time, but also comes at the expense of teachers’ own health. Hiring the appropriate personnel to conduct health work and improving legal safeguards are possible solutions to ameliorating the present working conditions of teachers. Yet, the larger and more long-term conversation demands the prioritiziation of teachers’ well-being and overall quality of life, and recognizing the cruciality of a healthy work-life balance for them. Future studies should involve more diverse geographic sites and teacher populations, and utilize more focused forms of analysis (e.g., comparative, policy-driven).

7.Knowledge, attitudes, and practices of barangay health workers in Marawi City regarding Hansen’s disease.

OBJECTIVE
To assess the knowledge, attitudes, and practices of Barangay Health Workers (BHWs) in Marawi City regarding Hansen’s Disease (HD).
METHODSA cross-sectional study was conducted among the Barangay Health Workers of Marawi City. This study was conducted in two phases. Phase 1 was questionnaire development where the knowledge, attitudes, and practices (KAP) questionnaire was formulated and administered to six Barangay Health Workers for pre-testing. Phase 2 of the study included the survey and focus group discussion (FGD). A questionnaire comprising of 27 questions was administered to BHWs to assess knowledge, attitude, and practices regarding HD.
RESULTSA total of 49 BHWs underwent the Phase 2 survey and six BHWs joined the FGD. The mean age of all the survey participants was 34.9 ± 19.3 years. Among the 49 participants, 40 (81.6%) were females. The knowledge of BHWs was found to be adequate only in six (12%) BHWs. Positive attitude was observed in 12 (24.5%) and adequate practices regarding HD were observed in 33 (67.3%) BHWs. First-hand experience of diagnosing HD patients was significantly associated with adequate knowledge (p < 0.001) and positive attitude of BHWs (p < 0.001). There was a significant association between > 5 years of experience as BHWs (p = 0.027) and first-hand experience in diagnosing leprosy (p = 0.005) with adequate practices of BHWs regarding HD. In the FGD, BHWs expressed their lack of training which highlighted the need for refresher courses on HD.
CONCLUSIONKnowledge about HD is low among Marawi BHWs due to their inadequate training. The attitudes and practices of BHWs are also affected because of deficient knowledge regarding etiology and transmission of HD. There is a need for adequate training and refresher courses on HD to increase the knowledge of BHWs regarding HD.

8.Physical activity and fitness level of healthcare workers in a tertiary teaching hospital

BACKGROUND AND OBJECTIVES
Physical activity (PA) and fitness level are considered key factors in public health promotion, and as such, healthcare workers (HCW) need to be physically well because they are not only responsible for themselves, but of their clients as well. Therefore, we aim to determine the PA and health-related fitness (HRF) level of HCW in a tertiary hospital, disaggregated to age, sex, work assignments, and job tenure.
METHODSA cross-sectional research design was utilized to assess HCW from the administrative, medical, nursing, and paramedical cohorts. ALPHA-FIT Test Battery was used to assess cardiorespiratory fitness, muscular strength, lower extremity power and strength, upper body muscle endurance, balance, and body composition.
RESULTSOur study concurred with the results of previous studies on HCW yielding lower PA levels compared with the general population. We tested 282 participants (administrative: 97; medical: 36; nursing: 55; paramedical: 94), mean age 37.4 years, 64.54% females, 52.13% perceived average health status. Ninety percent of the HCW had PA levels below the World Health Organization’s recommended levels. The ALPHA-FIT mean score per category revealed: one-legged stance, 2.67/3; figure-of-8 run, 2.45/3; shoulder-neck mobility, 4.58/5; modified push-up, 2.05/4; hand-grip strength, 2.5/5; jump and reach, 4/4; dynamic sit-up, 2.62/3; six-minute walk test, 475.38 meters covered. There were no significant differences in PA and HRF levels across cohorts except for modified sit-ups (p < 0.001) and figure-of-8 run (p = 0.012). The results showed significant inverse correlation between balance and shoulder-neck mobility and age (p < 0.001), and modified push ups (p = 0.004). Males had significantly higher sit-up scores (p < 0.001), one-legged stance scores (p = 0.001), and faster figure-of-8 run (p = 0.011), while females had better jump and reach scores (p < 0.001).
CONCLUSIONPhysical activity levels of HCW did not meet the World Health Organization’s recommended PA levels. Healthcare professionals who are expected to be aware of the benefits of PA and HRF have shown low to midfit levels of grip strength, upper extremity endurance, core strength, and cardiorespiratory endurance. Information on PA and HRF may aid in policy making on employee wellness that could highly impact health service delivery.

9.Implementation of Total Laboratory Automation and its Outcome Analysis for Laboratory Productivity.

BACKGROUND: To cope with demands of ever-increasing test volumes, laboratories have implemented the total laboratory automation (TLA) system. The main goals of TLA are superproductivity, shortened turnaround time and universal availability of laboratory data throughout the hospital and/or hospital network. The TLA system was installed at Samsung Medical Center (SMC) in 1994; at Asan Medical Center (AMC) in 1995. To evaluate the benefits of TLA system, the comparative study between the laboratories with varying degree of TLA installation and Seoul National University Hospital (SNUH) without TLA installation was conducted. METHODS: Three laboratories of tertiary care university hospitals participated in this study; SMC (with the full TLA installation), AMC (with the TLA installation in outpatient laboratory) and SNUH (without TLA installation). To evaluate productivity of a TLA system, the productivity of a technologist and a laboratory person between laborotories with and without TLA systems, the comparison of productivity between TLA laboratories and non-TLA laboratories in AMC, and test numbers per hospital bed were obtained. To evaluate the qualitative aspects, the turnaround time (TAT) analysis was carried out. For overall performance index, the length of hospital stay was calculated and analyzed. RESULTS: The test numbers per technologist in AMC, SMC, and SNUH were 177,082.2, 161,966.7 and 95,752.5, repectively in 1997. The test numbers per laboratory personnel including physicians in AMC, SMC, and SNUH were 108,287.7, 118,682.5, and 67,304.3, repectively in 1997. Those per hospital bed were 6841.8, 12194.1, and 6192.0, repectively. The turnaround times of routine hematologic tests in AMC (Outpatient) and SMC were 75 minutes and 102 minutes, and those of routine chemistry tests were 49 minutes and 106 minutes. CONCLUSIONS: The laboratories with TLA installation showed much improved productivities in all categories analyzed; especially test numbers per hospital bed. The TLA system would be the vehicle of the laboratory to gain the superproductivity without sacrificing the quality of laboratory and to ensure the short turnaround time.

10.Serogroup Distribution of Beta-hemolytic Streptococci Isolated from a Tertiary Care Hospital at Wonju Area during the Recent 4 Years (1997~2000).

BACKGROUND: Beta-hemolytic streptococci (BHS) are frequently isolated pathogens in the clinical microbiology laboratory, and the isolation frequency of each serogroup of BHS are influenced by patient type, nation and region, study period, and investigators. The aim of this study is to investigate the isolation rates and distribution of BHS by serogroup at Wonju area during the recent periods. METHODS: Isolation rates of BHS isolated from 1997 to 2000 at Wonju Christian Hospital were investigated. The distribution of each serogroup of BHS according to the types of clinical specimens was also studied. Serogroups A, B, C, F, and G were determined by latex agglutination test (Murex Streptex kit). RESULTS: Of 627 BHS isolated during the study periods, 16.3% were group A, 49.3% were group B, 5.1% were group C, 4.3% were group F, 21.0% were group G, and 4.0% were nongroupable. Isolation rates of BHS by specimen type were as follows:wound/pus was 44.4%; cervix, urine, and respiratory specimen ranged from 10% to 20%; blood was 4.0%; body fluid was 2.9%; and cerebrospinal fluid was 0.5%. The prevalent isolation sites by serogroup were:group A, wound/ pus (78.4%); group B, cervix (32.7%), urine (28.5%), and wound/pus (26.2%); group C and G, wound/pus and respiratory specimen; and group F, wound/pus and urine. From blood, group B streptococci (44.0%) were the most frequent isolate. CONCLUSIONS: Half of BHS isolated from clinical specimens were group B streptococci in this study, which reflects the increasing trend of group B streptococcal infections during the recent years at Wonju area.
